Reproducibility of the Mayo and Schatzker classification systems in proximal ulna fractures.

INTRODUCTION: A fracture classification system should provide a reliable and reproducible means of communication between different parties. It should be logical and understandable, with few categories to memorize. The aim of this study was to determine the intra- and interobserver reliability of the Schatzker and Mayo classification systems for the assessment of proximal ulna fractures. MATERIALS AND METHODS: Intra- and interobserver reliability studies were conducted on 39 X-rays of injured elbows drawn randomly from 74 cases previously used in a series on predictors of ulnohumeral osteoarth

Outcomes of ulna shortening osteotomy: a cohort analysis of 106 patients.

BACKGROUND: Ulna shortening osteotomy (USO) for ulnar impaction syndrome (UIS) aims to improve pain and function by unloading the ulnar carpus. Previous studies often lack validated patient-reported outcomes or have small sample sizes. The primary objective of this study was to investigate patient-reported pain and hand function at 12 months after USO for UIS. Secondary objectives were to investigate the active range of motion, grip strength, complications, and whether outcomes differed based on etiology. MATERIALS AND METHODS: We report on 106 patients with UIS who received USO between 2012

Isolated cryptococcal osteomyelitis of the ulna in an immunocompetent patient: A case report.

BACKGROUND: Cryptococcal osteomyelitis is a bone infection caused by cryptococcus. As an opportunistic infection, bone cryptococcosis usually occurs in patients with immunodeficiency diseases or in those undergoing immunosuppressive therapy and often displays characteristics of disseminated disease. Isolated cryptococcal osteomyelitis is extremely unusual in immunocompetent person. The pathogenic fungus often invades vertebrae, femur, tibia, rib, clavicle, pelvis, and humerus, but the ulna is a rare target. CASE SUMMARY: A 79-year-old woman complaining of chronic pain, skin ulceration and a s

Three-dimensional cortical and trabecular bone microstructure of the proximal ulna.

INTRODUCTION: The three-dimensional (3D) microstructure of the cortical and trabecular bone of the proximal ulna has not yet been described by means of high-resolution 3D imaging. An improved characterization can provide a better understanding of their relative contribution to resist impact load. The aim of this study is to describe the proximal ulna bone microstructure using micro-computed tomography (micro-CT) and relate it to gross morphology and function. MATERIALS AND METHODS: Five dry cadaveric human ulnae were scanned by micro-CT (17 μm/voxel, isotropic). Both qualitative and quantitat

Crista Supinatoris Fractures of the Proximal Part of the Ulna: Surgical Technique.

INTRODUCTION: Open reduction and internal fixation of crista supinatoris fractures is required when the elbow is unstable despite appropriate nonoperative management and when a patient is undergoing surgical treatment of a periarticular elbow fracture-dislocation. STEP 1 SKIN INCISION AND SURGICAL APPROACH: Use a posterior or lateral skin incision according to your preference and then utilize the Kocher interval to access the joint, lateral collateral ligament, and crista supinatoris or, in the setting of a proximal ulnar fracture, use the Boyd interval. STEP 2 MANAGEMENT OF ASSOCIATED INJUR

Feasibility and fidelity of practising surgical fixation on a virtual ulna bone.

BACKGROUND: Surgical simulators provide a safe environment to learn and practise psychomotor skills. A goal for these simulators is to achieve high levels of fidelity. The purpose of this study was to develop a reliable surgical simulator fidelity questionnaire and to assess whether a newly developed virtual haptic simulator for fixation of an ulna has comparable levels of fidelity as Sawbones. METHODS: Simulator fidelity questionnaires were developed. We performed a stratified randomized study with surgical trainees. They performed fixation of the ulna using a virtual simulator and Sawbones.

Intraosseous lipoma in the ulna and radius of a two-year-old Leonberger.

This report describes a case of intraosseous lipoma in a two-year-old Leonberger. The dog was presented with a history of ten month lameness in the right forelimb. A massive swelling from the elbow to the carpus of the right forelimb was visible. Treatment with anti-inflammatory medications by the local veterinarian for ten months was unsuccessful and the dog was presented at the university clinic. Radiographic images showed that the diaphyseal part of the ulna was affected by extensive cyst-like osteolysis. Furthermore, the distal metaphysis of the radius showed cyst-like osteolytic changes.

Acute dislocations of the distal radioulnar joint and distal ulna fractures.

This article reviews acute dislocations of the distal radioulnar joint (DRUJ) and distal ulna fractures. Acute dislocations can occur in isolation or in association with a fracture to the distal radius, radial metadiaphysis (Galeazzi fracture), or radial head (Essex-Lopresti injury). Distal ulna fractures may occur in isolation or in combination with a distal radius fracture. Both injury patterns are associated with high energy. Outcomes are predicated on anatomic reduction and restoration of the stability of the DRUJ.

Management and outcome of 103 acute fractures of the coronoid process of the ulna.

A series of 103 acute fractures of the coronoid process of the ulna in 101 patients was reviewed to determine their frequency. The Regan-Morrey classification, treatment, associated injuries, course and outcomes were evaluated. Of the 103 fractures, 34 were type IA, 17 type IB, ten type IIA, 19 type IIB, ten type IIIA and 13 type IIIB. A total of 44 type-I fractures (86%) were treated conservatively, while 22 type-II (76%) and all type-III fractures were managed by operation. At follow-up at a mean of 3.4 years (1 to 8.9) the range of movement differed significantly between the types of fractu

Bilateral stress fracture of the ulna in an adult weightlifter: a case report.

Bilateral stress fractures of the ulna are extremely rare and have only been described in non-adults. In this case report we present a young adult weightlifter who reported bilateral stress fractures of both ulnae after increasing his training frequency and load. Stress fractures of the ulnae are examples of bone stress injuries due to repetitive muscle pull. The diagnosis can be made by radiography, isotope bone scan, CT or MRI. Thorough training analysis and adaptation is essential to treat this rare injury. Return to full activity can take up to three months.

Osteolysis of the radius and ulna induced by a circumferential foreign body in a cat.

CASE DESCRIPTION: A 1-year-old neutered male cat was examined because of a 6-month history of recurrent swelling and draining wounds affecting the craniomedial aspect of the distal portion of the left forelimb. CLINICAL FINDINGS: No lameness or neurologic deficits were evident. Examination of craniocaudal and lateromedial radiographic views revealed nonprogressive circumferential osteolysis and a mildly radiopaque, ring-shaped foreign body surrounding the radius and ulna. TREATMENT AND OUTCOME: During surgery, a tight elastic band surrounded by a thick fibrous tissue capsule was found encirc

Wide excision of the distal ulna: biomechanical testing of a salvage procedure.

PURPOSE: To study changes in the relationship of the distal radius and ulna after progressive resection of the distal ulna. METHOD: Six-degree-of-freedom motion sensors were used to determine relative motion and a displacement transducer was used to determine strain changes in the interosseous membrane (IOM). These were applied in cadaveric specimens loaded to simulate wrist motion and axial compressive forces. RESULTS: Strain in the IOM decreased with increasing resection of the distal ulna. The distance between the radius and ulna decreased with increasing resection up to 20% of ulna lengt

Derotational osteotomy at the shafts of the radius and ulna for congenital radioulnar synostosis.

PURPOSE: The purpose of this study was to describe the results of a newly developed method of correction osteotomy for congenital radioulnar synostosis. With this method the osteotomy is performed at the distal one third of the radius and proximal one third of the ulna. After K-wires are inserted intramedullarly into both bones the forearm is derotated manually to the position planned before surgery followed by cast immobilization. METHODS: Four patients with an average age of 3.9 years underwent surgery with this method and were followed-up for 45.8 months. All of their forearms were fixed b

Physeal fractures of the distal radius and ulna: long-term prognosis.

OBJECTIVES: The long-term prognosis of injuries to the distal physis of forearm bones, including complications such as radioulnar length discrepancy and styloid nonunion, has not been extensively studied. Reliable radiographic prognostic criteria to predict physeal disturbance at trauma are also lacking. The aim of this study is to investigate both issues. DESIGN: Retrospective study. SETTING: University hospital. PATIENTS/PARTICIPANTS: One hundred sixty-three lesions to the distal physis of the forearm bones in 157 patients were available for a long-term follow-up. An analysis of the constraint properties of the distal radioulnar ligament attachments to the ulna.

Nine cadaver upper extremities were tested to evaluate the constraint properties of the dorsal and palmar radioulnar ligaments at their foveal and styloid attachments to the ulna. The specimens were tested by anterior and posterior displacement of the radius relative to the ulna with the forearm in pronation, supination, and neutral rotation. There were no statistically significant differences in the relative percent of constraint contribution of the styloid and foveal ligament insertions. In neutral forearm rotation total displacement after sectioning both ligament insertions tended to be lar
