Definitive ureteroscopy and intracorporeal lithotripsy in treatment of ureteral calculi during pregnancy.

OBJECTIVE: To evaluate the outcome of using semi-rigid ureteroscopy with or without intracorporeal pneumatic lithotripsy vs. temporary ureteric JJ stenting in the management of obstructing ureteric calculi in pregnant women. PATIENTS AND METHODS: This prospective comparative study comprised 43 pregnant women with obstructing ureteric calculi. The diagnosis was based on the acute flank pain as the main symptom, microscopic haematuria, and unilateral hydronephrosis on abdominal ultrasonography (US). The patients were randomly divided into two groups; those in group 1 (22 patients) were treated

Ureteroscopy for treatment of upper urinary tract stones in children: technical considerations.

The incidence of pediatric urolithiasis is increasing. While many smaller stones may pass spontaneously, surgical therapy is sometimes warranted. Surgical options include shock wave lithotripsy, ureteroscopy, percutaneous nephrolithotomy, and open surgery. Ureteroscopy represents a minimally invasive approach, and it is increasingly being used to treat pediatric upper tract calculi. Ureteroscopy is performed under anesthesia and fluoroscopic guidance, with basket extraction or lithotripsy of the calculi. Technical considerations include active or passive ureteral dilatation, the use of uretera

Changes in renal Doppler ultrasonographic parameters in patients managed with rigid ureteroscopy.

BACKGROUND: There has been no study evaluating the intrarenal hemodynamic changes after ureteroscopy in the published literature. PURPOSE: To determine preoperative and postoperative intrarenal vascular parameters such as resistive index (RI), pulsatility index (PI), peak systolic velocity (PSV), end-diastolic velocity (EDV), measure changes on these values (ΔRI, ΔPI, ΔPSV, ΔEDV) after ureteroscopy (URS) and compare the outcomes with the results of normal contralateral kidneys, and finally investigate possible parameters that would affect renal vascular resistance changes. A practical pressure measuring method for the upper urinary tract during ureteroscopy.

PURPOSE: The purpose of this study was to develop and test a method for measuring pressure in the upper urinary tract during ureteroscopic operations and to evaluate its efficacy and clinical significance. METHODS: A total of 44 patients, each with a ureteral calculus in the proximal ureteral segment, were enrolled in the study group: 21 patients with an acute and 23 with a chronic obstruction. The ureteroscope was passed forward to the upper segment of the obstructed ureter immediately after the calculus was broken and the intraluminal ureteral pressure was then transmitted along with the ir

Use of ureteroscopy and holmium:yttrium-aluminum-garnet laser in the treatment of an infundibular stenosis.

An infundibular stenosis is defined by a dilated calyx draining through a narrowed infundibulum into a nondistended renal pelvis. We describe the use of ureteroscopy and the holmium:yttrium-aluminum-garnet (YAG) laser to successfully treat an infundibular stenosis in a 27-year-old woman who presented with left flank pain. The holmium:YAG laser is well suited for this application. It can be applied with precise control of the direction and depth of the cut. The retrograde approach avoids the morbidity of a percutaneous nephrostomy and is well suited for treating an anterior infundibular stenosi

Transurethral ureteroscopy. Safety guide wire as an aid to a more aggressive approach.

In an unselected, consecutive series of patients, ureteroscopy was carried out for removal or manipulation of stones in 48 patients and for evaluation of suspected ureteric tumours in two. A success rate of 92% was achieved. The use of an intra-ureteric safety guide wire, indwelling throughout the procedure, facilitated ureteroscopic manipulation, minimised complications, reduced the need for subsequent open surgery and has been a prerequisite for our comparatively aggressive approach to ureteric disorders. The procedure may be time-consuming but prolonged manipulation within the ureter seems

Extending cystoscopic techniques into the ureter and renal pelvis. Experience with ureteroscopy and pyeloscopy.

The availability of extended-length, small-caliber endoscopes allows the use of many cystoscopic techniques in the ureter and renal pelvis. Although the instruments are rigid, they can be passed transurethrally in both male and female patients to allow the urologist to diagnose and treat many lesions of the upper urinary tract. The success of our series demonstrates the utility and safety of the technique, which often eliminates the need for surgical exposure of the lesions.

Does Continuous Intrarenal Pressure Predict Post-Ureteroscopy Sepsis? A Novel Pilot Study.

PURPOSE: Postoperative sepsis is a known complication of ureteroscopy, but whether intrarenal pressure (IRP) is indicative of postoperative sepsis remains unclear. Pyelovenous backflow is believed to occur when pressure exceeds 30 mmHg. Despite this, IRP is not routinely monitored during ureteroscopy. The present pilot study was designed to evaluate the relationship between IRP during ureteroscopy and postoperative sepsis. MATERIALS AND METHODS: Patients undergoing ureteroscopy for kidney stones (N = 100, 34% male, M = 56 years of age) had IRP continuously measured using the COMET™ II Pressur

BMI-stratified nomograms to predict early SIRS-defined sepsis after flexible ureteroscopy.

BACKGROUND: Postoperative sepsis is a serious complication of flexible ureteroscopy (FURS). Body mass index (BMI) may modify risk, but BMI-stratified prediction models are lacking. We developed and internally validated BMI-specific nomograms for early SIRS-defined sepsis after FURS. METHODS: This single-center retrospective cohort included 1,254 adults undergoing FURS (2020-2024), stratified by BMI (<25 vs. ≥ 25 kg/m2). Multivariable logistic regression identified independent predictors of SIRS-defined sepsis within 48 h in each stratum. BMI-specific nomograms were constructed and internally

Endoscopic visual cues of intrarenal pressure during ureteroscopy: in vivo porcine studies.

OBJECTIVES: To characterize the endoscopic visual appearance of the collecting system during ureteroscopy in a porcine in vivo model across a range of steady state intrarenal pressures (IRPs). MATERIALS AND METHODS: A flexible ureteroscope with pressure sensor was advanced into the renal pelvis of anaesthetised female porcine subjects. Endoscopic images were captured at IRPs from 8 to 200 cmH2O, and the collecting system was categorized as follows: insufficiently distended (ID), partially distended (PD), or fully distended (FD). The colour of the urothelium and the prominence of urothelial ve

Novel scoring system for predicting stone-free rate after flexible ureteroscopy lithotripsy.

This study aims to investigate the factors affecting the stone-free rate (SFR) of flexible ureteroscopy and laser lithotripsy (fURSL) for renal stones and establish predictive models by identifying their prognostic factors. We retrospectively examined 252 patients with renal stones who were treated with fURSL between July 2020 and April 2022. We analyzed the relationship between the patient's clinical data (sex, age, and body mass index), stone status (side, size, location, stone/transverse process pixel ratio [STPR], and the CT value of stone [SCTV]), and SFR to determine the relevant factors

Ureteroscopy during pregnancy under local anesthesia and light sedation: Technique and video.

Ureteroscopy during pregnancy has become increasingly recognized as a safe intervention. Performing it under local anesthesia and light sedation reduces the risks associated with general or regional anesthesia, such as difficult airway scenarios, hypothermia, and hypotension. In addition, this approach reduces the total amount of fetal exposure to medications and anesthetic agents. Performing ureteroscopy in this manner requires a number of adjustments and modifications to the standard technique. This article provides a summary in a step-by-step format, as well as an accompanying video demonst

Robot-assisted ipsilateral ureteroureterostomy in a duplex system utilizing intraoperative flexible ureteroscopy.

Iatrogenic injury to the ureter during pelvic surgery is an uncommon but well-documented complication. Accurate identification of the ureter during pelvic surgery is made far more complex in the presence of a duplex or ectopic system, an anomaly occurring in up to 2% of the population. In this article we present a technique for robot-assisted ipsilateral ureteroureterostomy for treatment of iatrogenic injury of a lower pole moiety ureter in a complete duplex system.

Two-part silicone mold. A new tool for flexible ureteroscopy surgical training.

INTRODUCTION AND OBJECTIVES:: Flexible ureteroscopy is a common procedure nowadays. Most of the training programs use virtual reality simulators. The aim of this study was to standardize the building of a three-dimensional silicone mold (cavity) of the collecting system, on the basis of polyester resin endocasts, which can be used in surgical training programs. MATERIALS AND METHODS:: A yellow polyester resin was injected into the ureter to fill the collecting system of 24 cadaveric fresh human kidneys. After setting off the resin, the kidneys were immersed in hydrochloric acid until total co

Placement of ureteral stent after uncomplicated ureteroscopy: do we really need it?

OBJECTIVE: To investigate the role of stenting after uncomplicated ureteroscopy. METHODS: This was a systematic review and meta-analysis of randomized controlled trials from MEDLINE, the Cochrane Central Search Library, and the EMBASE database. All of the studies reported various outcomes with or without stenting after ureteroscopy. RESULTS: Fourteen trials were identified. As there was significant heterogeneity in pain assessment, difference in postoperative pain between the stent and nonstent groups was not clear. The incidence of dysuria, frequency, and hematuria was statistically signifi
