Urethral calculi.

The diagnosis and management of 34 men and 13 women with urethral calculi are reviewed. A majority of patients presented with dysuria, weak stream and perineal pain. Diagnosis was made radiographically in only 42 per cent but was confirmed in all patients who underwent cystourethroscopy. All patients had lower urinary tract lesions or previous prostatic operations and 47 per cent were infected. Migrant urethral calculi were encountered 10 times more often than were native stones and a metabolic cause was found in 10 of 18 patients studied. In men penile urethral calculi were removed transureth

Urethral Calculi.

Urolithiasis is a condition with calculi commonly found within the kidney, ureter, or bladder. The urethra is an uncommon location of urolithiasis, with limited case reports and literature reviews of its presentation and management. Here we discuss a 24-year-old female who presented with urinary urgency, flank pain, and urinary retention for 12 hours. Physical exam showed a calculus at the urethral meatus. This case discusses the manual removal of a urethral calculus in a female patient with use of forceps, resulting in complete resolution of symptoms and urinary retention.

Managing prostate disorders in middle age and beyond.

Diseases of the prostate may be benign or malignant. It is important for the clinician to realize that a complete urinalysis and digital rectal exam are the cornerstones of management in the diagnosis of prostate disease. Surgery for benign disease is indicated if the patient has compromised renal function, hydronephrosis, recurrent urinary tract infections, urinary retention, or bladder calculi. Surgery is also indicated if the obstructive urinary symptoms interfere with the patient's quality of life. Prostate cancer is a common malignancy in elderly men and treatment options must be guided b

Are prostatic calculi independent predictive factors of lower urinary tract symptoms?

We determined the correlation between prostatic calculi and lower urinary tract symptoms (LUTS), as well as the predisposing factors of prostatic calculi. Of the 1 527 patients who presented at our clinic for LUTS, 802 underwent complete evaluations, including transrectal ultrasonography, voided bladder-3 specimen and international prostatic symptoms score (IPSS). A total of 335 patients with prostatic calculi and 467 patients without prostatic calculi were divided into calculi and no calculi groups, respectively. Predictive factors of severe LUTS and prostatic calculi were determined using un

[Functional urology: synopsis of the 101st Congress of the French Association of Urology].

Functional urology knows more and more a marked development. Regarding benign prostatic hyperplasia, combined treatments are used often and new molecules such as inhibitor of phosphodiesterase of type 5 are appearing. The gold standard treatment of urinary incontinence with sphincteric deficiency remains the artificial urinary sphincter, in men and in women, where the rate of success is nearly of 95 %. The use of botulinic toxin in neurogenic bladder and especially in overactive bladder is growing remarkably, notably in patients of whom quality of life is improve against a very low toxicity. H

Benign prostatic hypertrophy: When should you consider prostatectomy for your patient?

Watching the way a patient voids is the easiest way to determine the presence of prostatic obstruction. Uroflowmetry can objectively measure the amount of obstruction. Clinical evaluation of prostatism should include an intravenous urogram, serum creatinine determination, urinalysis, urine culture, and cystoscopy. Suspicious areas of induration in the prostate should be biopsied. Surgery is not required for most men with prostatic hypertrophy. Indications for prostatectomy are severe symptoms of obstruction, significant renal or bladder decompensation, recurrent urinary tract infection, bladde

[Experimental bladder stone production by human uropathogenic bacteria].

To investigate the formation of infection stones by 5 bacterial species isolated from patients with urinary calculi, such as Proteus mirabilis, Staphylococcus saprophyticus, Pseudomonas aeruginosa, Klebsiella pneumoniae and Escherichia coli, a zinc disc was implanted and 10(6) bacteria were surgically inoculated at the same time into the bladder of male Wistar rats (10 rats per group). Urinary pH was measured on days 1, 3, 5 and 7. The rats were killed on day 7 and the bladder examined for the formation of infection stones. Infection stones were formed in all rats inoculated with P. mirabilis

The giant calculus within the prostatic urethra.

The giant calculus within the prostatic urethra is a rare clinical entity in the young population. Most of the calculi within the urethra migrate from the urinary bladder and obliterate the urethra. These stones are often composed of calcium phosphate or calcium oxalate. The decision of treatment strategy is affected by the size, shape and position of the calculus and by the status of the urethra. If the stone is large and immovable, it may be extracted via the perineal or the suprapubic approach. In most cases, the giant calculi were extracted via the transvesical approach and external urethr

Urothelial lesions in mice given 4-ethylsulfonylnaphthalene-1-sulfonamide, acetazolamide, and oxamide.

4-Ethylsulfonylnaphthalene-1-sulfonamide, acetazolamide and oxamide were administered in the diet to female BALB/c mice for varying periods of time from three to eighty weeks. All three compounds induced lesions in the urothelium. In the bladder, these included simple hyperplasia, nodular hyperplasia, inflammation and calculi. Similar lesions were observed in the ureter and urethra, along with a novel lesion, diverticulum, in the ureter. The diverticular lesions existed as down-growths of the transitional epithelium which often extended from the mucosa through the muscle layers to the adventit

Multiple Uric Acid Bladder Stones: Clinical Presentation and Endoscopic Management.

Background: Bladder urinary calculi occur in 3%-8% of men with bladder outlet obstruction, and although most of them are composed of calcium, in a few cases uric acid bladder stones are diagnosed. Case Presentation: We present clinical images and therapeutic management of a 65-year-old diabetic man with significant prostate enlargement and >30 bladder stones, the largest being 17 mm. Despite the large stone burden, the patient was managed by cystolithotripsy. Remarkably, stone composition analysis revealed 100% uric acid stone. Intraoperative and postoperative course were uneventfully. Conclus

Giant prostatic calculus in patient treated with perineal prostatotomy: case report.

BACKGROUND: Urolithiasis (kidney stone) is a common condition that often leads patients to urgent or emergency care services. Urinary calculi are generally found in the kidneys, ureters, or bladder. Urethral calculi are uncommon and can result from the migration of a calculus in the upper urinary tract or vesicle or may be primary of the urethra. Prostatic calculi are a rarity. CASE DESCRIPTION: A 34-year-old male was admitted to the emergency service of a university hospital reporting dysuria and pain in the hypogastrium with a 4-day history. The patient reported that the pain sometimes irra

The anatomy of a jackstone: a novel morphometric analysis of a rare urinary calculus.

Jackstone calculi are rare urinary stones characterized by multiple spiculated arms, yet the mechanisms underlying their distinctive morphology remain incompletely understood, in part due to the challenge of quantifying microstructural growth patterns along their complex three-dimensional geometry. In this study, we applied a non-destructive micro-computed tomography morphometric method to analyze a jackstone retrieved from the bladder of a human donor. This approach enabled measurement of incremental layering along individual arms and branches. We hypothesized that distal arm regions would sh

How I do it: percutaneous cystolitholapaxy for bladder stones with complex lower urinary tract anatomy.

While cystolitholapaxy for bladder stones is commonly performed using a transurethral approach, large or complex stone burdens in patients with complex lower urinary tract anatomy may make this inefficient or infeasible. Percutaneous cystolitholapaxy is a safe, effective, minimally invasive alternative for diverse indications, including patients with benign prostatic hyperplasia, urethral stricture disease, closed bladder neck, continent catheterized channel, or other urinary diversion. In this article, we review the indications for and advantages of percutaneous cystolitholapaxy and describe

Successful engraftment of bladder organoids in de-epithelialized mouse colon.

PURPOSE: To engraft bladder organoids (BO) on de-epithelialized mouse colon using an epithelial replacement technique. METHODS: BO cultured using bladder specimens from enhanced green fluorescent protein (EGFP) transgenic mice were engrafted to replace proximal colon epithelium stripped from an approximately 1 cm long target site in syngeneic wild-type recipient mice (n = 9) by exposure to ethylenediaminetetraacetic acid by infusion and flushing with phosphate buffered saline. Target sites were harvested on postoperative days 2, 7, and 28 for hematoxylin-eosin staining and immunofluorescence.
