[Ursodeoxycholic acid treatment in intrahepatic cholestasis of pregnancy. A case report].

INTRODUCTION: Intrahepatic cholestasis of pregnancy is a rare disorder of unknown origin with a symptomatically distressing maternal course with pruritus and icterus as the chief complaints. The disease poses little medical risk to the mother, but poses significantly risk to the fetus of perinatal mortality, preterm delivery, fetal distress, and meconium staining. Beside the pruritus and the high level of bilirubin in the serum the increased values of parameters showing the liver function and of alkaline phosphatase activity are characteristic. Ursodeoxycholic acid is the only therapeutic moda

Successful immunomodulating in AIDS patients with ursodeoxycholic acid--a pilot study.

Based on the positive therapeutic results with ursodeoxycholic acid (UDCA) in patients with primary biliary cirrhosis, in whom we observed a clinical improvement in conjunction with the normalization of the low pretreatment dipeptidyl peptidase (DPIV, CD26) expression of peripheral blood lymphocytes (PBL), we hypothesized that the very low DPIV expression in AIDS patients could be positively influenced by UDCA. Four young male AIDS patients were therefore treated with 750 mg of UDCA for 4 months. The low CD26 expression (2-8% of the PBL versus 18-28% in healthy controls) at the beginning of th

Ursodeoxycholic acid inhibits eosinophil degranulation in patients with primary biliary cirrhosis.

Eosinophilia is a distinctive feature of primary biliary cirrhosis (PBC), especially in its early stages. Intriguingly, treatment with ursodeoxycholic acid (UDCA) ameliorates eosinophilia as well as liver tests in patients with PBC. It remains unknown, however, whether eosinophils in PBC patients are functionally activated and whether UDCA inhibits eosinophil activation. In the present study, we systematically examined eosinophil dynamics in the blood and liver in patients with stage I to II PBC before and after UDCA treatment. We determined serum concentrations of eosinophil granule proteins

Long-term ursodeoxycholic acid delays histological progression in primary biliary cirrhosis.

Primary biliary cirrhosis (PBC) is a progressive cholestatic liver disease frequently leading to development of cirrhosis and its complications. Ursodeoxycholic acid (UDCA) is a beneficial medical therapy for patients with PBC. Improvement in some histological features, but not in histological stage, has been reported after 2 years of UDCA therapy. Thus, longer follow-up may be necessary to determine whether UDCA has a favorable effect on histological stage of disease and progression to cirrhosis. Our aim was to determine the long-term effects of UDCA therapy on histological stage and progress

Ursodeoxycholic acid therapy of chronic cholestatic conditions in adults and children.

Cholestasis can be defined as the manifestation of defective bile acid transport from the liver to the intestine. Most chronic cholestatic conditions can progress towards cirrhosis. At this stage, liver transplantation is the treatment of choice. Most of the drugs so far evaluated show some degree of efficacy but have major side effects. Given that ursodeoxycholic acid (UDCA) has no apparent toxicity in humans, it was postulated that long-term treatment with this drug might displace endogenous bile acids and thus reverse their suspected toxicity. We demonstrated that long-term UDCA therapy slo

[Eosinophilia in primary biliary cirrhosis: regression under therapy with ursodeoxycholic acid].

Eosinophilia can be observed in up to 40% of patients with primary biliary cirrhosis (PBC). Eosinophilia is transient, tends to occur in the early stages of the disease and seems to be associated with episodes of florid bile duct destruction [42-45]. 14 Patients with PBC were examined before and during treatment with ursodeoxycholic acid (UDCA) (10 mg/kg/d) for episodes of eosinophilia and the number of eosinophilic granulocytes in differential white blood cell counts. Group A consisted of 5 patients with one or several episodes of eosinophilia before the start of UDCA. Group B included 9 pati

Combination therapy with ursodeoxycholic acid and colchicine for primary biliary cirrhosis.

Twelve patients with primary biliary cirrhosis (PBC), stages I to III, received long-term therapy with a combination of 600 mg ursodeoxycholic acid (UDCA) and 1 mg colchicine given daily for more than 2 years. Drug toxicity was mild; one patient experienced diarrhoea that was probably due to colchicine. Serum levels of bilirubin, alkaline phosphatase (ALPase), gamma-glutamyl transpeptidase and alanine aminotransferase decreased by more than 50% of the initial values. Serum albumin and cholesterol levels also improved, but immunoglobulins and anti-mitochondrial antibody titre did not change. Hi

Effect of different doses of ursodeoxycholic acid in chronic liver disease.

Recent clinical studies have indicated that ursodeoxycholic acid (ursodiol), administered at dosages ranging between 10 and 15 mg/kg/day, improves liver function indices in both cholestatic and inflammatory chronic liver diseases. These dosages would be considered high for the use of ursodiol in gallstone dissolution therapy. To investigate the dose-response relationship to ursodiol administration, we planned a few studies in patients with primary biliary cirrhosis (PBC), primary sclerosing cholangitis (PSC), and chronic hepatitis (CH). Patients with PBC were subdivided into two groups on the

Effectiveness of Ursodeoxycholic Acid in the Prevention of Cholelithiasis After Sleeve Gastrectomy.

PURPOSE: The use of ursodeoxycholic acid (UDCA) to prevent gallstone formation after sleeve gastrectomy (SG) is still debated. Furthermore, no study has assessed the effectiveness of UDCA on gallstone formation after the first postoperative year. Our aim was to compare the incidence of cholelithiasis (CL) at 1 and 3 years after SG between patients treated or not treated with UDCA. MATERIALS AND METHODS: From January 2008, a postoperative ultrasound monitoring was scheduled for all patients who underwent SG in our institution. Patients with a preoperative intact gallbladder who performed at le

The role of ursodeoxycholic acid on cholestatic hepatic fibrosis in infant rats.

The aim of the present study was to identify the impact of ursodeoxycholic acid (UDCA) on liver function and fibrosis markers in infant rats by establishing a cholestatic‑induced hepatic fibrosis model. α‑naphthylisothiocyanate (ANIT) was administrated by gavage to induce cholestatic hepatic fibrosis in infant rats. UCDA treatment was performed to assess its impact on biochemical indicators of liver function, four serum biomarkers of hepatic fibrosis, hepatic fibrosis indices in liver tissues and the pathology of liver tissues. Colorimetric assays and biochemical assays based on the initial ra

Rationale for therapy with ursodeoxycholic acid in patients with cholestatic liver disease.

Ursodeoxycholic acid is increasingly used in the treatment of a variety of cholestatic diseases such as primary biliary cirrhosis. The present article summarizes current understanding of the mechanism of action of ursodeoxycholic acid in these diseases. Apart from a direct hepatoprotective effect, ursodeoxycholic acid also influences the enterohepatic circulation with decreased ileal reabsorption and increased faecal excretion of potentially hepatotoxic endogenous bile salts. In addition, a cholehepatic shunt mechanism and immune modulating effects may also be important.

Dual Flow Reactor Approach for the Chemoenzymatic Production of Chenodeoxycholic Acid, a Precursor to Ursodeoxycholic Acid.

Chenodeoxycholic acid, a precursor to ursodeoxycholic acid, can be synthesized from inexpensive cholic acid through a two-step chemoenzymatic route. Here, a dual flow reactor system was developed to intensify this process. First, a packed-bed reactor with coimmobilized enzymes achieved complete conversion of 50 mM cholic acid to 12-oxochenodeoxycholic acid, achieving a space-time yield of 5831 g·L-1·d-1, significantly surpassing the batch mode (1632 g·L-1·d-1). Subsequently, a PTFE flow reactor facilitated the Wolff-Kishner-Huang reduction, producing chenodeoxycholic acid with a space-time yie

Bile acid patterns in meconium are influenced by cholestasis of pregnancy and not altered by ursodeoxycholic acid treatment.

BACKGROUND: Data on meconium bile acid composition in newborn babies of patients with intrahepatic cholestasis of pregnancy (ICP) are relatively scant, and changes that occur on ursodeoxycholic acid (UDCA) administration have not been evaluated. AIMS: To investigate bile acid profiles in meconium of neonates from untreated and UDCA treated patients with ICP. Maternal serum bile acid composition was also analysed both at diagnosis and delivery to determine whether this influences the concentration and proportion of bile acids in the meconium. PATIENTS/METHODS: The population included eight he

[Taurodeoxycholic acid in the treatment of primary biliary cirrhosis. A controlled study in comparison to ursodeoxycholic acid].

Thirty patients with primary biliary cirrhosis were enrolled; 25 completed the study period (6 months). Of these, 12 were randomised to TUDCA treatment and 13 to UDCA. Dosage range 12-15 mg/kg daily for both groups which were comparable at to age, sex, duration and stage of the disease, as well as for all liver laboratory tests. Altogether, the results obtained were satisfactory in both groups. What was surprising was that clinical experience failed to confirm the greater efficacy of TUDCA compared to UDCA. On the contrary, UDCA appeared to be more effective than TUDCA in improving liver funct

Omega-3 Polyunsaturated Fatty Acid: A Pharmaco-Nutraceutical Approach to Improve the Responsiveness to Ursodeoxycholic Acid.

Ursodeoxycholic acid (UDCA) is the first line therapy for the treatment of cholestatic and autoimmune liver diseases. Its clinical use is currently limited by a significant proportion of non-responder patients. Polyunsaturated fatty acids (n-3 PUFAs) possess important anti-inflammatory properties and protect liver cells against bile acid (BA)-induced toxicity. The present study was designed to rapidly evaluate whether combining n-3 PUFAs (i.e., eicosapentaenoic [EPA] and docosahexaenoic [DHA] acids) to UDCA would provide additional benefits when compared to the drug alone. The parameters evalu

Ursodeoxycholic acid administration on bile acid metabolism in patients with early stages of primary biliary cirrhosis.

Ursodeoxycholic acid has been proposed for the treatment of primary biliary cirrhosis. The aim of this study was to evaluate the effect of ursodeoxycholic acid administration on bile acid metabolism in patients with early-stage primary biliary cirrhosis. Biliary bile acid composition, primary bile acid pool sizes, synthesis, and fractional turnover rate were measured before and after four weeks of ursodeoxycholic acid administration (600 mg/day) in nine patients with biopsy-proven primary biliary cirrhosis (stages I-III). Molar percentages of chenodeoxycholic, cholic, and deoxycholic acids in

Unusual binding of ursodeoxycholic acid to ileal bile acid binding protein: role in activation of FXRα.

Ursodeoxycholic acid (UDCA, ursodiol) is used to prevent damage to the liver in patients with primary biliary cirrhosis. The drug also prevents the progression of colorectal cancer and the recurrence of high-grade colonic dysplasia. However, the molecular mechanism by which UDCA elicits its beneficial effects is not entirely understood. The aim of this study was to determine whether ileal bile acid binding protein (IBABP) has a role in mediating the effects of UDCA. We find that UDCA binds to a single site on IBABP and increases the affinity for major human bile acids at a second binding site.

Ursodeoxycholic acid prevents hepatic cytochrome P450 isozyme reduction in rats with deoxycholic acid-induced liver injury.

BACKGROUND/AIMS: Hydrophobic bile acids, such as deoxycholic acid produce cholestatic liver injury. Ursodeoxycholic acid has been shown to be useful in the treatment of cholestatic liver disease. METHODS: In this study, we investigated the effects of deoxycholic acid or ursodeoxycholic acid (1% of diet, for 14 days) and their combination (1% each) on expression of hepatic cytochrome P450 isozymes, their related enzyme activities and mRNA level in rats. RESULTS: Adding 1% deoxycholic acid to chow caused a marked increase in serum total bilirubin (47-fold) and total bile acid (8-fold) concentr
