Accuracy of Preliminary Birth Defect Surveillance in Utah: A Validation Study Supporting Timely Family Referrals.

BACKGROUND: Public health surveillance is critical for understanding disease burden, yet rigorous case confirmation often introduces significant latency. The Utah Birth Defect Network (UBDN) utilizes a multistage active surveillance process that, while highly accurate, is a long and labor-intensive process. This delay creates a barrier to timely family referrals for early intervention services. This study evaluates whether preliminary surveillance data is sufficiently accurate to initiate public health actions before final clinician confirmation. METHODS: We conducted a retrospective analysis

Association of Wender Utah Rating Scale (WURS)-61 items with clinical psychiatric diagnosis in adulthood.

BACKGROUND: The Wender Utah Rating Scale (WURS) is a widely used retrospective scale in adults presenting for ADHD evaluations which features items relating to childhood symptoms. AIMS: The aim of this study is to establish if certain childhood symptoms (including ADHD) as identified by the WURS-61 are associated with specific mental health disorders in adulthood. METHODS: Case-control study of N=630 attending Adult Mental Health Services (AMHS) and a control group without mental disorders (N=96). RESULTS: The mean age of the participants was 39.81 (SD 12.94) of which 387 (53.3%) were femal

Community-Associated Outbreak of COVID-19 in a Correctional Facility - Utah, September 2020-January 2021.

Transmission of SARS-CoV-2, the virus that causes COVID-19, is common in congregate settings such as correctional and detention facilities (1-3). On September 17, 2020, a Utah correctional facility (facility A) received a report of laboratory-confirmed SARS-CoV-2 infection in a dental health care provider (DHCP) who had treated incarcerated persons at facility A on September 14, 2020 while asymptomatic. On September 21, 2020, the roommate of an incarcerated person who had received dental treatment experienced COVID-19-compatible symptoms*; both were housed in block 1 of facility A (one of 16 o

Evaluation of Online Risk Assessment To Identify Rabies Exposures Among Health Care Workers - Utah, 2019.

On November 7, 2018, the Utah Department of Health (UDOH) reported the first confirmed human rabies death in the state since 1944 (1). The case occurred in a person who had been treated over a period of 19 days at four health care facilities and an emergency medical transport service across three counties and two states. Human rabies is preventable through preexposure or postexposure vaccination but is invariably fatal upon symptom onset. Timely identification of persons who might have been exposed to rabies virus is therefore crucial to administer postexposure prophylaxis (PEP). Because of th

Characteristics of patients having telemedicine versus in-person informed consent visits before abortion in Utah.

OBJECTIVE: This study aimed to evaluate demographic and service delivery differences between patients using telemedicine relative to an in-person visit to satisfy Utah's state-mandated informed consent visit, which must occur at least 72 h before the abortion. STUDY DESIGN: We conducted a retrospective cohort study with data from Planned Parenthood Association of Utah (PPAU), which included all informed consent and abortion encounters from January 1, 2015-March 31, 2018. We evaluated the following for each encounter by informed consent type (telemedicine vs in-person): demographics, distance

Restoration from acute urinary dysfunction using Utah electrode arrays implanted into the feline pudendal nerve.

OBJECTIVES: To investigate intrafascicular pudendal nerve stimulation in felines as a means to restore urinary function in acute models of urinary incontinence, overactive bladder, and underactive bladder. MATERIALS AND METHODS: Felines were anesthetized, and high-electrode-count (48 electrodes; 25 electrodes/mm(2) ) electrode arrays were implanted intrafascicularly into the pudendal nerve trunk. Electrodes were mapped for their ability to selectively or nonselectively excite the external anal sphincter, external urethral sphincter, and the detrusor bladder muscle. Statistical analysis was ca

Crocodyliform feeding traces on juvenile ornithischian dinosaurs from the Upper Cretaceous (Campanian) Kaiparowits Formation, Utah.

Crocodyliforms serve as important taphonomic agents, accumulating and modifying vertebrate remains. Previous discussions of Mesozoic crocodyliform feeding in terrestrial and riverine ecosystems have often focused on larger taxa and their interactions with equally large dinosaurian prey. However, recent evidence suggests that the impact of smaller crocodyliforms on their environments should not be discounted. Here we present direct evidence of feeding by a small crocodyliform on juvenile specimens of a 'hypsilophodontid' dinosaur from the Upper Cretaceous (Campanian) Kaiparowits Formation of so

Familial Associated Idiopathic Urethral Stricture Disease: An Assessment Using Pedigree Data From the Utah Population Database.

OBJECTIVE: To elucidate the role of familial transmissibility of idiopathic urethral stricture disease (USD) using the Utah Population Database. MATERIALS AND METHODS: To identify patients with idiopathic USD, we excluded: post-infective, post-traumatic, post-operative, and hypospadias-related USD. Each idiopathic stricture case was matched to a control (1:1). All male relatives (first to third degree) of cases and controls were identified. A Cox proportional hazards model was performed to assess the effect of being an idiopathic USD case relative vs control relative. Models including all mal

Mapping genetic susceptibility to spontaneous preterm birth: analysis of Utah pedigrees to find inherited genetic factors.

BACKGROUND: Spontaneous preterm birth (SPTB) is the leading cause of neonatal morbidity and mortality. It is a final common pathway for multiple etiologies, some of which are well known while others likely remain to be identified. Despite recent advancements in identifying genetic risk factors, the mechanisms of many SPTBs remain poorly understood due to the phenotypic heterogeneity and complexity. Large family-based studies decrease heterogeneity and improve power to identify genetic causes of SPTB. OBJECTIVE: To identify inherited genetic factors in SPTB etiology using large families. STUD

Psychometric data and versions of the Wender Utah Rating Scale including the WURS-25 & WURS-45.

Our associated paper presented a psychometric evaluation of the Wender Utah Rating Scale (WURS) and its abbreviated version, the WURS-25. Instead of actual factors scores, we employed "item averages" calculated by the average score of each item comprising that factor. We did not present a factor analysis of the WURS-25. Herein we identify items of the full WURS that are redundant or not part of any of the scale's five factors. Removing these items produced a shortened version, the WURS-45. We performed a logistic regression using actual factor loadings as well as factors based on item averages

A Five-Year Review of Newborn Screening for Spinal Muscular Atrophy in the State of Utah: Lessons Learned.

Spinal muscular atrophy (SMA) is an autosomal recessive condition characterized by alpha motor neuron degeneration in the spinal cord anterior horn. Clinical symptoms manifest in the first weeks to months of life in the most severe cases, resulting in progressive symmetrical weakness and atrophy of the proximal voluntary muscles. Approximately 95% of SMA patients present with homozygous deletion of the SMN1 gene. With multiple available therapies preventing symptom development and slowing disease progression, newborn screening for SMA is essential to identify at-risk individuals. From 2018 to

One Health Investigation of SARS-CoV-2 in People and Animals on Multiple Mink Farms in Utah.

From July−November 2020, mink (Neogale vison) on 12 Utah farms experienced an increase in mortality rates due to confirmed SARS-CoV-2 infection. We conducted epidemiologic investigations on six farms to identify the source of virus introduction, track cross-species transmission, and assess viral evolution. Interviews were conducted and specimens were collected from persons living or working on participating farms and from multiple animal species. Swabs and sera were tested by SARS-CoV-2 real-time reverse transcription polymerase chain reaction (rRT-PCR) and serological assays, respectively. Wh

Incidence and Prevalence of Pediatric Eosinophilic Esophagitis in Utah Based on a 5-Year Population-Based Study.

BACKGROUND & AIMS: Eosinophilic esophagitis (EoE) is often detected in children and is considered to be a rare disease, with prevalence values reported to be below 60 cases per 100,000 persons. To determine whether the incidence of EoE in children in Utah exceeds estimates from regional reports, we calculated incidence and prevalence values over a 5-year period. METHODS: Using consensus guidelines for the diagnosis of EoE, we reviewed pathology records from the Intermountain Healthcare pathology database, from July 1, 2011 through June 31, 2016. We collected data on 10,619 pediatric patients

Association of 2009 pandemic influenza A (H1N1) infection and increased hospitalization with parapneumonic empyema in children in Utah.

BACKGROUND: During previous influenza pandemics, many deaths were associated with secondary bacterial infection. In April 2009, a previously unknown 2009 influenza A virus (2009 H1N1) emerged, causing a global influenza pandemic. We examined the relationship between circulating 2009 H1N1 and the occurrence of secondary bacterial parapneumonic empyema in children. METHODS: Children hospitalized with parapneumonic empyema from August 2004 to July 2009, including a period when the 2009 H1N1 circulated in Utah, were identified using International Classification of Diseases, Ninth Revision codes.

The Utah paradigm of skeletal physiology: an overview of its insights for bone, cartilage and collagenous tissue organs.

In a 1960 paradigm of skeletal physiology, effector cells (chondroblasts, fibroblasts, osteoblasts, osteoclasts, etc.) regulated by nonmechanical agents wholly determined the architecture, strength, and health of bones, joints, fascia, ligaments, and tendons. Biomechanical and tissue-level phenomena had no roles in that paradigm. Subsequent studies and evidence slowly revealed skeletal tissue-level mechanisms and their functions, including biomechanical ones, as well as "game rules" that seem to govern them. That slow discovery process found that effector cells are only parts of tissue-level m

Growth hormone and osteoporosis: an overview of endocrinological and pharmacological insights from the Utah paradigm of skeletal physiology.

Multidisciplinary advances in skeletal physiology offer a new paradigm for the effects of growth hormone (GH) and other agents on bone and osteoporosis. The still-evolving Utah paradigm of skeletal physiology supplements earlier ideas with later discovered roles of the skeleton's tissue-level 'nephron equivalents' and muscle strength in skeletal development, physiology and disorders. This article summarizes how these factors could influence the effects of GH on bone strength and bone 'mass', and the use of GH in the treatment of osteoporoses. Although the cellular and molecular biological mech

Ethnic Differences in Unmet Care Needs Among Cancer Survivors in Utah Using a Mixed Methods Approach.

BACKGROUND: Cancer survivors' unmet care needs can negatively affect well-being and quality of life, yet few studies focus on how Hispanic/Latino (Hispanic) survivors' needs may differ from those of non-Hispanic White (NHW) survivors. AIMS: To explore unmet needs of cancer survivors by ethnicity. METHODS: A population-based sample of adult survivors in Utah was surveyed. Demographic and cancer characteristics of survivors reporting unmet needs and those without needs were compared using chi-square, Wilcoxon rank sum, and Fisher's exact tests. Summary of Utah Project on Exfoliation Syndrome (UPEXS): using a large database to identify systemic comorbidities.

The purpose of the Utah Project on Exfoliation Syndrome (UPEXS) is to identify associations between exfoliation syndrome (XFS) and other diseases that share the commonality of abnormalities in elastin and Lysyl Oxidase-Like 1 gene regulation. The UPEXS is unique because it uses the Utah Population Database, which is linked to the Utah genealogy, that contains a compilation of large pedigrees of most families in the state of Utah that go back multiple generations (3 to ≥11). The health and medical records of these family members are linked to vital records and can be used effectively in studies

Hepatitis A Virus Outbreaks Associated with Drug Use and Homelessness - California, Kentucky, Michigan, and Utah, 2017.

During 2017, CDC received 1,521 reports of acute hepatitis A virus (HAV) infections from California, Kentucky, Michigan, and Utah; the majority of infections were among persons reporting injection or noninjection drug use or homelessness. Investigations conducted by local and state health departments indicated that direct person-to-person transmission of HAV infections was occurring, differing from other recent, large HAV outbreaks attributed to consumption of contaminated commercial food products. Outbreaks with direct HAV transmission among persons reporting drug use or homelessness signals
