Impact of the type of vaginal assisted delivery on the pelvic floor and OASI - Ultrasound study.

OBJECTIVE: To assess the prevalence of pelvic floor and anal sphincter trauma in women after assisted vaginal delivery. METHODS: Retrospective study on 201 primiparous women after assisted vaginal delivery, control group 43 women after normal vaginal delivery. 4D translabial ultrasound examination of the levator ani and the anal sphincter was performed according to standard methodology at least 3 months postpartum. Ultrasound classification of trauma was performed by two independent evaluators blinded to clinical data. A third evaluator was asked to confirm findings in case of discrepancy. R

Vacuum-assisted delivery: a review.

In 1992, the number of vacuum deliveries overtook the number of forceps deliveries performed in the USA. Most clinical experiences report that the vacuum is safe for both the fetus and the mother when used properly. Correct cup placement on the fetal head and knowing when to abandon the procedure, appear to be key components to conducting a safe and successful vacuum delivery. However, the focus and training that has been afforded forceps deliveries in the past has not been given to the vacuum, because of its perceived 'ease of use'. This apparent lack of understanding has led to increasing nu

Modifiable risk factors of obstetric anal sphincter injury in primiparous women: a population-based cohort study.

OBJECTIVE: To determine modifiable risk factors and incidence of obstetric anal sphincter injury (OASIS) in primiparous women. STUDY DESIGN: We performed a population-based retrospective cohort study, using data from the Danish Medical Birth Registry. The population consisted of primiparous women with a vaginal delivery in the time period 2000-2010. Univariable and multivariable logistic regressions were used to determine risk factors of OASIS. Main outcome measures were incidence of OASIS in first vaginal delivery, odds ratios for possible risk factors: age, body mass index, birthweight, hea

Operative vaginal delivery: practice patterns and outcomes at a tertiary general hospital.

INTRODUCTION: There has been a global decrease in operative vaginal deliveries, with a marked shift towards the vacuum extractor. However, little is known about the trends in operative vaginal delivery in Singapore. METHODS: A retrospective study was conducted on all operative vaginal deliveries performed from 2012 to 2017 at Singapore General Hospital (SGH). Maternal outcomes in terms of postpartum haemorrhage and obstetric anal sphincter injuries were compared between forceps- and vacuum-assisted deliveries. Neonatal outcomes in terms of neonatal intensive care unit (NICU) admission and cli

Choice of instruments for assisted vaginal delivery.

BACKGROUND: Instrumental or assisted vaginal birth is commonly used to expedite birth for the benefit of either mother or baby or both. It is sometimes associated with significant complications for both mother and baby. The choice of instrument may be influenced by clinical circumstances, operator choice and availability of specific instruments. OBJECTIVES: To evaluate different instruments in terms of achieving a vaginal birth and avoiding significant morbidity for mother and baby. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (31 May 2010). SEL

[Obstetric spoons (author's transl)].

The article describes "obstetric spoons" which are easy to apply and which can be used as well as the forceps and the vacuum extractor for vaginal-operative termination of labor. These spoons offer the following advantages: They are an effective and relatively harmless instrument which can be applied rapidly and easily even by those who are less skilled. During extraction, the spoons are only slightly pressed onto the head of the child by the operator's hands; the additional pressure required is exercised by the soft parts of the mother. It is hardly critical if the spoons are not applied exac

A randomized comparison of vacuum extraction delivery with a rigid and a pliable cup.

OBJECTIVE: To compare the effectiveness and complications of obstetric vacuum extraction with a rigid and a pliable cup, with a focus on neonatal retinal hemorrhage. METHODS: One hundred women requiring assisted delivery who met predefined criteria for vacuum extraction were randomly assigned to be delivered by the classic rigid Malmström cup or the pliable Silastic cup. RESULTS: Because of the faster induction of vacuum, delivery occurred more rapidly with the pliable cup, but the pliable cup detached significantly more often than the rigid cup. The overall failure rate was not significantl

Temporal trends and morbidities of vacuum, forceps, and combined use of both.

OBJECTIVE: To assess trends over time of operative vaginal delivery and compare delivery-related morbidity between vacuum delivery, forceps delivery, or combined use of both in California. METHODS: California ICD-9 discharge data from 2001 to 2007 were used to identify cases of forceps and vacuum delivery. RESULTS: There was a decline in all operative delivery types (9.0% in 2001 to 7.6% in 2007), with the decline in the use of forceps most pronounced (7.26/1000 deliveries in 2001 to 3.85/1000 in 2007). Higher rates of third/fourth degree lacerations, postpartum hemorrhage, manual extraction

Deliveries in short-stay hospitals: United States, 1980.

During 1980 3.8 million women with deliveries were discharged from short-stay non-Federal hospitals in the United States. These women made up a sizable portion--9.9 percent--of all the discharges (excluding newborn infants) during that year. Women with deliveries remained hospitalized an average of 3.8 days and used 14.2 million days of inpatient hospital care. This was only 5.2 percent of the total days spent in hospitals by all patients discharged during the year. Most of the women who had a delivery were in their twenties, were white, and were married. The largest percent of deliveries occu

The role of mediolateral episiotomy during vacuum-assisted vaginal delivery with soft cup devices.

PURPOSE: This study evaluated whether episiotomy during vacuum-assisted delivery leads to fewer third- and fourth-degree tears. METHODS: This was a retrospective cohort study of all nulliparas who underwent a singleton, soft cup, vacuum-assisted vaginal delivery in one institution, from January 2014 to August 2019. Failed vacuum deliveries were excluded. Based on power analysis calculation, a sample size of 500 women in each group was sufficient to detect an advantage of episiotomy, if present. Primary outcome was third- or fourth-degree perineal tear. Secondary outcomes were other maternal c

A randomised controlled trial of a new handheld vacuum extraction device.

OBJECTIVE: To evaluate the effectiveness of a new handheld vacuum delivery device. DESIGN: Randomised controlled trial. SETTING: Southmead Hospital, Bristol, UK. POPULATION: One hundred and ninety-four women requiring delivery with vacuum extraction. METHODS: Women were recruited into the study while still in early labour. If during delivery, vacuum extraction was indicated they were then randomly allocated to either a new handheld vacuum device, the Kiwi Omnicup, or to a 'standard' vacuum cup selected by the obstetrician. The 'standard group' consisted of the silastic (66/98, 67%) or the

Continuous epidural anaesthesia with a low frequency of instrumental deliveries.

The results of 204 cases of regional analgesia by means of continuous epidural block during labour and delivery are reported. The frequency of instrummental intervention using forceps or the vacuum extractor is low, less than 15% for primi- and multi-gravidas combined, although pathological conditions were recognized prior to blockade in almost one-third of the parturients. Moreover, the frequency of emergency Caesarean section has dropped significantly since epidural anaesthesia was introduced into the clinic. The 1 min Apgar scores of babies born to mothers given epidural anaesthesia are sim

Understanding the underuse: Barriers to vacuum-assisted birth among clinicians in Tanzania: An exploratory qualitative study.

BACKGROUND: Vacuum-assisted birth (VAB) is a critical obstetric intervention for saving the lives of the mothers and newborns, particularly in cases of prolonged second-stage labor, maternal exhaustion, or fetal distress. Despite its proven benefits, VAB remains underutilized, contributing to an increased rate of unnecessary cesarean sections, some of which could be avoided with timely vacuum extraction. This study aimed to explore the barriers to the use of VAB among clinicians at a tertiary hospital in Tanzania. MATERIALS AND METHODS: An exploratory qualitative study using in-depth intervie

[The Neonatal Subgaleal Hematoma - A Neonatal Emergency].

Subgaleal hematoma (SGH) is a rare complication in neonates that may lead to hemorrhagic shock due to significant blood loss into the subgaleal space. We report of two neonates who developed subgaleal hematoma with severe hemorrhagic shock and encephalopathy. In the first case of a mature female neonate, the development of the subgaleal hematoma was promoted by early-onset sepsis and delivery by vacuum extraction. The second case, of a male preterm neonate, was a complicated fetal development followed by secondary cesarean section. Both cases highlight that a subgaleal hematoma is a severe neo

Abdominal rescue using the vacuum extractor after entrapment of the aftercoming head.

BACKGROUND: Fetal replacement into the uterus for ceasarean delivery after failed vaginal delivery has been reported for both vertex and breech presentations. Although an option, this mode of delivery frequently is the last maneuver to deliver a viable infant after other methods to allow vaginal delivery have failed. We report the adjunctive use of a vacuum extractor to facilitate abdominal rescue after entrapment of the aftercoming head during an attempted vaginal breech delivery. CASE: A multiparous woman presented at term with two fetal feet bulging through the membranes at the introitus.
