INTRODUCTION: Thrombosis is the most important access-related complication. Several declotting procedures have been suggested falling mainly into two categories; thrombolysis-dependent and thrombectomy-dependent.
AREAS COVERED: Seventeen studies after 2001 have been published on percutaneous treatment of thrombosed vascular access. Authors performed a systematic review of these studies together with a parametric meta-analysis of data available investigating clinical success, postintervention assisted primary patency (PAPP) and independent factors that could influence outcome measures.

The proposed introduction of outpatient electrophysiological procedures challenges physicians with the task of safely discharging patients after only a few hours of monitoring. Despite the widely established use of ultrasound-guided vascular access in many centers, access-site complications remain the most common complications associated with electrophysiological interventions. Modern vascular closure devices have made postprocedural care safer and more comfortable. Nevertheless, the traditional Z‑suture continues to play an important role, particularly in cases of individual anatomical challe

BACKGROUND: Vascular access specialist teams (VASTs) are established worldwide to enhance the safety and efficiency of vascular access procedures. In Poland, nurse-led VASTs are a novel concept.
OBJECTIVE: The aim of the study was to evaluate whether implementation of a nurse-led VAST improved the efficiency and safety of vascular access devices in a large academic hospital in Poland.
METHODS: This retrospective analysis included long peripheral intravenous catheters inserted from 2021 to 2024 and midline catheters inserted from 2022 to 2024. Data were obtained from hospital records and the

Peripheral venous access is a cornerstone of anesthesiology, traditionally performed with a clean but non-sterile technique. The increasing use of ultrasound for vascular access has sparked concerns about infection control, particularly regarding whether ultrasound probe covers ensure sterility. We argue that while probe covers provide a physical barrier, they do not render the procedure sterile and may lead to a false sense of security if basic hygiene practices-such as proper probe disinfection, hand hygiene, and skin antisepsis-are neglected. Confusion on this topic stems from confusion in

Vascular access is required to draw the patient's blood into the dialysis machine and return the filtered blood to the patient during hemodialysis to treat end-stage renal disease. The most reliable vascular access is the arteriovenous fistula (AVF), which unfortunately may develop significant stenosis or obstruction as a major complication. To evaluate the AVF geometry for potential pathological features, this study aims to develop and validate a free-hand 3D ultrasound imaging system using conventional 2D ultrasound scanning with scanner motion data from an electromagnetic (EMT) sensor to sp

The patient was a boy born at 23 weeks and 0 days of gestation weighed 401 g at birth. For treatment, an umbilical venous catheter was placed but the catheter perforated a blood vessel. We thought that prompt removal of the catheter would lead to massive bleeding, so we kept the catheter in place at the umbilicus, waited for weight gain, and removed it after confirming that the catheter tip had spontaneously become shallow and was in the umbilical vein. This procedure allowed us to handle the patient without major problems.

Large-caliber access to the arterial system is more frequently required in the age of thoracic endovascular aneurysm repair, endovascular aneurysm repair, and transaortic valve implantation. Frequently either anatomical or vessel size constraints preclude use of common access points such as the common femoral, radial, and brachial arteries. Alternative approaches include percutaneous access at alternate sites (subclavian, axillary, and carotid), open surgical access to the heart (left ventricular apex), open surgical access to large-caliber vessels (ascending aorta, subclavian, and axillary ar

Now widely used, totally implantable venous access devices allow mid- and long-term, frequent, repeated, or continuous injection of therapeutic products by vascular, cavitary, or perineural access. The effective flushing of these devices is a key factor that ensures their long-lasting use. We present experimental results and a numerical simulation to demonstrate that the implementation of rounded edge wall cavities improves flushing efficiency. We use the same approaches to suggest that the deposit amount may be reduced by the use of rounded edge wall cavities.

Even with current generation mechanical circulatory support (MCS) devices, vascular complications are still considerable risks in MCS that influence patients' recovery and survival. Hence, efforts are made to reduce vascular trauma and obtaining safe and adequate arterial access using state-of-the-art techniques is one of the most critical aspects for optimizing the outcomes and efficiency of percutaneous MCS. Femoral arterial access remains necessary for numerous large-bore access procedures and is most commonly used for MCS, whereas percutaneous axillary artery access is typically considered

This review demonstrates the feasibility of the transradial approach (TRA) to address different peripheral vascular lesions. The TRA has long been used to address practically all coronary artery lesion subsets. It has shown significant benefits when compared with transfemoral approach (TFA), particularly a reduction in puncture-site related bleeding complications. TRA can be utilized effectively to address peripheral vascular lesions, including the renal, iliac, subclavian, carotid, vertebrobasilar, and superficial femoral systems. Utility of TRA for addressing peripheral vascular lesions usin

Safe vascular access is integral to anaesthetic and critical care practice, but procedures are a frequent source of patient adverse events. Ensuring safe and effective approaches to vascular catheter insertion should be a priority for all practitioners. New technology such as ultrasound and other imaging has increased the number of tools available. This guidance was created using review of current practice and literature, as well as expert opinion. The result is a consensus document which provides practical advice on the safe insertion and removal of vascular access devices.

Permanent vascular closure systems are commonly used for achieving hemostasis after transfemoral percutaneous procedures. However, there exists conflicting data regarding the safety of re-puncturing these devices during subsequent procedures. Because percutaneous procedures have become more prevalent, so have the use of vascular closure devices, emphasizing the need to understand their limitations. The following case report describes a situation where a micropuncture catheter fractured within a closure device during inadvertent repeat device access and the challenges encountered with its retri

A growing number of studies have explored EUS-guided vascular catheterization due to the relative proximity of the gastrointestinal tract to the major blood vessels of the mediastinum and abdomen. In particular, EUS-guided access of the portal vein (PV) may be favorable given the relative difficulty of PV access through standard percutaneous routes. Two major diagnostic applications of EUS-guided vascular access include angiography and assessment of intravascular pressure. This review will outline the different devices and techniques employed to obtain angiographic visualization and/or direct

BACKGROUND: Vascular Closure primarily utilizing suture-based devices (SBDs) are frequently used for large bore percutaneous arterial access in patients undergoing Transcatheter Aortic Valve Implantation and Endovascular Aneurysm Repair. Little is known about the intentional combination of SBD with plug-based device (PBD). Main objective was to determine the efficacy of adopting a combined SBD and PBD strategy compared to double SBD for large bore arterial access.
METHODS: We searched randomized and observational studies comparing both strategies and that reported the outcomes of major vascul
