Ninety patients with chronic venous ulceration were investigated to determine the extent of the incompetence of the venous valves. They were then treated by established surgical methods to control reflux. All wore fitted compression stockings and were regularly followed up. Forty-two patients had valvular incompetence confined to the perforating and superficial veins. All their ulcers healed and only one developed a minor recurrence during the three year follow-up. Nineteen patients had additional but limited incompetence of the deep veins; all their ulcers healed, but three developed a recurr

INTRODUCTION: The thoracic duct drains both lymph and chyle into the confluence of the great veins in the root of the neck. The venous termination of the duct is protected from retrograde flow of blood by valves. However, little attention has been focused on the presence or absence of valves in the subsidiary lymph trunks in the neck.
AIMS: The aim of this study was to determine whether there are valves in the subsidiary lymphatic system in the neck.
MATERIAL AND METHODS: The left side of the neck was explored in ten formalin-fixed cadavers. Subsidiary lymphatic trunks were carefully preserv

It is commonly believed that valves are absent in veins smaller than two millimetres in diameter. Consequently, current investigations on the pathophysiology of chronic venous disease (CVD) consider and evaluate only the valvular competence of large veins. The authors review literature from their own collections as well as from medical database searches to assess the functional relevance of these valves. Microscopic venous valves (MVVs) were first described in 1934 in the human digits and have subsequently been demonstrated in other parts of the human body as well as in many tissues and organs

Leonardo da Vinci's anatomical drawings of quadricuspid, tricuspid, and bicuspid aortic valves underscored the hydraulic superiority of a three leaflet valve with cuspal equality. William Harvey demonstrated that venous valves were designed for unidirectional flow and to prevent reflux from the heart, observations that served as the basis of his immortal de Mortu Cordis. Joseph Rouanet of Paris proposed that heart sounds originated from the closing movements of cardiac valves. The Cardiodynamics of Mitral Insufficiency by Wiggers and Feil was followed three decades later by Paul Wood's An Appr

Chronic venous disease (CVD) is an insufficiency of distal veins caused by their partial or total obstruction, endothelial distension and functional disorders. Chronic venous disease of lower limbs is common problem and affects millions of people. In this article we suggest that inflammatory process is involved in the structural remodeling in venous valves and in the venous wall, leading to valvular incompetence and the development of varicose veins.

Luminal valves of collecting lymphatic vessels are critical for maintaining unidirectional flow of lymph and their dysfunction underlies several forms of primary lymphedema. Here, we report on the generation of a transgenic mouse expressing the tamoxifen inducible CreERT2 under the control of Cldn11 promoter that allows, for the first time, selective and temporally controlled targeting of lymphatic valve endothelial cells. We show that within the vasculature CLDN11 is specifically expressed in lymphatic valves but is not required for their development as mice with a global loss of Cldn11 displ

BACKGROUND: The aim of this explorative morphologic study was to determine the position and frequency of the valves in the axillary and subclavian veins.
METHODS: The position and frequency of the valves in the subclavian and axillary veins were studied macroscopically in 59 limbs from 30 cadavers. We measured in situ with a measuring tape, starting from the venous angle toward the initiation of the axillary vein. All cadavers were bequeathed by informed consent.
RESULTS: A terminal valve existed in all subclavian veins within the range of 0.0 to 27.5 mm (mean: left, 13.87 mm; right, 9.78 mm

BACKGROUND: Mutations in the FOXC2 gene cause lymphedema distichiasis, an inherited primary lymphedema in which a significant number of patients have varicose veins. Because lymphedema distichiasis is believed to be caused by lymphatic valve failure (reflux), and FOXC2 is highly expressed on venous valves in mouse embryos, we tested the hypothesis that FOXC2 mutations may be linked to venous valve failure and reflux.
METHODS AND RESULTS: The venous system of the leg was investigated with Duplex ultrasound. Pathological reflux was recorded by color Duplex ultrasound in all 18 participants with

Sequelae of the postphlebitic syndrome can new be treated by direct valve surgery. The present surgical treatment of stasis ulcer, including removal of the incompetent perforators, ulcer excision, and skin grafting, remains essential. Excision of perforators and ulcer care are effective but are associated with a high rate of ulcer recurrence. Experimental studies to restore venous valve function include autogenous or homologous vein valve transplantation, valvuloplasty, and valve transposition. In 23 cases of vein valve transplantation and two transpositions, a normal autogenous vein valve fro

Chronic venous disease (CVD) is common. Its manifestations include varicose veins; skin changes such as dermatitis, hyperpigmentation, and lipodermatosclerosis; and chronic leg ulcers. Recent advances in the understanding of its pathophysiology have shown how molecular mechanisms in the inflammatory cascade are involved in these diverse findings. Venous hypertension and associated fluid shear stress alterations on the endothelial surface may initiate this cascade and may lead to adverse changes in the venous wall, venous valves, and skin that can eventually result in varicose veins and in veno

This paper is a detailed case study of the persistent anastomotic channel between the cephalic vein and the external jugular vein, running anterior to the clavicle, corresponding to the jugulocephalic vein present at early stages of the ontogenesis in humans. This connection is not only a relic of early foetal development seldom occurring in adults, but it may also be of clinical significance, increasing the risk of complications during the cephalic vein catheterisation, clavicular fractures or head and neck surgery. The novelty in this paper was to determine the presence and distribution of v

Cor triatriatum dexter is a rare cardiac anomaly with division between the sinus and primitive atrial portions of the right atrium. It is supposed that the failure of the venous valves to regress appropriately may create abnormalities in fetal circulation predisposing the fetus to maldevelopment of the right heart structures. The underdevelopment of the right ventricle with hypoplasia or atresia of the tricuspid valve is a well known abnormality, but underdevelopment of the right atrium with tricuspid hypoplasia due to multiple venous valves is extremely rare. This is a case of complex type of

We investigated correlations among the superficial veins, cutaneous nerves, arteries, and venous valves in 128 cadaveric arms in order to choose safe venipuncture sites in the cubital fossa. The running patterns of the superficial veins were classified into four types (I-IV) and two subtypes (a and b). In types I and II, the median cubital vein (MCV) was connected obliquely between the cephalic and basilic veins in an N-shape, while the median antebrachial vein (MAV) opened into the MCV in type I and into the basilic vein in type II. In type III, the MCV did not exist. In type IV, additional s

The intramural pathways of the venous blood outflow from the cardiac wall have been studied histologically, histochemically and micrometrically in 20 control and 84 experimental dogs with an artificially produced circulatory disturbances, peculiar for congenital heart disease (open arterial canal, coarctation of the aorta and stenosis of the pulmonary trunk). The experimental animals have been observed for 6-12 months. In the venous line of the coronary basin several morphologically differed parts, anatomically and functionally connected between themselves and ensuring blood outflow from the m

Phlebograms for sclerotherapy of varicoceles were performed in 659 patients. In 484 cases no valves of the left spermatic vein could be demonstrated. However, 172 patients showed sufficient valves or absent insertions of the spermatic veins at typical point with retrograde flow through collaterals (persistent intercardinal anastomoses). The value of nutcracker phenomena I and II is discussed. Ontogenetic disorders in the development of the secondary abdominal venous system are responsible for the occurrence of the idiopathic left-sided varicocele.

The authors report on their experience with venous bypass in situ performed in 54 patients. For disruption of venous valves they constructed a simple stripper of the Connolly type. The method of bypass in situ is described in detail and its advantages are discussed. For reconstructive operations below the knee the bypass in situ should be preferred over the conventional reversed graft.

The terminal segments of vertebral, right supreme intercostal and accessory hemiazygos veins from 28 cadavers of various ages were histologically examined to assess the possible presence of intrinsic mechanisms facilitating the delivery of venous blood from the upper vertebral and spinal cord territories. In these veins, blood flow is assisted by gravity (veins of receptive type); however, the tunica adventitia showed a significant amount of longitudinally oriented bundles of smooth muscle fibers together with elastic fibers, i.e., an intrinsic structure similar to that of propulsive type vein

Non-reversed autovein graft was prepared by using a long saphenous vein. Disposable enema-syringe was used for vein graft to be turned inside out easily, and venous valves were resected out completely. After these procedure, a graft was put with syringe again, and turned outside in with same maneurver. Then, a non-reversed autovein graft without valves was prepared. We believe this is an excellent method to prepare non-reversed valveless autovein graft to be used as bypass grafting for cardiovascular surgery.

The medical history in a young man who developed varicose veins and venous insufficiency secondary to an acquired arteriovenous fistula in the thigh is presented. The radiological and clinical findings are discussed. The disease process in the development of varicose veins is not fully understood and cannot be satisfactorily explained as a consequence of arterial pressure within the venous system or a defect in the venous valves alone. The widespread ectatic changes seen within the venous system in our patient suggests the activation of an unknown, possibly humoral factor, resulting in morphol
