[Dilated cardiomyopathy in tachycardiac atrial flutter].

HISTORY AND CLINICAL FINDINGS: We present a 38-year-old male patient with progressive exertional dyspnoea and intermittent palpitations. The initial examination detected a regular tachycardia with a heart rate of 160 bpm. EXAMINATIONS, DIAGNOSIS: In association with intermittent palpitations the 12-lead ECG detected a typical type of atrial flutter. A transthoracic echocardiography and cardiac catheterization revealed a left ventricular dilation and marked systolic dysfunction (EF 39 %) with normal coronaries. The diagnosis of dilated cardiomyopathy was made on these findings. TREATMENT AND C

Study of pathogenesis of ventricular arrhythmia in experimental rats by separation of sinus and ventricular substitutional rhythms.

Pathogenesis of cardiac arrhythmia was determined by EEC changes after ATP-induced complete atrioventricular block. The re-entry mechanism underlays extrasystoles with equal coupling intervals with complexes of ventricular substitution rhythms, which transformed into paroxysmal tachycardia with equal R-R intervals, ventricular flutter, and ventricular fibrillation. Ectopic automaticity was characterized by extrasystole unrelated to the complexes of substitutional rhythms, which was transformed into accelerated idioventricular rhythm and asystole. During trigger activity, the extrasystoles were

Repaired tetralogy of Fallot in the adult.

The majority of persons living with tetralogy of Fallot are now adults and may face a number of long-term cardiac problems that necessitate reoperation. These problems include pulmonary regurgitation, tricuspid regurgitation, ventricular tachycardia, atrial flutter and/or fibrillation, pulmonary artery branch stenoses, right ventricular aneurysms, right ventricular outflow tract obstruction, residual ventricular septal defects, and coronary artery disease. Management approaches to these potential problems are discussed. Issues related to genetics, pregnancy, infective endocarditis, insurabilit

Complex dysrhythmias in infants and children.

Dysrhythmias in infants and children are, in many ways, similar to those in adults, yet several important differences exist in their presentation and management. Complex dysrhythmias most frequently encountered in pediatrics include sinus node dysfunction, chaotic atrial rhythm, atrial flutter, supraventricular tachycardia (including Wolff-Parkinson-White syndrome and junctional ectopic tachycardia), complete atrioventricular block (congenital and acquired), and ventricular dysrhythmias (premature ventricular contractions and ventricular tachycardia). Newer approaches to the diagnosis and mana

A tale of two tachycardias.

A patient with non-ischaemic cardiomyopathy, and pre-existing atypical atrial flutter and left bundle branch block, developed broad complex tachycardia. In this unique and uncommon case of double tachycardia, we discuss the diagnostic approach of ventricular tachycardia in patients with broad complex tachycardia, and the use of different contemporary algorithms to help diagnose ventricular tachycardia and differentiate it from supraventricular tachycardia with aberrant conduction.

[Cardiac arrhythmias during pregnancy--what to do?].

METHODS: Atrial premature beats are frequently diagnosed during pregnancy, supraventricular tachycardia (atrial tachycardia, AV nodal reentrant tachycardia, circus movement tachycardia) less frequently. For acute therapy, electrical cardioversion with 50-100 J is indicated in all unstable patients. In stable supraventricular tachycardia, initial therapy includes vagal maneuvers to terminate breakthrough tachycardias. For short-term management, when vagal maneuvers fail, intravenous adenosine is the drug of first choice and may safely terminate the arrhythmia. For long-term therapy, beta-blocki

[Arrhythmia and conduction defects in polymyositis].

The prevalence of cardiac complications of dermatomyositis and polymyositis is generally underestimated. The authors report the case of heart block as the presenting symptom of the disease, situated in the atrioventricular node. Initially paroxysmal, it eventually became permanent. It was associated with atrial (fibrillation and flutter) and ventricular hyperexcitability (ventricular tachycardia in runs). Myocardial biopsy provided histological proof of the cardiac disease and the definitive link between the nodal conduction defect and the polymyositis.

Stabilization of cardiac rhythm in subsequently fatal ventricular tachycardia and fibrillation by calcitonin gene-related peptide.

A potentially fatal arrhythmia was encountered in a patient with a previous myocardial infarction, who was resistant to treatment with lidocaine, amiodarone and electric defibrillation. Calcitonin gene-related peptide stabilized ventricular fibrillation and flutter, but the patient subsequently died.

Predictors of ventricular tachyarrhythmia in patients with implantable cardioverter-defibrillator and non-ischemic systolic heart failure.

BACKGROUND: The benefit derived from implantable cardioverter-defibrillators (ICD) in subjects with non-ischemic systolic HF (NICM) is less well-established. AIM: The study aimed to determine the incidence, predictors, and prognostic impact of ventricular arrhythmias in patients with ICD and NICM. METHODS: The study sample included 377 consecutive patients with ICD or cardiac resynchronization cardioverter-defibrillators (CRT-D, 74% of patients) and NICM implanted and monitored remotely in a university hospital. RESULTS: During the median (interquartile range [IQR]) follow-up of 1645 (960-2

Pulsed-field ablation for refractory typical atrial flutter resistant to multiple radiofrequency ablations: A technical report.

This technical report presents a case with longstanding, recurrent typical atrial flutter who experienced multiple relapses despite four prior radiofrequency (RF) ablation procedures targeting the cavotricuspid isthmus (CTI). Initial RF attempts failed to achieve durable bidirectional CTI block, leading to persistent arrhythmia and significant symptom burden. Upon current admission, comprehensive electrophysiological mapping confirmed a scar-related, clockwise right atrial flutter with residual conduction across the CTI, notably on both the ventricular and inferior vena cava (IVC) sides. Given

A Quarter of a Century Later: What is Dofetilide's Clinical Role Today?

Dofetilide is a class III antiarrhythmic agent approved by the Food and Drug Administration for the conversion of atrial fibrillation and atrial flutter and maintenance of sinus rhythm in symptomatic patients with persistent arrhythmia. Drug trials showed neutral mortality in post-myocardial infarction patients and those with heart failure. This is a review of postmarket data, including real-world efficacy and safety in a variety of populations. Dofetilide has been used off-label with success in patients with paroxysmal atrial fibrillation and atrial flutter, as well as atrial tachycardia and

False arrhythmia alarms reduction in the intensive care unit: a multimodal approach.

The purpose of this study was to develop algorithms to lower the incidence of false arrhythmia alarms in the ICU using information from independent sources, namely electrocardiogram (ECG), arterial blood pressure (ABP) and photoplethysmogram (PPG). Our approach relies on robust adaptive signal processing techniques in order to extract accurate heart rate (HR) values from the different waveforms. Based on the quality of available signals, heart rate was either estimated from pulsatile waveforms using an adaptive frequency tracking algorithm or computed from ECGs using an adaptive mathematical m

[Effects of diltiazem in supraventricular paroxysmal tachyarrhythmias].

Diltiazem (0.3 mg/kg body weight intravenous in 2 minutes) was administered to 40 patients (24 males, 16 females, mean age 51.55 years) with paroxysmal supraventricular tachyarrhythmias: 7 patients with atrial fibrillation, 6 patients with atrial flutter, 25 patients with paroxysmal supraventricular tachycardia, 2 patients with uncommon atrioventricular reciprocating tachycardia. In patients with atrial fibrillation intravenous diltiazem produced a significant decrease of ventricular response (from 160 +/- 11 to 113.57 +/- 10.34--p less than 0.01). In patients with atrial flutter intravenous d

Narrow-QRS and Wide-QRS Tachycardias.

In a woman with rheumatic heart disease, atrial flutter with a rapid ventricular response, and congestive heart failure, treatment with digoxin slows conduction in the atrioventricular node and thus allows atrioventricular conduction to occur by way of a previously unrecognized accessory pathway.

Characterization of atrial flutter after pulmonary vein isolation by cryoballoon ablation.

PURPOSE: Pulmonary vein isolation (PVI) by cryoballoon ablation (CBA) has emerged as a commonly used technique for the treatment of atrial fibrillation. We sought to explore the incidence, risk factors for, and characterization of post-CBA-PVI atrial flutter. METHODS: We analyzed a prospective registry of patients who underwent CBA-PVI at a single institution. We included patients with more than 3 months of follow-up data and excluded those with a history of cavotricuspid isthmus (CTI) ablation. Locations of post-CBA-PVI atrial flutters were determined by analysis of intracardiac electrograms

Concomitant Short QT Syndrome and Sick Sinus Syndrome: A Case Report.

Short QT syndrome (SQTS) is a rare inherited cardiac channelopathy disease resulting from mutations in potassium channels. It has been associated with several dysrhythmias, including ventricular tachycardia, ventricular fibrillation, atrial fibrillation, and atrial flutter. We report, for the first time, a case of a 19-year-old man presenting with recurrent syncope due to SQTS concomitant with sick sinus syndrome, who underwent implantation of an implantable cardioverter-defibrillator.

Double pacemaker implantation.

A demand-type pacemaker and an inductive radiofrequency atrial pacemaker were implanted in four patients presenting with sick sinus syndrome, bradycardia-tachycardia syndrome, Wolff-Parkinson-White syndrome, atrial flutter, and paroxysmal atrial tachycardia, and in one patient having ventricular tachycardia with ectopic ventricular beats. Our follow-up of these patients showed that double implantation is feasible and effective in controlling different types of arrhythmia occurring in the same patient.

Sudden unexpected death during ambulatory electrocardiographic monitoring. The Framingham Study.

This report documents the clinical and electrocardiographic events of sudden unexpected death in a 52-year-old man without known heart disease during ambulatory electrocardiographic monitoring. This death occurred two and a half hours after detailed noninvasive testing, including treadmill exercise, that was unrevealing. The sequence of S-T change suggesting epicardial injury, multiform ventricular premature depolarizations, and frequent and early cycle ventricular premature depolarizations followed by ventricular tachycardia-flutter-fibrillation precipitated by an early cycle ventricular prem

[Intravenous sotalol in the acute treatment of supraventricular tachycardias].

The effects of intravenous sotalol (0.5 mg/kg in 6 minutes) were studied in 23 patients with supraventricular tachycardia (ventricular rate greater than 120 bpm) after failure of placebo (isotonic dextrose). Thirteen patients had atrial flutter or fibrillation and 10 a junctional tachycardia of recent onset. Sinus rhythm was restored in 4 of the patients with atrial flutter or fibrillation and the ventricular rate was slowed significantly in 3 patients (less than 100 bpm). In 3 other patients the ventricular rate decreased (31 to 35 p. 100) but remained above 100 bpm. Sinus rhythm was restored
