[Echocardiographic evaluation of left ventricular function in mitral valve prolapse].

Echocardiographic assessment of left ventricle functions was performed in 52 cases with idiopathic prolapse of mitral valve, confirmed on the base of the simultaneous presence of the well known auscultatory, phonocardiographic and echocardiographic criteria. Mitral prolapse was established not to be a homogenous group as regards left ventricle function. The majority of the cases (80.7%) had normal EchoC-indices for the pump and total and local contraction function of left ventricle. The cases without (25%) or with light stage (55.7%) mitral regurgitation are included here. In the presence of h

Effect of digitalis on left ventricular function in exercising dogs.

The effect of ouabain on left ventricular function in nonfailing hearts was assessed in 14 chronically instrumented dogs during graded treadmill exercise. At rest, ouabain increased the maximum first derivative of the left ventricular pressure (dp/dtmax) and stroke volume by 38% and 16%, respectively. No change occurred in end-diastolic left ventricular diameter or peak systolic left ventricular pressure. During exercise, ouabain reduced maximum running speed and limited the increments in heart rate and systolic pressure but did not alter dp/dtmax, stroke volume, or end-diastolic diameter. Whe

Echocardiographic assessment of left ventricular function in mitral valve disease.

Echocardiography was used to study left ventricular size and contraction in 90 patients with isolated mitral valve disease--47 patients with mitral stenosis, 26 with mixed mitral valve disease and 89 with mitral regurgitation. Left ventricular measurements included the end-diastolic internal dimension (LVIDd), mural thickness (PWTd), an index of circumferential myocardial contraction--fractional shortening (see article)--and stroke volume (LVSV). The left ventricle was abnormally small only when mitral stenosis was severe. Reduced myocardial contraction was common in patients with rheumatic va

Quantitative Assessment of Left Ventricular Function by Left Ventricular Pressure-Strain Loop in Patients with End-Stage Renal Disease.

INTRODUCTION: A new and noninvasive technology of left ventricular pressure-strain loop (LV-PSL) has recently been used to provide information on myocardial work (MW) and identify subtle modifications in cardiac function. This study aimed to use LV-PSL for early identification of changes in left ventricular (LV) structure and MW in patients with end-stage renal disease (ESRD). METHODS: Seventy-two patients with ESRD were divided into two groups based on undergoing maintenance hemodialysis (MHD), namely, the dialysis group (ESRD-D group) and non-dialysis group (ESRD-ND group). Thirty age- and

Left ventricular systolic function assessment in secondary mitral regurgitation: left ventricular ejection fraction vs. speckle tracking global longitudinal strain.

AIMS: Left ventricular (LV) ejection fraction (LVEF) is currently considered for the decision making of patients with mitral regurgitation (MR). However, LVEF represents change in LV volume between end-diastole and end-systole but does not characterize the intrinsic function of the myocardium. In contrast, speckle-tracking global longitudinal strain (GLS) characterizes myocardial deformation. The present study evaluated whether LV GLS may detect further impairment in LV systolic function in dilated cardiomyopathy patients with and without severe secondary MR matched based on LVEF. METHODS AND

Outcomes, arrhythmic burden and ambulatory monitoring of pediatric patients with left ventricular non-compaction and preserved left ventricular function.

Pediatric patients with left ventricular noncompaction (LVNC) and severe ventricular dysfunction are at risk for sudden death. The aims of this study were to (1) evaluate outcomes, (2) describe arrhythmic burden on Holter monitoring, and (3) analyze the utility of Holter monitoring and its impact on care in pediatric patients with LVNC and preserved or mild ventricular dysfunction. This was a retrospective study including patients <21 years of age with LVNC and ejection fractions ≥45%. Demographic and outcome data were analyzed. Individual and cumulative Holter data were evaluated for all pati

Impact of azelnidipine and amlodipine on left ventricular mass and longitudinal function in hypertensive patients with left ventricular hypertrophy.

BACKGROUND: The impact of long-acting calcium channel blocker (CCB) administration on serial changes in left ventricular (LV) function and morphology in hypertensive patients with LV hypertrophy remains unclear. This study attempted to clarify this impact by comparing the effects of administration of azelnidipine with that of amlodipine using conventional and speckle tracking echocardiography. METHODS: An equal number (16) of 32 hypertensive patients was prospectively assigned to a group administered 5 mg of amlodipine/day or a group administered 16 mg of azelnidipine/day. Marked improvement of left ventricular function after parathyroidectomy in a hemodialysis patient with secondary hyperparathyroidism and left ventricular dysfunction.

A 52-year-old woman, a hemodialysis patient, was admitted because of exertional dyspnea. Echocardiography showed left ventricular (LV) dilatation and reduced contraction. Coronary angiography showed no fixed stenosis. She had elevated levels of parathyroid hormone (PTH) as a result of secondary hyperparathyroidism with advanced renal failure. After parathyroidectomy, marked improvement of LV function following immediate decrease of blood levels of PTH was observed. It is suggested that PTH might have a significant role in the pathogenesis of LV dysfunction and that parathyroidectomy might be e

Carvedilol therapy improved left ventricular function in a patient with arrhythmogenic right ventricular cardiomyopathy.

An asymptomatic 35 year-old man was referred to our hospital because of abnormal ECG findings. The ECG showed complete right bundle branch block and left anterior hemiblock. Echocardiography revealed a moderately enlarged right ventricle (RV) and an apical aneurysm. RV wall motion showed diffusely moderate impairment, while the systolic function of the left ventricle (LV) was slightly decreased. The ejection fractions (EF) of the RV and LV were calculated as 28.1% and 41.9% by Simpson's method using multiple cardiac computed tomography (CT) scans. A 24 hour ambulatory ECG showed only 372 singl

[Maximum rate of left ventricular volume change--a parameter of ventricular function (author's transl)].

Peak left ventricular ejection rate (dV/dtsyst) and peak left ventricular filling rate (dV/dtdiast) were determined from biplane cineangiographies in patients with normal left ventricular function (n = 8), pressure overload (n = 11), aortic regurgitation (n = 7), mitral regurgitation (n = 9), mitral stenosis (n = 6), hypertrophic obstructive cardiomyopathy (n = 10), congestive cardiomyopathy (n = 9) and coronary heart disease (n = 17). dV/dtsyst (normal 642 +/- 187 ml s-1) was reduced in mitral stenosis (447 +/- 77 ml s-1) and was increased significantly in aortic regurgitation (1085 +/- 162 m

[Left atrial booster pump function in left ventricular blood filling: clinical and experimental analyses].

Left atrial booster pump function produces variable effects on cardiac output. Generally, cardiac output decreases by only 15-20% when atrial fibrillation occurs, however, in some cases, hemodynamic collapse occurs through loss of left atrial contraction. We evaluated the relative significance of left atrial booster pump function in acute or chronic load and in myocardial ischemia using the left ventricular volume curve. Blood entering into the left ventricle during the left atrial contraction phase (FVLA) represents the left atrial volume work, and the ratio of FVLA to the left ventricular fi

Automated Deep Learning Pipeline for Characterizing Left Ventricular Diastolic Function.

INTRODUCTION: Left ventricular diastolic dysfunction (LVDD) is most commonly evaluated by echocardiography. However, without a sole identifying metric, LVDD is assessed by a diagnostic algorithm relying on secondary characteristics that is laborious and has potential for interobserver variability. METHODS: To characterize concordance in clinical evaluations of LVDD, we evaluated historical echocardiogram studies at two academic medical centers for variability between clinician text reports and assessment by 2016 American Society of Echocardiography (ASE) guidelines. We then developed a workfl

Right Ventricular Remodeling and Function in Hypoplastic Left Heart Syndrome.

The right ventricle (RV) in hypoplastic left heart syndrome (HLHS) becomes the systemic ventricle pumping against systemic afterload. It also has to adapt to an initially increased volume load followed by a decrease in volume load after Fontan completion. Anatomical HLHS subtype, therapeutic strategy, tricuspid valve regurgitation, recoarctation, and genetics influence RV size and function. The resulting remodeling process can be maladaptive and lead to ventricular systolic and diastolic dysfunction. While systolic dysfunction is a strong predictor for mortality before Fontan, there is increas

Distinctive kidney function trajectories following left ventricular assist device implantation.

BACKGROUND: The aim of this study was to assess for distinct kidney function trajectories following left ventricular assist device (LVAD) placement. Cohort studies of LVAD recipients demonstrate that kidney function tends to increase early after LVAD placement, followed by decline and limited sustained improvement. Inter-individual differences in kidney function response may be obscured. METHODS: We identified continuous flow LVAD implantations in US adults (2016-2017) from INTERMACS (Interagency Registry for Mechanically Assisted Circulatory Support). Primary outcomes were estimated glomerul

Right Ventricular Systolic Function Parameters in Hypoplastic Left Heart Syndrome.

Assessment of the systolic function of the right ventricle (RV) in patients with hypoplastic left heart syndrome (HLHS) is important. The asymmetric shape and heavy trabeculations make accurate assessment of RV systolic function challenging. Novel measures of RV function could be helpful in distinguishing reduced versus preserved function in HLHS and may also be worse in HLHS with preserved function compared to normal controls. These novel methods offer promise, but research and clinical applicability is hindered as no cut-off values for normal function have been established. We performed a re
