Negative ionotropic agents for the treatment of left ventricular outflow tract obstruction due to sigmoid septum and concentric left ventricular hypertrophy.

BACKGROUND: Negative ionotropic agents are established treatments for left ventricular outflow tract obstruction (LVOTO) in hypertrophic cardiomyopathy (HCM). However, their benefit in those with LVOTO without typical HCM is unclear. We evaluated if negative ionotropic agents are beneficial in treatment of LVOTO in patients without typical HCM. METHOD: In this retrospective cohort study, we evaluated echocardiograms and reviewed records of 15 patients with resting LVOT obstruction due to sigmoid septum (SS, n=9) and mild concentric left ventricular hypertrophy (LVH, n=6). Patients received tr

Left ventricular remodelling in outflow tract obstructive lesions during fetal life.

OBJECTIVE: Left ventricular (LV) remodelling in prenatally diagnosed LV outflow tract obstructive lesions such as aortic stenosis and aortic coarctation is important for prenatal counselling and postnatal management. The purpose of this study was to document the spectrum and the progression of different LV remodelling patterns and to identify prenatal markers of hypoplastic left heart syndrome (HLHS). METHODS: We studied 29 fetuses with LV outflow tract obstruction: 13 with isolated aortic stenosis, 14 with isolated aortic coarctation and two with combined aortic stenosis and aortic coarctati

Incidence and management of the left ventricular outflow obstruction in patients with atrioventricular septal defects.

OBJECTIVES: Left ventricular outflow tract obstruction (LVOTO) is a recognized complication after complete repair of atrioventricular septal defect (AVSD). This study reviewed the incidence and management of LVOTO following AVSD repair at a single institution. METHODS: From 1975 to 2019, 24 patients (3.3%, 24/730) underwent reoperation due to LVOTO following partial AVSD (pAVSD) and complete AVSD (cAVSD) repair. The data were retrospectively reviewed. RESULTS: The incidence of LVOTO following pAVSD and cAVSD repair was 4.4% (12/275) and 2.6% (12/455). Freedom from LVOTO reoperation following

Spontaneous variability of left ventricular outflow tract gradient in hypertrophic obstructive cardiomyopathy.

BACKGROUND: Improvement in the left ventricular outflow tract (LVOT) gradient has been used as a means of assessing response to therapy in patients with hypertrophic obstructive cardiomyopathy (HOCM). To our knowledge, no data exist regarding the spontaneous day-to-day variability of the LVOT gradient in patients with HOCM. Defining the magnitude of such variability is critical to properly understand how much improvement in LVOT gradient must be present to invoke a therapeutic response. METHODS AND RESULTS: We studied the spontaneous variation in the continuous-wave, Doppler-derived pressure

Clinical Spectrum and Management Implications of Left Ventricular Outflow Obstruction With Mild Ventricular Septal Thickness in Hypertrophic Cardiomyopathy.

Hypertrophic cardiomyopathy (HC) has historically been characterized as a disease of substantial left ventricular hypertrophy, often associated with dynamic left ventricular outflow tract obstruction. However, we have recently encountered patients with subaortic obstruction and only minimal basal septal thickness, raising important management implications. Thereby, we sought to characterize the natural history and treatment strategies for this under-recognized subgroup of HC patients with dynamic obstruction. Of 1,591 consecutive patients with a HC diagnosis, 113 (7%) were identified with outf

Management of infants with left ventricular outflow obstruction by conduit interposition between the ventricular apex and thoracic aorta.

On the basis of experience with older children, creation of a double-outlet ventricle by interposition of a valved conduit between the apex of the left ventricle and the thoracic aorta appeared to be an option for treating life-threatening left ventricular outflow tract obstruction even in the small heart of the infant. Over the recent 3 year period, nine infants ranging in age from 3 days to 13 months with various forms of left ventricular outflow tract obstruction underwent placement of an apical-aortic conduit to decompress the hypertensive left ventricle. There were two early and two late

Right Ventricular Pacing as Rescue Therapy for Cardiogenic Shock in Takotsubo Cardiomyopathy With Left Ventricular Outflow Tract Obstruction.

Takotsubo cardiomyopathy (TC) is a disease characterized by chest pain that must be differentiated from acute coronary syndrome, yet the coronary arteries are normal, and left ventricular systolic function is impaired. Historically, it had been recognized as a disease with a relatively favorable prognosis that improved spontaneously, but recent reports have made it clear that this is not necessarily the case. Particularly challenging situations arise when left ventricular outflow tract obstruction (LVOTO) is present, and this tendency is especially pronounced when accompanied by cardiogenic sh

Intermediate-term clinical outcomes of primary biventricular repair for left ventricular outflow tract obstruction and ventricular septal defect.

OBJECTIVE: Primary biventricular repair for left ventricular outflow tract obstruction and ventricular septal defect remains challenging. The intermediate-term outcomes and risk factors for mortality remain undefined. METHODS: All patients undergoing primary biventricular repair of left ventricular outflow tract obstruction and ventricular septal defect from 1995 to 2008 at the C. S. Mott Children's Hospital, University of Michigan Health Systems were analyzed. RESULTS: Thirty-one patients (mean age, 18 days; 20 male) with a median follow-up of 6.7 years (range, 0.3-13.5 years) were identifi

Predictors of left ventricular outflow obstruction following single-stage repair of interrupted aortic arch and ventricular septal defect.

This study looked at echocardiographic predictors of left ventricular outflow obstruction after primary neonatal repair of interrupted aortic arch and ventricular septal defect. Results of this study indicate that the only significant independent predictor of left ventricular outflow obstruction is aortic valve diameter; all patients with an aortic valve diameter <4.5 mm (Z score <-5) subsequently developed obstruction, whereas patients with annuli >4.5 mm (Z score >-5) remained free from obstruction.

[Surgical treatment of transposition of the great arteries with intact ventricular septum associated with left ventricular outflow obstruction].

Thirteen patients have undergone surgical treatment for transposition of the great arteries (TGA) with intact ventricular septum (IVS) associated with left ventricular outflow obstruction (LVOTO) in our institute. Ages at operation ranged from three months to six years (mean 9.2 +/- 4.7 months). Of these patients, seven had dynamic type LVOTO (group I), and six had organic LVOTO (group II). Preoperative left ventricular end-diastolic volume was significantly smaller in group II (78 +/- 13% of normal) than control group (135 +/- 53% of normal). Preoperative pressure gradient between the pulmona

Complicated third sternotomy for left ventricular outflow tract obstruction secondary to mitral valve prosthesis strut.

Introduction: LVOT obstruction after surgical bioprosthetic MVP placement is uncommon and the influence of the MVP strut width has seldom been examined. Case presentation: A 74-year-old female presented with palpitations, chest pain, and shortness of breath. She had MVR with 31-mm bioprosthetic valve 2 years prior. TEE confirmed LVOT obstruction secondary to a MVP strut, causing a peak gradient of 55 mmHg. The redo operation was complicated by iatrogenic Type A aortic dissection. The patient was immediately cooled for DHCA with RCP and ascending aortic replacement was performed. MVR was then p

Predictors and prognostic impact of dynamic left ventricular outflow tract obstruction during dobutamine stress echocardiography.

BACKGROUND: Dynamic left ventricular outflow tract obstruction (DLVOTO) is a recognized phenomenon during dobutamine stress echocardiography (DSE), but its predictors and long-term prognostic significance remain uncertain. OBJECTIVES: To identify clinical and echocardiographic predictors of DLVOTO during DSE and to assess the association between DLVOTO and major adverse cardiovascular events (MACE). METHODS: We conducted a retrospective, single-center study included 355 consecutive patients undergoing DSE for ischemia assessment. DLVOTO was defined as a peak left ventricular outflow tract gr

Haemodynamic response to exercise in patients with hypertrophic cardiomyopathy without left ventricular outflow tract obstruction.

BACKGROUND: Dyspnoea and reduced exercise capacity are common features in hypertrophic cardiomyopathy (HCM) without left ventricular outflow tract (LVOT) obstruction, yet the haemodynamic mechanisms contributing to these symptoms are poorly described. This study aimed to characterise the haemodynamic response to exercise in patients with HCM without LVOT obstruction. METHODS: In this observational study, 121 subjects underwent right heart catheterisation with haemodynamic measurements during rest and supine incremental exercise: 59 patients with HCM without LVOT obstruction (58±12 years) and

Detection of Left Ventricular Outflow Obstruction From Standard B-Mode Echocardiogram Videos Using Deep Learning.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) affects 20 million individuals globally, with increased risk of sudden death and heart failure. Although cardiac myosin inhibitors show great promise as disease-specific treatment, current indications are for obstructive HCM. Obstruction is not always well characterized by echocardiography. Artificial intelligence might assist in the improving the underdiagnosis of left ventricular outflow tract (LVOT) obstruction. OBJECTIVES: The authors aimed to develop a deep learning model to detect LVOT obstruction from non-Doppler B-mode echocardiography. M

Fabry cardiomyopathy presenting as hypertrophic phenotype with left ventricular outflow tract obstruction: a case series.

BACKGROUND: Fabry disease is a cardiomyopathy with multisystemic manifestations that is easily to be misdiagnosed as another type of cardiomyopathy. Fabry cardiomyopathy may present with left ventricular outflow tract obstruction (LVOTO), mimicking obstructive hypertrophic cardiomyopathy (oHCM). We present three cases of genetically confirmed Fabry disease that were initially misdiagnosed as oHCM, highlighting the importance of early recognition and tailored management. METHODS: We performed Fabry disease screening on patients diagnosed with oHCM and ultimately identified three individuals wh
