Efficacy of endoscopic third ventriculostomy in fourth ventricular outlet obstruction.

OBJECTIVE: Fourth ventricular outlet obstruction (FVOO), an uncommon cause of obstructive hydrocephalus, is most commonly associated with prior intraventricular hemorrhage or intraventricular infection in children. There have been few reports of FVOO in an adult population. METHODS: Twenty-two patients with FVOO treated with endoscopic third ventriculostomy (ETV) were analyzed retrospectively. RESULTS: Of the 22 patients, 10 were younger than 2 years, 6 were between 2 and 18 years, and 6 were older than 18 years of age. Predisposing factors included tubercular meningitis (1 patient), ventric

Initial experience with endoscopic third ventriculostomy in Nigeria, West Africa.

OBJECTIVES: Endoscopic third ventriculostomy (ETV) as an alternative to traditional shunt surgery in the management of hydrocephalus of different etiologies is new in Nigeria and West Africa, with no published data till date. This initial study was done to determine the success rate and complication among our patient population. MATERIALS AND METHODS: This series consists of a prospective study of the clinical and radiological features and outcome of the first 25 consecutive patients who underwent ETV. ETV was considered to be successful when there was no event occurring during surgery that r

Recurrent intracranial germinoma along the endoscopic ventriculostomy tract. Case report.

The authors report a case of a recurrent intracranial germinoma along the site of an endoscopic third ventriculostomy (ETV) after complete local tumor control using 3D conformal radiation therapy. A 13-year-old girl presented with sudden left upward gaze limitation for 4 days. A pineal region tumor and obstructive hydrocephalus were noted on magnetic resonance (MR) images. An ETV and tumor biopsy procedure were performed, which revealed the lesion to be a germinoma. The patient's visual symptoms and hydrocephalus disappeared postoperatively. Chemotherapy using cisplatin, etoposide, vincristine

Audits can improve neurosurgical practice--illustrated by endoscopic third ventriculostomy.

OBJECTIVE: A single-center, retrospective study was performed to evaluate the effect of audit on the patient selection for endoscopic third ventriculostomy (ETV). MATERIALS AND METHODS: Between 01.01.99 and 07.31.01, 134 patients underwent ETV (group 1). During this period, there was no consensus within the neurosurgical community as to patient selection criteria for ETV. A review of our clinical practice in August 2001 demonstrated significantly lower ETV success rates for patients <6 months of age, patients with communicating hydrocephalus (HC) and for patients with prior shunt surgery. Neisseria sicca meningitis following intracranial hemorrhage and ventriculostomy tube placement.

A normal component of the flora of the oropharynx, Neisseria sicca was first isolated in 1906 and has since been reported as a rare cause of various human infections including endocarditis, pneumonia, sinusitis, sepsis, and urethritis. We report the case of a 44-year-old African-American female with a history of hypertension who presented with complaints of right frontal headache, nausea, photophobia, and vomiting. A computed tomography scan of the patient's brain showed a large subarachnoid hemorrhage, and an arteriogram confirmed a large posterior communicating artery aneurysm. A ventriculos

[Intratumoral hemorrhage in choriocarcinoma during neuroendoscopic third ventriculostomy: case report].

The authors report a case of intratumoral hemorrhage in a pineal region choriocarcinoma during neuroendoscopic third ventriculostomy. A 12-year-old boy who presented with headache and vomiting had precocious puberty. Neuroimagings revealed a pineal region tumor with obstructive hydrocephalus and his serum HCG level was 4,280 mIU/ml. He was diagnosed as having choriocarcinoma and underwent neuroendoscopic third ventriculostomy for obstructive hydrocephalus. There were many tumor vessels observed on the tumor surface, some of which bled subcapsularly. Postoperative CT scan showed the tumor incre

Outcome prediction of third ventriculostomy: a proposed hydrocephalus grading system.

An important factor in making a recommendation for different treatment modalities in hydrocephalus patients (VP shunt versus endoscopic third ventriculostomy) is the definition of the underlying pathology which determines the prognosis/outcome of the surgical procedure. Third ventriculostomies (3rd VS) are successful mainly in obstructive hydrocephalus but also in some subtypes of communicating hydrocephalus. A simple, easily applicable grading system that is designed to predict the outcome of 3rd VS is proposed. The hydrocephalus is graded on the basis of the extent of downward bulging of the

Mechanisms of failure after endoscopic third ventriculostomy in young infants.

OBJECT: The cause of failed endoscopic third vetriculostomy (ETV) surgery in the treatment of hydrocephalus may be a poor absorption of cerebrospinal fluid (CSF) or a new or continuing obstruction of CSF pathways. Particularly in infants, ETV failures often are ascribed to a still poorly developed CSF absorptive capacity. METHODS: The authors report on a series of 11 infants younger than 1 year of age undergoing at least one repeated endoscopic surgery after a failed initial ETV for aqueductal stenosis. The following three patterns of endoscopic findings were observed: 1) reclosure of the ven

Hydrocephalus and racemose cysticercosis: surgical alternative by endoscopic third ventriculostomy.

BACKGROUND AND PURPOSE: Hydrocephalus is a frequent and potentially serious complication of neurocysticercosis. Its treatment often requires ventricular shunting. The complication rate is high due to obstruction or material infection, which may justify endoscopic third ventriculostomy (ETV). OBSERVATION: We report a case of obstructive hydrocephalus in a 46-year-old man in the context of racemose cysticercosis, presenting with headaches and transient disorders of consciousness. Imaging showed cystic lesions of the cisterna magna, responsible for hydrocephalus which was treated effectively by

[Spontaneous ventriculostomy developing in non-communicating hydrocephalus of unestablished etiology].

The authors report a case of exceptionally rarely occurring spontaneous ventriculostomy in the region of epiphyseal recess in a patient with increasing non-communicating hydrocephalus. This case is the fourth one diagnosed intravitally in a group of 22 cases published as yet. Positive ventriculography was the method establishing the diagnosis of this condition during life. Restoration of communication in intracranial cerebrospinal pathways with bypassing of the blockade by development of spontaneous ventriculostomy is regarded as a trial of self-treatment of the organism. The observations demo

Endoscopic Third Ventriculostomy with Choroid Plexus Coagulation for Infant Hydrocephalus.

Endoscopic third ventriculostomy with choroid plexus coagulation/cauterization (ETV-CPC) is a well-established technique for treating certain types of infant hydrocephalus-a significant global issue. Since its first use by Dr. Benjamin Warf in Uganda over 20 years ago, pediatric neurosurgeons around the world have incorporated it into their practices. The primary benefit of the technique is the avoidance of indwelling shunts, which carry upfront costs that can be prohibitive in many parts of the world, and unpredictable long-term failure rates that are undesirable in any setting. Questions rem

Robot-assisted endoscopic third ventriculostomy under intraoperative CT imaging guidance.

BACKGROUND: The robot-assisted neurosurgical procedures have recently benefited of the evolution of intraoperative imaging, including mobile CT unit available in the operating room. This facilitated use paved the way to perform more neurosurgical procedures under robotic assistance. Endoscopic third ventriculocisternostomy requires both a safe transcortical trajectory and a smooth manipulation. METHOD: We describe our technique of robot-assisted endoscopic third ventriculocisternostomy combining robotic assistance and intraoperative CT imaging. CONCLUSION: Robot-assisted endoscopic third ven

Ideal trajectory for frontal ventriculostomy: Radiological study and anatomical study.

OBJECTIVE: Several techniques have been described to improve the accuracy of the freehand procedure for frontal ventriculostomy and reduce complications due to suboptimal placement or misplacement of the catheter tip. To date, none of the available studies have found a reliable, low cost and consistent technique. We aimed to provide a standardized protocol for freehand frontal ventriculostomy. METHODS: In the first part of the radiological study, 125 CT scans were used to measure the length of the catheter using 2 right-sided entry points. In the second part, a grid of 24 entry points on the

Re-Do endoscopic third ventriculostomy. Retrospective analysis of 13 patients.

OBJETIVES: Indication for endoscopic third ventriculostomy (ETV) in the treatment for noncommunicating hydrocephalus is widely accepted. There is controversy regarding the indication of a second procedure (re-ETV) when the first has failed. The objective of this work is to revise ETV failures in a series in which re-ETV was performed and to describe the factors related to its prognosis. METHOD: Retrospective study of pediatric patients with ETV failure treated by re-ETV between 2003 and 2018. Gender, age in first and second ETV, time to failure of first ETV, etiology of hydrocephalus, previou

Endoscopic third ventriculostomy in patients with myelomeningocele after shunt failure.

PURPOSE: Myelomeningocele (MMC) is the most frequent form of spina bifida. Moreover, 90% of patients with MMC have hydrocephalus and require shunt placement. However, shunt failure management in such patients typically involves several shunt revisions, increasing the risk for morbidity and mortality. In this study, we report our experience with endoscopic third ventriculostomy (ETV) in MMC patients with shunts in a Brazilian institution. METHODS: Patients with MMC who presented with shunt failure and underwent ETV alone between January 1996 and December 2016 were included in this study. Patie

Endoscopic Third Ventriculostomy in Failed Ventriculoperitoneal Shunt in Pediatric Population.

INTRODUCTION: Ventriculoperitoneal (VP) shunt malfunction is common in pediatric age group patients. There is a high complication rate and revision rate of VP shunt. Endoscopic third ventriculostomy (ETV) can alleviate these complications and can act as an effective alternative for the treatment of hydrocephalus in this age group of patients. MATERIALS AND METHODS: The authors retrospectively reviewed the management and outcome of 36 failed VP shunts in pediatric patients for the treatment of hydrocephalus. The surgeries were performed between November 2010 and January 2016 in a tertiary care

Robot-assisted endoscopic third ventriculostomy: institutional experience in 9 patients.

OBJECTIVE The endoscopic third ventriculostomy (ETV) is an established and effective treatment for obstructive hydrocephalus. In its most common application, surgeons plan their entry point and the endoscope trajectory for the procedure based on anatomical landmarks, then control the endoscope freehand. Recent studies report an incidence of neural injuries as high as 16.6% of all ETVs performed in North America. The authors have introduced the ROSA system to their ETV procedure to stereotactically optimize endoscope trajectories, to reduce risk of traction on neural structures by the endoscope

Long-term results of endoscopic third ventriculostomy: an outcome analysis.

OBJECT: Endoscopic third ventriculostomy (ETV) is the procedure of choice in the treatment of obstructive hydrocephalus. The excellent clinical and radiological success rates are well known. Nevertheless, very few papers have addressed the very long term outcomes of the procedure in very large series. The authors present a large case series of 113 patients who underwent 126 ETVs, and they highlight the initial postoperative outcome after 3 months and long-term follow-up with an average of 7 years. METHODS: All patients who underwent ETV at the Department of Neurosurgery, Mainz University Hosp
