INTRODUCTION: The objectives of surgical interventions for tumoral lesions of the spine include the establishment and improvement of tumor-related symptoms. Anterior tumor resection followed by reconstruction indicated if surgical treatment allowed a marginal removal of the tumor or could extend the individual survival rate in combination with adjuvant therapy options. Sufficient re-stabilization depends on adequate anterior column reconstruction. The purpose of this retrospective study was to present our experiences and results after anterior tumor resection followed by reconstruction with th

Kummell's disease is a rare spinal disorder characterized as avascular necrosis of a vertebral body occurring in a delayed fashion after minor trauma. The disease derives its name from the German surgeon Hermann Kummell, who described a series of patients with the disorder in 1895. Kummell's disease is distinguished from typical osteoporotic compression fractures, by virtue of the fact that patients with Kummell's disease develop symptoms in delayed fashion. In addition, Kummell's disease exhibits different signal characteristics on magnetic resonance imaging when compared to osteoporotic comp

Congenital dermal sinus is a type of closed spinal dysraphism. Cervical and thoracic regions are the rare sites for dorsal dermal sinuses. Dermal sinuses are frequently associated with dermoid or epidermoid tumor and osseous abnormalities such as bifid spinous process. The association of dorsal dermal sinuses with vertebral body anomalies is very rare. We present MR imaging features of a case of thoracocervical dorsal dermal sinus associated with multiple vertebral body anomalies.

We evaluated the safety and histological results of percutaneous transpedicular biopsy in patients undergoing vertebroplasty for vertebral collapse. Over a 6 year period, we carried out biopsies in 46 patients who underwent percutaneous injection of acrylic surgical cement for 57 collapsed vertebrae, because the diagnosis was not clearly established on clinical or imaging grounds. All procedures were performed under fluoroscopic guidance via a coaxial bitranspedicular approach used for vertebroplasty. We performed a clinical examination and CT after every procedure and approximately 6 months t

Upper instrumented vertebra (UIV) fracture in adult spinal deformity surgery leads serious complications, such as spinal cord injury in 0.5-0.8%. Although tip-apex distance is important for preventing screw cut-out in proximal femoral fracture surgery, this suggest that the screw occupancy rate for bone fragments is also important. The purpose of this study was to investigate how the occupancy rate of pedicle screws (ORPS) affects UIV fracture. Patients with UIV fracture 1 year after surgery were defined as the fracture group (F); others were defined as the no fracture group (NF). ORPS, cut-ou

For compression fracture, vertebral body height loss (VBHL) and kyphotic angle (KA) are two important imaging parameters for determining the prognosis and appropriate treatment. This study used previous measurement methods to assess the degree of VBHL and KA, compare and examine differences between various measurement methods, and examine the correlation between relevant measurement parameters and intravertebral cleft (IVC) in the vertebral body. The radiographic images (lateral view of the T-L spine) of 18 patients with a single-level vertebral compression fracture were reviewed. We measured

PURPOSE: To identify the appropriate intraoperative correction in lumbar vertebral body tethering (VBT) to optimize outcomes and to avoid under- or over-correction at skeletal maturity by accounting for preoperative deformity size and stiffness, body weight, and skeletal maturity.
METHODS: Fifteen cases of pediatric idiopathic scoliosis (average lumbar Cobb angle: 48° (40-59°) were used to build validated 3D patient-specific finite element models, which were calibrated to various preoperative characteristics. Physics-based numerical simulations of VBT were performed in the intraoperative late

INTRODUCTION: Vertebral body tethering (VBT) is growing in popularity for skeletally immature patients with scoliosis because of presumed preservation of spinal motion. Although results have shown preserved thoracic motion, there is minimal data to support motion over the lumbar instrumented segments after VBT. The purpose of this study was to analyze the range of motion of the thoracolumbar and lumbar spine after lumbar VBT.
METHODS: Retrospective review of patients treated with lumbar VBT underwent low-dose biplanar flexion-extension and lateral bending radiographs at 1 year after surgery t

We report a case of vertebral osteonecrosis after chemotherapy in a 24-year-old man with non-Hodgkin lymphoma, with transient avidity on 18F-FDG PET initially misinterpreted as recurrent extranodal disease. The patient demonstrated a partial metabolic response on midtreatment restaging PET; however, posttreatment PET showed an increase in uptake in T4 and T5 vertebrae, interpreted as recurrent vertebral disease. Repeat PET performed 9 days later showed resolution of thoracic vertebral uptake without interval treatment. On follow-up PET study, thoracic vertebrae demonstrated photopenia with scl

PURPOSE: Growth-friendly spinal implants (GFSI) were established for scoliotic children as an interim solution until definite spinal fusion could be performed during puberty. While deformity control was clearly proven, the effects on vertebral shape and morphology are still unclear. Our prospective study assesses the effect of GFSI with continuous distraction on vertebral body shape and volume in SMA children in comparison with previously untreated age-matched SMA patients.
METHODS: Cohort I (n = 19, age 13.2 years) were SMA patients without prior surgical scoliosis treatment. Cohort II (n =

RATIONALE: Gout occurs mainly in joints, but rarely in the spine. In the spine, urate crystals can cause intervertebral space instability but rarely lead to retrolisthesis. Here, we present an extremely rare disease with gout invaded the intervertebral disc with lumbar retrolisthesis.
PATIENT CONCERNS: A 61-years male patient with gout history has suffered from severe low back pain and intermittent claudication. Physical examination showed the level of muscle strength of his left first toe was 3/5. Images illustrated a destruction of the intervertebral space, and a retrolisthesis at L4/5 inte

Bone extraction and division can enhance the accuracy of diagnoses based on whole-body bone SPECT data. This study developed a method for using conventional SPECT for automatic recognition of the vertebral column. A novel feature of the proposed approach is a novel "bone graph" image description method that represents the connectivity between these image regions to facilitate manipulation of morphological relationships in the skeleton before surgery. By tracking the paths shown on the bone graph, skeletal structures can be identified by performing morphological operations. The performance of t

The use of stereotactic body radiotherapy for metastatic spinal tumours is increasing. Serious adverse events for this treatment include vertebral compression fracture (VCF) and radiation myelopathy. Although VCF is a fairly low-risk adverse event (approximately 5% risk) after conventional radiotherapy, crude risk estimates for VCF after spinal SBRT range from 11% to 39%. In this Review, we summarise the evidence and predictive factors for VCF induced by spinal SBRT, review the pathophysiology of VCF in the metastatic spine, and discuss strategies used to prevent and manage this potentially di

Common bony spinal pathologies that could present with progressive spasticity include vertebral body tumors or chronic infections of the spine. Cysticercosis of the spine commonly has an intramedullary occurrence. The authors discuss the presentation and management of a rare case of solitary vertebral cysticercosis that presented with lower-limb spasticity and sphincter involvement.

Delayed post-traumatic osteonecrosis, also known by its eponym Kummell's disease, is a rarely reported clinical entity that likely occurs with higher frequency than recognized. We highlight a case of a 75-year-old female household ambulator who presented with significant thoracolumbar pain and delayed T12 collapse after a ground-level fall. The patient had sustained a trivial fall at home 4 months prior to this presentation and had been hospitalized in our institution at that time for a general medical workup. Dedicated spine radiographs were not obtained during this visit. However, lateral ch

OBJECTIVE: Superior sulcus tumors with involvement of the spine have historically been considered unresectable. We have previously documented a 2-year survival of 54% in patients treated with a multimodality approach. This work builds on our previous experience and examines the long-term outcomes.
METHODS: A retrospective review was performed on patients with superior sulcus non-small cell lung cancer tumors with involvement of the vertebral column (n = 39) treated at The University of Texas MD Anderson Cancer Center from 1990 to 2006. Their clinical and pathologic data were analyzed for shor

We present the case report of a 32-year-old female suffering from severe low back pain because of local vertebral body destruction at L3/4 after migration of PDN-SOLO nucleus replacement. Removal of the dislocated implant via a posterior transforaminal approach was not possible. After posterior stabilisation the implant could be salvaged from the anterior direction, but stabilisation with a vertebral body replacement was necessary.

OBJECTIVE AND IMPORTANCE: Granuloma formation has been reported as a rare complication of pencil lead injury. Insufficient data exist regarding pencil lead injuries of the cervical spine. We present the findings in an 18-year-old male patient with secondary granuloma formation after a penetrating transoral pencil injury. We suggest that imaging characteristics and a detailed history will assist with the diagnosis of such lesions.
