Development of the vestibular system and balance function: differential diagnosis in the pediatric population.

Dizziness is a rare complaint among children. In this article, the authors present the embryology and development of the vestibular system, and offer a rational approach to taking a careful history and ordering and interpreting appropriate vestibular and balance testing in children. A differential diagnosis is presented, so that the likely cause of the balance disorder can be elucidated even in the most complex pediatric patients.

Evolutionary modularity of the primate vestibular system: Morphological distinction of otolithic and canalicular organs.

The mammalian inner ear is traditionally divided into two systems: the peripheral vestibular system (for balance) and the cochlea (for hearing). This bipartite model has shaped our understanding of inner ear evolution, function, and pathology. However, it groups the semicircular ducts (which detect angular velocity) and otolithic organs (which detect linear acceleration, vibration, and head tilt) into a single peripheral vestibular system, overlooking key differences in their structure and function. Using advances in imaging technology, we investigate whether the otolithic system is evolutiona

Progression of Peripheral Vestibular System Degeneration Secondary to Otitis Media in the Chinchilla Model.

HYPOTHESIS: Untreated otitis media (OM) may associate with progressive histopathological changes in the peripheral vestibular system structures in the chinchilla model. BACKGROUND: Previous studies have suggested a link between OM and vestibular pathology. However, the mechanisms underlying this relationship are not well understood. OBJECTIVES: To evaluate the progression of peripheral vestibular pathology in chinchilla temporal bones with OM. METHODS: Chinchilla temporal bones, both control and Streptococcus pneumoniae -inoculated, were obtained from the Paparella Otopathology & Ear Pathog

Agreement between functional and electrophysiologic measures in patients with unilateral peripheral vestibular system impairment.

This investigation was conducted to determine whether there was congruence between "physiology-based" definitions of compensated and uncompensated unilateral peripheral vestibular system impairment and "functional" measures of self-perceived dizziness disability/handicap. A retrospective analysis was performed on data obtained from 122 patients evaluated in the Balance Function Laboratory at Henry Ford Hospital over a 4-year period. Both electronystagmography and rotational test data were tabulated. Additionally, results of a self-report measure of dizziness disability/handicap were tabulated.

Temporal bone studies of the human peripheral vestibular system. Normative Scarpa's ganglion cell data.

Scarpa's ganglion cell counts were performed in 106 serially sectioned, normal human temporal bones from 75 individuals. Of these 106 bones, 15 were from neonates less than 30 days old, 14 were from infants between the ages of 1 and 12 months, and the remainder were distributed throughout each decade of life, with sample sizes ranging from 4 to 10 per decade. All temporal bones had to meet 2 criteria: no symptoms or signs of inner ear disease except for presbycusis in the medical case history and no abnormality in the inner ear on light microscopy. The total ganglion cell counts declined signi

Evaluation of vestibular system with vHIT in industrial workers with noise-induced hearing loss.

PURPOSE: Industrial workers are frequently exposed to noise and the noise can cause hearing loss. The relationship between vestibular abnormalities and long-term exposure to occupational noise has been reported. The aim of this study was to use vHIT (video head impulse test) to evaluate the semicircular canal's function in industrial workers with noise-induced hearing loss. METHODS: In the present study, we included 36 male patients (aged 28-55 years, mean 44.4 ± 6.1 years) with hearing loss due to noise exposure from working in the steel and metal industry for at least 4 years, and a control

The effects of vestibular system lesions on autonomic regulation: observations, mechanisms, and clinical implications.

The loss of labyrinthine inputs in patients or animal models has been demonstrated to affect autonomic regulation. Considerable evidence suggests that vestibular-autonomic responses serve to adjust blood pressure and respiratory activity during movement and postural alterations. However, following peripheral vestibular lesions, compensation rapidly occurs, such that autonomic disturbances are not readily evident in patients with chronic labyrinthine dysfunction. This manuscript summarizes the evidence suggesting that vestibular inputs influence autonomic regulation, but that cardiovascular and

[Effect of pathologic and induced peripheral vestibular balance disturbance on the central nervous system].

The authors investigated the central projection of excitement in acute stage vestibular neuronitis using positron emission tomography. The changes in the pattern of regional cerebral blood flow caused by the disease were compared with the effect of cold caloric stimulation known to provoke similar signs. It was concluded that the involved brain regions overlapped each other only partially. The mismatch could be explained by the compensatory processes developing during the disease. These processes do not normally develop during the caloric vestibular stimulation because of its short duration.

Expression of inducible nitric oxide-synthase in the vestibular system of hydropic guinea pigs.

Immunohistochemical investigations of the guinea pig vestibular system, using a specific antibody to the inducible isoform of NO-synthase (iNOS/NOS II), have been performed 3 weeks after surgical closure of the right endolymphatic duct (n = 7). Endolymphatic hydrops (ELH) of the right temporal bone became evident by excavation of the Reissner's membrane in all seven animals. Those animals revealed iNOS-expression in ganglion cells, in the wall of blood vessels and in nerve fibers of the right vestibular system, while the corresponding left temporal bones and temporal bones of non-operated cont

Post-spaceflight orthostatic intolerance: possible relationship to microgravity-induced plasticity in the vestibular system.

Even after short spaceflights, most astronauts experience at least some postflight reduction of orthostatic tolerance; this problem is severe in some subjects. The mechanisms leading to postflight orthostatic intolerance are not well-established, but have traditionally been thought to include the following: changes in leg hemodynamics, alterations in baroreceptor reflex gain, decreases in exercise tolerance and aerobic fitness, hypovolemia, and altered sensitivity of beta-adrenergic receptors in the periphery. Recent studies have demonstrated that signals from vestibular otolith organs play an
