Fungal endocarditis at the aortotomy site after aortic valve replacement.

This brief report presents a patient with fungal endocarditis involving the suture location in the ascending aorta after aortotomy to replace a stenotic aortic valve. It emphasizes the importance of expanding the diagnostic investigation for endocarditis beyond the prosthesis, which was normal in this case. It also reiterates the value of transesophageal echocardiography during the evaluation of prosthetic valves and reminds the echocardiographer to visualize the aortotomy site carefully.

Classifying prosthetic use via accelerometry in persons with transtibial amputations.

Knowledge of how persons with amputation use their prostheses and how this use changes over time may facilitate effective rehabilitation practices and enhance understanding of prosthesis functionality. Perpetual monitoring and classification of prosthesis use may also increase the health and quality of life for prosthetic users. Existing monitoring and classification systems are often limited in that they require the subject to manipulate the sensor (e.g., attach, remove, or reset a sensor), record data over relatively short time periods, and/or classify a limited number of activities and body

Continuous clinical and radiological follow-up of isainlay reverse shoulder prostheses: results from the isa registry with a minimum of 1-year follow-up.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has been the subject of numerous studies reporting promising clinical outcomes which has led to its widespread use as the most common implant in shoulder arthroplasty today. However, few mid-term studies have reported outcomes of newer stem designs. This study evaluates the clinical and radiographic outcomes of a reverse shoulder prosthesis with an inlay stem design, through a multicenter, retrospective cohort analysis. METHODS: This retrospective study included 856 patients who underwent primary implantation of the isainlay, a reverse to

A Novel Height-Adjustable Nano-Hydroxyapatite/Polyamide-66 Vertebral Body for Reconstruction of Thoracolumbar Structural Stability After Spinal Tumor Resection.

BACKGROUND: Reconstruction of thoracolumbar structural stability is a formidable challenge for spine surgeons after vertebral body tumor resection. Various disadvantages of the currently used expandable or nonexpandable cages have limited their clinical applications. We sought to develop a novel prosthesis for clinical use and assess its preliminary clinical outcome in reconstruction of thoracolumbar structural stability after spinal tumor resection. METHODS: Using data obtained from a retrospective analysis of the morphological characteristics of the thoracolumbar vertebrae and endplates in

Damage Assessment of Retrieved Birmingham Monoblock Cups: Is Conversion to Dual-Mobility Head a Viable Revision Option?

BACKGROUND: A novel revision technique for failed hip resurfacings involves retention of the acetabular cup, if well-fixed, which is mated to dual-mobility (DM) prosthesis in a traditional hip replacement configuration. It is unknown whether existing damage on the retained cup will result in unacceptable wear of the DM prosthesis. METHODS: Thirty retrieved Birmingham (Smith & Nephew) monoblock cups were visually scored for damage features and area of coverage. Surface roughness measurements were obtained within each damage feature as well as reference points on each cup. Analysis of prior met

Echocardiographic assessment of prosthetic heart valves.

Valvular heart disease is a global health problem. It is estimated that more than 280,000 prosthetic heart valves are implanted worldwide each year. As the world's population is aging, the incidence of prosthetic heart valve implantation and the prevalence of prosthetic heart valves continue to increase. Assessing heart valve prosthesis function remains challenging, as prosthesis malfunction is unpredictable but not uncommon. Transthoracic two-dimensional and Doppler echocardiography is the preferred method for assessing prosthetic valve function. Clinically useful Doppler-derived measures for

Temporally-Varying Stimulations for Cortical Visual Neuroprosthetics using Spiking Neural Networks.

Visual neuroprosthesis can help to restore a rudimentary form of sight in visually impaired subjects via electrical stimulation. These systems receive camera input images processed by a artificial neural network (ANN) to output stimulation patterns for driving a large set of electrodes. Previous optimization approaches for visual neuroprosthesis stimulation patterns provided a fixed stimulation amplitude for each electrode. In this work, we look at the feasibility of using a spiking neural network (SNN) instead of the ANN allowing the model to provide time-varying stimulation patterns. During

Improvement in reading performance through training with simulated thalamic visual prostheses.

Simulations of artificial vision are used to provide the researcher an opportunity to explore different aspects of visual prosthesis device design by observing subject performance on various tasks viewed through the simulation. Such studies typically use normal, sighted subjects to measure performance at a given point in time. Relatively few studies examine performance changes longitudinally to quantitatively assess the benefits from a training plan that would be akin to post-implantation rehabilitation. Here, we had six normal, sighted subjects use a standard reading task with daily practice

[Keratoprosthesis with biocolonizable support].

Keratoprosthesis consists in replacing the central cornea with an optical system. The optic requires a support which was usually located in front of or behind the cornea. In our opinion microporous materials can hold the prosthesis in place and prevent melting and leakage. The prosthesis is implanted in a single procedure. The optical core is made of clinical quality PMMA. The core is 5 mm in diameter and 2.5 mm long. To promote tissue stability, 9 mm of a new microporous fluorocarbon (200 microns thick and 80 microns pore size) is used. The material becomes transparent and wettable after impl

Quantitative analysis of phosphenes induced by navigation-guided repetitive transcranial magnetic stimulation.

OBJECTIVE: Though a cortical visual prosthesis is a promising method for treating severe visual disturbances, long-term blindness is known to depress visual cortex activity. We examined the use of repetitive transcranial magnetic stimulation (rTMS) with a navigation system as a direct functional assessment tool for the visual cortex. METHODS: We performed rTMS of the occipital cortex at three different stimulus frequencies (1 Hz, 5 Hz, and 20 Hz), on five stimulus targets around the calcarine fissure in 10 healthy subjects and 3 patients with visual impairment. RESULTS: In the subjects with

Optimal Radial Head Prosthesis Sizing in Elbow Trauma: A Review of Anatomical Principles and Clinical Techniques.

» This review synthesizes current evidence and techniques for optimal spherical radial head arthroplasty, focusing on the importance of accurate prosthesis sizing to ensure a successful outcome. » Replication of the native radial head's diameter and height is important. Mismatch can lead to pain, stiffness, capitellar wear, altered joint kinematics, and accelerated arthritis. » For diameter, the minimal outer diameter of the reconstructed native head is the most reliable guide. If comminution prevents this, the capitellar width on computed tomography scans provides a strong correlative measure

Cardiac CT Beyond Coronaries: Focus on Structural Heart Disease.

PURPOSE OF REVIEW: Cardiac computed tomography (CT) is an established non-invasive imaging tool for the assessment of coronary artery disease. Furthermore, it plays a key role in the preinterventional work-up of patients presenting with structural heart disease. RECENT FINDINGS: CT is the gold standard for preprocedural annular assessment, device sizing, risk determination of annular injury, coronary occlusion or left ventricular outflow tract obstruction, calcification visualization and quantification of the target structure, and prediction of a co-planar fluoroscopic angulation for transcat

Visual cortical potentials of the mouse evoked by electrical stimulation of the retina.

An electrical evoked potential (EEP) of the visual system is a summed electrical record of the visual cortex in response to electrical stimulation of the retina. It is the primary method in the assessment of a visual prosthesis which targets at restoring vision to individuals with disease of the outer retina. However despite marked enthusiasm in the fabrication of such devices, little is known about the feasibility of such devices with the visually deprived brain. Recent research in visual plasticity has demonstrated that the adult brain retains marked plasticity following visual deafferentati

Spatial Restriction of Neural Activation Using Focused Multipolar Stimulation With a Retinal Prosthesis.

PURPOSE: The resolution provided by present state-of-the-art retinal prostheses is severely limiting for recipients, partly due to the broad spread of activation in the retina in response to monopolar (MP) electrical stimulation. Focused multipolar (FMP) stimulation has been shown to restrict neural activation in the cochlea compared to MP stimulation. We extended the FMP stimulation technique to a two-dimensional electrode array and compared its efficacy to MP and hexapolar (HP) stimulation in the retina. METHODS: Normally-sighted cats (n = 6) were implanted with a suprachoroidal electrode a

Transformer-based light-evoked retinal spiking activity prediction.

Retinal visual prosthetic devices aim to restore vision via electrical stimulation delivered on the retina. While a number of devices have been commercially available, the stimulation strategies applied have not met the expectations of end-users. These stimulation strategies involve the neurons being activated based on their spatial properties, regardless of their functions, which may lead to lower visual acuity. The ability to predict light-evoked neural activities thus becomes crucial for the development of a retinal prosthetic device with better visual acuity. In addition to temporal nonlin

High early failure rate of the Columbus posterior stabilized high-flexion knee prosthesis.

BACKGROUND: Most studies report high survivorship rates for TKAs, however, we observed higher than anticipated rates of dislocation and femoral component loosening after implanting a Columbus posterior stabilized prosthesis. QUESTIONS/PURPOSE: We therefore determined (1) the incidence of dislocation and aseptic loosening that occurred after implantation of posterior stabilized high-flexion prostheses in TKAs, (2) the causative factors of dislocation and aseptic femoral component loosening when comparing two designs of prostheses, and (3) the mechanisms of dislocation. METHODS: We retrospecti
