Pulmonary involvement in systemic lupus erythematosus.

Several series have suggested that pulmonary function abnormalities are common in systemic lupus erythematosus. However, only isolated studies have attempted to relate these abnormalities to immunological aspects of the diseases. In the present study respiratory symptoms, pulmonary function tests, and immunological data were reviewed in 22 patients with systemic lupus erythematosus. Seventeen subjects had either clinical evidence or abnormalities of lung function suggestive of pulmonary involvement. A restrictive ventilatory defect or reduction in pulmonary diffusing capacity for carbon monoxi

Evidence for expiratory flow limitation of extrathoracic origin in patients with obstructive sleep apnoea.

BACKGROUND: A study was undertaken to examine the expiratory flow response to a negative pressure (NEP) applied at the airway in patients with no abnormalities of the intrathoracic airway but suffering from the obstructive sleep apnoea syndrome (OSAS). METHODS: Nineteen patients with OSAS with normal spirometric values were studied. NEP of -5 cm H(2)O and -10 cm H(2)O was applied to the mouth when sitting and when supine. RESULTS: Thirteen patients exhibited expiratory flow limitation when supine (group 1). Of these, three also had flow limitation in the sitting position. The remaining six p

Development of a model-based clinical trial simulation platform to optimize the design of clinical trials for Duchenne muscular dystrophy.

Early clinical trials of therapies to treat Duchenne muscular dystrophy (DMD), a fatal genetic X-linked pediatric disease, have been designed based on the limited understanding of natural disease progression and variability in clinical measures over different stages of the continuum of the disease. The objective was to inform the design of DMD clinical trials by developing a disease progression model-based clinical trial simulation (CTS) platform based on measures commonly used in DMD trials. Data were integrated from past studies through the Duchenne Regulatory Science Consortium founded by t

The effect of brace treatment on pulmonary functions in adolescent idiopathic scoliosis: An 8-month follow-up study.

BACKGROUND: The instant effect of a brace on pulmonary functions of patients with adolescent idiopathic scoliosis (AIS) is known. However, the permanent effects of its regular use are still unclear. OBJECTIVE: This study aimed to determine whether a brace in patients with AIS had a permanent effect on respiratory functions. METHODS: Fifteen patients with a mean age of 13.2 ± 1.6 years, and a major Cobb angle of 25.8∘± 7.7∘ participated in this study. Lung volumes and respiratory muscle strength were measured with and without thoracolumbosacral brace, at the end of first month and follow-up p

Does pulmonary function improve after surgical correction of adult idiopathic scoliosis?

PURPOSE: The aim was to investigate pulmonary function after surgical correction of adult idiopathic scoliosis. METHODS: This study included 146 adult scoliosis patients aged 20-50 years (main curve in thoracic spine). Respiratory function was assessed as predicted forced vital capacity (%FVC) and the ratio of forced expiratory volume in 1 s / FVC (%FEV1) preoperatively and 2 years postoperatively and classified as a normal function (≥ 80%), mild impairment (≥ 65% and < 80%), and moderate impairment (< 65%). RESULTS: Preoperative %FVC and %FEV1 were 85.3% and 85.4%, which were 81.5% and 87.5

Is there any difference in pulmonary testing parameters due to spirometry reference values? A study in adolescents with idiopathic scoliosis.

The Global Lung Function Initiative (GLI 2012) presented multi-ethnic spirometry reference values that are recommended to be used instead previous Zapletals' reference values. The study aimed to compare the values of the pulmonary parameters calculated according to the Zapletals' versus the GLI 2012 reference values in adolescents with idiopathic scoliosis. Preoperative pulmonary testing and radiographic evaluation were performed in 39 adolescents (29 females, 10 males) aged 12-17 years. The thoracic Cobb angle ranged 50°-104°. Forced vital capacity (FVC) and forced expiratory volume in one se

Natural variability in the disease course of SSc-ILD: implications for treatment.

Interstitial lung disease (ILD) affects approximately 50% of patients with systemic sclerosis (SSc) and is the leading cause of death in SSc. Our objective was to gain insight into the progression of SSc-associated ILD (SSc-ILD). Using data from longitudinal clinical trials and observational studies, we assessed definitions and patterns of progression, risk factors for progression, and implications for treatment. SSc-ILD progression was commonly defined as exceeding specific thresholds of lung function worsening and/or increasing radiographic involvement. One definition used in several studies

Changes in resting pulmonary function testing over time after the Nuss procedure: A systematic review and meta-analysis.

BACKGROUND: Pectus excavatum, the most common congenital chest wall deformity in pediatric patients, leads to pulmonary dysfunction. There is no consensus regarding the effectiveness of the Nuss procedure for recovering pulmonary function. In this meta- analysis, we focused on the changes that occur in pulmonary function after the Nuss procedure. METHODS: We performed a literature search in the MEDLINE, Embase, Cochrane library and PubMed databases. The included studies were required to contain pulmonary function tests with results adjusted to predicted values both before and after the Nuss p

Hypoxic ventilatory depression in a patient with mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes.

We describe a case of a 21-year-old man with mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes (MELAS) who presented with hypoxic ventilatory depression. He had chronic hypoventilation, which was not explained by weakness of respiratory muscles. His hypercapnic ventilatory response was not impaired. In contrast, hypoxic ventilatory depression was observed in the isocapnic progressive hypoxic response test. After exposure to hypoxic conditions, his respiratory frequency decreased and tidal volume was unchanged. The hypoxic ventilatory depression was partially bloc

Influence of lung volume on the airflow-intensity relationship.

This study reports airflow-intensity relationships observed in crescendo and decrescendo phonations produced at three fundamental frequency levels by 10 men. The purpose of the investigation was to study the possible influence of lung volume on airflow-intensity relationships. Of the 30 decrescendo phonations, 26 showed statistically significant correlation coefficients and none showed significant negative correlations. The crescendo phonations, however, showed greater inter- and intrasubject variability. Of the 30 crescendo phonations, 16 showed statistically significant positive correlation

Effect of left pneumonectomy on lung mechanics in rabbits.

Left pneumonectomy was performed in nine rabbits at 10 wk of age. Detailed pulmonary mechanics studies were performed at 14 wk of age in these rabbits and in nine littermate controls. Lung weight, total lung capacity, and static lung pressure-volume curves of the pneumonectomized rabbits were not different from those of the controls, but the forced vital capacity was smaller (P less than 0.05). Tantalum bronchograms of excised right lungs revealed no difference between controls and pneumonectomized rabbits in terms of bronchial diameter or pressure-area behavior of central or peripheral airway

Trapped air in extrinsic allergic alveolitis.

Non-ventilated lung volume (trapped air) during tidal breathing and several other lung function parameters were determined in 19 patients with extrinsic allergic alveolitis, most of them with farmer's lung, and in 28 non-smoking healthy subjects. The trapped air was significantly larger (p < 0.01) in the patients (mean 0.73 litres) than in normal subjects (mean 0.32 litres); in 7 patients (37%) it was definitely enlarged. The results suggest that the frequent increase of trapped air may be caused by patchy small airways occlusion due to bronchiolitis.
