We report voriconazole levels in an infant with disseminated Candida glabrata infection who received combination antifungal therapy and rescue voriconazole treatment. Serum and cerebrospinal fluid voriconazole levels were higher than anticipated and above target. Dose reduction did not lead to a reduction in the blood or cerebrospinal fluid levels. The patient did not exhibit identifiable drug toxicity.

CASE REPORT: A 70-year-old woman, with a history of diabetes and persistent corneal ulceration, developed a severe keratitis caused by Candida albicans. It evolved rapidly to corneal perforation in spite of treatment with topical amphotericin B and oral itraconazole. An amniotic membrane transplant was performed as an emergency, associated with systemic administration of voriconazole. We have evaluated the efficacy of voriconazole (VRC) in a murine model of systemic infection by Scedosporium apiospermum. The survival of mice treated with VRC at 5, 20, or 40 mg/kg/day was greater than that of the control group (P< or =0.0009). VRC reduced the tissue burden in the spleen and brain (P<0.001 in both organs) in comparison with that of the control group.

The susceptibility of 1763 yeast isolates (from 22 species and seven genera) was tested using Clinical and Laboratory Standards Institute M27-A2 microdilution methodology. Candida spp. predominated (97.1%), mainly C. albicans (51.4%), C. glabrata (16.4%) and C. tropicalis (13.7%), followed by Trichosporon spp. (1.1%) and Cryptococcus neoformans (1.0%). Most isolates came from blood/catheters (72.0%) or the oesophagus/oropharynx (11.3%). The voriconazole, itraconazole, fluconazole and amphotericin B MIC90 values (minimum inhibitory concentration for 90% of the isolates) for all isolates were 1.
