Lymphangioma circumscriptum is a rare benign disorder of lymphatic channels in deep dermal and subcutaneous layers. It can occur either as a congenital abnormality or as acquired damage to previously normal lymphatic channels. It occurs in different parts of the body, and the vulva is one of the parts which is commonly affected. Here, we presented a 39 years old para 2 who presented with vulvar swelling. She was diagnosed with acquired lymphangioma circumscriptum of the vulva and superficial vulvectomy was done.

This publication describes the second known reported case of benign giant condyloma acuminatum of the vulva and anal canal (Buschke-Loewenstein tumour). The diagnosis of squamous cell carcinoma was made initially on clinical examination and could not be excluded by punch biopsy. A full pathological study of the tumour established the diagnosis. A defunctioning colostomy and a perineo-ano-vulvectomy with groin gland dissection was performed and the patient is free of disease 36 months later. The biology of this type of tumour is discussed.

Iatrogenic immune suppression following renal transplantation is frequently associated with certain neoplasms, including vulvar carcinoma. We describe a patient with a vulvar carcinoma less than 1 mm depth of invasion and less than 3 mm superficial spread 12 years after renal transplantation. A simple vulvectomy was performed but 4 months later disease recurred in the inguinal nodes. The patient died 20 months later with progressive disease in the retroperitoneum and liver metastases.

En bloc radical vulvectomy with bilateral inguinofemoral lymphadenectomy has now been replaced by radical wide excision and selective inguinal lymphadenectomy based on the stage and location of invasive vulvar cancer. Early stage lateral cancers can be effectively treated by radical wide excision and ipsilateral superficial inguinal lymphadenectomy. Lymph node mapping using perilesional injection of radiocolloid and blue dye may identify sentinel lymph nodes which can be removed, thereby avoiding the morbidity of full inguinal lymphadenectomy in selected patients with early stage disease.

Primary malignant melanoma of the female urethra is a rare tumour with great agressivity and poor prognosis. An early diagnosis may benefit of radical surgery and adjuvant immunotherapy with curative effect. We present a sixty-five years old female with a malignant melanoma of urethra and a secondary additional melanoma in vulva, that was treated with both radical ureterectomy and vulvectomy. A bilateral inguinal lymphadenectomy and immunotherapy with alpha-interferon were added. The patient is free of disease one year postoperatively.

Inguinal intranodal blue nevus is a rare lesion, but awareness of the condition will avoid a mistaken diagnosis of metastatic melanoma. The authors describe the case of a 40-year-old woman in whom an inguinal node blue nevus was discovered incidentally during radical vulvectomy for squamous cell carcinoma. They describe the characteristic light and electron microscopic features. With increased awareness of this lesion the authors believe it will be found in lymph-node chains other than the inguinal and axillary ones previously reported.

Neuroendocrine tumours are rare in the gynaecologic tract, comprising approximately 2% of all gynaecological tumours. They have an aggressive behaviour and are a diagnostic and clinical challenge, due to their rarity and the lack of standardized therapeutic approaches. There are a few case reports. It is defined as a high-grade carcinoma exhibiting neuroendocrine differentiation. The authors describe the case of a 70-year-old woman, with vulvar neuroendocrine small cell carcinoma after superficial vulvectomy. The patient was submitted to a surgery with wide local excision and adjuvant radiatio

Radical pelvic surgery including pelvic lymphadenectomy in the obturator fossa has become a routine endoscopically performed procedure in patients with gynecologic cancer. Nerve injury during these procedures is rare. However, to choose the best surgical procedure, the surgeon must be aware of the anatomical landmarks of the obturator fossa and of various injury mechanisms. Herein is presented the case of obturator nerve transection during laparoscopic pelvic lymph node dissection, radical vulvectomy, and inguinal lymphadenectomy and its immediate laparoscopic repair in a 56-year-old patient.

Thirty-five vulvectomy specimens were obtained from post-mortem material and studied to assess the distribution of melanin pigment. The information obtained was compared with the melanin distribution in melanoma-in-situ and melanosis vulvae. A lesion had to be larger than 3mm before it was recognised clinically. Melanosis vulvae is but an exaggeration of the normal distribution of melanin pigment. Melanoma-in-situ, however, cannot be reliably distinguished from the benign causes of pigmentation on clinical grounds. The correct diagnosis can only be made histologically. Other causes of vulval p

Mammary-like vulvar carcinoma represents an extremely rare disease. Here, the authors report a case of mammary-like ductal carcinoma of the vulva. A 71-year-old woman affected by stage T2N1M0 mammary-like vulvar carcinoma underwent neoadjuvant chemotherapy, radical vulvectomy, and adjuvant antioestrogen hormone therapy. The patient is alive, 24 months after surgery, with no clinical and radiological evidence of disease. To provide a better insight in to the clinical approach for this rare disease, a review of the available literature was performed.

We present the first case of primary vulvar mucinous adenocarcinoma of ectopic breast origin. The patient is an 84-year-old woman with a mass on the left side of her vulva. A left partial vulvectomy with bilateral inguinal lymph node dissections revealed a mucinous adenocarcinoma that involved the dermis and subcutaneous tissue. The tumor cells were positive for estrogen receptors (ERs), progesterone receptors (PRs), and BRST-1 markers. The clinical and pathologic features, differential diagnosis, and treatment are discussed.

Although lichen sclerosus was first described almost a century ago, a grading system was not devised, and the minimal histologic criteria required for diagnosis were not defined. The present study was based on the sequential sectioning of nine vulvectomy specimens removed for invasive carcinoma. Both epidermal and dermal changes were noted, but only the dermal ones were unique to the disease. Early changes included subtle densification of the basement membrane as well as hyalinization of the papillary dermis and superficial capillaries. Based on these findings, a grading system was developed.

INTRODUCTION: Lichen sclerosus et atrophicus is an inflammatory, scarring dermatosis of the female anogenital area and may lead to pain and sexual dysfunction. In select cases which are refractory to conservative therapy, surgery may provide significant symptom improvement. The objective of this study was to expand the range of surgical treatment options for these patients by presenting the operative outcomes of a specialised reconstructive method using the anterior obturator artery perforator (aOAP) flap.
METHODS: A retrospective cohort study was conducted on sexual outcomes following the ex

OBJECTIVE: To demonstrate near-infrared fluorescence image-guided inguinal sentinel lymph node (SLN) biopsy in patients with vulvar cancer.
METHODS: A 40-year-old woman with a 3-cm-sized palpable left vulvar mass was diagnosed with vulvar cancer on biopsy with protrusion into the vaginal cavity. Pelvic contrast-enhanced magnetic resonance imaging and F-18 fluorodeoxyglucose positron-emission tomography-computed tomography showed a small ulcerative enhancing lesion confined to the left vulva without distant metastasis. The patient was scheduled for radical vulvectomy with a left inguinal SLN b

Adnexal carcinomas of the vulva are rare tumors. A case of an 83-year-old woman with a 3.5 cm vulvar lobulated mass that grew over an 18-month period is reported. Histopathological examination of the initial biopsy revealed a poorly differentiated infiltrating carcinoma. Treatment included radical vulvectomy with bilateral inguinal lymphadenectomy. The histological examination of the vulvectomy specimen resulted in eccrine porocarcinoma (EPC) diagnosis. A review of the literature disclosed eight vulvar cases previously reported. The study of the nine cases including the present revealed that t
