Recent trends in adolescent substance use, primary care screening, and updates in treatment options.

PURPOSE OF REVIEW: To provide a brief overview on trends in common substances of abuse in adolescents, a summary of tools to evaluate adolescent substance abuse in an outpatient setting, and an update of outpatient and inpatient treatment options. RECENT FINDINGS: Recent national data suggest an overall slight decline in the use of tobacco, crystal methamphetamine, heroin, and club drugs. No significant change was noted in the use of alcohol, marijuana, and cocaine. Yearly screening of all adolescents for substance use is recommended. This can be accomplished in an office setting using mnemon

"I struggle at times to see her struggle": Mothers' perspectives on dyslexia-related school struggles and the inter-connected nature of mother and child well-being.

INTRODUCTION: Parents of children with dyslexia may be at elevated risk for parenting stress and mental health concerns. Our aim was to explore the emotional experience of growing up with dyslexia in Australia from parents' perspectives. In so doing, we also developed an understanding of parents' own mental health and support needs informed by their lived experience. METHODOLOGY: Seventeen interviews with mothers of children (9-14 years; 16 with a diagnosis of dyslexia) were analysed using Braun and Clarke's reflexive thematic analysis approach. ANALYSIS: Five themes were developed to addres

Fleas on roof rats in six areas of Los Angeles County, California: their potential role in the transmission of plague and murine typhus to humans.

Roof rats (Rattus rattus) in southern California are rarely involved with plague epizootics and murine typhus. Little evidence exists implicating these rodents as sources of human infection. This might be explained by the absence of fleas capable of transmitting these 2 diseases. From February 1981 through January 1982, roof rats were live-trapped and examined for fleas each month in 4 areas of Los Angeles County. Two other areas were trapped for 9 and 3 months respectively. Areas sampled were in or near the suburban-wilderness fringe where plague and murine typhus occur, and where roof rats c

[Redistribution of glossina in a forest area of Ivory Coast?].

Historically the region of Abengourou is a well-known of sleeping sickness in the forest area of Cote d'Ivoire. However data from epidemiologic studies carried out since 1980 show that this area is currently disease-free. This finding warrants study of glossina vectors to clarify the epidemiology of the disease in this area. Entomologic surveys were carried out over a period of one year. Traps were used to capture glossina in ten natural habitats: villages with or without pigs, coffee, cocoa and rice plantations, grazing lands, camping areas, uncultivated farmlands, trails, forest borderlands

Management of multiple traumas in emergency medicine department: A review.

One of the main causes of adults' disability during their working age is multiple trauma. The process of medical care of patients who are injured seriously is still a challenging job. The primary treatment of these patients in the emergency medicine departments is the most required choice after the wilderness first aid and also would be very required before definitive care in the hospital. The main aim of emergency medicine departments is quick recognition and treatment of injuries which pose severe threat to patients' life in appropriate order of priority. The procedure of primary evaluation

Resilience in a hotter world.

As the planet warms, storms and floods are becoming wilder--killing thousands, disrupting supply chains and power grids, and causing billions of dollars in damage. Meanwhile, supplies of all kinds of resources are dwindling, and demand is rising. These two megachallenges--extreme weather and resource scarcity--could have an unprecedented impact on corporate profits and global prosperity, warns Winston. To manage these threats, companies must do what he calls "the big pivot". The big pivot represents a radical change in strategy, operations, and mind-set. Instead of focusing first on short-term

The Montreal Heart Institute: the idea and the man behind it.

The Montreal Heart Institute (MHI) opened on January 11, 1954. It was the work of a single individual, a young cardiologist with a vision. Paul David (1919-1999) trained in the late 1940s -- first, in Boston with Paul Dudley White and, second, in Paris with Jean Lenègre, two of the well-known cardiologists of the times. In the early 1950s, the only heart institute in the world was the one that Ignacio Chavez had founded in Mexico in 1944. Montreal had a medical institute of its own, the Montreal Neurological Institute, which Wilder Penfield had established in 1934. David probably got his inspi

Therapeutic hypothermia and N-PASS; results from implementation in a level 3 NICU.

BACKGROUND: Neonates that have been subjected to perinatal asphyxia and fulfill criteria for therapeutic hypothermia are cooled to 33.5 °C for 72 h. There is no consensus regarding sedation and analgesic use during hypothermia, but there is evidence supporting the importance of pain relief and adequate sedation. There is a need for assessment of the neonates need for pain relief and sedation, and for adjustments of medication to ensure adequate treatment. There are many different scoring tools available. We found the N-PASS (Neonatal Pain, Agitation and Sedation Scale) scoring tool to be the m

Recommendations on the Appropriate Level of Medical Support at Ultramarathons.

Participation in ultramarathons continues to grow, especially among older individuals and among younger runners who may have less running and wilderness experience than many past participants. While ultramarathons tend to have relatively few serious medical issues, adverse medical incidents do occur. These factors make it increasingly important that appropriate safety precautions and medical support are defined and implemented at these events to enhance the safety of participants, spectators, and volunteers. This document establishes the minimum recommended level of medical support that should

Seroprevalence of Brucella antibodies in harbor seals in Alaska, USA, with age, regional, and reproductive comparisons.

Populations of harbor seal Phoca vitulina in the Gulf of Alaska have dramatically declined during the past 4 decades. Numbers of seals in Glacier Bay, in southeast Alaska, USA, have also declined despite extensive protection. Causes of the declines and slow recovery are poorly understood. Brucellosis is a zoonotic disease that adversely affects reproduction in many domestic species. We measured the seroprevalence of Brucella antibodies in 554 harbor seals in 3 Alaska locations: Prince William Sound (PWS), Glacier Bay (GB), and Tracy Arm Fords Terror (TAFT) Wilderness Area. Objectives included

Pesticides in blood from spectacled caiman (Caiman crocodilus) downstream of banana plantations in Costa Rica.

Spectacled caiman (Caiman crocodilus) are fish-eating crocodilians that inhabit freshwater habitat in tropical regions of the Americas. To assess the exposure of caiman to pesticides from banana plantations, the authors collected whole blood samples (30 mL) from 14 adult caiman that were captured in the North Atlantic region of Costa Rica. Blood samples were analyzed for 70 legacy- and current-use pesticides and breakdown products using newly developed ultra-trace, high-resolution mass spectrometry (HRMS). Caiman accumulated pesticides ranked by concentration as dieldrin > permethrin > mirex >

Tick-borne relapsing fever in the Pacific Northwest: an underdiagnosed illness?

Some 30 cases of tick-borne relapsing fever due to Borrelia are known to have occurred between 1965 and 1978 in the Pacific Northwest. This disease was found more frequently in young men with a history of wilderness exposure during the summer months. Recurrent fever was the most common symptom with temperatures reaching higher than 39.5 degrees C (103.1 degrees F) in all cases, and many patients had three or more febrile episodes. Splenomegaly was the second most common finding reported. Diagnosis of relapsing fever was made in 20 cases by identifying spirochetes on peripheral blood smears. In

Modeling long-term dynamics of carbon dioxide and oxygen in high-altitude wetlands.

Altiplanic wetlands are located above 3000 m.a.s.l. in endorheic basins in South America. Extreme environmental conditions and great ecological value characterize these systems. However, they are threatened because they host great mineralogical wealth. In addition, climate change could impact these environments, unbalancing water cycles and biochemical processes. High-altitude wetlands play a key role in recycling greenhouse gasses through microbial activity, which can alter the effects of meteorological forcings. To analyze these effects, we constructed a transient 0-D model that simulates in

The Disease Ecology, Epidemiology, Clinical Manifestations, and Management of Trichinellosis Linked to Consumption of Wild Animal Meat.

Historically, human trichinellosis was caused by Trichinella spiralis and transmitted to humans by consumption of undercooked domestic pork. Today, most cases of trichinellosis are caused by other Trichinella species and transmitted by consumption of raw or undercooked wild game meats. Given the increasing global prevalence of wild animal meat-linked trichinellosis, the objectives of this review are: 1) to describe the life cycle and global distribution of Trichinella worms; 2) to describe the changing epidemiology of trichinellosis; 3) to describe the clinical phases of trichinellosis; 4) to

Beyond the Cyst: The Many Contributions of Isadore Tarlov (1905-1977) to Contemporary Neurosurgical Practice.

: Isadore Max Tarlov (1905-1977) is primarily remembered for his 1938 description of the eponymous perineural "Tarlov cyst." However, during his long career as a neurosurgeon and researcher, he was responsible for many other observations and inventions that influenced the development of neurosurgery in the 20th century. While studying at Johns Hopkins Medical School he was acquainted with Walter Dandy, and he became the first resident to study under Wilder Penfield at the newly formed Montreal Neurological Institute. He made many novel observations about peripheral and cranial nerve anatomy, p

Adult and Pediatric Emergency Medical Services Ambulance Response Times.

BACKGROUND: The routine use of lights and sirens (L&S) by emergency medical services (EMS) may be associated with increased risk of ambulance crashes. Recently, the National Association of EMS Physicians and American College of Emergency Physicians, among others, released a joint statement recommending decreased use of L&S when appropriate. OBJECTIVES: Given the recent shifting culture in recommended L&S use, our objective was to describe current EMS response times across the U.S., stratified by urbanicity and patient age. METHODS: This was a cross-sectional evaluation of the 2023 National E

Flare guns and trauma: Patterns, severity, and clinical implications from case and national surveillance data.

Flare guns are pyrotechnic devices capable of causing serious injury due to high-temperature combustion and projectile discharge. Though intended for signaling in maritime or wilderness settings, improper use can result in trauma patterns distinct from those seen with typical burns or fireworks. Despite their severity, flare gun injuries remain underreported and poorly characterized. We conducted a scoping review of PubMed, Scopus, and Ovid to identify 12 case reports involving 13 patients, and reviewed US National Electronic Injury Surveillance System records from 2015 to 2024, identifying 18
