Night-shift work is associated with peripheral clock gene dysregulation and circadian transcriptional network remodeling in female healthcare workers.

BACKGROUND: Night-shift work disrupts the circadian-melatonin axis through chronic light-at-night exposure and has been associated with metabolic, inflammatory, cardiovascular, and cancer-related disorders, but peripheral molecular signatures of occupational chronodisruption remain incompletely characterized. Female healthcare workers (HCWs), frequently exposed to prolonged night-shift schedules, represent a relevant study population. In a previous computational study, we identified a deregulated circadian gene panel (CLOCK, TEF, PER1, and PER3) suggestive of transcriptional remodeling in expo

Effect of 12 weeks with a 30-min nap opportunity during the night shift of healthcare workers on early cardiovascular risk biomarkers: the NAPWORK study protocol of a randomised controlled trial.

BACKGROUND: Night shift work is well known to cause health disruption in the short and long term. Among healthcare workers, sleep deprivation is a common concern with many nurses reporting sleep of <6 hour per 24-hour period. Consequences of this chronic sleep restriction have been associated with long-term health issues such as endothelial dysfunction, which plays a key role in the development of cardiovascular diseases, arterial hypertension and type 2 diabetes. While countermeasures such as nap at work have been suggested to mitigate these deleterious effects, the impact of such interventio

Data-driven insights into predictors of stress and sleep health among Pakistani healthcare workers under rotational shifts.

Healthcare professionals in low- and middle-income countries often deal with persistent stress at work, driven by heavy workloads, irregular shift schedules, and limited institutional support. This cross-sectional study explored levels of stress, emotional well-being, sleep issues, and shift work-related challenges among healthcare workers in Pakistan. Overall, participants reported moderate levels of stress and sleep disturbances. Interestingly, positive emotions outweighed negative ones. Age stood out as the strongest predictor of stress; those in their mid to late careers experienced signif

Longitudinal trajectories of sleep inertia among intern nurses with shift work in China: Prevalence, risk factors and outcomes.

There is a growing consensus regarding the significant individual differences in responses to sleep inertia. However, little is known about the longitudinal trajectories of sleep inertia. Therefore, we aimed to investigate the incidence rates of sleep inertia, longitudinal trajectories, influencing factors and outcomes among intern nurses with shift work. A total of 1079 intern nurses with shift work completed online surveys at 2 weeks (T1) and 16 weeks (T2) after their clinical internship, respectively. The demographics and willingness to continue working as nurses, circadian type, sleep prob

The interplay between personal traits and coping strategies on shift-work tolerance: a cross-sectional study.

The ability to adapt to night shift work varies greatly among individuals, but little is known about how personal traits and coping strategies interact to affect shift-work tolerance. This study aimed to identify how certain personal traits (e.g., flexibility, languidness, chronotype, and neuroticism) and behavioral and coping strategies influence shift-work tolerance. Additionally, it explored whether behavioral and coping strategies moderate the association between neuroticism and shift-work tolerance. In this cross-sectional study, nurses (N=297) working rotating shifts consisting of either

Determinants of sleepiness at work among railway control workers.

In the last two decades the control of the Portuguese railway network has become much more centralized in three centres, there integrating the functions of route flow management, electrical control and signalling. This study aimed to investigate the influence of work and individual determinants in sleepiness among railway control workers, namely socio-demographic factors, work ability, psychosocial factors, shiftwork characteristics, fatigue perception, and sleep. Sleepiness by shift was associated with quality of sleep, job satisfaction, fatigue perception, quantitative demands, and age. The

Fixed or Rotating Night Shift Work Undertaken by Women: Implications for Fertility and Miscarriage.

This review summarizes the evidence concerning effects of night shift work on women's reproductive health, specifically difficulty in conceiving and miscarriage. We distinguish between fixed night shift and rotating night shift, as the population subgroups exposed, the social and biological mechanisms, and the magnitude of effects are likely to differ; of note, women working fixed night shift are known to have high tolerance for this schedule. We identified two relevant systematic reviews with meta-analyses and five additional studies. Night shift work may give rise to menstrual cycle disturba

The implications of isolation for remote industrial health workers.

INTRODUCTION: Recent literature has explored the health and social implications of industrial workers who are involved in a variety of long-distance commute (LDC) work arrangements including fly-in, fly-out; bus-in, bus-out; and drive-in, drive-out. However, the role of an industrial health worker in caring for this special population of workers is poorly understood and documented in current literature. In Australia, the health role has existed primarily to meet minimum standards of safety legislation and carry out compliance activities. The combination of low social risk tolerance, increasing

Effect of reducing interns' work hours on serious medical errors in intensive care units.

BACKGROUND: Although sleep deprivation has been shown to impair neurobehavioral performance, few studies have measured its effects on medical errors. METHODS: We conducted a prospective, randomized study comparing the rates of serious medical errors made by interns while they were working according to a traditional schedule with extended (24 hours or more) work shifts every other shift (an "every third night" call schedule) and while they were working according to an intervention schedule that eliminated extended work shifts and reduced the number of hours worked per week. Lifetime exposure to shift work and risk of multiple sclerosis: retrospective and prospective cohort studies in UK Biobank.

BACKGROUND: Multiple sclerosis (MS) is an autoimmune disease with complex aetiology involving genetic, environmental and lifestyle factors. Shift work disrupts circadian rhythms and is a potential occupational risk factor for MS, with exposure before age 20 thought to be of additional risk. OBJECTIVE: To investigate associations between retrospective lifetime shift work exposure and MS diagnosis and between prospective shift work exposure and incident MS over 13 years of follow-up in the UK Biobank cohort. METHODS: Participants that were in work and free of MS at baseline were followed up fo

Slim larks and overweight owls? A two years dietary intervention in shift workers.

Previous studies have shown that shift workers are more prone to non-communicable diseases. The aim of the present crossover study is to investigate whether it is possible to improve the health status of shift workers. Nineteen male shift workers (38.5 years ± 7.4) received every other month a dietary counseling for one year. All subjects kept a seven-day diet diary during a night shift, received bioelectrical impedance analysis, and a laboratory examination was performed at the beginning of the study, after one year and at the end of the study. The laboratory blood test included the main meta

Shift working and cardiovascular health.

Adequate sleep is an important component of cardiovascular health. Due to circadian misalignment and irregular shifts, inadequate sleep is common in shift workers. Adverse consequences include daytime sleepiness, decreased cognitive performance, increased sympathetic tone, inflammation, impaired glucose tolerance, and altered leptin and ghrelin levels. When circadian adjustment has not occurred, daytime sleepers often experience decreased sleep duration and increased wakefulness during displaced sleep; additionally, skipping sleep is common in shift workers, further contributing to sleep depri
