[Heliox in pediatrics].

Heliox is composed of oxygen and helium and its low specific gravity allows a modification of the gas flow within the airway. Breathing heliox favors a laminar flow and therefore decreases the work of breathing. Its usefulness in the child is established in croup or in post-extubation stridor. It can be considered if conventional treatment fails to improve the child's breathing pattern. Its major goal is to avoid invasive manoeuvers as much as possible.

Role of carotid chemoreceptors and pulmonary vagal afferents during helium-oxygen breathing in ponies.

Our purpose was to assess compensatory breathing responses to airway resistance unloading in ponies. We hypothesized that the carotid bodies and hilar nerve afferents, respectively, sense chemical and mechanical changes caused by unloading, hence carotid body-denervated (CBD) and hilar nerve-denervated ponies (HND) might demonstrate greater ventilatory responses when decreasing resistance. At rest and during treadmill exercise, resistance was transiently reduced approximately 40% in five normal, seven CBD, and five HND ponies by breathing gas of 79% He-21% O2 (He-O2). In all groups at rest, He

Global notes: mysteries of 'the first' and the 'last breath' and the gift of Neonatal Resuscitation Program.

First and last breaths define the life span of a living animal including humans. Humans have been fascinated more with the last breath-the death. Neonatologists are focused on initiating the first breath. Millions of babies around the world do not have a chance of survival beyond a few moments after birth for lack of successful initiation of their first breath. Both neonatologists and other health care providers work hard to separate the last breath from the first breath using resuscitative measures thanks to the NRP developed and propagated two decades ago. This paper is dedicated to the init

Work-physiological synchronization as a determinant of performance in repetitive computer work.

The present study tested the hypothesis that performance would improve when the work rhythm of a highly repetitive task was synchronous with a worker's internal physiological rhythms. Experienced office workers (n = 20) used video display terminals (VDTs) to perform a repetitive, self-paced data-entry task in a simulated office environment over a 2-day period. Each work day consisted of six 40-min work periods. Work rhythm changes were induced by varying input data field lengths (3-13 characters) across eleven of the twelve work periods. The degree of synchronization between the work and breat

Respiratory muscle limitation in patients after pneumonectomy.

Exercise capacity is significantly impaired in postpneumonectomy patients who have relatively normal remaining lungs. Our objectives are to determine (1) the nature and extent of mechanical ventilatory abnormalities and oxygen cost of breathing in such patients, and (2) the efficacy of a selective respiratory muscle training program in improving ventilatory and exercise performance. A group of eight postpneumonectomy and eight normal subjects (mean ages 59 and 50 yr, respectively) were studied during steady-state exercise and resting voluntary hyperventilation. Detection of breathing segments in respiratory signals.

The typical approach for analysis of respiratory records consists of detection of respiratory pauses and elimination of segments corrupted by movement artifacts. This is motivated by established rules used for manual scoring of respiratory events, which focus on pause segmentation and do not define criteria to identify breathing segments. With this strategy, breathing segments can only be inferred indirectly from the absence of abnormalities, yielding an unclear and ambiguous definition. In this work we present novel detectors for synchronous and asynchronous breathing, and compare them with A

Energetics of nose and mouth breathing, body size, body composition, and nose volume in young adult males and females.

This research tests the relative ability of body size factors and sex to explain variation in nose volume and to explain metabolic measures during rest and moderate exercise in 50 young adults. It uses standard anthropometric measures plus two new measures of nose morphology (nose/face angle and nose volume) and evaluates body composition with a device for air displacement plethysmography. Standard physiological measures were obtained in nose-only and mouth-only breathing conditions with subjects at rest and using moderate exercise. A factor representing linear and bulk measures of body size a

Inspiratory pressure waveform influences time to failure, respiratory muscle fatigue, and metabolism during resistive breathing.

Increased ventilatory work beyond working capacity of the respiratory muscles can induce fatigue, resulting in limited respiratory muscle endurance (Tlim ). Previous resistive breathing investigations all applied square wave inspiratory pressure as fatigue-inducing pattern. Spontaneous breathing pressure pattern more closely approximate a triangle waveform. This study aimed at comparing Tlim , maximal inspiratory pressure (PImax ), and metabolism between square and triangle wave breathing. Eight healthy subjects (Wei = 76 ± 10 kg, H = 181 ± 7.9 cm, age = 33.5 ± 4.8 years, sex [F/M] = 1/7) comp

A noninvasive method for recording central and obstructive apnea with bradycardia in infants.

A noninvasive method for recording central and obstructive apnea with bradycardia is described. A polygraphy was used to record impedance pneumography, heart rate, and air flow with a nasally positioned thermistor. Episodes of apnea both central and obstructive were measured. In addition, episodes of bradycardia were recorded and the relationship to respiratory events documented. The application of the method to 12 patients with 13 clinical diagnosis for observed episodes demonstrated that the clinical diagnoses were inaccurate in 10 of 13. The method improved diagnoses by detecting apnea, bot

Physiological Effects of High-Flow Tracheal Oxygen in Tracheostomized Patients Weaning From Mechanical Ventilation.

BACKGROUND: High-flow tracheal oxygen (HFTO) is being used as supportive therapy during weaning in tracheostomized patients difficult to wean from invasive mechanical ventilation. There is, however, no clinical evidence for such a strategy. Therefore, we conducted a systematic review to summarize studies evaluating the physiologic effects of HFTO during tracheostomy-facilitated weaning and to identify potential areas for future research in this field. METHODS: Observational and interventional studies on critically ill subjects weaning from mechanical ventilation via tracheostomy published unt

A novel method to quantify breathing effort from respiratory mechanics and esophageal pressure.

Breathing effort is important to quantify to understand mechanisms underlying central and obstructive sleep apnea, respiratory-related arousals, and the timing and effectiveness of invasive or noninvasive mechanically assisted ventilation. Current quantitative methods to evaluate breathing effort rely on inspiratory esophageal or epiglottic pressure swings or changes in diaphragm electromyographic (EMG) activity, where units are problematic to interpret and compare between individuals and to measured ventilation. This paper derives a novel method to quantify breathing effort in units directly

Lung- and Diaphragm-Protective Ventilation by Titrating Inspiratory Support to Diaphragm Effort: A Randomized Clinical Trial.

OBJECTIVES: Lung- and diaphragm-protective ventilation is a novel concept that aims to limit the detrimental effects of mechanical ventilation on the diaphragm while remaining within limits of lung-protective ventilation. The premise is that low breathing effort under mechanical ventilation causes diaphragm atrophy, whereas excessive breathing effort induces diaphragm and lung injury. In a proof-of-concept study, we aimed to assess whether titration of inspiratory support based on diaphragm effort increases the time that patients have effort in a predefined "diaphragm-protective" range, withou

Characterization of the Airflow within an Average Geometry of the Healthy Human Nasal Cavity.

This study's objective was the generation of a standardized geometry of the healthy nasal cavity. An average geometry of the healthy nasal cavity was generated using a statistical shape model based on 25 symptom-free subjects. Airflow within the average geometry and these geometries was calculated using fluid simulations. Integral measures of the nasal resistance, wall shear stresses (WSS) and velocities were calculated as well as cross-sectional areas (CSA). Furthermore, individual WSS and static pressure distributions were mapped onto the average geometry. The average geometry featured an ov

[Noninvasive ventilation in medical intensive care medicine].

Noninvasive ventilation refers to mechanical respiratory assist not requiring an endotracheal airway. Potential advantages include the reduction of complications resulting from intubation and invasive long-term mechanical ventilation and possibly a reduced time demand on medical personnel in the long run. Patients with additional severe organ failure, lacking ability to cooperate or high risk of aspiration are not suitable for noninvasive ventilation. Noninvasive ventilation has proven to be effective in reducing symptoms, need for intubation and mortality in selected patients with exacerbatio

Acetazolamide-induced inhibition of carbonic anhydrase influences energy metabolism and respiratory work in healthy subjects.

To assess the usefulness of acetazolamide in weaning a patient from a respirator, we monitored the changes in the respiratory quotient ratio (RQ ratio), the ventilation volume (VE; l/min.), carbon dioxide elimination (VCO2; ml/min.), the oxygen consumption (VO2; ml/min.) and the metabolic energy expenditure (EE; Cal/day) for 6 hours before (baseline) and after the intravenous administration of acetazolamide, 6 mg/kg, in 12 healthy adult volunteers. The RQ ratio decreased significantly from 0.88 to 0.82 after the injection of acetazolamide, 6 mg/kg, and remained below baseline throughout the 6

Effects of increased end-expiratory lung volume on breathing in awake ponies.

We studied the changes in breathing and respiratory muscle electromyograms (EMG) during passively induced increases in end-expiratory lung volume (EELV) in awake normal (N), hilar nerve-denervated (HND), carotid body-denervated (CBD), and HND + CBD ponies. EELV was increased by applying continuous negative pressure (-10 and -20 cmH2O) around the torso of the standing pony. In all groups, negative pressure produced sustained increases in EELV that were linearly related to the degree of negative pressure. Elevated EELV decreased breathing frequency (f) in N and CBD ponies but increased f in HND

Dyspnea and diaphragmatic fatigue in patients with chronic obstructive pulmonary disease. Responses to theophylline.

This study sought answers to 2 questions: (1) Is severe dyspnea to the point of exhaustion regularly accompanied by diaphragmatic fatigue in patients with moderately severe chronic obstructive pulmonary disease (COPD)? (2) When diaphragmatic fatigue occurs in such patients, does theophylline prevent or delay its onset? Eight eucapnic patients with moderately severe COPD were subjected to 2 different stresses to the point of severe dyspnea requiring cessation of the stress. The stresses were cycle exercise and inspiratory resistive breathing, the latter requiring a tidal Pdi equal to 60% of Pdi
