Wound infection after median sternotomy during the war in Croatia.

From 1990 to 1994 at Clinical Hospital Center, Zagreb, 1904 median sternotomies were performed for cardiac operations. Patients shared the same intensive care unit (ICU) with the wounded persons, admitted to the hospital from battlefield. Infection developed in 124 patients, an incidence of 6.51%. Methicillin resistant Staphylococcus aureus (MRSA) was isolated from 90, methicillin resistant Staphylococcus epidermidis (MRSE) from 19, and gram negative bacilli (GNB) from 56 patients, Pseudomonas aeruginosa in 2, and Clostridium pneumoniae in 1 case. Ninety-six patients (5.04%) developed superfic

Surgical wound infection by Aspergillus fumigatus in liver transplant recipients.

Cutaneous aspergillosis is generally associated with immunosuppression, burns, and major trauma. Most cases are acquired by direct inoculation, although cutaneous involvement does occasionally occur with disseminated disease. Surgical wound infections caused by Aspergillus species are very unusual and to our knowledge have not been described in the setting of organ transplantation. We describe two liver transplant recipients who developed wound aspergillosis during a nosocomial outbreak of Aspergillus infection. Infection developed in the second and fourth postoperative week respectively, and

The effect of endogenous skin bacteria on burn wound infection.

Burn wound sepsis can be due to exogenous or endogenous bacteria. When rare organisms cause infection, exogenous sources are implicated. This sets into motion hospital infection control team searches, which are both exhausting and harassing to patients and staff. This study examines the skin bacteria present at admission and the frequency of endogenous infection in burn patients. Sixty-two patients with burns up to 92% of the total body surface area underwent unburned skin bacterial surveillance on admission and at weekly intervals using RODAC contact plates. Burn wounds were biopsied for quan

Wound infection rates after gastric surgery in a Melbourne hospital.

In four groups of 25 patients in whom perioperative prophylactic antibiotics were not used, the wound infection rates after partial gastrectomy were 40% in gastric cancer, 44% in chronic gastric ulcer, 24% in chronic duodenal ulcer and after trunkal vagotomy and pyloroplasty, the wound infection rate was 12%. In gastric cancer and chronic gastric ulcer, most wound infections were caused by enterobacteria, while in chronic duodenal ulcer almost all infections were caused by Staphylococcus aureus. Appropriate short-term perioperative prophylactic antibiotic use is likely to decrease wound sepsis

Assessment of Severe Extremity Wound Bioburden at the Time of Definitive Wound Closure or Coverage: Correlation With Subsequent Postclosure Deep Wound Infection (Bioburden Study).

Infection remains the most common and significant complication after high-energy fractures. The Bioburden Study is a multicenter, prospective, observational cohort study of wound bacterial bioburden and antibiotic care in severe open lower extremity fractures. The aims of this study are to (1) characterize the contemporary extremity wound "bioburden" at the time of definitive wound closure; (2) determine the concordance between polymerase chain reaction results and hospital microbiology; (3) determine, among those who develop deep infections, the concordance between the pathogens at wound clos

Enzyme responsive antimicrobial hyaluronan-nanocellulose hybrid wound dressings for the treatment of infected wounds.

Wound infections pose a substantial clinical challenge and an escalating healthcare burden, further complicated by the rapid increase in multidrug-resistant bacteria. Antimicrobial peptides (AMPs) offer an alternative to conventional antibiotics, but their rapid degradation, hemolytic activity, and potential cytotoxicity complicate systemic delivery and can have negative impact on wound healing. Here we show a bacterial nanocellulose hyaluronan (BC-HA) hybrid hydrogel wound dressing functionalized with mesoporous silica nanoparticles (MSNs) for localized, enzyme responsive delivery of a sequen

Managing High Exudate Wounds: Improvised Negative Pressure Wound Therapy in Clostridial Infection.

We report a rare and life-threatening case of pediatric gas gangrene caused by Clostridium septicum following an intramuscular injection. The child presented with severe pain and blisters over the left gluteal region. Rapid progression of surgical emphysema coupled with systemic toxicity necessitated immediate surgical debridement, intensive care, and broad-spectrum antibiotic therapy. The causative pathogen was C. septicum. Wound management was particularly challenging due to high exudate volumes. A cost-effective negative pressure wound therapy system was devised using sterilized, readily av

Systematics, diagnosis and treatment of wound infections in chronic wounds: A position paper from WundDACH.

Wound infections are still an interdisciplinary and interprofessional challenge, because of numerous complications, particularly in people with chronic wounds. There are many different concepts and approaches in this field today. Therefore, WundDACH, the umbrella organization of the German-speaking wound healing societies, wrote a position paper on this important topic. An interdisciplinary and interprofessional group of experts from German-speaking countries developed definitions and procedures for nomenclature, diagnosis and treatment of wound infections in people with chronic wounds in a mo

Controlled release of antimicrobial peptides from nanocellulose wound dressings for treatment of wound infections.

Wounds are highly prone to infection, which can delay healing and lead to severe complications such as gangrene and sepsis. Non-healing wounds significantly impact patients' physical and mental well-being and place a substantial financial burden on healthcare systems. Timely and effective treatment of wound infections is critical, but the rise of antibiotic-resistant pathogens complicates this process. In this study, we investigate a potent protease resistant antimicrobial peptide (AMP), PLNC8 αβ, for the treatment of wound infections and present a strategy for localized AMP delivery using fun

Negative pressure wound therapy for sternal wound infections following congenital heart surgery.

OBJECTIVE: This study examines the efficacy of a comprehensive, multidisciplinary wound management team and negative pressure wound therapy (NPWT) for the treatment of sternal wound infections in congenital heart surgery patients. METHOD: A single-institution retrospective review of all congenital heart surgery patients with post-operative sternal wound infections who were treated with NPWT was performed. Patients were evaluated based on (a) whether NPWT occurred before or after the establishment of a multidisciplinary wound management team, and (b) whether NPWT was initiated early (within 2

Prospective analysis of postoperative wound infections using an early exposure method of wound care.

The management of operative wounds in most surgical procedures is important in preventing many complications, most notably, infection. Among most surgical subspecialties, postsurgical dressings vary in type of materials utilized as well as duration left in place. The authors propose that the contemporary method of postoperative wound management (i.e., dressings left in place until sutures are removed) is overly conservative when dealing with the uncomplicated patient population. This study involves 100 patients who underwent forefoot, rearfoot, or ankle procedures in which skin incisions were

Incidence of Wound Infection After Abdominoplasty: A Retrospective Observational Study.

Aim Abdominoplasty is a popular aesthetic and reconstructive procedure aimed at contouring the abdomen, addressing diastasis, and removing excess abdominal skin. Although it is generally safe, the incidence of wound infection remains a significant concern. Wound infections can increase medical costs, impact patients' quality of life, and lead to further complications, including the need for reoperation. This retrospective questionnaire-based study investigates the incidence and associated symptoms of wound infection after abdominoplasty at Salmaniya Medical Complex. Objective  To determine the

Paralysis and Necrotic Wound Infection Resulting From Monocled Cobra Envenomation.

Non-native snake envenomations can be difficult to manage because of challenges obtaining appropriate antivenom and unfamiliarity with the expected clinical effects. This case report describes a 37-year-old man who was envenomated by his pet monocled cobra (Naja kaouthia). He experienced respiratory failure, requiring intubation and mechanical ventilation. Thai king cobra antivenom, recommended by a toxinologist consultant, was obtained from the nearby zoo and administered. The patient was extubated on hospital day (HD) 2. He returned to the hospital two days after initial discharge with worse

Unlocking Wearable Microbial Fuel Cells for Advanced Wound Infection Treatment.

Better infection control will accelerate wound healing and alleviate associated healthcare burdens. Traditional antibacterial dressings often inadequately control infections, inadvertently promoting antibacterial resistance. Our research unveils a novel, dual-functional living dressing that autonomously generates antibacterial agents and delivers electrical stimulation, harnessing the power of spore-forming Bacillus subtilis. This dressing is built on an innovative wearable microbial fuel cell (MFC) framework, using B. subtilis endospores as a powerful, dormant biocatalyst. The endospores are

RISK FACTORS FOR SURGICAL WOUND INFECTION AFTER ELECTIVE LAPAROSCOPIC CHOLECYSTECTOMY.

BACKGROUND: One of the ways to avoid infection after surgical procedures is through antibiotic prophylaxis. This occurs in cholecystectomies with certain risk factors for infection. However, some guidelines suggest the use of antibiotic prophylaxis for all cholecystectomies, although current evidence does not indicate any advantage of this practice in the absence of risk factors. AIMS: This study aims to evaluate the incidence of wound infection after elective laparoscopic cholecystectomies and the use of antibiotic prophylaxis in these procedures. METHODS: This is a retrospective study of 4
