X-ray mammograms reveal abnormal tissue densities, while metabolic images identify regions of abnormal metabolism. Conventional nuclear medicine and radiologic breast images must be acquired at different times with different patient positions making coregistration difficult. Accurate coregistration of metabolic and x-ray images of the breast is likely to be important when acquiring information about the location and diagnosis of suspicious lesions or tumors. It has been assumed that all patients with injury, deformity or pain in the spine should be X-rayed and the films interpreted in the same way regardless of the condition suspected. This is not an effective approach and in part 1 of this article I will discuss the interpretation of the spinal X-ray of the injured patient. The interpretation of spinal X-rays of the patient who complains of pain or deformity will be covered in part 2, to be published in a subsequent issue of the Journal.

In a retrospective analysis, 2,006 X-ray examinations of the skeleton obtained after trauma in 1,386 children were examined; 354 fractures were diagnosed. X-ray films of the skull were requested in 596 (29.7%); fractures were diagnosed in only 13 patients (2.2%). The radiological result changed the medical management in none of these cases. On the other hand, X-ray after trauma in the extremities is indispensable, except for the knee region. Examinations of the lower arm showed a fracture in 54.9%, X-ray of the shoulder 47.7%.

We classified the radiologic findings of 93 patients with acute or subacute farmer's lung (FL) disease by type and severity of the change in chest x-ray film. Despite a number of advanced imaging modalities, plain film x-ray is essential for diagnostic evaluation of the elbow. Although computed tomography and magnetic resonance imaging continue to provide many uses in subtle processes or advanced evaluation, x-rays should typically provide initial, and often all, necessary imaging. Plain film imaging is used to evaluate trauma including fractures and dislocations, occult or suspected bony injury, instability patterns, tumor, arthritis and degenerative disease, and causes of associated pathology such as compression neuropathy.

A possible relationship of upper genital tract abnormalities in the diethylstilbestrol-exposed woman and infertility was studied. Six hundred thirty-two women had roentgenography performed. Three hundred sixty-seven of these women were not using contraception and were actively trying to become pregnant. One hundred thirty-four (36%) of these women reported difficulties conceiving for a period of 1 or more years. Seventy-three percent of these women had abnormal hysterosalpingograms. A review of charts and x-ray films of individuals who sustained ankle sprains was carried out to determine the presence of calcaneonavicular coalition. Coalitions were present in 63% of 215 patients with 223 ankle sprains. An analysis of plain X-ray films, tomography and myelography in 30 patients with proven cervical spinal cord injury, whose X-ray films were negative, has been performed. Features considered were the sagittal diameter of the spinal canal, associated cervical spondylosis, soft tissue changes, mobility studies and the value in cases of cervical injury of two recently described radiological signs. The only consistent feature in the majority of patients was a large posterior osteophytic spur in the lower cervical spine. No other radiological feature was found to be of particular value in diagnosis.

OBJECTIVES: Pediatric thoracolumbar spinal injuries (TLSIs) are rare and uncommonly require intervention. In adults, plain x-ray is not adequate to screen for spinal injury.
