Optimal target localization for subthalamic stimulation in patients with Parkinson disease.

OBJECTIVE: To further determine the causes of variable outcome from deep brain stimulation of the subthalamic nucleus (DBS-STN) in patients with Parkinson disease (PD). METHODS: Data were obtained from our cohort of 309 patients with PD who underwent DBS-STN between 1996 and 2009. We examined the relationship between the 1-year motor, cognitive, and psychiatric outcomes and (1) preoperative PD clinical features, (2) MRI measures, (3) surgical procedure, and (4) locations of therapeutic contacts. RESULTS: Pre- and postoperative results were obtained in 262 patients with PD. The best motor out

Treating post-traumatic tremor with deep brain stimulation: report of five cases.

BACKGROUND: Post-traumatic tremor is one of the most common movement disorders resulting from severe head trauma. However, literature regarding successful deep brain stimulation (DBS) treatment is scarce, resulting in ambiguity regarding the optimal lead location. Most cases support the ventral intermediate nucleus, but there is evidence to defend DBS of the zona incerta, ventral oralis anterior/posterior, and/or a combination of these targets. We report five patients with disabling post-traumatic tremor treated with DBS of the ventral intermediate nucleus and of the globus pallidus internus.

Amylin regulates testosterone levels via steroidogenesis-related enzymes in the central nervous system of male mice.

Amylin is a peripheral satiation signal polypeptide co-secreted with insulin by pancreatic β-cells in response to nutrient ingestion. Amylin participates in the eating-inhibitory effect and regulates energy metabolism by acting on the central nervous system (CNS). However, the role of amylin in regulating the biosynthesis of steroid hormones, such as testosterone, through the hypothalamic-pituitary-gonadal axis (HPG) remains unexplored. However, only limited evidence is available on the involvement of amylin in steroid synthesis, we hypothesize that amylin regulates testosterone levels via ste

Impact of localisation of deep brain stimulation electrodes on motor and neurobehavioural outcomes in Parkinson's disease.

BACKGROUND: Deep brain stimulation (DBS) of the subthalamic nucleus (STN) represents a well-established treatment in advanced Parkinson's disease (PD) for motor signs, but it is still debated concerning psychiatric effects. OBJECTIVE: Exploration of relation between position of active electrode contacts and neuropsychological and motor change after STN DBS procedure for PD. METHODS: A cohort of 34 patients who underwent STN DBS was followed for 6 months. Preoperative and postoperative assessments included mood evaluation (depression and mania) and motor status. Active contact localisation wa

Thalamotomy-Like Effects From Partial Removal of a Ventral Intermediate Nucleus Deep Brain Stimulator Lead in a Patient With Essential Tremor: Case Report.

BACKGROUND AND IMPORTANCE: The ventral intermediate nucleus of the thalamus is a primary target of deep brain stimulation (DBS) in patients with essential tremor. Despite reliable control of contralateral tremor, there is sometimes a need for lead revision in cases of infection, hardware malfunction, or failure to relieve symptoms. Here, we present the case of a patient undergoing revision after ventral intermediate nucleus (Vim) DBS failed to control his tremor. During the electrode removal, the distal portion of the lead was found to be tightly adherent to tissue within the deep brain. Parti

Afferent projections to the rostral anterior pretectal nucleus of the rat: a possible role in the processing of noxious stimuli.

The afferent inputs to the rostral pole of the anterior pretectal nucleus have been examined by utilizing the retrograde axonal transport of a fluorescent dye, Fast Blue. After unilateral injection of the dye into the rostral anterior prectectal nucleus, large numbers of labelled neuronal somata were found in the somatosensory cortex, the ventrolateral geniculate nucleus, the zona incerta, the superior colliculus, the deep mesencephalic nuclei, the pedunculopontine tegmental nucleus and the medial vestibular nucleus. In addition, the contralateral parabigeminal nucleus provided a major input t

Determining an efficient deep brain stimulation target in essential tremor - Cohort study and review of the literature.

INTRODUCTION: Deep brain stimulation (DBS) is a highly efficacious treatment for essential tremor (ET). Still, the optimal anatomical target in the (sub)thalamic area is a matter of debate. The aim of this study was to determine the optimal target of DBS for ET regarding beneficial clinical outcome and impact on activities of daily living as well as stimulation-induced side effects and compare it with previously published coordinates. METHODS: In 30 ET patients undergoing bilateral DBS, severity of tremor was assessed by blinded video ratings before and at 1-year follow-up with DBS ON and OFF

Targeting the posterior subthalamic area for essential tremor: proposal for MRI-based anatomical landmarks.

OBJECTIVE: Deep brain stimulation (DBS) of the posterior subthalamic area (PSA) is an alternative to thalamic DBS for the treatment of essential tremor (ET). The dentato-rubro-thalamic tract (DRTT) has recently been proposed as the anatomical substrate underlying effective stimulation. For clinical purposes, depiction of the DRTT mainly depends on diffusion tensor imaging (DTI)-based tractography, which has some drawbacks. The objective of this study was to present an accurate targeting strategy for DBS of the PSA based on anatomical landmarks visible on MRI and to evaluate clinical effectiven

Pathological activity in mediodorsal thalamus of rats with spinal cord injury pain.

Spinal cord injury (SCI) results not only in motor deficits, but produces, in many patients, excruciating chronic pain (SCI pain). We have previously shown, in a rodent model, that SCI causes suppression of activity in the GABAergic nucleus, the zona incerta (ZI), and concomitant increased activity in one of its main targets, the posterior nucleus of the thalamus (PO); the increased PO activity is correlated with the maintenance and expression of hyperalgesia after SCI. Here, we test the hypothesis that SCI causes a similar pathological increase in other thalamic nuclei regulated by the ZI, sp

Cerebral activity during the anesthesia-like state induced by mesopontine microinjection of pentobarbital.

Microinjection of pentobarbital into a restricted region of rat brainstem, the mesopontine tegmental anesthesia area (MPTA), induces a reversible anesthesia-like state characterized by loss of the righting reflex, atonia, antinociception, and loss of consciousness as assessed by electroencephalogram synchronization. We examined cerebral activity during this state using FOS expression as a marker. Animals were anesthetized for 50 min with a series of intracerebral microinjections of pentobarbital or with systemic pentobarbital and intracerebral microinjections of vehicle. FOS expression was com

Characteristics and distribution of somatosensory evoked potentials in the subthalamic region.

OBJECT: The aim of the present study is to evaluate the topographical distribution of somatosensory evoked potentials (SSEPs) in the subthalamic area, including the zona incerta (ZI). Determination of this distribution may help in the correct placement of deep brain stimulation (DBS) leads. METHODS: Intraoperative SSEPs were recorded from contacts of DBS electrodes at 221 sites in 41 patients: three patients with essential tremor and 38 with Parkinson disease who underwent implantation of DBS electrodes for the relief of severe tremor or parkinsonism. RESULTS: Two distinct SSEPs were recorde

Electric field and stimulating influence generated by deep brain stimulation of the subthalamic nucleus.

OBJECTIVE: The goal of this project was to develop a quantitative understanding of the volume of axonal tissue directly activated by deep brain stimulation (DBS) of the subthalamic nucleus (STN). METHODS: The 3-dimensionally inhomogeneous and anisotropic tissue medium surrounding DBS electrodes complicates our understanding of the electric field and tissue response generated by the stimulation. We developed finite element computer models to address the effects of DBS in a homogeneous isotropic medium, and a medium with tissue conductivity properties derived from human diffusion tensor magneti

Brain activation during human male ejaculation.

Brain mechanisms that control human sexual behavior in general, and ejaculation in particular, are poorly understood. We used positron emission tomography to measure increases in regional cerebral blood flow (rCBF) during ejaculation compared with sexual stimulation in heterosexual male volunteers. Manual penile stimulation was performed by the volunteer's female partner. Primary activation was found in the mesodiencephalic transition zone, including the ventral tegmental area, which is involved in a wide variety of rewarding behaviors. Parallels are drawn between ejaculation and heroin rush.

Neurturin and glial cell line-derived neurotrophic factor receptor-beta (GDNFR-beta), novel proteins related to GDNF and GDNFR-alpha with specific cellular patterns of expression suggesting roles in the developing and adult nervous system and in peripheral organs.

Cloning strategies were used to identify a gene termed glial cell line-derived neurotrophic factor receptor-beta (GDNFR-beta) related to GDNFR-alpha. In situ hybridization was then used to map cellular expression of the GDNF-related trophic factor neurturin (NTN) and GDNFR-beta mRNA in developing and adult mice, and comparisons with GDNFR-alpha and RET were made. Neurturin is expressed in postnatal cerebral cortex, striatum, several brainstem areas, and the pineal gland. GDNFR-beta mRNA was more widely expressed in the developing and adult CNS, including cerebral cortex, cerebellum, thalamus,

Extensive brain mapping of calcitonin-induced anorexia.

The purpose of this study was to compare the localization in the brain of calcitonin-induced anorexia to the distribution of calcitonin binding sites (as described by others). We, thus, performed an extensive mapping of brain structures to determine those involved in calcitonin-induced anorexia. A significant anorexia is found after injection of calcitonin (15 ng in 0.3 microliters) into several brain areas. Forebrain: lateral septum, lateral part of the anterior commissure, and bed nucleus of the stria terminalis; hypothalamus: floor of the anterior part of the hypothalamus, paraventricular n

Autoradiographic distribution of cerebral blood flow increases elicited by stimulation of the nucleus basalis magnocellularis in the unanesthetized rat.

The nucleus basalis magnocellularis (NBM) of the rat, equivalent of Meynert's nucleus in the primate, is the origin of the main cholinergic innervation of the cerebral cortex. Stimulation of this area has been previously shown to induced marked, cholinergically mediated, blood flow increases in the frontal and parietal cortices. However, the complete distribution of the cerebrovascular effects of NBM stimulation within the whole brain has not been determined. In the present study, we used the [14C]iodoantipyrine autoradiographic method to measure local cerebral blood flow (CBF) in the unanesth

The organization of the crossed geniculogeniculate pathway of the rat: a Phaseolus vulgaris-leucoagglutinin study.

The intergeniculate leaflet of the thalamus is known to give rise to neuronal projections to the suprachiasmatic nuclei and the rostral part of the pineal gland. Via these projections the intergeniculate leaflet is considered to play a role in regulation of circadian rhythms. Iontophoretic injections of the anterograde tracer Phaseolus vulgaris-leucoagglutinin were placed in various subnuclei of the lateral geniculate nucleus in order to study the topographical organization of the crossed geniculogeniculate pathway in the rat. Injections involving neurons in the intergeniculate leaflet or the

Subcortical projections to the pontine nuclei in the cat.

In a systematic attempt to trace all projections from the brainstem and diencephalon to the pontine nuclei of the cat, implantations and injections of horseradish peroxidase-wheat germ agglutinin (HRP-WGA) or Fluoro-Gold were placed in the pontine nuclei of 21 cats. In most of the cases there was no evidence of spread of tracer outside the pontine nuclei. Retrogradely labeled cells in the brainstem and diencephalon were carefully mapped and counted. The number labeled cells in the brainstem and diencephalon ranged from 24 in cases with very small implantations to 3,490 in cases with large inje

Specific somatic sensory relays in the mammalian diencephalon.

There are five major domains within the diencephalon that receive input from ascending somatic sensory pathways serving the limbs, trunk and viscera. These domains include the ventroposterolateral nucleus (VPL), the posterior group complex (PO), the zona incerta (ZI), the intralaminar complex and the thalamic reticular nucleus. Of these five domains, VPL is commonly considered a "specific" somatic "relay". The reasons for this view are that VPL receives dense, synaptically secure and precisely organized input from the equally precisely organized dorsal column nuclei (DCN), that most of VPL's n

The diencephalon of the vervet monkey (Cercopithecus aethiops). Part II: epithalamus, subthalamus and hypothalamus.

The nuclear configuration and topography of the epithalamus, subthalamus and hypothalamus of the vervet monkey (Cercopithecus aethiops) are described and compared with those of other primates, particularly the macaque monkey. The epithalamus does not show any striking structural differences, except some architectonic differentiation in the lateral habenular nucleus. The subthalamus is a phylogenetically stable structure throughout the primate scale; it does not show any significant changes, except that it extends less rostrally and that the nuclei entopeduncularis and peripeduncularis are much
