Ovarian sensitization to gonadotrophins by human growth hormone. Persistence of the effect beyond the treated cycle.

OBJECTIVE: To document the persistence of a sensitizing effect of human growth hormone on the ovarian responsiveness to human menopausal gonadotrophin in anovulatory patients resistant to standard gonadotrophin doses. DESIGN: We performed an open study of three patients given 4, 12 or 24 IU recombinant growth hormone on alternate days for 5-7 doses, concomitantly with gonadotrophin, and assessed gonadotrophin dose requirements before, during and after the cycle of growth hormone therapy. PATIENTS: We studied two with isolated gonadotrophin deficiency and primary amenorrhoea and one with a pi

Long-term cabergoline use does not predict degree of prolactinoma fibrosis nor significantly impact surgical outcomes.

PURPOSE: Prolactinomas are often treated initially with dopamine agonists (DA). For patients subsequently treated with surgery, the effect of cabergoline (CAB) on tumor fibrosis and its potential impact on surgical outcomes is largely unexplored. METHODS: Records of patients with prolactinoma treated by a single surgeon between 2006 and 2024 were examined. Analyses considered relationships among duration and cumulative dose of presurgical DA (DA + vs. DA-), extent of fibrosis measured quantitatively by collagen volume fraction (CVF) and qualitatively by surgeon assessment, and remission statu

Pharmacologic Management of Central Fever in Patients With Acute Brain Injury.

BACKGROUND: Central fever is a noninfectious increase in body temperature to >37.5 °C that commonly occurs in patients with acute brain injury. Because central fever has detrimental effects on the injured brain and is associated with prolonged neurointensive care unit stays, timely diagnosis and treatment are imperative. Management includes both pharmacologic and nonpharmacologic methods. However, the optimal pharmacologic approach for the management of central fever remains unclear, so we conducted a scoping review to evaluate the pharmaceutical therapies currently being used. METHODS: We sc

Functional Lactotrophs in Induced Adenohypophysis Differentiated From Human iPS Cells.

Prolactin (PRL), a hormone involved in lactation, is mainly produced and secreted by the lactotrophs of the anterior pituitary (AP) gland. We previously reported a method to generate functional adrenocorticotropic hormone-producing cells by differentiating the AP and hypothalamus simultaneously from human induced pluripotent stem cells (iPSCs). However, PRL-producing cells in the induced AP have not been investigated. Here, we confirmed the presence of PRL-producing cells and evaluated their endocrine functions. We differentiated pituitary cells from human iPSCs using serum-free floating cultu

Uterine Notch2 facilitates pregnancy recognition and corpus luteum maintenance via upregulating decidual Prl8a2.

The maternal recognition of pregnancy is a necessary prerequisite for gestation maintenance through prolonging the corpus luteum lifespan and ensuring progesterone production. In addition to pituitary prolactin and placental lactogens, decidual derived prolactin family members have been presumed to possess luteotropic effect. However, there was a lack of convincing evidence to support this hypothesis. Here, we unveiled an essential role of uterine Notch2 in pregnancy recognition and corpus luteum maintenance. Uterine-specific deletion of Notch2 did not affect female fertility. Nevertheless, th

Dopamine D2 receptor signaling modulates pancreatic beta cell circadian rhythms.

Antipsychotic drugs (APD) have clinically important, adverse effects on metabolism that limit their therapeutic utility. Pancreatic beta cells produce dopamine and express the D2 dopamine receptor (D2R). As D2R antagonists, APDs alter glucose-stimulated insulin secretion, indicating that dopamine likely plays a role in APD-induced metabolic dysfunction. Insulin secretion from beta cells is also modulated by the circadian clock. Disturbed circadian rhythms cause metabolic disturbances similar to those observed in APD-treated subjects. Given the importance of dopamine and circadian rhythms for b

Dopamine outside the brain: The eye, cardiovascular system and endocrine pancreas.

Dopamine (DA) and DA receptors (DR) have been extensively studied in the central nervous system (CNS), but their role in the periphery is still poorly understood. Here we summarize data on DA and DRs in the eye, cardiovascular system and endocrine pancreas, three districts where DA and DA-related drugs have been studied and the expression of DR documented. In the eye, DA modulates ciliary blood flow and aqueous production, which impacts on intraocular pressure and glaucoma. In the cardiovascular system, DA increases blood pressure and heart activity, mostly through a stimulation of adrenocepto

Clinical and Laboratory Characteristics of Hyperprolactinemia in Children and Adolescents: National Survey.

OBJECTIVE: We aimed to report the characteristics at admission, diagnosis, treatment, and follow-up of cases of pediatric hyperprolactinemia in a large multicenter study. METHODS: We reviewed the records of 233 hyperprolactinemic patients, under 18 years of age, who were followed by different centers. The patients were divided as having microadenomas, macroadenomas, drug-induced hyperprolactinemia and idiopathic hyperprolactinemia. Complaints of the patients, their mode of treatment (medication and/or surgery) and outcomes were evaluated in detail. RESULTS: The mean age of the patients with

Dopamine inhibits somatolactin gene expression in tilapia pituitary cells through the dopamine D2 receptors.

Dopamine (DA) is an important neurotransmitter in the central nervous system of vertebrates and possesses key hypophysiotropic functions. Early studies have shown that DA has a potent inhibitory effect on somatolactin (SL) release in fish. However, the mechanisms responsible for DA inhibition of SL gene expression are largely unknown. To this end, tilapia DA type-1 (D1) and type-2 (D2) receptor transcripts were examined in the neurointermediate lobe (NIL) of the tilapia pituitary by real-time PCR. In tilapia, DA not only was effective in inhibiting SL mRNA levels in vivo and in vitro, but also

Dopamine Agonists: Time Pattern of Adverse Effects Reporting in Australia.

BACKGROUND: In Australia, there is voluntary reporting of suspected adverse events (AEs) of therapeutic medicines. Some dopamine agonists (DAs) have serious AEs. OBJECTIVE: We aimed to explore the pattern of DA AE reporting over two decades. METHODS: We analysed AE case line listings obtained from the Australian Committee on the Safety of Medicines (ACSOM) for bromocriptine, cabergoline, pergolide, pramipexole and ropinirole, and related these to drug utilisation data (1992-2012). We noted the AE nature, frequency, onset, novelty, severity and outcome. RESULTS: The 220 suspected AEs fell in

Diminished adrenal sensitivity and ACTH efficacy in obese premenopausal women.

BACKGROUND: The ACTH-cortisol axis in women is activated and associated with decreased ACTH potency, estimated by relating ACTH and cortisol pulse masses. Recently, a new accurate method for constructing the endogenous dose-response relationship was introduced, which is based on the relation between ACTH concentrations and associated cortisol secretion rates within cortisol bursts. HYPOTHESIS: The endogenous dose-response relation between ACTH and cortisol in obesity is changed, leading to diminished responsiveness. SUBJECTS: Twenty-five obese premenopausal women and 16 normal weight premeno

[A case of multiple sclerosis with galactorrhea-amenorrhea syndrome].

Patients with multiple sclerosis sometimes show subthalamic lesions presenting syndrome of inappropriate secretion of ADH (SIADH), hypothermia, hyperprolactinemia, weight loss, and cachexia. Hyperprolactinemia also has been found in the patients with active systemic lupus erythematosus, because prolactin can be produced from human activated lymphocytes. We described a case of multiple sclerosis showing galactorrhea-amenorrhea syndrome with hyperprolactinemia. A 31-year-old woman showed a high level of prolactin in the serum (79.6 ng/ml) during remission stage 5 months after the onset of multip
