The dynamic reconstruction of the medial patellofemoral ligament shows good subjective outcomes but high rates of recurrent instability: a systematic review and meta-analysis.

PURPOSE: The surgical reconstruction of the medial patellofemoral ligament (MPFL) is a commonly used treatment for recurrent patellar dislocations. A surgical method which is frequently used is the dynamic reconstruction of the MPFL (dMPFLr), which involves attaching the released end of a hamstring muscle to the medial patella using a pulley. The aim of this systematic review and meta-analysis is to evaluate the evidence of this method in terms of patient-reported outcomes, the rate of recurrent instabilities, and other complications. METHODS: MEDLINE® and Web of Science™ were used to identif

Use of semi-constrained total knee arthroplasty in gonarthrosis with collateral ligament insufficiency: Clinical and functional outcomes.

INTRODUCTION: Achieving stability in total knee arthroplasty (TKA) is crucial for long-term implant survival. In cases of severe deformity or ligament laxity, constrained implants may be required. Traditionally, increasing constraint involved intramedullary stems. However, there are intermediary alternatives, including employing a constrained polyethylene insert without stems, thereby avoiding complications related to them. The study aims to evaluate our experience with a non-modular constrained (NMC) implant in primary TKA. MATERIAL AND METHODS: We conducted a retrospective review of the cli

[Knee joint hemarthrosis--differential diagnostic considerations for planning an operation].

From 1980 to the end of 1988, arthroscopy has been performed on 620 patients with acute hemarthrosis of the knee joint. Of all the intraarticular lesions, 89.4% required surgery. Arthroscopy has changed in the last decade from diagnostic screening to invasive instrumentation for exact operative planing and alternative operative techniques. Hemarthrosis in stable knee joints was caused by minor lesions (7.84%) that did not require surgical procedures. Seventy-three patients had traumatic patellar dislocations--in 33 cases associated with chondral or osteochondral fractures. In 54.8% of the isol

Mobile-bearing total knee prosthesis: a 5- to 9-year follow-up of the first 110 consecutive arthroplasties.

This study regards the total articulating cementless knee. This is a mobile-bearing knee, the tibial component of which consists of 2 parts: a highly conforming polyethylene insert freely rotating on a metal tray. Our case study relating to the implant of the first 110 knees operated on consecutively from 1991 to 1995 is reported, with an average follow-up of 6.3 years (range, 5-9 years). The average preoperative Knee Society Score was 78 points, and the average postoperative score was 156 points. The complications specifically related to the prosthetic components and which required revision s

Articular cartilage repair by gene therapy using growth factor-producing mesenchymal cells.

OBJECTIVE: To investigate the repair of partial-thickness lesions in rat articular cartilage by combining cell transplantation with transfer of growth factor complementary DNA (cDNA). METHODS: Mesenchymal cells isolated from rib perichondrium were infected ex vivo with adenoviral vectors carrying bone morphogenetic protein 2 (BMP-2) or insulin-like growth factor 1 (IGF-1) cDNA. The cells were suspended in fibrin glue and applied to mechanically induced partial-thickness cartilage lesions in the patellar groove of the rat femur. The filling of the defects was quantified and the quality and int

Optimizing medial patellofemoral ligament reconstruction: a randomized trial of 3D-printed guide plates for enhanced femoral tunnel accuracy and early functional recovery.

This study aimed to determine whether patient-specific 3D-printed guide plates enhance femoral tunnel placement accuracy, reduce operative time, and promote faster functional and kinematic recovery in medial patellofemoral ligament (MPFL) reconstruction compared to conventional techniques. Prospective, randomized controlled trial. Single academic tertiary care hospital. Sixty patients diagnosed with recurrent patellar dislocation were enrolled and randomly assigned to either a 3D-printed guide plate group (n = 30) or a conventional fluoroscopy-guided group (n = 30). Patients in the interventio

Bilateral double osteotomy in severe torsional malalignment syndrome: 16 years follow-up.

BACKGROUND: Torsional malalignment syndrome (TMS) is a well defined condition consisting of a combination of femoral antetorsion and tibial lateral torsion. The axis of knee motion is medially rotated. This may lead to patellofemoral malalignment with an increased Q angle and chondromalacia, patellar subluxation and dislocation. Conservative management is recommended in all but the most rare and severest cases. In these cases deformity correction requires osteotomies at two levels per limb. MATERIALS AND METHODS: From 1987 to 2002 in our institution three patients underwent double femoral and

Age at the time of surgery does not compromise the outcome of deepening trochleoplasty: Results from under and over 30 years old.

PURPOSE: This study aimed to evaluate whether age at the time of surgery influences patient-reported outcome measures (PROMs) in patients undergoing tailored surgical treatment for lateral patellar dislocation (LPD), including deepening trochleoplasty (TP). METHODS: This retrospective cohort study used a prospectively maintained database. The Banff Patella Instability Instrument 2.0 (BPII 2.0) and a numerical analogue scale (0-10) for patellofemoral pain (PFP) and subjective knee joint function were used to assess patients' disease-specific quality of life before and after surgery. Suboptimal patellofemoral alignment is associated with poor clinical outcome scores after primary total knee arthroplasty.

BACKGROUND: Proper patellofemoral alignment is an important goal in total knee arthroplasty (TKA). Acceptable patellar alignment is defined as patellar tilt less than or equal to 5° and patellar displacement less than or equal to 5 mm. Previous studies reported an incidence of post-operative patellar malalignment in TKA from 7 to 35%. However, correlation between patellar malalignment and clinical outcome after TKA remains unclear. The purpose of the present study was to evaluate the effect of patellar tilt and displacement on the clinical outcome of TKA. METHODS: A retrospective review of 13

Patella incarceration with associated patella fracture: a rare case of a paediatric patient with no associated femoral fracture.

We describe the case of a 15-year-old girl who presented to the Accident and Emergency Department with right knee pain and a tense effusion following a twist and fall directly onto her right knee. An MRI scan demonstrated that she had an incarcerated dislocated patella with an associated patella avulsion fracture. This required open reduction. Open reduction and fixation was performed using suture anchors. We feel that this case is particularly pertinent since nearly all previous case reports describe an incarcerated patella with an associated femur fracture. Furthermore, no previous case repo

The Use of Mesh Plates for Difficult Fractures of the Patella.

Patella fractures present some of the more complicated fracture patterns in orthopaedic trauma care. This is partially due to the small size of the fragments but also the articular nature of each fragment. Fixation methods such as cerclage wiring, excision of smaller fragments, and screw fixation of larger fragments all have their own challenges. Our study examined our Level I trauma center's experience with variable angle locked 2.7 mm titanium plates for treatment of comminuted patella fractures or treatment of patellar nonunion. After Institutional Review Board approval, we used billing rec

Modern synthetic material is a safe and effective alternative for medial patellofemoral ligament reconstruction.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction is often performed using gracilis autografts, which may be associated with donor site morbidity and complications. The use of a synthetic material can circumvent a harvest operation and has previously been demonstrated to be effective in other types of reconstructive procedures and may be effective in MPFL reconstruction. This study reports the clinical result with the use of a modern ultra-high molecular weight polyethylene with a braided jacket of polyester tape (FT) in MPFL reconstruction compared to using standard autografts. Anteromedialisation tibial tubercle osteotomy for recurrent patellar instability in young active patients: A retrospective case series.

INTRODUCTION: Recurrent patellar instability can be a source of continued pain and functional limitation in the young, active patient population. Instability in the setting of an elevated tibial tubercle-trochlear groove (TT-TG) distance can be effectively managed with a tibial tubercle osteotomy. At the present time, clinical outcome data are limited with respect to this surgical approach to patellar instability. METHODS: A retrospective chart review was performed to identify all cases of tibial tubercle osteotomy for the management of patellar instability performed at our institution with a
