Successful management of pediatric cardiac emergencies requires an accurate diagnosis to institute an appropriate plan of therapy. The diagnosis, however, is not always straightforward, as evidenced by the nonspecific clinical picture that can be presented by congenital heart defects. Entertaining the possibility of a cardiac problem in neonates with pulmonary symptoms unresponsive to standard therapies is crucial for successful management of patients with congenital heart disease. In addition to ventilatory support, prostaglandin E1 infusions or emergency interventional cardiac catheterizatio

We report two cases of chronic intractable pericardial effusion associated with pulmonary hypertension. Case 1. A 35-year-old women was admitted to our hospital because of dyspnea and edema. Chest X-ray examination showed enlargement of cardiac, and pulmonary artery shadow. An electrocardiogram with high voltage of R in V1 and deep S in V5 suggested right ventricular hypertrophy. Pericardial echo-free space with dilated right ventricle was demonstrated by echocardiography. Cardiac catheterization revealed an elevated pulmonary systolic pressure of 120 mmHg. No intracardiac shunt was calculated

BACKGROUND: Advances in pediatric cardiology and congenital heart surgery have increased the adult population with congenital heart disease (CHD), now facing long-term complications like atrial arrhythmias. Given the limited data and safety concerns in this unique and vulnerable patient group, this study analyzes in-hospital outcomes of atrial catheter ablation in CHD patients versus non-CHD patients from a German nationwide real-world registry.
METHODS: Using health records, all atrial catheter ablation procedures in Germany from 2008 to 2021 were analyzed. After adjustment for confounders,

BACKGROUND: Primary pericardial mesothelioma (PMPM) is a rare, aggressive, and lethal form of cancer. Due to its rarity, low incidence and poor prognosis, PMPM has no accepted standard-of-care treatment options with management and outcomes often extrapolated from diffuse pleural mesothelioma. Disease-specific studies are needed to better define PMPM. We report a case of PMPM highlighting the potential role for multimodal combined therapy.
CASE REPORT: The patient is a 62 years old female who had nonspecific syndromes and inconclusive image findings in May 2023. Then monthly follow-up echocard

Cardiac tamponade is an uncommon complication of systemic sclerosis (SSc) with a high mortality rate. Here, we report a case of a 58-year-old patient with limited cutaneous systemic sclerosis (lcSSc), gastroesophageal reflux disease (GERD), diabetes mellitus, pulmonary hypertension (PHTN), and COVID-19 infection, which occurred one month ago, presenting with a large hemorrhagic pericardial effusion and early cardiac tamponade. The patient had an acute onset of progressive dyspnea and anasarca. On examination, she was tachypneic, tachycardic, desaturating on room air, and hypotensive. Pitting e

Background: Primary cardiac angiosarcomas, especially those originating in the pericardium, are extremely rare and aggressive tumors with poor prognosis. These types of malignant tumors have diverse clinical presentations and are often masked by other comorbidities. Case Summary: Our hospital reported a 59-year-old woman who initially presented with pulmonary thromboembolism (PTE) and was subsequently treated with low-molecular-weight heparin. However, she experienced acute pericardial tamponade after anticoagulation therapy, where no obvious mass was primarily identified upon imaging, both in

BACKGROUND: Takotsubo cardiomyopathy is characterized by transient dysfunction of the medial to apical segment of the left ventricle. Recurrence within a few months or years has been reported and serious complications, including arrhythmia, acute cardiac shock and cardiac rupture, can arise; however, recurrence is rare and takotsubo cardiomyopathy is typically a reversible functional disorder.
CASE PRESENTATION: A 91-year-old Japanese woman with a past medical history of angina pectoris, hypertension and uterine carcinoma noted bilateral axillary pain and presented herself to an emergency roo

We report a rare case of a 75-year-old man with a history of mild-to-moderate pericardial effusion that was detected on echocardiography performed in October 2011 when the patient was 69 years old. Follow-up echocardiography was performed every 6 months thereafter, showing that the pericardial effusion gradually subsided. However, in April 2017 he started experiencing several episodes of dyspnea, which prompted him to visit our hospital's outpatient department on June 22, 2017. Echocardiography revealed a large amount of pericardial effusion; thus, he was immediately hospitalized. After underg

AIM: The aim of this study is to report recent trends in the performance of endomyocardial biopsy (EMB) and its in-hospital complications (and their predictors) in the United States (US).
METHOD: We analyzed Nationwide Inpatient Sample (NIS) database from years 2007 through 2014 to identify patients who underwent EMB. Once identified, the patients were subdivided into those with no history of heart transplant (HT) (cohort 1) and those with history of HT (cohort 2). We then studied the major complication of pericardial effusion, hemopericardium or/and cardiac tamponade that required a pericard

The aim of the study was to identify the determinants of pericardial effusion (PE) after a first myocardial infarction (MI). Cardiac magnetic resonance enables early analysis of multiple post-MI parameters; 193 patients with a first ST-elevation MI admitted to the Angers University Hospital (France) were enrolled prospectively. Cardiac magnetic resonance was performed at baseline (median of 5 days [4 to 7]) and repeated at a 3-month follow-up to investigate left ventricular (LV) volumes, LV ejection fraction, infarct size, microvascular obstruction (MVO), systolic wall stress (SWS), and PE pre

A man in his fifties was admitted to our hospital because of syncope and hypotension. In the emergency room he reported abdominal pain, but focused assessment with sonography for trauma (FAST) was negative. His systolic blood pressure varied between 60 and 90 mmHg and an arterial blood gas revealed lactic acidosis with normal haemoglobin, which still might indicate acute bleeding. An ECG did not indicate myocardial infarction. In the absence of an obvious bleeding focus, the patient was urgently transported to a CT lab nearby, to identify possible bleeding and rule out other causes of circulat

OBJECTIVE: To determine the mortality rate and its predictors in patients with a presumptive diagnosis of tuberculous pericarditis in sub-Saharan Africa.
DESIGN: Between 1 March 2004 and 31 October 2004, we enrolled 185 consecutive patients with presumed tuberculous pericarditis from 15 referral hospitals in Cameroon, Nigeria and South Africa, and observed them during the 6-month course of antituberculosis treatment for the major outcome of mortality. This was an observational study, with the diagnosis and management of each patient left at the discretion of the attending physician. Using Cox

BACKGROUND: Acute chest pain is very frequent in medicine and caused by several cardiac disorders or disorders of the great vessels. In addition, chest pain is well known as "noncardiac chest pain" or due to functional abnormalities. For prognosis and long-term follow-up, acute coronary syndromes (ACS) and ascending aortic dissections are important disorders.
RESULTS: The ACS represents a heterogeneous group of patients along a continuum of risk from unstable angina (UA) to non-ST segment elevation (NSTEMI) to ST segment elevation myocardial infarction (STEMI). In patients with NSTEMI/UA, bet

BACKGROUND: Previous studies have suggested that cardiac resynchronization achieved through atrial-synchronized biventricular pacing produces clinical benefits in patients with heart failure who have an intraventricular conduction delay. We conducted a double-blind trial to evaluate this therapeutic approach.
METHODS: Four hundred fifty-three patients with moderate-to-severe symptoms of heart failure associated with an ejection fraction of 35 percent or less and a QRS interval of 130 msec or more were randomly assigned to a cardiac-resynchronization group (228 patients) or to a control group

BACKGROUND: Contributing factors to cardiac tamponade after contemporary thermal ablation remain poorly defined, especially in relation to ablation strategies. We aimed to clarify the incidence and identify the risk factors associated with cardiac tamponade following thermal atrial fibrillation (AF) ablation.
METHODS: Data from consecutive AF ablation procedures at 20 cardiovascular centers between 2022 and 2024 were analyzed.
RESULTS: Among 12 820 AF ablation procedures (mean age 68 ± 11 years; 8877 men; 6,963 paroxysmal AF; 9824 index procedures), radiofrequency and cryoballoon ablation we

BACKGROUND: Left atrial appendage occlusion (LAAO) using the Watchman device is an established strategy for stroke prevention in atrial fibrillation (AF) patients at high thromboembolic risk who cannot tolerate long-term oral anticoagulation (OAC).
OBJECTIVES: To evaluate short-term clinical and echocardiographic outcomes of Watchman implantation in AF patients with contraindications to long-term OAC.
METHODS: We conducted a retrospective single-center study of AF patients who underwent Watchman implantation between September 01, 2023, and March 01, 2025. Baseline demographics, comorbidities

BACKGROUND: The current numerical standards for procedural competency in Emergency Medicine (EM) lack strong evidentiary support. In the absence of more validated competency standards, self-reported confidence has been used as a learner-centered measure of perceived procedural readiness. Characterizing procedural confidence, patterns across procedures, and factors associated with confidence may help inform improvements in residency curricula and assessment.
OBJECTIVE: To characterize self-reported confidence in key emergency medicine procedures, identify patterns across procedures, and examin

BACKGROUND: Performing atrial fibrillation (AF) catheter ablation and percutaneous left atrial appendage occlusion (LAAO) during a single procedure may improve efficiency and reduce cumulative procedural risk. However, concerns remain regarding potential increases in periprocedural and longer-term complications, including stroke, pericardial effusion, device leak, and device-related thrombus. Evidence evaluating the safety of this concomitant strategy remains limited.
OBJECTIVES: The objectives of the study was to compare the safety and short- and long-term outcomes of concomitant AF ablation

Primary malignant pericardial mesothelioma is an exceptionally rare and aggressive malignancy that frequently presents with nonspecific cardiopulmonary symptoms, recurrent pericardial effusion, constrictive physiology, or cardiac tamponade. Early diagnosis is challenging because an infiltrative pericardial tumor may remain occult on initial imaging studies performed for acute cardiopulmonary evaluation. A 67-year-old woman presented with chest discomfort and shortness of breath. CT pulmonary angiography and thoracic aortic computed tomography angiography (CTA) were performed to evaluate for pu
