Outcomes of double-incision posterior knee release for severe knee flexion contracture.

Many surgical solutions for knee flexiondeformity in the pediatric population alter the anatomical bony alignment in the distal femur. Posterior knee capsule release has been presented as an alternative surgical procedurethat maintains the anatomical shape of relevant bones while solving the issue of knee flexion contracture. The aim of this study is to assess the results of a double-incision posteriorknee capsulotomy release performed on pediatric patients with neuromuscular or congenital severe knee flexion deformity. Thirty cases (24 patients, mean age 7.4 years) of severe knee flexion cont

Patients with Ehlers Danlos syndrome and CRPS: a possible association?

Rare patients are left with chronic pain, vasodysregulation, and other symptoms that define complex regional pain syndrome (CRPS), after limb traumas. The predisposing factors are unknown. Genetic factors undoubtedly contribute, but have not yet been identified. We report four CRPS patients also diagnosed with the classical or hypermobility forms of Ehlers Danlos syndrome (EDS), inherited disorders of connective tissue. These patients had been diagnosed using standard diagnostic criteria for CRPS and for EDS. All had sustained joint injury; in three this had been surgically treated. The associ

Knee size chart nomogram for evaluation of tibial tuberosity-trochlear groove distance in knees with or without history of patellofemoral instability.

PURPOSE: Lateralized tibial tubercle is a cause of patellar instability. Before proceeding with reduction of the tibial tubercle-trochlear groove (TT-TG) distance, surgeons prefer to know whether this distance is pathologic. However, the pathological value remains discussed and may vary with the size of the knee. METHODS: We sought to determine variability in the traditional TT-TG distance versus the anthropometric knee size, using dimensions of the distal part of the femur and proximal part of the tibia of 85 CT scans of the knees in two groups of knees, one normal group without history of p

[Ligamentous knee injuries in children and adolescents].

Due to an increase in sporting activities, the number of injuries of the immature knee is continuously increasing. These injuries necessitate a special approach regarding the particular anatomical situation with open growth plates. Three of the most commonly occurring injuries are rupture of the anterior cruciate ligament, patella dislocation and meniscus injuries. The clinical results for conservative treatment of ruptures of the anterior cruciate ligament in the growth phase are inferior to operative treatment. Transepiphyseal reconstruction has been shown to be a safe treatment method and p

Combined trochleoplasty and MPFL reconstruction for treatment of chronic patellofemoral instability: a prospective minimum 2-year follow-up study.

PURPOSE: Excessive trochlear dysplasia may be responsible for recurrent patellofemoral instability (PFI) due to a missing bony guidance for the patella in early flexion. Thus, an isolated medial patellofemoral ligament reconstruction (MPFLR) can be insufficient, since it mainly addresses instability close to extension and additionally can increase patellofemoral pressure leading to pain in flat or convex trochlear dysplasia. Therefore, in combination with a trochleoplasty, an anatomical trochlear groove is created, resulting in patellofemoral stability also in flexion, while patellofemoral pre

Patellar tracking after isolated medial patellofemoral ligament reconstruction: dynamic evaluation using computed tomography.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction offers good clinical results with a very low rate of instability recurrence. However, its in vivo effect on patellar tracking is not clearly known. The aim of this study is to investigate the effects of MPFL reconstruction on patellar tracking using dynamic 320-detector-row CT. METHODS: Ten patients with patellofemoral instability referred to isolated MPFL reconstruction surgery were selected and subjected to dynamic CT before and ≥6 months after surgery. Patellar tilt angles and shift distance were analysed using computer software

A prospective evaluation of trochleoplasty for the treatment of patellofemoral dislocation and instability.

Trochlear dysplasia is a developmental condition characterised by an abnormally flat or dome-shaped trochlea and is an important contributory factor to patellofemoral instability and recurrent dislocation. We studied prospectively a series of 54 consecutive patients (59 knees) with patellofemoral instability secondary to trochlear dysplasia, who were treated by a trochleoplasty by a single surgeon between June 2002 and June 2007. Pre- and post-operative scores were assessed by the patients and a satisfaction questionnaire was completed. Of the 54 patients (59 knees) in the series, 39 (44 knees

The position of the tibia tubercle in 0°-90° flexion: comparing patients with patella dislocation to healthy volunteers.

PURPOSE: The aim of this study was to measure the tibia tubercle trochlea groove distance (TT-TG) as a function of knee flexion. Our hypothesis was that there is a different pattern in healthy volunteers and patients with patella instability (PFI). METHODS: Thirty-six knees of 30 patients with at least one dislocation of the patella and 30 knees of 30 healthy volunteers as control group were analysed with magnetic resonance imaging by three different observers. The TT-TG was measured in steps of 15° between 0° and 90° of knee flexion. Furthermore, the alignment of the leg (MA), the femur tors

Is there an association between chronicity of patellar instability and patellofemoral cartilage lesions? An arthroscopic assessment of chondral injury.

The purpose of this study was to investigate the association between chronicity of patellar instability on the prevalence, grade, and location of chondral lesions in patients with recurrent patellar instability. Patellofemoral chondral status was documented and graded according to the Outerbridge classification in 38 patients who underwent arthroscopic examination at the time of a medial patellofemoral ligament reconstruction procedure. Chondral lesions of any location were observed in 63.2% of patients. Patellar and trochlear lesions were observed in 57.9 and 13.2% of patients, respectively.

The double-pulley technique for anatomical double-bundled medial patellofemoral ligament reconstruction.

INTRODUCTION: Majority of the current medial patellofemoral ligament (MPFL) reconstruction techniques vary with respect to methods of fixation on the patella. In an effort to maximise the area of patellar footprint coverage and minimise the complication of patellar fracture, we developed a procedure that implemented the double-pulley suture anchors technique. DESIGN: Prospective case series. SETTING: A primary medical centre in Kaohsiung, Taiwan. PATIENTS AND METHODS: Eighteen consecutive patients (18 knees) who met the inclusion criteria underwent the aforementioned operation between Janua

Fulkerson procedure for chronic patella component dislocation after total knee arthroplasty.

Chronic patella component dislocation after primary knee arthroplasty can be a challenging problem. The purpose of this case series was to review our results of a chronic patella dislocation after total knee arthroplasty treated with a Fulkerson procedure (anteromedialization of the tibial tubercle). Five patients with an average age of 68.8 years (range, 60-76 years) underwent a Fulkerson procedure to restore extensor mechanism alignment. The tibial tubercle osteotomy was secured with lag screws with an average medialization of approximately 1.5 cm. The average preoperative Knee Society score

Reduced quadriceps deficit following dry arthroscopy compared to standard fluid arthroscopy at the time of MPFL reconstruction.

PURPOSE: Recurrent patellar dislocations (RPDs) are prevalent, particularly among adolescents. The 'Menu à la carte' technique facilitates personalized management of each patient through reconstruction of the medial patellofemoral ligament (MPFL) while simultaneously addressing other anatomical factors that may contribute to instability. Arthroscopy, frequently performed prior to stabilization surgery to identify and treat intra-articular pathology, carries specific risks. Fluid arthroscopy can increase post-operative effusion, while dry arthroscopy may expose cartilage to potential damage due

Knee Chondral Shear Injury Repair: A Biomechanical Laboratory Comparison of Suture Bridge to Chondral Pin Fixation.

BACKGROUND: Delamination of chondral fragments in pediatric patients is common and is usually caused by osteochondritis dissecans (OCD) or patellar dislocations. For fragments with minimal to no bone, fixation with screws may not be ideal due to hardware prominence on the cartilage or suboptimal screw purchase, which can result in hardware migration and loss of fixation. Lower-profile chondral fixation devices may be ideal in these circumstances, such as the suture bridge fixation construct and bioabsorbable chondral pin fixation. HYPOTHESIS: Suture bridge constructs would provide superior fi

High wear resistance of femoral components coated with titanium nitride: a retrieval analysis.

PURPOSE: The objective of this study was to evaluate the in vivo wear resistance of cobalt-chromium femoral components coated with titanium nitride (TiN). Our null hypothesis was that the surface damage and the thickness of the TiN coating do not correlate with the time in vivo. METHODS: Twenty-five TiN-coated bicondylar femoral retrievals with a mean implantation period of 30.7 ± 11.7 months were subjected to an objective surface damage analysis with a semi-quantitative assessment method. A visual examination of scratches, indentations, notches and coating breakthroughs of the surfaces was p

Higher sensitivity with the lever sign test for diagnosis of anterior cruciate ligament rupture in the emergency department.

INTRODUCTION: The objective of this study was to assess the diagnostic value of the "lever sign test" to diagnose ACL rupture and to compare this test to the two most commonly used, the Lachman and anterior drawer test. METHOD: This prospective study was performed in the ED of the Cliniques Universitaires Saint-Luc (Brussels, Belgium) from March 2017 to May 2019. 52 patients were included undergoing knee trauma, within 8 days, with an initial radiograph excluding a fracture (except Segond fracture or tibial spine fracture). On clinical investigation, patients showed a positive lever sign test
