Pericardial Effusion and Compressive Disorders of the Heart: Influence of New Technology on Unraveling its Pathophysiology and Hemodynamics.

Constrictive pericarditis and cardiac tamponade cause severe diastolic dysfunction, but do not depress systolic function until the agonal state has been reached. Multimodality cardiovascular imaging has brought the nuances of pericardial disease to the domain of the practicing cardiologist. This introduction is a revised article originally written by the late Dr Shabetai for a pericardial diseases textbook which was not published. He was the editor of previous Pericardial Diseases issue for Cardiology Clinics in the 1980s, it is most appropriate to begin our issue with his insights. The remain

A risk score for pericarditis recurrence.

BACKGROUND: Currently, we remain uncertain about which patients are at increased risk for recurrent pericarditis. We developed a risk score for pericarditis recurrence in patients with acute pericarditis. MATERIALS AND METHODS: We prospectively recruited 262 patients with a first episode of acute pericarditis. Baseline patients' demographics, clinical, imaging and laboratory data were collected. Patients were followed up for a median of 51 months (interquartile range 21-71) for recurrence. Variables with <10% missingness were entered into multivariable logistic regression models with stepwise

Tuberculous and Infectious Pericarditis.

Viral pericarditis is the most common cause of acute pericarditis and it is typically responsive to aspirin or nonsteroidal anti-inflammatory drugs. Tuberculous pericarditis is common in immunocompromised patients or in immunocompetent patients in endemic areas. The diagnosis of tuberculous pericarditis usually requires a multidisciplinary approach, and presumptive treatment should be started for people with suspected infections living in endemic areas. Antituberculous treatment along with corticosteroid therapy can reduce complications from constrictive pericarditis. Purulent pericarditis is

Infantile major airway stenosis and acute respiratory distress associated with cardiac tamponade.

Coxsackie virus pericarditis caused cardiac tamponade in a 45-day-old infant with corrected total anomalous pulmonary venous drainage and a hypodynamic left heart. The pathophysiology comprised reduced heart compliance, venous return impairment, acute pulmonary hypertension, and increased airway microvascular permeability. Tracheal edema and external compression caused tracheal lumen narrowing and respiratory failure. Laryngoscopy was difficult because of laryngeal inlet swelling. Endotracheal intubation was accomplished with a 3.0-mm tube. Pericardial cavity evacuation resulted in rapid recov

I Can See Your HALO: Consensus-based List of High Acuity, Low Occurrence Procedures for Critical Care Medicine Fellowship Training.

BACKGROUND: The Accreditation Council of Graduate Medical Education (ACGME) requires proficiency in procedures such as endotracheal intubation, central line insertion, and bronchoscopy for certification in critical care medicine (CCM). These are common in the intensive care unit and gaining sufficient exposure to attain competence during fellowship training is generally not challenging. However, intensivists are also expected to perform several high-risk procedures that are less frequent. Such high-acuity, low-occurrence (HALO) procedures lack a clear designation, possibly due to a lack of con

When Pneumonia Is Not Pneumonia: T-cell Acute Lymphoblastic Leukemia Presenting With Mediastinal Mass, Airway Compression, and Pericardial Effusion in a 4-Year-Old.

Pediatric leukemia may initially present in a way that mimics common respiratory infections, particularly when fever, cough, chest pain, and radiographic findings are observed. This overlap may obscure the diagnosis of malignancy, especially when the initial imaging is interpreted as community-acquired pneumonia. T-cell acute lymphoblastic leukemia (T-ALL) is commonly linked with anterior mediastinal masses that may result in airway compression, atelectasis, pleural or pericardial effusions, and cardiopulmonary instability. We describe a previously healthy four-year-old boy who presented with

Outcomes of Left Atrial Appendage Occlusion Treatment With Amulet After Unsuccessful Watchman FLX Device: A Multicenter Observational Study.

INTRODUCTION: Watchman FLX (W-FLX) is the most widely used left atrial appendage occlusion (LAAO) device to prevent atrial fibrillation (AF) related strokes in the United States. However, successful LAAO with W-FLX can be challenging in patients with complex left atrial appendage (LAA) anatomies. This analysis aimed to assess the procedural success rates of LAA occlusion (immediate and short-term outcomes) using the Amulet device, in patients with prior failed LAAO with W-FLX device. METHODS AND RESULTS: This was a multicentric retrospective analysis of 62 consecutive patients with unsuccessf

Safety and effectiveness of pulsed field ablation for pulmonary vein isolation in atrial fibrillation patients: One-year single center experience.

INTRODUCTION AND OBJECTIVES: Pulmonary vein (PV) isolation is one of the cornerstones of rhythm-control therapy for symptomatic atrial fibrillation (AF) patients. Pulsed field ablation (PFA) is a novel ablation modality that involves the application of electrical pulses causing cellular death, and it has preferential tissue specificity. In this study, we aimed to share a one-year single center experience of AF ablation with PFA. METHODS: Single center, retrospective study of consecutive patients undergoing PVI using the pentaspline PFA catheter between June 2022 and July 2023. Data on demogra

Anakinra-Dependent Recurrent Pericarditis: The Role of the R202Q Variant of the MEFV Gene.

Background: the role of the R202Q (c.605G>A, p.Arg202Gln) missense variant of the MEFV gene has been debated as either a benign polymorphism or a potentially pathogenic mutation. We report and discuss here the case of a young female with corticosteroid-dependent recurrent pericarditis carrying the homozygous R202Q variant, exhibiting distinctive clinical features possibly influenced by this genetic variant. Methods: a 30-year-old woman with a previous diagnosis of cancer and recent respiratory infection presented with severe pleuritic chest pain, hypotension, tachycardia, and fever. Initial di

Intra-pericardial thrombin injection in iatrogenic cardiac tamponade: a case report.

BACKGROUND: Nowadays, percutaneous procedures are expanding in use, and this comes with complications associated with the procedure itself. Cardiac tamponade is rare but may be life threatening since it can involve hemodynamic instability. It is known that after pleural effusion during a percutaneous procedure, pericardiocentesis should be used as drainage of the cavity. However, that does not achieve hemostasis in some cases, and in those patients who are hemodynamically unstable, a sealing agent to promote hemostasis might be useful, like thrombin. CASE PRESENTATION: We present a case repor

Primary Hypothyroidism Unleashing Severe Pericardial Effusion: Lessons Learnt From an Atypical Presentation of a Common Endocrine Condition.

Primary hypothyroidism is a commonly encountered endocrine disorder and can be associated with pericardial effusion and cardiac tamponade in severe cases. Early detection of hypothyroidism is key since it is a potentially treatable and reversible cause of pericardial effusions. A 53-year-old female was admitted following a fall. The clinical history was remarkable, with symptoms of persistent tiredness and fatigue for six months. She had no known medical conditions and was not taking any regular medications. Vital signs were stable. Physical examination revealed bilateral pitting pedal oedema

Procedural outcomes of chronic total occlusion percutaneous coronary interventions in patients with acute kidney injury.

BACKGROUND: The prognostic impact of contrast-associated acute kidney injury (CA-AKI) in patients undergoing chronic total occlusion (CTO) percutaneous coronary intervention (PCI) remains underestimated. METHODS: We examined 2707 consecutive procedures performed in a referral CTO center between 2015 and 2019. CA-AKI was defined as an increase in serum creatinine ≥ 0.3 mg/dL or ≥ 50% within 48 h post-PCI. Primary endpoints were in-hospital major adverse cardiac and cerebrovascular events (MACCE, composite of all-cause death, myocardial infarction, target vessel revascularization, stroke) and a
