Surgical treatment of multiple knee ligament injuries in 44 patients: 2-8 years follow-up results.

The purpose of the study was to evaluate the mid-term results of surgical treatment in different groups of patients with multiple knee ligament injuries. Review of our patients' records revealed that 48 acute and chronic patients were surgically treated for combined knee injury. Due to severe capsular damage in these injuries, open techniques were used. In our treatment protocol, avulsed ligaments and tears of the posterolateral and posteromedial corner were repaired if possible, whereas midsubstance tears of cruciate ligaments and chronic cases were reconstructed with autografts. Postoperativ

Anatomic Multiligament Knee Reconstruction With a Medial Meniscal Repair in the Setting of a Valgus Hyperextension Injury.

BACKGROUND: Simultaneous anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), and medial collateral ligament (MCL) tears represent a severe form of multiligament knee injury (MLKI). This combination of injuries often results from high-energy trauma and leads to gross instability, impaired joint biomechanics, and long-term degenerative changes if not addressed appropriately. Untreated or improperly treated MLKIs may result in chronic pain, instability, and early onset osteoarthritis. INDICATIONS: Combined ACL, PCL, and MCL reconstruction is indicated in patients with complete t

Hinged total knee arthroplasty for fracture cases: Retrospective study of 52 patients with a mean follow-up of 5 years.

INTRODUCTION: Hinged total knee arthroplasty (hTKA) is one of the treatment options available for complex fractures around an intact knee or an existing implant. The primary objective of this multicenter study was to evaluate the medium-term outcomes of hTKA for fractures. The secondary objective was to analyze the complications and risk factors based on fracture type, predisposition and time to surgery. We hypothesized that outcomes would be satisfactory, despite the context, and comparable to published data although impacted by significant mortality and morbidity. MATERIALS AND METHODS: Wit

Injuries and conditions of the extensor mechanism of the pediatric knee.

The extensor mechanism of the knee is essential to ambulation and is subject to a number of traumatic, congenital, and inflammatory processes. In the pediatric population, the spectrum of pathologic conditions affecting the extensor mechanism is specific to skeletally immature patients. In addition, certain congenital and developmental disorders may further predispose the knee extensor mechanism to injury. The pathologic processes can be subdivided into categories: conditions of the attachments and insertions of the quadriceps and patellar tendons, conditions of the patella, conditions of the

Predictors of Graft Failure After Primary Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: The tibiofemoral rotation angle has been found to be higher in patients with recurrent patellar dislocations (RPDs) than in healthy people; however, little is known about the clinical significance of this finding. PURPOSE: To determine whether an increased tibiofemoral rotation angle is associated with graft failure after primary medial patellofemoral ligament reconstruction (MPFL-R) and to investigate the role of the tibiofemoral rotation angle in predicting MPFL-R failure in patients with RPDs. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: We retrospectively

Staging of Charcot neuroarthropathy along the medial column of the foot in the diabetic patient.

Diabetes mellitus is the leading cause of Charcot neuroarthropathy. The most common location is along the medial column of the foot. Over a 2-year period, the process can result in a severely deformed foot, which is highly prone to ulcers, infection, and subsequent amputation. To help identify the early stages of the disease process, the histories, physical examinations, and radiographs of 40 patients with 51 neuropathic feet were evaluated. We were able to identify five stages of Charcot deformities. Stage 0 is a clinical stage in which the patient presents with a locally swollen, warm, and o

Anatomic Multiligament Knee Reconstruction With Biceps Femoris Tendon and Medial Meniscal Repair.

BACKGROUND: Multiligament knee injuries are defined as tears of ≥2 of the following ligament structures: anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), posteromedial corner, or posterolateral corner (PLC). These injuries occur in various settings and may be associated with knee dislocations. INDICATIONS: The literature has reported that anatomic technique and a single-;stage approach for treating multiligament knee injuries are optimal because they may lead to improved outcomes. An anatomic double-;bundle PCL reconstruction is suggested to improve knee kinematics compare

Breaking down tibial tuberosity to trochlear groove distance into two components to enable patient-specific treatment strategies.

OBJECTIVE: Tibial tuberosity to trochlear groove (TT-TG) distance serves as one of the main metrics for patellofemoral instability (PFI) surgical decision-making. The purpose of this study is to split TT-TG into translational (caused by bony morphology) and rotational (external tibiofemoral rotation) components, elucidate how those two components relate to each other, and determine how the components differ between recurrent PFI patients and controls. METHODS: Computed tomography (CT) scans of PFI patients with at least two reported dislocation events, seen by our institution's orthopedic dep

[MR imaging of the knee].

Since its introduction into clinical use, magnetic resonance imaging (MRI) has become well established in the diagnosis of injuries of the knee joint and has replaced diagnostic arthroscopy as the primary evaluation method. Traumatic lesions of the ligaments, menisci, and articular surfaces are common injuries that can be confidently detected using MRI. This article reviews the basic principles of the examination technique of the knee, the normal MRI anatomy of the internal joint structures, and the typical findings in injuries of the cruciate ligaments, collateral ligaments, and mensci, in tr

Surgical reconstruction of severe patellofemoral maltracking.

Dysfunction of the patellofemoral mechanism presents in many ways. Results from different realignment procedures show great variability in patient outcome. A surgical technique is presented that attempts to correct all the abnormalities of patellofemoral maltracking. The procedure consists of a lateral release, a vastus medialis (obliquus) tendon advancement, and a tibial tubercle transfer. Along with being moved medially, the tubercle also is moved distally to correct patella alta and elevated anteriorly to reduce patellofemoral joint reaction forces. One hundred seven knees in 84 patients we
