Structural and molecular determinants of medial temporal lobe network vulnerability in aging and Alzheimer's disease.

BACKGROUND: The medial temporal lobe is organized into two memory-critical networks: the anterior-temporal and posterior-medial systems. While these systems show selective vulnerability to aging and Alzheimer's disease, the underlying structural and molecular determinants of this susceptibility remain unclear. We aimed to characterize the specific white matter pathways supporting these networks, investigate how amyloid-β and neuroinflammation interact to impact their integrity, and determine if structural changes relate to functional connectivity alterations. METHODS: In 88 cognitively unimpa

Cytotoxic Lesions of the Corpus Callosum: A Single-Center Case Series of Eight Patients With an Emphasis on Infectious Etiologies.

Background and aim Cytotoxic lesions of the corpus callosum (CLOCCs) are transient lesions characterized by restricted diffusion on MRI and are associated with various infectious and noninfectious conditions. However, the clinical spectrum of infection-related CLOCCs, particularly those associated with bacterial infections, remains incompletely defined. This study aimed to describe the clinical, etiological, and neuroimaging characteristics of CLOCCs diagnosed at a single center, encompassing both infectious and noninfectious etiologies, with particular attention to infection-related cases. The clinical and genetic spectrum of inherited glycosylphosphatidylinositol deficiency disorders.

Inherited glycosylphosphatidylinositol deficiency disorders (IGDs) are a group of rare multisystem disorders arising from pathogenic variants in glycosylphosphatidylinositol anchor pathway (GPI-AP) genes. Despite associating 24 of at least 31 GPI-AP genes with human neurogenetic disease, prior reports are limited to single genes without consideration of the GPI-AP as a whole and with limited natural history data. In this multinational retrospective observational study, we systematically analyse the molecular spectrum, phenotypic characteristics and natural history of 83 individuals from 75 uni

FRONTAL WHITE MATTER CHANGES INDICATE RECOVERY WITH INPATIENT TREATMENT IN HEROIN ADDICTION.

IMPORTANCE: Amidst an unprecedented opioid epidemic, identifying neurobiological correlates of change with medication-assisted treatment of heroin use disorder is imperative. Distributed white matter (WM) impairments in individuals with heroin use disorder (iHUD) have been associated with increased drug craving, a reliable predictor of treatment outcomes. However, little is known about the extent of whole-brain structural connectivity changes with inpatient treatment and abstinence in iHUD. OBJECTIVE: To assess WM microstructure and associations with drug craving changes with inpatient treatm

The Impact of Multimorbidity Burden, Frailty Risk Scoring, and 3-Directional Morphological Indices vs. Testing for CSF Responsiveness in Normal Pressure Hydrocephalus.

Objective: Multimorbidity burden across disease cohorts and variations in clinico-radiographic presentations within normal pressure hydrocephalus (NPH) confound its diagnosis, and the assessment of its amenability to interventions. We hypothesized that novel imaging techniques such as 3-directional linear morphological indices could help in distinguishing between hydrocephalus vs. non-hydrocephalus and correlate with responsiveness to external lumbar drainage (CSF responsiveness) within NPH subtypes. Methodology: Twenty-one participants with NPH were recruited and age-matched to 21 patients wi

Unilateral neglect: a theory of proprioceptive space of a stimulus as determined by the cerebellar component of motor efference copy (and is autism a special case of neglect).

Unilateral neglect is a devastating condition, which manifests as a loss of a person's spatial awareness opposite the damaged side of the brain. It challenges our conception of the seat of the soul and its explanation is at the heart of the mind-body problem. A heuristic definition of the dorsal stream of a modality is here based on the categorization of parietal networks by the cerebellar component of motor efference copy. Taking this premise, the proprioceptive space of a stimulus is established as a concept in this paper. It is proposed that unilateral neglect is typically a dysfunction of

Human cingulate cortex: surface features, flat maps, and cytoarchitecture.

The surface morphology and cytoarchitecture of human cingulate cortex was evaluated in the brains of 27 neurologically intact individuals. Variations in surface features included a single cingulate sulcus (CS) with or without segmentation or double parallel sulci with or without segmentation. The single CS was deeper (9.7 +/- 0.81 mm) than in cases with double parallel sulci (7.5 +/- 0.48 mm). There were dimples parallel to the CS in anterior cingulate cortex (ACC) and anastomoses between the CS and the superior CS. Flat maps of the medial cortical surface were made in a two-stage reconstructi

Functional reorganization of the visual cortex in blindness: a connectivity perspective.

Visual deprivation provides a critical model for understanding how sensory experience and developmental constraints shape cortical organization. In blindness, the occipital cortex is recruited for tactile and auditory perception as well as language, memory, mathematical processing, and executive control. The core question is how visual cortex becomes functionally embedded in non-visual and cognitive systems, and how this embedding gives rise to particular forms of occipital recruitment. Connectivity is central to this question because cortical functions are shaped, in part, by the information

EIPR1 variants cause a neurodevelopmental disorder with endolysosomal and dense core vesicle defects.

EIPR1 (EARP-interacting protein 1, formerly known as tumor suppressing subtransferable candidate 1 or TSSC1) is a WD40-domain protein that interacts with the EARP (endosome-associated recycling protein) and GARP (Golgi-associated retrograde protein) complexes in the process of delivering endosome-derived transmembrane cargos to the plasma membrane and the trans-Golgi network (TGN), respectively. Additionally, EIPR1 cooperates with EARP in the biogenesis of dense core vesicles. While these properties of EIPR1 were established in cultured cells and model organisms, the physiological and patholog

Peripheral immunity patterns, imaging features, and clinical outcomes in patients with gait impairment and ventriculomegaly on brain MRI.

INTRODUCTION: Ventricular enlargement is a common finding on brain MRI in patients with gait impairment, particularly in those with idiopathic normal pressure hydrocephalus (iNPH). However, iNPH shares several clinical and radiological features with neurodegenerative diseases, which complicates accurate diagnosis. This study aimed to explore the associations between peripheral immune markers, imaging biomarkers, and final diagnosis in patients with gait disturbance and ventriculomegaly. METHODS: We retrospectively analyzed 55 patients with gait impairment and ventriculomegaly, and 40 age-comp

Microstructural Degeneration of the Corpus Callosum in Parkinson's Disease with Unilateral Onset: A Free-Water Imaging Study.

Background: Motor symptom laterality is an important clinical feature of PD that not only manifests as lateral limb dysfunction but also affects the nonmotor symptoms and the prognosis in PD patients. Previous studies suggested that the compensatory mechanisms in the dominant hemisphere of the brain may be an underlying explanation. The corpus callosum (CC) is the largest fiber connecting the two hemispheres of the brain. Considering the CC as the pointcut may help elucidate the mechanisms underlying how motor symptom laterality affects nonmotor symptoms and prognosis in PD patients. Purpose:

Utility of MRI-based disproportionately enlarged subarachnoid space hydrocephalus scoring for predicting prognosis after surgery for idiopathic normal pressure hydrocephalus: clinical research.

OBJECTIVE The presence of disproportionately enlarged subarachnoid space hydrocephalus (DESH) on brain imaging is a recognized finding of idiopathic normal pressure hydrocephalus (iNPH), but the features of DESH can vary across patients. The aim of this study was to evaluate the utility of MRI-based DESH scoring for predicting prognosis after surgery. METHODS In this single-center, retrospective cohort study, the DESH score was determined by consensus between a group of neurosurgeons, neurologists, and a neuroradiologist based on the preoperative MRI findings of the patients with suspected iNP

Susac Syndrome: A differential diagnosis of white matter lesions.

INTRODUCTION: Susac Syndrome is an autoimmune endotheliopathy affecting capillaries and precapillary arterioles of the brain, inner ear and retina. The classic symptom triad includes visual disturbances, hypoacusia, and encephalopathy, but is rarely fully manifest at onset. The syndrome typically follows an active fluctuating monophasic course. Typical imaging findings on brain magnetic resonance show central fiber microischemias/microinfarctions of the corpus callosum known as "snow balls", as well as lineal infarcts and upper callosal fiber involvement. Fluorescein angiography and tonal audi

Widespread grey matter pathology dominates the longitudinal cerebral MRI and clinical landscape of amyotrophic lateral sclerosis.

Diagnosis, stratification and monitoring of disease progression in amyotrophic lateral sclerosis currently rely on clinical history and examination. The phenotypic heterogeneity of amyotrophic lateral sclerosis, including extramotor cognitive impairments is now well recognized. Candidate biomarkers have shown variable sensitivity and specificity, and studies have been mainly undertaken only cross-sectionally. Sixty patients with sporadic amyotrophic lateral sclerosis (without a family history of amyotrophic lateral sclerosis or dementia) underwent baseline multimodal magnetic resonance imaging

Prenatal neurologic anomalies: sonographic diagnosis and treatment.

The low prevalence of fetal CNS anomalies results in a restricted level of exposure to, and limited experience for most obstetricians involved in, prenatal ultrasound. Sonographic guidelines for screening the fetal brain in a systematic way may increase the detection rate of fetal CNS anomalies, thus promoting correct referral to tertiary care centers offering patients a multidisciplinary approach to the condition. The aim of this review is to elaborate on the prenatal sonographic diagnosis and outcome of various CNS malformations. Detailed neurosonographic investigation has become available t
