The advantage of point-of-care ultrasound in central venous catheterization and related pericardial effusion in infants in the NICU.

BACKGROUND: Central venous catheterization (CVC) is broadly used in neonatal intensive care units (NICUs) for efficient vascular access; however, its establishment and maintenance are associated with numerous risks and complications. Here, we focus on investigating the value of point-of-care ultrasound (POCUS) in the early diagnosis and treatment of pericardial effusion associated with CVC and compare the differences in ultrasound and radiography in CVC localization and monitoring in the NICU. METHODS: Twenty-five infants with CVC-associated pericardial effusion (PCE) who were hospitalized in

Cardiac tamponade - an unexpected "long COVID-19" complication.

INTRODUCTION: Year 2020 has been a cornerstone in medical research due to the COVID-19 pandemic outbreak. The process of understanding the condition brought to light certain organ involvement like pulmonary or kidney damage or endocrine disbalances, while connection to other types of organ impairment remain unclear. SARS-CoV-2 has previously been incriminated in cardiac involvement, ranging from mild symptoms to more severe occurrences such as myocarditis, arrythmias or heart failure, thus complicating the acute-phase management and worsening patients' prognosis. Despite being reported as an a

Unexplained intraoperative cardiac tamponade during thoracoscopic lobectomy: a case report.

BACKGROUND: Cardiac tamponade is a life-threatening emergency characterized by reduced cardiac output and circulatory failure. It most commonly occurs during cardiac surgery, interventional cardiology procedures, pericardiocentesis, or central venous catheterization, as well as in the setting of acute myocardial infarction or aortic dissection. To the best of our knowledge, cardiac tamponade during video-assisted thoracoscopic surgery (VATS) lobectomy has not been reported and may has an insidious presentation, making diagnosis challenging in the absence of obvious pericardial injury. Cardiac Adaptation in Lupus: A Case of Massive Pericardial Effusion With Preserved Hemodynamics.

Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease; cardiac involvement is a recognized complication, with pericardial effusion being one of the most frequent manifestations. Here, we present a patient with massive pericardial effusion in a known SLE patient without hemodynamic instability, highlighting concepts of pericardial compliance and physiological adaptation in autoimmune disease. We present a case of a 33-year-old female with a known history of SLE who presented with progressively worsening pleuritic chest pain over three weeks. She was hemodynamically stable, with

Ablation of Refractory Ventricular Tachycardia Using Intramyocardial Needle Delivered Heated Saline-Enhanced Radiofrequency Energy: A First-in-Man Feasibility Trial.

BACKGROUND: Ablation of ventricular tachycardia (VT) is limited by the inability to create penetrating lesions to reach intramyocardial origins. Intramural needle ablation using in-catheter, heated saline-enhanced radio frequency (SERF) energy uses convective heating to increase heat transfer and produce deeper, controllable lesions at intramural targets. This first-in-human trial was designed to evaluate the safety and efficacy of SERF needle ablation in patients with refractory VT. METHODS: Thirty-two subjects from 6 centers underwent needle electrode ablation. Each had recurrent drug-refra

Prolonged Drainage and Intrapericardial Bleomycin Administration for Cardiac Tamponade Secondary to Cancer-Related Pericardial Effusion.

Malignant pericardial effusion (MPE) is a serious complication of several cancers. The most commonly involved solid tumors are lung and breast cancer. MPE can give rise to the clinical picture of cardiac tamponade, a life threatening condition that needs immediate drainage. While simple pericardiocentesis allows resolution of the symptoms, MPE frequently relapses unless further procedures are performed. Prolonged drainage, talcage with antineoplastic agents, or surgical creation of a pleuro-pericardial window are the most commonly suggested ones. They all result in MPE resolution and high rate

Single center experience: Implantation failures, early, and late complications after implantation of a partially biodegradable ASD/PFO-device (BioStar®).

INTRODUCTION: In the search for a biodegradable device that leaves nothing but the tissue of the patient after complete endotheliazation and absorption, the BioSTAR® device was introduced in 2007 (CE Mark in European community and HPB in Canada) for ASD and PFO closure. It consists of a metal framework covered by a biodegradable membrane generated from a layer of porcine collagen that is broken down and absorbed over time. In a sheep model, the results were promising, showing complete closure of the defect with degradation of approximately 90% of the implanted membrane material after two years
