Neurostimulants for disorders of consciousness in the intensive care unit: A systematic review.

BACKGROUND: Disorders of consciousness (DoC; encompassing coma, unresponsive wakefulness syndrome, and minimally conscious state) are sequelae of critical illness that prolong hospitalization and interfere with engagement in care. Due to heterogeneous and incompletely understood pathophysiology, no pharmacologic standard of care has been established. We aimed to systematically examine the efficacy, safety, and dosing of neurostimulants among adult ICU patients with DoC. METHODS: We conducted a systematic search of PubMed, Embase, CINAHL, and Web of Science from inception to December 2025. Stu

Severe Iatrogenic Tricuspid Regurgitation.

Tricuspid regurgitation (TR) is a valvular heart disease mostly associated with structural heart disease or pulmonary hypertension (PH). Rare cases of TR secondary to exposure to specific medications, such as dopamine agonists, have been described. A 50-year-old woman presented with a pituitary microprolactinoma (without surgical indication) and a history of pulmonary embolism (PE) in 2021 (risk factors included bromocriptine, which was switched to cabergoline). An echocardiogram performed one year after the PE showed no significant abnormalities. She was maintained on rivaroxaban 10 mg due to

Sleep duration and association with cardiometabolic health in adolescents and adults with type 1 diabetes: Results from the BCQR-T1D study.

AIM: Type 1 diabetes (T1D) increases the risk of morbidity and mortality from cardiovascular disease, and insufficient sleep is prevalent. Emerging evidence suggests a link between sleep and cardiometabolic health, but this has not been examined across the lifespan in individuals with T1D. We aimed to examine associations between sleep and cardiometabolic health in adolescents and adults with T1D in a secondary analysis of data from a 4-week double-blind, random-order, placebo-controlled crossover trial of bromocriptine quick release (BCQR) therapy with a 4-week washout in between conditions.

Opposing roles of Akt and STAT3 in the protection of the maternal heart from peripartum stress.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a pregnancy-associated cardiomyopathy in previously healthy women. Mice with a cardiomyocyte-restricted deletion of signal transducer and activator of transcription-3 (STAT3, CKO) develop PPCM. PI3K-Akt signalling is thought to promote cardiac hypertrophy and protection during pregnancy. We evaluated the role of activated Akt signalling in the maternal heart postpartum. METHODS AND RESULTS: CKO mice were bred to mice harbouring an Akt transgene, specifically expressed in cardiomyocytes (CAkt(tg)) generating CKO; CAkt(tg), CAkt(tg), CKO, and wild

Prolactin and the expression of suppressor of cytokine signaling-3 in the sheep adrenal gland before birth.

The fetal pituitary-adrenal axis plays a key role in the fetal response to intrauterine stress and in the timing of parturition. The fetal sheep adrenal gland is relatively refractory to stimulation in midgestation (90-120 days) before the prepartum activation, which occurs around 135 days gestation (term=147+/-3 days). The mechanisms underlying the switch from adrenal quiescence to activation are unclear. Therefore, we have investigated the expression of suppressor of cytokine signaling-3 (SOCS-3), a putative inhibitor of tissue growth in the fetal sheep adrenal between 50 and 145 days gestat

Wilson's disease with depression and parkinsonism.

Wilson's disease (WD) is an autosomal recessive disorder with reduced biliary excretion of copper plus impaired formation of ceruloplasmin, leading to copper accumulation in the liver, brain, kidney, and cornea. Clinical manifestations include liver damage, psychiatric symptoms, and neurological features. We report a 35-year-old woman with a history of deranged liver functions who had severe depression several years later and eventually presented with parkinsonian features. The underlying diagnosis is WD and family screening revealed WD in 2 other siblings. She could not tolerate penicillamine

Paliperidone-induced neuroleptic malignant syndrome: A case report.

RATIONALE: Neuroleptic malignant syndrome (NMS) is a rare, life-threatening complication of neuroleptic (antipsychotic) medications. Paliperidone is an atypical antipsychotic used in the treatment of schizophrenia. While current evidence suggests that atypical oral antipsychotics have a lower incidence of NMS compared to typical oral antipsychotics, there is limited information available on the incidence and management of NMS associated with long-acting injectable (LAI) antipsychotics. More case studies are needed to fully understand the true incidence of NMS associated with LAI atypical antip

"Off" painful dystonia in Parkinson's disease treated with botulinum toxin.

The "off" painful dystonia (OPD), usually concerning the feet, is a type of abnormal involuntary movement, induced by the chronic use of levodopa. It is mostly observed in the advanced stage of Parkinson's disease (PD), particularly in the early morning, in the evening, and late at night. Indeed, some patients have experienced OPD also during "on" periods when dystonic posture of the foot alternates with dyskinesia. The pain probably is due to sustained muscle contraction, which causes prolonged muscle spasm, as in primary dystonia or torticollis. Dopaminergic drugs like bromocriptine, pergoli

Pharmacological adjuncts in the treatment of opioid and cocaine addicts.

Addiction consists of a complex neuropharmacologic behavioral cycle. The positive reinforcement of the drug and the negative reinforcement of withdrawal serve to drive the behavior of obtaining and ingesting the drug. The pharmacological adjuncts that are available today work by interfering with one or another part of the cycle. The alpha 2-adrenergic agonists, such as clonidine and guanabenz, act to block noradrenergic activity in the locus coeruleus and therefore block the negative reinforcement of opioid withdrawal. Naltrexone, on the other hand, works by preventing the positive reinforceme

D-2 dopaminergic agonists and adenosine 3',5'-monophosphate directly regulate the synthesis of alpha-melanocyte-stimulating hormone-like peptides by cultured rat melanotrophs.

When placed in short term (2-day) tissue culture, the melanotrophs from the intermediate lobe of the rat pituitary gland synthesize a proopiomelanocortin-like material (POMC-LM). Exposure of these cells to bromocriptine (CB 154), an agonist upon their D-2 dopamine receptor, reduces the synthesis of POMC-LM; spiroperidol, an antagonist of the D-2 receptor, prevents this effect of CB 154. Cultured melanotrophs secrete an alpha MSH-like material. The amount of this alpha MSH-like material, either stored intracellularly or secreted into the culture medium, can be quantified in a specific RIA; the

Peripartum Cardiomyopathy: Literature Review.

Peripartum cardiomyopathy (PPCM) is a rare cause of heart failure that affects women during the last month of pregnancy to the first five months after delivery. The disease occurs in about one in 1,000 births in the U.S. Risk factors include advanced age, multiparity, twin pregnancy, African origin, preeclampsia or preexisting hypertension, and severe anemia. Heart failure in PPCM is treated similarly to heart failure from other causes, bearing in mind the pregnancy and lactation status. In this review article, we discuss the background, etiology, clinical evaluation, treatment, and natural hi
