Infected hepatic cyst treated with percutaneous transhepatic drainage.

Simple hepatic cysts are common benign lesions that are usually asymptomatic and require no treatment. However, complications can occur. This report describes a patient with an infected hepatic cyst treated with percutaneous transhepatic drainage. A 64-year-old woman presented at a nearby hospital because of acute right-upper-quadrant pain, mild left-lower-quadrant pain, diarrhea, and fever. She was admitted and received intravenous antibiotics for 1 week, but symptoms persisted. She was, therefore, referred to our hospital. On admission, ultrasonography demonstrated multiple hepatic cysts. On

Optical Microangiography: High-Definition Magnification Colonoscopy with Narrow Band Imaging (NBI) for Visualizing Mucosal Capillaries and Red Blood Cells in the Large Intestine.

BACKGROUND/AIMS: Recent advances in zoom endoscopy have enabled the subepithelial capillary network (SECN) in different organs of the gastrointestinal tract to be visualized. Ex vivo studies have suggested that the SECN demonstrates a honeycomb-like structure in the large intestine, but this has not yet been visualized in vivo. The high clarity and resolution of narrow-band imaging (NBI) may allow visualization at the single red-blood-cell (RBC) level and more accurate visualization of the SECN. We investigated whether high-definition magnification colonoscopy with NBI is useful for visualizin

Sacral nerve stimulation induces pan-colonic propagating pressure waves and increases defecation frequency in patients with slow-transit constipation.

OBJECTIVE: Colonic propagating sequences are important for normal colonic transit and defecation. The frequency of these motor patterns is reduced in slow-transit constipation. Sacral nerve stimulation (SNS) is a useful treatment for fecal and urinary incontinence. A high proportion of these patients have also reported altered bowel function. The effects of SNS on colonic propagating sequences in constipation are unknown. Our aims were to evaluate the effect of SNS on colonic pressure patterns and evaluate its therapeutic potential in severe constipation. METHOD: In eight patients with scinti

Diagnostic yield of advanced colorectal neoplasia at colonoscopy, according to indications: an investigation from the Korean Association for the Study of Intestinal Diseases (KASID).

BACKGROUND AND STUDY AIMS: The factors that more accurately predict the detection of colorectal cancers and adenomas at colonoscopy are different. We conducted a prospective multicenter study to evaluate which indications were most closely associated with advanced colorectal neoplasm (CRN), including colorectal cancer, in a group of patients undergoing colonoscopy. PATIENTS AND METHODS: The 17 468 patients were enrolled in this study between July 2003 and March 2004, from 11 tertiary medical centers in Korea. They were recruited according to 11 itemized colonoscopic indications. The term "adv

Nonradiologists as second readers for intraluminal findings at CT colonography.

RATIONALE AND OBJECTIVES: Multiple trials have documented wide interobserver variability between radiologists interpreting computed tomography colonography (CTC) exams. We sought to determine if nonradiologists could learn to interpret intraluminal findings at CTC with a high degree of sensitivity to determine if they could play a role as second readers in interpreting CTC exams. MATERIALS AND METHODS: Seven nonradiologists (five medical students, two radiologic technologists) undertook self-directed CTC training using a teaching file of 50 cases; thereafter, each reader blindly interpreted 5

Endoscopic resection of large colorectal polyps.

BACKGROUND: Endoscopic polypectomy is a common technique, but there are discrepancies over which treatment--surgical or endoscopic--to follow in case of polyps of 2 cm or larger. OBJECTIVES: To analyse the efficacy and complications of colonoscopic polypectomy of large colorectal polyps. PATIENTS AND METHODS: 147 polypectomies were performed on 142 patients over an eight-year period. The technique used was that of submucosal adrenaline 1:10000 or saline injection at the base of the polyp, followed by resection of the polyp using a diathermic snare in the smallest number of fragments. One-time screening for colorectal cancer with combined fecal occult-blood testing and examination of the distal colon.

BACKGROUND: Fecal occult-blood testing and sigmoidoscopy have been recommended for screening for colorectal cancer, but the sensitivity of such combined testing for detecting neoplasia is uncertain. At 13 Veterans Affairs medical centers, we performed colonoscopy to determine the prevalence of neoplasia and the sensitivity of one-time screening with a fecal occult-blood test plus sigmoidoscopy. METHODS: Asymptomatic subjects (age range, 50 to 75 years) provided stool specimens on cards from three consecutive days for fecal occult-blood testing, which were rehydrated for interpretation. Evaluation of endoscopic and histopathological features of serrated adenoma of the colon.

BACKGROUND AND STUDY AIMS: The aim of this study was to reveal the clinical features of serrated colonic adenoma by investigating its endoscopic features using a magnifying videoscope. MATERIALS AND METHODS: 68 colorectal serrated adenomas presented for colonoscopic examination were included in this study. A magnifying videoscope with a zoom ranging from x 1 to x 100 magnification was employed to observe the pit patterns of colonic lesions, and 39 of the serrated adenoma specimens were evaluated using this. Some pit patterns appeared as elongated oval and stellar-like orifices of the crypts;

In a resource-poor country, mutation identification has the potential to reduce the cost of family management for hereditary nonpolyposis colorectal cancer.

PURPOSE: Colonoscopic surveillance of family members at risk of hereditary nonpolyposis colorectal cancer is difficult in a resource-poor country because of its expense. For family members who live in remote areas, poor communication and limited access to sophisticated medical care make surveillance even more difficult. The identification of the mutation causing the disease will simplify surveillance. Our aim was to assess the impact of mutation analysis on the management of a South African family with more than 150 members at risk for hereditary nonpolyposis colorectal cancer. METHODS: We st

Hormone replacement therapy is associated with lower risk of adenomatous polyps of the large bowel: the Minnesota Cancer Prevention Research Unit Case-Control Study.

Evidence of a role for steroid hormones and reproduction in colon neoplasia remains tantalizing but unclear. Hormone replacement therapy (HRT) has been reported in a number of recent studies to be associated with a reduced risk of colon cancer. A case-control study was undertaken to establish whether HRT is associated with lower risk of adenomatous polyps. This case-control study was undertaken as a project of the Minnesota Cancer Prevention Research Unit. Cases (n = 219) were women, ages 30-74 years with colonoscopy-proven, pathology-confirmed, adenomatous polyps of colon and rectum recruited

BCP-YOLO: a lightweight and interpretable YOLOv8-based framework for real-time coloreactal polyp detection.

BACKGROUND: Colorectal cancer is one of the most prevalent malignant tumors worldwide. Early screening relies on accurate polyp detection during colonoscopy. Polyps in colonoscopic images exhibit diverse morphologies, indistinct boundaries, and low contrast. Specular reflections, fold occlusions, and imaging artifacts further complicate detection, which fail to meet the requirements of real-time clinical assistance. METHODS: To address these challenges, we propose BCP-YOLO (You Only Look Once), a high-precision, relatively lightweight polyp detection framework built upon an improved YOLOv8 ar

Prevalence of intestinal spirochaetosis in serrated polyposis syndrome compared with other colorectal polyp conditions: a case-control study in Japan.

OBJECTIVE: Serrated polyposis syndrome (SPS) is the most common form of colorectal polyposis syndrome. Most SPS cases are sporadic, and the environmental factors underlying its pathogenesis remain poorly defined. Intestinal spirochaetosis (IS), an infection of the colonic epithelium by Brachyspira species, is frequently asymptomatic and its clinical relevance is uncertain. Here, we examined the potential association between IS and SPS. METHODS: In this retrospective case-control study at two centres in Japan, IS prevalence was assessed in 64 patients with SPS and 382 non-SPS controls who unde

Somatic Mosaic AXIN2-associated Colonic Polyposis Predominantly Involving the Proximal Colon: A Case Report.

Colonic adenomatous polyposis is most commonly caused by germline pathogenic variants in the APC gene; however, recent genetic studies have identified patients without APC mutations. AXIN2-associated colonic polyposis is a rare condition related to dysregulation of the Wnt/β-catenin signaling pathway. Most reported cases have involved germline AXIN2 variants, whereas somatic mosaic AXIN2 alterations remain poorly documented. We report a rare case of somatic mosaic AXIN2-associated colonic polyposis predominantly involving the proximal colon. A 77-year-old asymptomatic man was referred for colo

Identification and Validation of Feature Genes Related to Mitochondrial Dysfunction and Oxidative Stress in Ulcerative Colitis.

BACKGROUND: The roles of mitochondrial dysfunction (MD) and oxidative stress (OS) in the pathogenesis of ulcerative colitis (UC) have received increasing attention. Given their close association, it is crucial to clarify the molecular characteristics and biological functions of MD and OS-related genes in UC. METHODS: Gene expression profiles, mitochondrial-related genes, and OS-related genes were obtained from the corresponding databases. Unsupervised clustering of UC samples was performed based on differentially expressed MD and OS-related genes (DEMORGs). The CIBERSORT algorithm was used to

Rare Cause of Lower Gastrointestinal Bleeding-Case Presentation and Literature Review.

Background and Clinical Significance: Gastrointestinal bleeding is a critical medical emergency, with upper gastrointestinal bleeding occurring approximately five times more frequently than lower gastrointestinal bleeding (LGIB). The incidence of LGIB tends to increase with age, likely due to a greater prevalence of vascular and diverticular diseases among older patients. However, there are rare or extremely rare causes of LGIB that demand significant diagnostic and therapeutic efforts, some of which may pose unexpected challenges during surgery. Case report: We present the case of a 75-year-o
