Ablation of Accessory Pathways with Challenging Anatomy.

Although catheter ablation of accessory pathways is deemed highly safe and effective, peculiar location of these pathways might lead to complex and potentially hazardous procedures requiring ablation in anatomic regions such as para-Hisian area, coronary sinus, and epicardial surface. The electrophysiologist should know these possible scenarios to plan the best strategy for safe and effective ablation of these uncommon accessory pathways.

Cryoablation of anteroseptal accessory pathways with a his bundle electrogram on the ablation catheter.

BACKGROUND: Radiofrequency catheter ablations of anteroseptal (AS) accessory pathways (AP) in pediatric patients have higher incidence of atrioventricular (AV) block than other AP locations. We report our experience using cryoablation in pediatric patients where a His bundle electrogram was noted on the ablation catheter at the site of the successful ablation. METHODS AND RESULTS: We retrospectively reviewed all patients ≤21 years that underwent cryoablation for an AS AP from 2005 to 2012 at our institution (n=70). Patients with a His bundle electrogram noted on the cryoablation catheter at t

Cryosurgical ablation of left parietal wall accessory atrioventricular connections through the coronary sinus without the use of extracorporeal circulation.

A simplified technique for surgical treatment of preexcitation syndrome caused by left-sided accessory atrioventricular connections without the use of extracorporeal circulation has been elaborated experimentally and used in 21 patients with tachycardia refractory to medical treatment. Cryoablation of the accessory bundles was performed by means of local cryothermia using a specially designed cryoinstrument introduced into the coronary sinus through the right atrial wall. The operations were performed after thoracotomy on the working closed heart. The signs of preexcitation and paroxysms of ta

Successful cryoablation of atrioventricular nodal reentrant tachycardia and coexisting accessory pathways without fluoroscopy.

We report the case of a 14-year-old boy patient admitted to our outpatient clinic with palpitations and documented supraventricular tachycardia. Electrophysiological study and ablation were planned. In the electrophysiological study, two tachycardias with different cycle lengths and morphologies were induced. After elimination of the slow pathway, left posterior accessory pathway was detected and successfully ablated. Another pathway was detected following that ablation. Due to the slow retrograde conduction of this pathway, diltiazem infusion was started to uncover the accessory pathway. The

[Severe atrioventricular block in 2 cases with pre-excitation syndrome].

In two patients with WPW syndrome Type A suffering from syncopes and dizziness intermittent high degree A-V block was observed. The analysis of the surface Ecg revealed in the first case a complete A-V block within the normal conduction system at the level of the A-V node. In the second case there was a constant left bundle branch block with intermittent block in the right fascicle (intermittent trifascicular block). In both cases the preexcitation syndromes could be best explained by accessory tracts bypassing the normal nodal system left side. One-to-one conduction through the bypass occurre

The course of the conduction system in single ventricle with inverted (L-) loop and inverted (L-) transposition.

The conduction systems of three hearts with single ventricle and inverted (L-) loop and inverted (L-) transposition were studied by serial section. The course of the conduction system was similar in all three hearts. The regular (posterior) atrioventricular (A-V) node was rudimentary and did not form the bundle of His. An accessory (anterior) A-V node was present in the roof of the right atrium. This node pierced the connective tissue adjacent to the right side of the pulmonary annulus to form the bundle of His. The bundle proceeded into the main ventricular chamber lying on the septum between

Notch signaling regulates murine atrioventricular conduction and the formation of accessory pathways.

Ventricular preexcitation, which characterizes Wolff-Parkinson-White syndrome, is caused by the presence of accessory pathways that can rapidly conduct electrical impulses from atria to ventricles, without the intrinsic delay characteristic of the atrioventricular (AV) node. Preexcitation is associated with an increased risk of tachyarrhythmia, palpitations, syncope, and sudden death. Although the pathology and electrophysiology of preexcitation syndromes are well characterized, the developmental mechanisms are poorly understood, and few animal models that faithfully recapitulate the human dis

Change in the H-V Interval Without a Change in QRS Morphology During Atrial Pacing in a Case of Wolff-Parkinson-White Syndrome-What is the Mechanism?

In the background of an accessory pathway (AP), the H-V interval can vary during atrial/coronary sinus pacing, but only with a concomitant change in the QRS morphology and the degree of pre-excitation. In an interesting case of a 62-year-old woman, the H-V interval varied during coronary sinus pacing despite a fixed pre-excitation. This appears to have happened due to infra-Hisian complete atrioventricular dissociation, which resulted from iatrogenic mechanical bumping of the left anterior fascicle in the background of right bundle branch block and left posterior hemiblock.

Wide QRS complex supraventricular tachycardia with negative precordial concordance.

The authors describe the case of a 49-year-old man who experienced an episode of palpitations and dizziness. The results of 24-hour Holter monitoring demonstrated an episode of wide QRS complex regular tachycardia. During the electrophysiological study, a wide QRS complex tachycardia with negative precordial concordance was induced. A diagnosis of orthodromic atrioventricular re-entrant tachycardia involving a left lateral accessory pathway with left bundle branch aberration was made. This case represents a rare exception to the rule that negative precordial QRS concordance is diagnostic of ve

Multiple Accessory Pathways as Substrate for Duodromic Tachycardia: Successful Ablation of the Key Pathway.

BACKGROUND: Atriofascicular pathways are uncommon bypass tracts (BTs) with decremental conduction properties. We report a case involving 2 tachycardia mechanisms sharing a common accessory pathway as the retrograde limb. CASE SUMMARY: A 42-year-old man had a narrow-complex, short-RP tachycardia and wide-complex tachycardia with left bundle branch morphology during emergency department visits. The electrocardiogram during sinus rhythm showed manifest ventricular pre-excitation. During the electrophysiology study, both tachycardias exhibited an identical retrograde atrial activation sequence an

Quadruple atrioventricular nodal pathways: involved in orthodromic atrioventricular reentrant tachycardia.

Supraventricular tachycardia can be caused by multiple atrioventricular nodal pathways or atrioventricular accessory pathways. Herein, we report the case of a patient who was diagnosed with an orthodromic atrioventricular reentrant tachycardia that was caused by an unusual combination of quadruple atrioventricular nodal pathways and an atrioventricular accessory pathway. Radiofrequency catheter ablation of the accessory pathway successfully eliminated the arrhythmias and the patient's symptoms. Careful analysis of complete electrophysiologic studies can help in the diagnosis of such rare clini

[Radiofrequency catheter ablation of supraventricular tachycardia in an adult with corrected tetralogy of Fallot].

An 18 year old female with Fallot's tetralogy had undergone complete repair at thirteen years of age. Two years later she first presented a wide complex right bundle branch block tachycardia at a rate of 220 beats/min which could not be controlled on intravenous verapamil. Electrical shock successfully converted tachycardia to sinus rhythm, which showed typical Wolff-Parkinson-White syndrome. On electrophysiological study, the ortodromic tachycardia was found to be due to left lateral atrioventricular accessory pathway, which was ablated by radiofrequency catheter ablation. One year later she

Low energies and Helifix electrodes in the successful ablation of atrioventricular conduction.

High energies delivered via standard pacing catheter electrodes produce permanent atrioventricular conduction block and generate high pressures. We investigate the use of lower energies and an active fixation electrode. Ten patients with refractory supraventricular tachycardias (six with paroxysmal atrial fibrillation, three with dual AV nodal pathways, and one with a concealed accessory atrioventricular pathway) were treated. A 6F Vitatron Helifix electrode was positioned to give the maximum His bundle deflection. Four shocks of only 50 joules each were delivered at 1-minute intervals. Long-t

Fasciculoventricular accessory pathway masked extensive atrioventricular conduction system disease in a patient with PRKAG2 syndrome.

A 23-year-old male with a history of ventricular pre-excitation and atrial flutter presented for evaluation after recurrent syncope. The possible mechanism of syncope erroneously attributed to pre-excited atrial flutter with fast heart rates in the first hospitalization. The patient was found to have advanced heart block and PRKAG2 genetic mutation in the second hospitalization. The genetic findings and clinical features are consistent with PRKAG2 syndrome (PS). PS is a rare, autosomal dominant inherited disease, characterized by ventricular pre-excitation, supraventricular tachycardia, and ca
