Establishment and validation of a dual qPCR method for the detection of carbapenem-resistant Acinetobacter baumannii in bloodstream infections.

OBJECTIVE: Bloodstream infections(BSIs) caused by carbapenem-resistant Acinetobacter baumannii (CRAB) have a high mortality rate due to the high levels of drug resistance. There is an urgent need to establish a sensitive and accurate detection method to rapidly detect CRAB in BSIs. METHODS: A new method was developed based on fluorescence quantitative PCR (qPCR) targeting the specific region of 16sRNA and OXA-23 gene from CRAB. The parameters were evaluated and optimized. This qPCR method was further applied in the detection of AB from 30 clinical specimens. RESULTS: The qPCR method establis

Clinical characteristics and treatment outcomes of Acinetobacter baumannii bloodstream infections in a setting with high carbapenem susceptibility among isolates.

UNLABELLED: In most settings, carbapenem-resistant Acinetobacter baumannii (CRAB) infections predominate over carbapenem-susceptible Acinetobacter baumannii (CSAB). Treatment guidelines focus on the management of CRAB and do not describe the optimal antibiotic choice for CSAB. We describe the clinical characteristics and outcomes in both CRAB and CSAB. We examined consecutive episodes of CRAB or CSAB bloodstream infections in our institution from February 2022 to July 2024. Clinical, laboratory, and microbiological data were tabulated, and adverse outcomes were defined as the need for intensiv

Triple threat: Sequential meningitis infections in an immunosuppressed patient with Morganella morganii, Acinetobacter baumannii, and vancomycin-resistant Enterococcus faecium.

Gram-negative bacilli and vancomycin-resistant enterococci (VRE) are rare causative agents in the etiology of bacterial meningitis and can lead to significant mortality due to the difficulty of treatment. This article reports three successive cases of different meningitis clinical presentations in an immunosuppressed patient diagnosed with diffuse large B-cell lymphoma. A 65-year-old male patient was initially followed with Morganella morganii meningitis. Later, the patient was diagnosed with Acinetobacter meningitis and VRE meningitis. Clinical and microbiological success was achieved followi

Alert Germ Infections: Chest X-ray and CT Findings in Hospitalized Patients Affected by Multidrug-Resistant Acinetobacter baumannii Pneumonia.

Acinetobacter baumannii (Ab) is an opportunistic Gram-negative pathogen intrinsically resistant to many antimicrobials. The aim of this retrospective study was to describe the imaging features on chest X-ray (CXR) and computed tomography (CT) scans in hospitalized patients with multidrug-resistant (MDR) Ab pneumonia. CXR and CT findings were graded on a three-point scale: 1 represents normal attenuation, 2 represents ground-glass attenuation, and 3 represents consolidation. For each lung zone, with a total of six lung zones in each patient, the extent of disease was graded using a five-point s

In vitro potential of Lycosin-I as an alternative antimicrobial drug for treatment of multidrug-resistant Acinetobacter baumannii infections.

The resistance of multidrug-resistant Acinetobacter baumannii (MDRAB) isolates to most traditional antibiotics results in huge challenges for infection therapy. We investigated the in vitro activities of both l- and d-lycosin-I against MDRAB. These two compounds displayed high antibacterial activities and rapid bactericidal effects against MDRAB. Moreover, the compounds retained their activity even at high salt (Mg(2+) or Ca(2+)) concentrations. These results demonstrate the potential of lycosin-I to be developed as a new antibiotic.

Prognostic Indicators of Carbapenem-Resistant Acinetobacter baumannii Infection: A Meta-Analysis and Systematic Review.

BACKGROUND: Carbapenem-resistant Acinetobacter baumannii (CRAB) infections are clinically difficult to treat owing to their lack of effective treatments and high mortality rates. This calls for the development of prognostic indicators to facilitate risk stratification and patient management. AIMS: To identify prognostic indicators of CRAB infections through a systematic review and meta-analysis. METHODS: A systematic search was performed on PubMed, Embase, and Cochrane Library up to November 2024. Studies that reported the CRAB infections and their adverse outcomes (e.g., mortality, acute ki

Potential Mechanisms of Mucin-Enhanced Acinetobacter baumannii Virulence in the Mouse Model of Intraperitoneal Infection.

Porcine mucin has been commonly used to enhance the infectivity of bacterial pathogens, including Acinetobacter baumannii, in animal models, but the mechanisms for enhancement by mucin remain relatively unknown. In this study, using the mouse model of intraperitoneal (i.p.) mucin-enhanced A. baumannii infection, we characterized the kinetics of bacterial replication and dissemination and the host innate immune responses, as well as their potential contribution to mucin-enhanced bacterial virulence. We found that mucin, either admixed with or separately injected with the challenge bacterial ino

In vivo bioluminescence imaging of labile iron accumulation in a murine model of Acinetobacter baumannii infection.

Iron is an essential metal for all organisms, yet disruption of its homeostasis, particularly in labile forms that can contribute to oxidative stress, is connected to diseases ranging from infection to cancer to neurodegeneration. Iron deficiency is also among the most common nutritional deficiencies worldwide. To advance studies of iron in healthy and disease states, we now report the synthesis and characterization of iron-caged luciferin-1 (ICL-1), a bioluminescent probe that enables longitudinal monitoring of labile iron pools (LIPs) in living animals. ICL-1 utilizes a bioinspired endoperox
