Surgical management of the symptomatic os acromiale.

Os acromiale is an uncommon cause of shoulder pain with symptoms often resulting from an unstable meso-acromion. The associated pain may be due to impingement from the unfused fragment, a concomitant rotator cuff tear, or gross motion at the os acromiale site. Currently, initial treatment includes physical therapy, nonsteroidal anti-inflammatory agents, and subacromial corticosteroid injections. Surgical intervention is reserved for patients who do not respond to nonoperative treatment. Treatment options include open fragment excision, open reduction and internal fixation, and arthroscopic dec

Is there an association between the individual anatomy of the scapula and the development of rotator cuff tears or osteoarthritis of the glenohumeral joint?: A radiological study of the critical shoulder angle.

We hypothesised that a large acromial cover with an upwardly tilted glenoid fossa would be associated with degenerative rotator cuff tears (RCTs), and conversely, that a short acromion with an inferiorly inclined glenoid would be associated with glenohumeral osteoarthritis (OA). This hypothesis was tested using a new radiological parameter, the critical shoulder angle (CSA), which combines the measurements of inclination of the glenoid and the lateral extension of the acromion (the acromion index). The CSA was measured on standardised radiographs of three groups: 1) a control group of 94 asymp

Changes in the cervical FRR, shoulder muscle pain and position after continuous detailed assembly work.

BACKGROUND: Several risk factors for upper-extremity muscle injuries are known, including repetitive and continuous-flow assembly work. However, no study has investigated the changes in the cervical flexion-relaxation ratio (FRR) and scapular position after continuous detailed assembly work. OBJECTIVE: This study investigated the changes in the cervical FRR, trapezius muscle pain, and acromion and scapular positions after continuous detailed assembly work. METHODS: Fifteen male workers were recruited. The cervical FRR, pressure-pain threshold (PPT) of the upper and middle trapezius muscle, a

Novel anatomical mapping of the anterior branch of the axillary nerve across the anterior and middle part of the deltoid: a fresh-cadaver study defining safe zones.

PURPOSE: To map the surface projection of the anterior branch of the axillary nerve (ABAN) over the anterior and middle part of the deltoid and to define safe and high-risk zones for proximal humeral approaches. METHODS: Fourteen whole adult fresh-frozen cadavers (28 paired shoulders) were dissected in this study. Acromial landmarks were defined as point A (anterolateral corner of acromion), point B (middle point of AC), point C (posterolateral corner of acromion), and insertion of middle part of the deltoid (point D) in a modified anatomical position. A delto-pectoral landmark connected D to

[The value of radiological examination in the diagnosis of rotator cuff tear].

The aim of this paper was to evaluate the correlation between radiological changes on a-p radiograph of the shoulder and rotator cuff tear on the ground of analysis of 328 patients (360 shoulders) aged between 11 and 73 years suffering from different forms of periarthritis humeroscapularis. Sonographic examination revealed rotator cuff tear in 54 shoulders; 38 confirmed at surgery and 16 at arthrography. There was very strong correlation between rotator cuff tear and irregularities of greater tubercle, narrowing of the distance between humeral head and acromion to 5 mm or less, greater tubercl

Analysis of the capsule and ligament insertions about the acromioclavicular joint: a cadaveric study.

PURPOSE: The purpose of this study was to analyze the capsular and ligamentous insertions about the acromioclavicular (AC) joint to determine the amount of bone that can be removed without destabilizing the joint. METHODS: We dissected 28 cadaveric shoulders. The AC ligament insertions were measured under loupe magnification with a digital caliper on the acromial and clavicular sides on the anterior, posterior, superior, and inferior edges. We measured the distance to the coracoacromial (CA) and coracoclavicular ligaments. In addition, the axial and coronal angle of the AC joint was measured.

Rotator cuff impingement associated with Type III acromial morphology in a young athlete-a case for early imaging.

Down-sloped or hooked acromion morphologies may cause bony encroachment on the soft tissues of the subacromial space, predisposing to shoulder impingement syndrome. Of the latter, a hooked or Type III acromion (T3A) has also been linked to rotator cuff (RC) pathology. However, as bony acromial impingement is typically thought to occur over the age of 40, its occurrence in younger shoulder athletes presenting with shoulder pain, impingement and RC pathology may be overlooked. This case serves to illustrate the occurrence of T3A in a younger shoulder athlete, and the importance of early imaging

Three-dimensional scapular dyskinesis in hook-plated acromioclavicular dislocation including hook motion.

BACKGROUND: The purpose of this study is to analyze the 3-dimensional scapular dyskinesis and the kinematics of a hook plate relative to the acromion after hook-plated acromioclavicular dislocation in vivo. Reported complications of acromioclavicular reduction using a hook plate include subacromial erosion and impingement. However, there are few reports of the 3-dimensional kinematics of the hook and scapula after the aforementioned surgical procedure. METHODS: We studied 15 cases of acromioclavicular dislocation treated with a hook plate and 15 contralateral normal shoulders using computed t

Acromioclavicular joint injury: enhanced technique of examination with dynamic maneuver.

Acromioclavicular (AC) sprains can be graded in 3 to 6 different types according to Tossy or Rockwell, respectively. In mild sprains (Tossy and Rockwell I), movements in the AC joint are minimal, because the coraco-clavicular ligaments are intact. In these patients, stress radiography is usually normal, and sonographic examination at rest can be normal as well, showing minimal or no displacement between the 2 extremities of the bones. We present a simple dynamic maneuver to enhance the diagnosis of these mild sprains known as the cross-arm maneuver, in which the hand is placed on the opposite

A simple method of shoulder arthrodesis.

A new technique of shoulder fusion is presented using a posterior approach. After removal of the articular cartilage, a Rush pin is introduced from the spine of the scapula, through the glenoid into the medullary canal of the humerus. This is supplemented by tension-band wiring from the acromion to the neck of the humerus and a muscle pedicle graft attached to the acromion. A shoulder spica is applied for four to six weeks. Four patients with injuries to the upper brachial plexus and 14 with paralysis of the upper arm due to anterior poliomyelitis have been followed for three years. One of the

[The enthesopathic shoulder].

61 shoulders of rheumatoid diseases, 23 of ankylosing spondylo-arthritis, 22 of psoriatic rheumatism and 30 of hyperostoses (Forestier's disease) were analysed and compared. There is a very significant difference between rheumatoid disease, on the one hand, and the other diseases, on the other hand. In ankylosing spondylo-arthritis, psoriatic rheumatism and hyperostosis, enthesophytes are observed; it is not the case in rheumatoid disease (unless associated with hyperostosis). The most frequent entesophyte is the one at the acromial insertion of the acromio-coracoid ligament ("bearded acromion

In vivo analysis of coracoid and subacromial shoulder impingement mechanism during clinical examination.

BACKGROUND: Understanding the mechanisms of shoulder impingement created by clinical tests is crucial to accurately evaluate the condition. The objective of this study was to relate mechanisms of subacromial and coracoid impingement occurring in positions of the shoulder during clinical examination, in quantitative and qualitative terms. METHODS: A 1.0T open magnetic resonance imaging system was used in 18 female and 19 male subjects, to determine the distances between the humeral head and the acromion or coracoid, and contact with the rotator cuff (RC). Measurements were taken with the shoul

The effects of arm elevation on the 3-dimensional acromiohumeral distance: a biplane fluoroscopy study with normative data.

HYPOTHESIS AND BACKGROUND: Narrowing of the subacromial space has been implicated in several shoulder pathologies. However, the location of the minimum distance points during clinical testing has not been defined. We sought to measure the in vivo minimum distance and location of the minimum distance points on the acromion and proximal humerus during arm elevation. METHODS: Eight healthy male subjects (mean age, 30 years) underwent a dynamic in vivo biplane fluoroscopy assessment of scaption and forward elevation. For each frame, the 3-dimensional position and orientation of the humerus and sc

Decreasing glenoid inclination improves function in shoulders with simulated massive rotator cuff tears.

BACKGROUND: A massive rotator cuff tear leads to poor shoulder function as evidenced by diminished glenohumeral abduction and superior translation of the humeral head compared to its normal position. The inclination angle of the glenoid has been associated with rotator cuff tears. The objective of this study was to quantify the effect of a decreased glenoid inclination angle on glenohumeral kinematics during active abduction in shoulders with a simulated, massive rotator cuff tear. METHODS: Eight fresh-frozen full upper extremities were tested using a dynamic shoulder testing apparatus. After

MRI evaluation of axillary neurovascular bundle: Implications for minimally invasive proximal humerus fracture fixation.

BACKGROUND: Percutaneous fixation of proximal humeral fractures places the axillary nerve and posterior humeral circumflex artery at risk for injury. Safe operative zones for the axillary nerve are described based on external measurements from anatomic landmarks, but no study to date has incorporated advanced imaging to help guide surgical procedures in the region of the axillary neurovascular bundle (ANVB). We sought to define the location and trajectory of the ANVB in relation to osseous landmarks using magnetic resonance imaging (MRI) measurements. METHODS: Retrospective review of 750 cons

Biodegradable spacer reduces the subacromial pressure: A biomechanical cadaver study.

BACKGROUND: Failure after rotator cuff repair remains a major clinical problem and could be related to excessive pressures from the acromion. Previous studies with irreparable tears showed good clinical results of tendon healing with arthroscopic insertion of a protective biodegradable spacer balloon between the repaired tendon and the acromion. One hypothesis is that compression pressures on the repaired tendon will be reduced by the spacer. This cadaver study aimed to investigate the effects of this subacromial spacer on compression pressures over a repaired supraspinatus tendon in passive m
