Recent studies using electrophysiologic testing have established that effective antiarrhythmic therapy does indeed prolong survival in the high-risk group of survivors of out-of-hospital cardiac arrest. Ideally, the initial antiarrhythmic agent is selected on the basis of its efficacy, side effects, ease of administration, cost, and interaction with other medications. To a large extent, however, selection remains empiric. In patients without QT prolongation, long-acting procainamide or quinidine are usually tried first.

A double-blind, placebo-controlled, cross-over study has been carried out to compare long-acting diethylpropion hydrochloride (Tenuate Dospan, Wm. S. Merrell Company, hereafter referred to as TD) in a rural general practice. TD was found to produce significantly more weight loss than placebo and side effects were not significantly greater. The need for the treatment of obese patients in general practice is discussed, as is the role of anorexiant therapy.

AIMS: Opportunities and needs for starting insulin therapy in Type 2 diabetes (T2D) have changed overtime. We evaluated clinical characteristics of T2D subjects undergoing the first insulin prescription during a 15-year-observation period in the large cohort of the AMD Annals Initiative in Italy.
METHODS: Data on clinical and laboratory variables, complications and concomitant therapies and the effects on glucose control after 12 months were evaluated in T2D patients starting basal insulin as add-on to oral/non-insulin injectable agents, and in those starting fast-acting in add-on to basal in

This study systematically compared the enzymatic hydrolysis of corn starch granules by porcine pancreas α-amylase (PA, endo-acting), maltogenic α-amylase (MA, multifunctional), glucan 1,4-α-maltotriohydrolase (AMTS, multifunctional), and amyloglucosidase (AMG, exo-acting). Multiscale structural analysis revealed that enzymatic hydrolysis produced distinct pore-cavity structures, ranging from large cavities (PA, MA) to increased pore number with moderate cavity (AMTS) and minimal cavity formation (AMG), following a consistent "inside-out" pattern of initial amorphous region hydrolysis followed

OBJECTIVE: The risk of inappropriate use of opioid drugs increases, especially the short-acting ones. The risk of addiction in patients with cancer with a relatively good prognosis is a challenge. The aim of the study is to evaluate the decision to continue therapy with a short-acting fentanyl.
METHODS: The study concerns a 49-year-old male patient with an advanced neuroendocrine tumour in the pre-sacral region. The research method includes the medical history and physical examination, an analysis of the patient's medical record and a self-designed questionnaire to assess the degree of depend

Orthodontic treatment can cause resorptions on teeth. Factors causing resorptions are forces and moments. Other factors that cause resorptions are less influenced by an orthodontic treatment. By carrying out experiments and finite element calculations, it was possible to demonstrate that forces and moments will produce stress in certain parts of the periodontal ligament and the root. This stress reaches or exceeds the blood pressure in some areas. To avoid high pressure in those areas, new treatment devices were constructed consisting of NiTi wires or a combination of NiTi and stainless steel.

In 1988 and 1990 screening for HbAlc and albumin excretion rate (AER) in diabetic children was carried out throughout Denmark. Each study included about 1,000 diabetic patients and found HbAlc and insulin requirements increased during maturation in both sexes. No significant differences in HbAlc were seen among adolescents treated with two, three or more daily insulin injections. The prevalence of persistent microalbuminuria was 4.3% which was associated only with age and diastolic blood pressure. An international survey on HbAlc and insulin treatment, involving 2,873 children, showed HbAlc to

Phylogenetic reconstruction methods are subject to two types of limitations: our knowledge about the true history of organisms and the gross simplification implied in the numerical simulation models of the relationships between them. In such a situation, experimental phylogenetics provides a way to assess the accuracy of the phylogenetic reconstruction methods. Nonetheless, this capacity is only feasible for organisms in which replication and mutation rates are high enough to provide valuable data. On the other hand, experimental phylogenetics also provides insights on the main evolutionary pr

Recent reports have demonstrated that glucose-6-phosphate dehydrogenase (G6PD) activity in mammalian cells is necessary in order to ensure cell survival when damage is produced by reactive oxygen intermediates. In this paper we demonstrate that oxidative stress, caused by agents acting at different steps in the biochemical pathway controlling the intracellular redox status, determines the increase in G6PD-specific activity in human cell lines of different tissue origins. The intracellular level of G6PD-specific mRNA also increases, with kinetics compatible with the induction of new enzyme synt

BACKGROUND: Cardiogenic shock (CS) complicating acute myocardial infarction (AMI) carries high mortality. Early revascularisation improves survival, but the effect of structured multidisciplinary care on outcomes remains underexplored.
METHODS AND RESULTS: ACT-SHOCK is a service evaluation at a UK tertiary cardiac centre. Between May 2023 and May 2024, 82 patients with AMI-related CS requiring emergent percutaneous coronary intervention (PCI) were identified using protocolised physiological criteria and managed by an Advanced Cardiogenic-Shock Team (ACT). The ACT comprised interventional card

The absence of disease modifying drugs in Parkinson's disease therapy urges for new chemical entities acting on relevant PD-associated biological targets. As a result, developing selective and reversible inhibitors targeting MAO-B is still a desirable line of therapeutic research. Within this framework, a small library of chromone derivatives was synthesized and screened towards human monoamine oxidases. Structural modifications on the chromone 3-phenylcarboxamide resulted in potent MAO-B inhibitors with an improved drug-like profile, and for the first time we obtained potent and selective chr

Asthma exacerbation is defined as a progressive increase in symptoms of shortness of breath, cough, or wheezing sufficient to require a change in therapy. After ruling out diagnoses that mimic an asthma exacerbation, therapy should be initiated. Short-acting β2 agonists and short-acting muscarinic antagonists are effective as bronchodilators for asthma in the acute setting. Systemic corticosteroids to reduce airway inflammation continue to be the mainstay therapy for asthma exacerbations, and, unless there is a contraindication, the oral route is favored. Based on the current evidence, nebuliz

A diverse group of processes are involved in central control of ventilation. Both fast acting neurotransmitters and slower acting neuromodulators are involved in the central respiratory drive. This review deals with fast acting neurotransmitters that are essential centrally in the ventilatory response to H(+)/CO(2) and to acute hypoxia. Data are reviewed to show that the central response to H(+)/CO(2) is primarily at sites in the medulla, the most prominent being the ventral medullary surface (VMS), and that acetylcholine is the key neurotransmitter in this process. PURPOSE: Advanced hepatocellular carcinoma (HCC) not eligible for local therapies has limited chances of cure. Sorafenib is a multikinase inhibitor with proven activity in advanced HCC. Octreotide is used in this setting with conflicting results. Treatment with sorafenib and long-acting octreotide was tested in advanced HCC to evaluate safety and activity.
METHODS: Fifty patients with advanced HCC, Child-Pugh A or B, received sorafenib at a dosage of 800 mg/day for 28 days with a following week of rest and long-acting octreotide at a dose of 40 mg, administered every 28 days.

A prospective study of postoperative pulmonary complications (PPC) appearing during the hospital stay was carried out in 417 patients undergoing surgery through a subcostal or midline incision. Postoperative pain was relieved either by intercostal block and centrally-acting analgesics on demand or by centrally-acting analgesics alone. Pulmonary complications were diagnosed from combined physical and radiological signs. After biliary surgery through a subcostal incision, PPC were less frequent (P less than 0.05) in patients receiving intercostal blocks (6%) than in those given centrally-acting

BACKGROUND: Assisted reproduction techniques (ART) such as in vitro fertilisation (IVF) and intracytoplasmic sperm injection (ICSI) can help subfertile couples to create a family. It is necessary to induce multiple follicles; this is achieved by follicle stimulating hormone (FSH) injections. Current treatment regimens prescribe daily injections of FSH (urinary FSH with or without luteinizing hormone (LH) injections or recombinant FSH (rFSH)).Recombinant DNA technologies have produced a new recombinant molecule which is a long-acting FSH, named corifollitropin alfa (Elonva) or FSH-CTP. Treatment-resistant hypertension is still common despite the availability of several types of antihypertensive agents acting by different mechanisms. The existence of refractory hypertension should lead to rule out "white-coat hypertension", poor adherence to prescribed drugs as well as classical causes of secondary hypertension such as renal artery stenosis, primary aldosteronism, pheochromocytoma and renal disease. It is also important to consider the possible existence of obstructive sleep apnea or the regular intake of vasopressive drugs or substances.

This case report describes a highly complicated case with combined chronic pain, substance use disorder (SUD) and social challenges following war participation in Ukraine. The patient had developed PTSD and SUD because of pain management and was enrolled in dual diagnosis treatment in out-patient psychiatry. The patient successfully transitioned to long-acting buprenorphine leading to better pain management and remission in SUD, improving both treatment relation and clinical treatment of PTSD by use of integrated dual diagnosis treatment.
