BACKGROUND: Our institutional emergency general surgery service is staffed by both trauma and critical care-trained surgeons and other boarded general surgeons and subspecialists. We compared efficiency of care for common emergency general surgery conditions between trauma and critical care-trained surgeons and boarded general surgeons and subspecialists.
METHODS: Adults admitted between February 2014 and May 2017 with acute appendicitis, acute cholecystitis, intestinal obstruction, incarcerated hernia, or other acute abdominal diagnoses seen by emergency general surgery service were included

OBJECTIVE: Converging evidence has demonstrated that there is aberrant connectivity of the default-mode network (DMN) in left temporal lobe epilepsy (lTLE) yet changes in the network homogeneity (NH) of the DMN in people with first-episode, treatment-naive lTLE remains unclear. In this study, we used an NH method to investigate the NH of the DMN in people with first-episode, treatment-naive, lTLE, at rest.
PATIENTS AND METHODS: We collected resting-state functional magnetic resonance imaging (rs-fMRI), and attention network test (ANT) data from 43 people with lTLE and 42well-matched, healthy

BACKGROUND: Recommendations for health care digitization as issued with the Riyadh Declaration led to an uptake in telemedicine to cope with the COVID-19 pandemic. Evaluations based on clinical data are needed to support stakeholders' decision-making on the long-term implementation of digital health.
OBJECTIVE: This health economic evaluation aims to provide the first German analysis of the suitability of video consultations in the follow-up care of patients in orthopedic and trauma surgery, investigate the financial impact on hospital operations and personnel costs, and provide a basis for d

BACKGROUND: Surgical resources are often shared across medical specialties, which can lead to conflicts over prioritisation during on-call hours. This study investigated factors contributing to inter-speciality conflict in the prioritisation of emergency surgical cases.
METHODS: A survey on views of surgical urgency and prioritisation of 22 hypothetical cases was distributed to Swedish physicians involved in emergency surgery via posters in operating departments and administrative areas, and by email. The Kruskal-Wallis test and Dunn's post-hoc test were used to compare responses across speci

The severity and extent of gunshot and blast injuries are often misjudged due to a lack of specialist knowledge. The aim of this article is to outline the characteristics of gunshot and blast injuries, including emergency diagnostics and initial surgical treatment.Due to multiple high-energy penetrating injuries, barotrauma, or blunt trauma, affected patients are usually polytraumatized, with multiple organ damage and an average Injury Severity Score (ISS) of > 36. In addition to the complexity of the wounds, they are characteristically severely contaminated. After pre-hospital treatment, life

BACKGROUND: Timing to surgery for acute cholecystitis remains variable, ranging from early (< 7 d) to delayed surgery (> 7 d). Accelerated surgery may result in better outcomes owing to reduced exposure to hypercoagulable and inflammatory states. We sought to determine the feasibility of a trial comparing accelerated surgery with standard care among patients with calculous acute cholecystitis.
METHODS: We conducted a multicentre pilot randomized controlled trial. We randomly assigned adult patients with acute cholecystitis to receive accelerated surgery (i.e., goal of surgery within 6 hours o

BackgroundEmergency general surgery (EGS) patients require surgical intervention and often critical care. Current literature suggests that interfacility transfer potentially face worse outcomes including increased mortality and complications. This study aims to evaluate patient transfers on mortality and the utilization of damage control laparotomy (DCL) in EGS.MethodsRetrospective cohort study of patients undergoing emergent exploratory laparotomy at an academic institution from 2013 to 2023. Patients were included if they were ≥18 years old and underwent emergent nontraumatic laparotomy. Or

In critical illness the regulation of inflammation and oxidative stress can improve patient outcomes, and thus omega-3 polyunsaturated fatty acids (PUFAs) have been used as part of parenteral nutrition (PN) owing to their potential anti-inflammatory effects. The international lipids in PN Summit, encompassed discussions and the production of consensus guidelines concerning PN intravenous lipid emulsion (ILE) use in critical care. The Lipid Summit participants agreed that the inclusion of fish oil in ILEs is associated with meaningful clinical benefits without signals of harm, based on a strong

INTRODUCTION: Postacute care services (PACS) are one of the fastest-growing programs funded by the Centers for Medicare and Medicaid (CMS), with potential promise to rural communities. Emergency general surgery (EGS) is essential to many hospitals. Yet little is known about post-EGS PACS utilization, especially among rural communities, which have longstanding barriers to healthcare access and greater travel distances. This study seeks to uncover drivers of PACS utilization after EGS among rural patients.
METHODS: Using the 2016 to 2022 National Inpatient Sample, we conducted a retrospective c

PURPOSE: Acute high-risk abdominal (AHA) surgery is associated with considerable morbidity and mortality. In the emergency department (ED), early identification of patients requiring AHA surgery remains challenging and is often based on non-standardized clinical judgment. This study aimed to identify clinical indicators that may support the timely recognition of patients in need of emergency abdominal surgery. METHODS: A modified Delphi study was conducted to achieve expert consensus on early clinical indicators of AHA surgery. Two Delphi rounds were performed. In round one, experienced surgeo

BackgroundThe Social Vulnerability Index (SVI) has also been used as a measure of social determinants of health (SDOH), with several studies showing worse health outcomes in patients with higher burdens of SDOH. This systematic review focuses on the application of SVI in Emergency General Surgery (EGS), exploring the impact of patient vulnerability on individual health outcomes.MethodsA systematic literature search was conducted using PubMed, EMBASE and Web of Science. Inclusion criteria consisted of studies that were peer reviewed, obtainable in English, used SVI as a measurement applied to E

The abrupt onset of the situation after a traumatic amputation and the preparatory discussions following unsuccessful attempts to preserve limbs with necessary amputation require a high level of empathy, attention and well-founded information individually tailored to the affected individuals. Optimization of the treatment process can only be achieved by considering these aspects. The self-motivation and cooperation of the patient should be encouraged. To achieve this goal, the professions involved are less suitable for counseling due to a lack of personal experience, whereas so-called peers, as

BACKGROUND: About 1%-2% of pregnant patients develop conditions that require emergency general surgery (EGS). The diagnosis and management of these conditions can be challenging, as surgeons must carefully balance the needs of the pregnant patient and the developing fetus. We sought to summarize the latest literature guiding surgical management of appendicitis, benign biliary disease, bowel obstruction, and hemorrhoids in pregnant patients.
METHODS: We performed a comprehensive scoping review using OVID Medline for articles published between January 2000 and June 2023 pertaining to EGS and pr

The challenge in treating traumatic hemipelvectomy is the dynamics of the complex and life-threatening consequences of the injury. These include skin and soft tissue defects, osseous, neural and vascular injuries as well as the subsequent hemostatic derangement and organ dysfunction as part of the shock process. The treatment requires rapid and targeted decisions to save the patient's life. In this particular case a 34-year-old farmer was trapped between a wheeled loader and a stationary trailer. Upon arrival at the hospital the patient was in a state of hemorrhagic shock with accompanying acu

BACKGROUND: Regionalized care has increased interfacility transfers, particularly for emergency general surgery (EGS) patients requiring urgent, specialized care. Air medical transport (AMT) provides rapid transfer with critical care capabilities and is increasingly used for EGS, but its time-saving benefits remain uncertain. This study aims to identify the population of EGS patients undergoing AMT and the distance thresholds where AMT is faster than ground transport.
METHODS: We conducted a retrospective cohort and analyzed EGS patients transferred to our quaternary center from January 2021

INTRODUCTION: An aging population and advances in surgery have led to increasing need for geriatric management of elderly patients undergoing emergency general surgical care. Integration of a proactive geriatric service for these patients may beneficially impact both traditionally measured hospital and patient-based metrics. Here, we synthesize evidence regarding measurable outcomes of embedded geriatric care within emergency general surgery.
METHODS: A comprehensive search was conducted across Medline, Embase, Cochrane Library, CINAHL, and Scopus databases by two independent reviewers. The i

AIM: To determine the current status of surgical rescue (SR) and examine the predictors of failure to rescue (FTR). In acute care surgery, predictors of FTR have only been reported in patients with trauma, not in SR. METHODS: This retrospective study included 142 patients who underwent emergency surgery at our institution between April 2019 and March 2023. The primary outcome was the identification of the FTR predictors using logistic regression analysis. The secondary outcome was the development of a Surgical Rescue Severity Score (SRSS) using these SR predictors and long-term outcomes. RESUL

BACKGROUND: During the first wave of the COVID-19 pandemic, several studies - including our own - demonstrated a significant decrease in emergency surgical presentations, including acute appendicitis (AA). While these early findings are well documented, little is known about how presentation patterns and clinical outcomes evolved across subsequent pandemic waves. This study examines longer-term trends in AA care across three distinct pandemic phases, highlighting how healthcare systems and patient responses adapted over time within a tertiary surgical setting.
METHODS: We conducted a retrospe

INTRODUCTION: Both acute care surgeons (ACS) and colorectal surgeons (CRS) manage patients with acute lower gastrointestinal (GI) surgical conditions. No consensus guidelines exist regarding which service is best suited to manage specific conditions. We investigated surgeon views and practice patterns regarding service allocation for the management of acute lower GI conditions.
METHODS: An anonymous survey was emailed to all 237 ACS and CRS at teaching hospitals in New England with both departments (n = 19) in April 2024. Respondents rated 20 conditions on which service should manage each con
