Transplantation of adult living donor kidneys into infants and small children.

HYPOTHESIS: We hypothesized that improved outcomes following renal transplantation in high-risk infants and small children primarily are due to advances in immunosuppression and accurate diagnosis of rejection. Optimizing renal allograft perfusion is critical to achieving good early graft function and decreasing early graft loss. DESIGN: Twenty-eight consecutive recipients (weighing <20 kg) of adult living donor kidneys transplanted at our center from 1984 to 1999 were reviewed. Two groups were identified based on differing immunosuppression protocols and clinical surveillance. Actuarial graf

Hepatocellular carcinoma in children: clinical review and comparison with adult cases.

BACKGROUND: Hepatocellular carcinoma (HCC) in children was rarely reported and usually included with hepatoblastoma in most studies of pediatric liver malignancies despite different clinical behaviors. The authors report their experience in pediatric HCC and discuss its differences from adult HCC. METHODS: A retrospective review of radiographic, laboratory, pathological, and therapeutic data in 55 children with HCC was performed. The liver function was graded by modified Child's classification. Kaplan-Meier survival curves in various therapeutic and Child's groups were plotted, and log-rank t

Adult-type respiratory distress syndrome in children. Experience with seven cases.

Adult respiratory distress syndrome (ARDS) is not well recognized in children. We report seven individuals, ranging in age from 1.5 to 16 years. The clinical picture of ARDS reached a peak in 72 hours from admission and was associated with a 28.5 per cent death rate (2/7). In no case was an organism isolated, either from bronchial washings via a flexible bronchoscope or from open lung biopsy specimen. ARDS is characterized by severe damage to the alveolar-capillary unit and probably inadequate production of lung surfactant, resulting in severe hypoxia, hypoxemia, intrapulmonary shunting, and m

Adult outcomes for children who sustained firearm-related spinal cord injuries.

OBJECTIVE: To describe the adult functional, participation, education, employment, and quality of life outcomes of children who incurred spinal cord injury (SCI) as the result of gun injuries vs. non-violent etiologies, as well as their utilization of health services. DESIGN: Retrospective-cohort study. Eligibility criteria were current age at least 18 years, at least 5 years after SCI, and injury prior to 19 years of age. After enrolling the gun injury group, we matched individuals with non-violent etiologies from the Midwest Regional SCI Model System database to the gun injury group's demog

Unrecognized pediatric and adult family members of children with acute brucellosis.

BACKGROUND AND AIM: Brucellosis is an infectious, contagious and zoonotic disease that occurs worldwide. The family members of an index case of brucellosis may be especially susceptible, due to sharing the same source of infection and similar risk factors for brucellosis. In this study, we propose to screen pediatric and adult family members of brucellosis index cases for detecting additional unrecognized infected family members. MATERIALS AND METHODS: 114 family members of 41 pediatric patients with brucellosis were evaluated. All family members completed a brief questionnaire and were teste

Assessment of children exposed to adult pulmonary tuberculosis in Cali, Colombia.

BACKGROUND: There is a lack of uniform criteria for the diagnosis and management of tuberculosis (TB) in children in Cali, Colombia. Addressing TB in children is a challenge in this setting, under both programmatic and research conditions. OBJECTIVES: To facilitate the diagnostic assessment of TB in a paediatric cohort of TB household contacts. METHODS: A diagnostic and management algorithm (DMA) was used to assess children exposed to adult TB cases, according to clinical and epidemiological findings and under programmatic conditions. On the basis of diagnostic tests, cases were classified a

Adult-type bimalleolar ankle fracture in children: report of two cases.

UNLABELLED: Ankle fractures patterns in children may vary depending on the maturity of the distal tibial and fibular physis. Bimalleolar ankle fracture is an exceedingly rare fracture pattern in children and has been reported once in the current English literature. Two further adolescents with bimalleolar ankle fracture are reported. Although these fractures are rare, surgeons should be aware of these atypical fracture patterns. Herein, the underlying physiology and the mechanism of injury of ankle fractures in children are discussed. LEVEL OF EVIDENCE: Therapeutic, Level IV, Case study.

Voriconazole pharmacokinetics and safety in immunocompromised children compared to adult patients.

The aim of this study was to investigate the pharmacokinetics and safety of voriconazole after intravenous (i.v.) administration in immunocompromised children (2 to 11 years old) and adults (20 to 60 years old) who required treatment for the prevention or therapy of systemic fungal infections. Nine pediatric patients were treated with a dose of 7 mg/kg i.v. every 12 h for a period of 10 days. Three children and 12 adults received two loading doses of 6 mg/kg i.v. every 12 h, followed by a maintenance dose of 5 mg/kg (children) or 4 mg/kg (adults) twice a day during the entire study period. Pain reduction by fibrin sealant in older children and adult tonsillectomy.

OBJECTIVES: Because tonsillectomy is a painful procedure in adults, this study was performed to see whether the addition of fibrin sealant (Tisseel) to the tonsillar fossa at the completion of tonsillectomy would reduce pain in adults and allow them to return to work sooner. STUDY DESIGN: Forty adult patients over the age of 12 were entered into a prospective randomized study to see whether the addition of fibrin sealant (Tisseel) to the tonsillar fossa at the completion of tonsillectomy for recurrent tonsillitis would reduce the time the patient needed to take narcotic analgesics after surge

Adult Toxocara cati infections in U.S. children: report of four cases.

We report four cases of passage of subadult or adult Toxocara cati worms by young children ages 20 months to seven years. Worms were expelled rectally in two cases and in two cases they were vomited. A single worm was passed in two cases, three worms in one case, and 15 worms in the fourth case. All worms that were available for study were identified as T. cati by morphologic criteria, including the arrow-shaped cervical alae and the digitiform shape of the male tail. None of the four children exhibited clinical signs of ocular or visceral larva migrans, and in two cases where serum samples we

Task switching hurts memory in adults but not in children.

Switching between tasks impairs not only ongoing task performance (lower accuracy, slower responses for switch vs. repeat trials) but also for subsequent memory (poorer memory for stimuli on switch vs. repeat trials). This study examined developmental differences in memory switch costs in children (ages 7-13) and young adults (ages 19-31). Participants (N = 240) categorized trial-unique pictures of animals and objects using alternating size and animacy tasks. A subsequent incidental recognition test assessed memory for pictures from repeat and switch trials after a short filler task and again

The effect of flooding on respiratory health in children and adults.

INTRODUCTION: The occurrence of severe storms and floods has steadily increased over the last decades world-wide because of climate change and global warming. Flooding events have important effects on respiratory health especially in vulnerable populations. Some health effects are immediate, such as respiratory infections, asthma attacks and chronic obstructive pulmonary disease exacerbations. An important immediate risk arises for patients with technology dependent respiratory conditions if power supplies fail. Post-flood, indoor damp and mold develop in flooded homes creating a high risk of

[Binge eating disorder in children, adolescents, and adults].

A 70-year-old woman presented with morning stiffness, groin and knee pain, weight loss, and night sweats-initially suggestive of polymyalgia rheumatica (PMR). Laboratory tests showed elevated inflammatory markers, but imaging revealed a renal tumour. Following nephrectomy, symptoms resolved without sustained corticosteroid treatment. This case underscores the need to consider malignancy in patients with atypical or treatment-resistant PMR-like symptoms.

Entecavir for children and adults with chronic hepatitis B.

RATIONALE: Chronic hepatitis B is a major worldwide public health concern. Entecavir, one nucleos(t)ide analogue antiviral therapy option, is recommended as the first-line drug for chronic hepatitis B in many clinical guidelines. However, none of the guideline recommendations are based on the findings of a systematic review with meta-analysis, where entecavir versus no treatment or placebo are compared directly. OBJECTIVES: To evaluate the benefits and harms of entecavir versus no treatment or placebo in children and adults with chronic hepatitis B, who are either hepatitis B e-antigen (HBeAg

[Myasthenia in adults, children, and adolescents: what's new?].

Therapy of myasthenia gravis (MG) is increasingly oriented to the patient's antibody status. In addition to symptomatic therapy, steroids, classic long-term immunosuppressive therapies and thymectomy are regularly used. In recent years, new therapeutic approaches have been developed that particularly benefit acetylcholine receptor (AChR) antibody (Abs) positive patients with highly active disease. While the C5 complement inhibitor eculizumab was reserved for treatment-refractory generalized courses of AChR-Abs positive MG, two new drugs, the neonatal Fc receptor inhibitor efgartigimod and the
