Modulating ILC2 function for treatment of type 2 airway diseases.

Type 2 airway diseases including chronic rhinosinusitis, allergic rhinitis, and asthma remain a major health concern. These disorders are largely characterized by an uncontrolled type 2 immune response with elevated cytokines of IL-4, IL-5 and IL-13, eosinophilic inflammation, goblet cell hyperplasia as well as tissue remodeling. In the last few decades, critical potential roles of innate lymphoid cells (ILCs) in type 2 human diseases have emerged. Unlike their lymphocyte counterpart T cells, ILCs lack antigen-specific receptors and are largely tissue resident. Specifically, group 2 innate lym

Airway Eosinophilopoietic and Autoimmune Mechanisms of Eosinophilia in Severe Asthma.

Eosinophils are critical in asthma biology, contributing to symptoms, airflow obstruction, airway hyperresponsiveness, and remodeling. In severe asthma, in addition to local maturation in bone marrow, in situ eosinophilopoiesis plays a key role in the persistence of airway eosinophilia. Local milieu of structural, epithelial and inflammatory cells contribute by generating eosinophilopoietic cytokines in response to epithelial-derived alarmins. Another mechanism of persistent airway eosinophilia is glucocorticosteroid insensitivity, which is linked to recurrent airway infections and presence of

The language of the immune system: Part 1: Cytokine conversations and conversationalists.

The immune system is always acting to protect the body from the microbial world, especially at the openings of the body but also helps to maintain homeostasis. Normal functions include self-regulation and regeneration, facilitating the maintenance of organs, skin and mucosal membranes, protecting the openings of the body, providing surveillance for abnormal and cancer cells and eliminating molecular and cellular debris. In addition to status quo maintenance functions, the immune system responds to damage, disruption or danger when challenged by trauma, infection, cancer, autoimmunity and other

NEUTROPHIL HETEROGENEITY IN SEPSIS: THE ROLE OF DAMAGE-ASSOCIATED MOLECULAR PATTERNS.

Sepsis is a severe inflammatory disease syndrome caused by the dysregulated host response to infection. Neutrophils act as the first line of defense against pathogens by releasing effector molecules such as reactive oxygen species, myeloperoxidase, and neutrophil extracellular traps. However, uncontrolled activation of neutrophils and extensive release of effector molecules often cause a "friendly fire" to damage organ systems. Although neutrophils are considered a short-lived, terminally differentiated homogeneous population, recent studies have revealed its heterogeneity comprising different

High Mobility Group Box 1: Biological Functions and Relevance in Oxidative Stress Related Chronic Diseases.

In the early 1970s, a group of non-histone nuclear proteins with high electrophoretic mobility was discovered and named high-mobility group (HMG) proteins. High-mobility group box 1 (HMGB1) is the most studied HMG protein that detects and coordinates cellular stress response. The biological function of HMGB1 depends on its subcellular localization and expression. It plays a critical role in the nucleus and cytoplasm as DNA chaperone, chromosome gatekeeper, autophagy maintainer, and protector from apoptotic cell death. HMGB1 also functions as an extracellular alarmin acting as a damage-associat

Impact of Low Humidity on Damage-associated Molecular Patterns at the Ocular Surface during Dry Eye Disease.

SIGNIFICANCE: Dry eye is one of the leading causes for individuals to seek eye care, whereas the pathogenesis is poorly understood. One mechanism in which dry eye inflammation may ensue is by the release of damage-associated molecular patterns (DAMPs) by damaged cells to stimulate the production of cytokines and matrix metalloproteinases. Examining DAMP levels on the ocular surface during dry eye disease (DED) will increase our understanding of their potential involvement in the pathogenesis of DED. PURPOSE: This study aimed to quantitate DAMPs, high-mobility group box 1 (HMGB1), and heat sho

Damage-Associated Molecular Patterns and Their Signaling Pathways in Primary Blast Lung Injury: New Research Progress and Future Directions.

Primary blast lung injury (PBLI) is a common cause of casualties in wars, terrorist attacks, and explosions. It can exist in the absence of any other outward signs of trauma, and further develop into acute lung injury (ALI) or a more severe acute respiratory distress syndrome (ARDS). The pathogenesis of PBLI at the cellular and molecular level has not been clear. Damage-associated molecular pattern (DAMP) is a general term for endogenous danger signals released by the body after injury, including intracellular protein molecules (HMGB1, histones, s100s, heat shock proteins, eCIRP, etc.), secret

COPD Biologics: Right Patient, Right Pathway, Right Time.

Chronic obstructive pulmonary disease (COPD) is a heterogeneous disease that is entering a new era in precision medicine. Advances in disease endotyping have challenged the traditional view of COPD as a uniformly neutrophilic disorder and revealed biologically distinct subgroups in whom targeted immunomodulation may be effective. Reproducible signatures of type 2 (T2) inflammation and epithelial-derived alarmin activation have emerged as actionable pathways, reshaping therapeutic development in COPD. This narrative review synthesises mechanistic insights and clinical trial evidence for biologi

Innate airway immune response to fungal allergens.

Fungi are ubiquitous in our environment, and inhaled exposure of their spores is associated with the development of upper and lower respiratory airway diseases including asthma and chronic rhinosinusitis (CRS). CRS and asthma associated with fungal sensitization also tend to be more severe compared with other endotypes. Alternaria and Aspergillus species have specifically been recognized as the primary fungal allergens that stimulate a robust innate immune response, potentially leading to allergic sensitization and type 2 inflammation. Other fungi, such as Cladosporium, Penicillium, and Candid

VIP modulates IL-22R1 expression and prevents the contribution of rheumatoid synovial fibroblasts to IL-22-mediated joint destruction.

Rheumatoid arthritis (RA) and osteoarthritis are two rheumatic diseases whose progression is associated with a chronic synovitis. Fibroblast-like synoviocytes (FLS) have been shown to play a pivotal role in initiating and perpetuating inflammatory and destructive processes in the rheumatoid joint. Recently, the stimulating role of IL-22 has been reported on RA-FLS contribution to joint destruction by means of the increase of proliferation and matrix-metalloproteinase-1 (MMP-1) and alarmin S100A8/A9 production. Besides, mediators potentially present in inflamed joints have been shown to increas

OPTN knockout alleviates OVA-induced airway inflammation in a mouse model of asthma.

OBJECTIVE: Asthma is a common inflammatory disease of the respiratory system. This study aimed to investigate the effect of optineurin (OPTN) gene knockout on airway inflammation in an ovalbumin (OVA)-induced asthma mouse model. METHODS: An OVA-induced chronic asthma model was established in 6-8-week-old C57BL/6 wild-type and OPTN knockout mice. Lung inflammation and goblet cell hyperplasia were respectively evaluated by hematoxylin and eosin (H&E) staining and periodic acid-Schiff (PAS) staining. Levels of epithelial-derived alarmins (IL-33, TSLP, and IL-25), Th2 cytokines (IL-4, IL-5, and I

Peripheral Biomarker Signatures in Rheumatoid Arthritis-Associated Interstitial Lung Disease.

OBJECTIVE: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is a significant cause of morbidity and mortality among patients with RA, yet effective risk stratification for RA-ILD is lacking. We sought to characterize unique peripheral blood biomarker signatures in RA that could improve RA-ILD discrimination beyond clinical and genetic risk factors. METHODS: We performed a cross-sectional study of participants in the Veterans Affairs Rheumatoid Arthritis Registry. ILD was validated through systematic record review. Autoantibodies (n = 14), proinflammatory cytokines/chemokines

New-onset allergic diseases after SARS-CoV-2 infection: mechanistic hypotheses and emerging strategies for risk stratification.

Multinational cohort studies consistently associate SARS-CoV-2 infection with elevated incidence of allergic diseases, with hazard ratios of 2.25 for asthma and 1.23 for allergic rhinitis persisting beyond six months post-infection; whether this excess risk reflects de novo allergic sensitization or preferential unmasking of pre-existing subclinical atopy remains to be established. Yet mechanisms bridging acute viral illness to delayed allergic phenotypes remain incompletely understood. This review synthesizes recent advances across epithelial biology, immunology, and neuroimmune interactions

Erythrocytes as immunomodulators in trauma: orchestrating clearance pathways, inflammation, and organ injury.

Acute traumatic injury triggers complex systemic inflammatory and immune responses and remains a leading global cause of mortality and disability. Traditionally viewed primarily as passive oxygen carriers, erythrocytes are now emerging as key participants with significant immunomodulatory functions in the post-traumatic milieu. This narrative review presents a conceptual framework for understanding the evolving and dual-faceted role of erythrocytes in immune clearance pathways following acute injury. We focus on the known and hypothesized mechanisms by which erythrocytes actively modulate infl

Natural modulators of epithelial-immune signaling in atopic dermatitis: Molecular immunological mechanisms and translational perspectives.

Atopic dermatitis (AD) is a self-reinforcing epithelial-immune disorder in which barrier failure, alarmin release, type 2 cytokines, oxidative stress and dysbiosis converge on interconnected signaling circuits. This review critically evaluates natural products by mode of action rather than by pathway name alone. The available evidence supports coordinated suppression of NF-kB/MAPK-driven inflammatory cascades, indirect attenuation of JAK/STAT amplification, activation of Nrf2/HO-1 and AHR-dependent barrier programs, and upstream modulation of microbiota-metabolite-immune signaling. We distingu

Upper and lower airway crosstalk in acute exacerbations of COPD: a clinical and biological overview.

INTRODUCTION: Acute exacerbations of chronic obstructive pulmonary disease (AE-COPD) are acute worsening events characterized by increased dyspnea, cough, and sputum production. Although traditionally viewed as lower airway events, growing evidence suggests that AE-COPD may reflect broader pan-airway dysfunction involving both upper and lower respiratory compartments. AREAS COVERED: This overview examines upper - lower airway crosstalk in AE-COPD across three domains: pan-airway inflammation, epithelial alarmin/cytokine networks, and the continuous airway microbiome. We discuss the coexistenc

FPR1 signaling aberrantly regulates S100A8/A9 production by CD14+FCN1hi macrophages and aggravates pulmonary pathology in severe COVID-19.

Excessive alarmins S100A8/A9 escalate the inflammation and even exacerbate immune-driven thrombosis and multi-organ damage. However, the regulatory mechanisms of S100A8/A9 expression in infectious diseases remain unclear. In this study, high-dimensional transcriptomic data analyses revealed a high proportion of CD14+FCN1hi macrophages within the pulmonary niche post-severe SARS-CoV-2 infection. By constructing the S100-coexpression gene list and supervised module scoring, we found that CD14+FCN1hi macrophages presented the highest scores of alarmin S100, and possibly served as the trigger and

Damage-associated molecular patterns in the pathogenesis of osteoarthritis: potentially novel therapeutic targets.

Osteoarthritis (OA) is a chronic disease that degrades the joints and is often associated with increasing age and obesity. The two most common sites of OA in adults are the knee and hip joints. Increased mechanical stress on the joint from obesity can cause the articular cartilage to degrade and release damage-associated molecular patterns (DAMPs). These DAMPs are involved in various molecular pathways that interact with nuclear factor-kappa B and result in the transcription of inflammatory cytokines and activation of matrix metalloproteinases that progressively destroy cartilage. This review

Circadian skin pH gates IL-33 and remote food sensitization.

Skin barrier disruption can initiate the atopic march, yet comparable injuries do not invariably trigger systemic alarmin bursts. The physiological gates determining when barrier perturbation generates an endocrine signal remain incompletely defined. We hypothesized that circadian oscillations in the acid mantle regulate the threshold for systemic interleukin-33 (IL-33) release and remote allergic sensitization. Using a transepidermal water loss (TEWL)-clamped tape-stripping model to match barrier injury independent of baseline permeability, we identified a nocturnal alarmin-competent state (Z
