Sex partners of Alaskan drug users: HIV transmission between white men and Alaska Native women.

This study describes patterns of sexual behavior and condom use in a sample of 1125 Black, White, and Alaska Native drug users. Data are self-reports of recent sexual behavior, including descriptions of (up to) the five most recent sex partners. This provided information on 1116 sex partner pairs, of which at least one partner was a drug user. Sex partner pairs involving a White man and an Alaska Native woman were frequently reported. Level of condom use within these pairs was significantly lower than within all other pairs. The implications of a potential vector of HIV transmission from White

Creating a culturally appropriate web-based behavioral intervention for American Indian/Alaska Native women in Southern California: the healthy women healthy Native nation study.

Health disparities in fetal alcohol spectrum disorders (FASD) are of high importance to American Indian/Alaska Native (AI/AN) communities. We conducted focus groups and interviews with 21 AI/AN women and key informants in Southern California to modify a brief, Web-based program for screening and prevention of prenatal alcohol use. This process resulted in several important program modifications and was essential for fostering partnerships between researchers and the community, engaging community members in research, and identifying community priorities.

Social Determinants of Health Among American Indians and Alaska Natives and Tribal Communities: Comparison with Other Major Racial and Ethnic Groups in the United States, 1990-2022.

BACKGROUND AND OBJECTIVE: Limited research exists on health inequities between American Indians and Alaska Natives (AIANs), tribal communities, and other population groups in the United States. To address this gap in research, we conducted time-trend analyses of social determinants of health and disease outcomes for AIANs as a whole and specific tribal communities and compared them with those from the other major racial/ethnic groups. METHODS: We used data from the 1990-2022 National Vital Statistics System, 2015-2022 American Community Survey, and the 2018-2020 Behavioral Risk Factor Surveil

Air Pollution and Pulmonary Health in American Indian/Alaska Native Children: Describing and Addressing Inequitable Environmental Exposures.

BACKGROUND: Air pollution is a significant modifiable risk factor that exacerbates asthma and compromises overall pulmonary health. American Indian/Alaska Native (AI/AN) populations may experience inequitable exposure to air pollution due to centuries of discrimination and forced relocation. AIMS: This review summarizes studies examining air pollution and its effects on pulmonary health in AI/AN children. MATERIALS AND METHODS: A narrative review was undertaken by searching the largest medical literature databases. The search yielded ~100 articles, of which 12 are included in the review. RE

Collaborative Implementation Planning to Jumpstart Advance Care Planning with Alaska Native and American Indian peoples in Primary Care.

BACKGROUND: Palliative care, including advance care planning (ACP), ensures a patient's medical care aligns with their values, goals, and priorities throughout serious illness or injury. The Alaska Native and American Indian (AN/AI) population is greatly increasing and less likely to have documentation on ACP conversations. AN/AI peoples are more likely to engage in ACP when it is culturally-tailored to their specific needs. Jumpstart AN/AI is a culturally-tailored tool to assist providers and customer-owners with starting ACP conversations. We conducted a qualitative study to gather feedback

Implementation of a Pilot Project Program to Expand Research on Alcohol Use Disorders in American Indian and Alaska Native Communities.

BACKGROUND: Pilot project programs offer early-stage and other investigators support to pursue emerging research areas, explore new methodologies, gain experience as principal investigators, and collect pilot data needed to pursue larger extramural research funding, such as from the National Institutes of Health. Pilot project programs may be particularly important to early-stage investigators from underrepresented backgrounds, who must overcome unique challenges to launching careers in community-based participatory research. This paper describes the structure, function, and impact of the Nati

Recruitment and retention in randomized controlled trials with urban American Indian/Alaska Native adolescents: Challenges and lessons learned.

BACKGROUND: Although the majority of American Indians/Alaska Natives reside in urban areas, there are very few randomized controlled trials analyzing culturally centered substance use prevention interventions for this population. METHODS: We describe methods employed to recruit and retain urban American Indian/Alaska Native adolescents into a randomized controlled trial, which was focused on testing the potential benefits of a substance use prevention intervention for this population. We also report challenges encountered in recruitment and retention of participants and strategies employed ad

Sexual Health, STI and HIV Risk, and Risk Perceptions Among American Indian and Alaska Native Emerging Adults.

Emerging adulthood (18-25) is a period of increased risk for adverse sexual health outcomes. While anyone in this age group is at elevated risk, American Indian and Alaska Native (AI/AN) youth and emerging adults face unique factors that influence their risk for sexually transmitted infections (STIs). To address this increased risk among AI/AN youth, culturally appropriate interventions are necessary. This study reports the results of a video-based sexual health intervention designed specifically for AI/AN youth and emerging adults (15-24 years old) on risk changing perceptions. This intervent

Colonized Silence: Confronting the Colonial Link in Rural Alaska Native Survivors' Non-Disclosure of Child Sexual Abuse.

Though there is evidence linking the history of colonialism and oppression of Indigenous people to the high rates of rape and child sexual abuse experienced by this population today, it is less understood how colonial processes, past and present, condition the decision to disclose or report victimization. Drawing on a survivorship storytelling study of rural Alaska Native survivors of child sexual abuse, this paper underscores the importance of centering colonialism in understanding the culture of silence surrounding sexual victimization among Indigenous people. Results show that reasons for n

Impact of home remediation and household education on indoor air quality, respiratory visits and symptoms in Alaska Native children.

Alaska Native children experience high rates of lower respiratory tract infections (LRTIs) and lung conditions, which are associated with substandard indoor air quality (IAQ). We conducted an intervention of home remediation and education to assess the impact on IAQ, respiratory symptoms and LRTI visits. We enrolled households of children 1-12 years of age with lung conditions. Home remediation included improving ventilation and replacing leaky woodstoves. We provided education about IAQ and respiratory health. We monitored indoor airborne particles (PM2.5), CO2, relative humidity and volatile

Resources, roadblocks and turning points: a qualitative study of American Indian/Alaska Native adults with type 2 diabetes.

Type 2 diabetes is a worldwide health problem that has reached epidemic proportions in some communities. Alaska Native and American Indian (AN/AI) people are disproportionately diagnosed with type 2 diabetes and incidence is increasing in many Alaska communities. Developing effective interventions requires understanding the social and psychological factors that impact effective management of diabetes, yet little is known about these factors in AN/AI communities. The objective of this study was to explore perceived psychosocial needs and barriers to management of diabetes among AN/AI adults wit

A workshop considering genetic research and data collection with American Indian and Alaska Native people outside of Tribal jurisdiction.

INTRODUCTION: In the United States (US), Tribes are sovereign nations and have the right to oversee research conducted with Tribal citizens. However, it is unclear who should approve research protocols when data from American Indian and Alaska Native (AIAN) people are collected off Tribal lands. As genetic research continues to advance and transform the delivery of healthcare, equitable inclusion of AIAN people is necessary, but oversight of research needs clarity. METHODS: We held a 3-day workshop with US thought leaders on genetic and other health research with AIAN people in urban areas to

Engagement and completion of evidence-based psychotherapy for PTSD among American Indian/Alaska Native, Latino, and White veterans.

OBJECTIVE: Among psychotherapeutic treatments for posttraumatic stress disorder, prolonged exposure, and cognitive processing therapy have distinguished themselves with their high efficacy and strong backing in the treatment literature. However, low patient retention undermines the benefit of these evidence-based psychotherapies. Underrepresentation of Latinx and American Indian/Alaska Native (AI/AN) veterans in posttraumatic stress disorder research has rendered it difficult to determine ethno-racial disparities in patient retention. Despite serving in the U.S. military at high rates, Latino
