Deep dyslexic phenomena in a letter-by-letter reader.

Numerous accounts of pure alexia have suggested that prelexical impairment precludes rapid access to orthographic information in patients with the disorder. We report a patient with features of both pure and partially recovered deep dyslexia in whom we demonstrate prelexical deficits in maintaining a reliable abstract representation of the right side of letter arrays, as well as in modulating a "spotlight" of visual attention. These deficits, we suggest, encourage the patients's use of a letter-by-letter reading strategy; despite them, however, he demonstrates rapid, accurate reading of some,

[Pure agraphia with topographical disorientation caused by right hemisphere lesion].

A 69-year-old, right-handed, Japanese male patient presented with pure agraphia with topographical disorientation after hemorrhage in the right parietal lobe. Upon developing cerebral hemorrhage, he was referred to our hospital for close examination of agraphia. There was no paresis or clumsiness in his extremities. His speech was fluent and well-articulated. Neither aphasia nor reading impairments was found, although there was a clear writing impairment with effort and hesitation. His writing of both kanji and kana letters contained additional, absent or deformed strokes or parts. No hemianop

Reversed laterality of cerebral functions in a non-right-hander: neuropsychological and spect findings in a case of 'atypical' dominance.

A 54-year-old non-right-handed man with positive familial sinistrality showed a pure right hemisphere syndrome following a left hemisphere stroke. Severe right side hemineglect, transcortical motor dysprosodia, spatial dysgraphia and visuo-constructive impairments were observed. At no time were the expected left hemisphere abnormalities such as aphasia, alexia, right-left disorientation or finger agnosia noted. A left fronto-temporal subcortical lesion was documented on CT scan. A Tc-99m HM-PAO SPECT study revealed no cerebral blood flow changes in the right hemisphere while in the left hemisp

Progressive Wallerian Degeneration of the Corpus Callosal Splenium in a Patient with Alexia Without Agraphia: Advanced MR Findings.

The corpus callosal splenium is an uncommon location for Wallerian degeneration (WD), which may be mistaken for new pathology on magnetic resonance imaging (MRI). We describe the case of a 69-year-old woman with a left posterior cerebral artery infarct in whom progressive WD of the splenium of the corpus callosum seen on MRI was misinterpreted as new infarction or neoplasm. We address how magnetic resonance spectroscopy, perfusion MRI, diffusion tensor MRI, and serial imaging were utilized in establishing the correct diagnosis. Interestingly, the patient also presented with alexia without agra

Hyperacute cognitive stroke syndromes.

Cognitive syndromes are common clinical manifestations of hyperacute stroke and may be the single or dominant presenting features. They are related to acute dysfunction of complex integrated distributed functional networks serving different cognitive domains. The most common cortical syndromes include nonfluent or fluent aphasia, neglect, collor agnosia, pure alexia and Balint's syndrome. Disturbances of declarative memory are common following posterior cerebral artery and thalamic strokes. Abulia can follow thalamic, caudate and capsular lesions. Intraventricular and subarachnoid haemorrhages

Why are digits easier to identify than letters?

Beginning with Dejerine's report of pure alexia in 1892, numerous researchers have noted that individuals with acquired impairments of reading may show spared digit identification performance. This digit advantage has also been found in unimpaired adult readers across a number of tasks, and five main hypotheses have been proposed to explain how it arises. In this paper I consider these hypotheses in the context of recent theories of a unified alphanumeric character identification system, and evaluate them according to relevant empirical evidence. Despite some promising findings, none of the hy

How Does iReadMore Therapy Change the Reading Network of Patients with Central Alexia?

Central alexia (CA) is an acquired reading disorder co-occurring with a generalized language deficit (aphasia). The roles of perilesional and ipsilesional tissue in recovery from poststroke aphasia are unclear. We investigated the impact of reading training (using iReadMore, a therapy app) on the connections within and between the right and left hemisphere of the reading network of patients with CA. In patients with pure alexia, iReadMore increased feedback from left inferior frontal gyrus (IFG) region to the left occipital (OCC) region. We aimed to identify whether iReadMore therapy was effec

Visual word recognition in the left and right hemispheres: anatomical and functional correlates of peripheral alexias.

According to a simple anatomical and functional model of word reading, letters displayed in one hemifield are first analysed through a cascade of contralateral retinotopic areas, which compute increasingly abstract representations. Eventually, an invariant representation of letter identities is created in the visual word form area (VWFA), reproducibly located within the left occipito-temporal sulcus. The VWFA then projects to structures involved in phonological or lexico-semantic processing. This model yields detailed predictions on the reading impairments that may follow left occipitotemporal

Two systems for colour-naming defects: verbal disconnection vs colour imagery disorder.

Two subjects affected by pure alexia and showing no central dyschromatopsia or generalized aphasia, performed poorly on traditional tasks with visually-presented colour stimuli and on tasks with objects presented verbally. Three experiments were conducted to evaluate the possible role of mental colour imagery in recalling the colours of objects from memory. It was concluded that Case I, with left occipital lobe softening, had preserved imagery systems, but failed to recode the colours of mentally generated colour images, just as he failed to name visually presented colours, suggesting a langua

The Visual Agnosias and Related Disorders.

BACKGROUND: There are many disorders of higher visual processing that result from damage to specific areas of the cerebral cortex that have a specific role in processing certain aspects (modalities) of vision. These can be grouped into those that affect the ventral, or "what?", pathway (e.g., object agnosia, cerebral achromatopsia, prosopagnosia, topographagnosia, and pure alexia), and those that affect the dorsal, or "where?", pathway (e.g., akinetopsia, simultanagnosia, and optic ataxia). EVIDENCE ACQUISITION: This article reviews pertinent literature, concentrating on recent developments i

Face and Word Recognition Can Be Selectively Affected by Brain Injury or Developmental Disorders.

Face and word recognition have traditionally been thought to rely on highly specialised and relatively independent cognitive processes. Some of the strongest evidence for this has come from patients with seemingly category-specific visual perceptual deficits such as pure prosopagnosia, a selective face recognition deficit, and pure alexia, a selective word recognition deficit. Together, the patterns of impaired reading with preserved face recognition and impaired face recognition with preserved reading constitute a double dissociation. The existence of these selective deficits has been questio

Using errorless learning to treat letter-by-letter reading: contrasting word versus letter-based therapy.

Some pure alexic readers have been shown to activate lexical and semantic knowledge under brief presentation conditions. This ability is not seen when letter-by-letter reading accuracy is high or the reading impairment is very severe. It is also unlikely to occur under normal untimed presentation because the pure alexic will make deliberate use of their letter-by-letter strategy. This paper presents data from a moderately severe letter-by-letter reader, FD, who had visual processing problems affecting reading. He also had other mild aphasic characteristics. FD showed implicit reading abilities

Transitory alexia without agraphia: a disconnection syndrome due to neurocysticercosis.

We describe a 65-year-old male who presented with acute onset inability to read, without any difficulty in writing. A clinical diagnosis of alexia without agraphia was made and the patient was subjected to routine investigations including contrast MRI. MRI showed a ring-enhancing lesion in left occipital area, suggestive of neurocysticercosis supported by quantitative enzyme-linked immunosorbant assay from purified cell fraction of taenia solium cysticerci (PCF-ELISA). Patient was treated with albendazole and prednisolone for one week. The clinical manifestation as well as the radiological fin

Consciousness of perception after brain damage.

Disturbances of visual perception after brain damage provide clues to understanding consciousness and the brain. In this article we review six visual disorders in which perception and consciousness are dissociated as a result of brain damage: blindsight, implicit shape perception in apperceptive visual agnosia, covert recognition of faces in prosopagnosia, unconscious perception in neglect and extinction, implicit reading in pure alexia, and implicit object recognition in associative visual agnosia. We consider these six disorders from the standpoint of three main schools of thought concerning

[The reader brain: natural and cultural story].

The report of cases of pure alexia suggests that some regions of the neural system are dedicated to reading-specific visual processing abilities. Pure alexia results from the disruption or the disconnection of the visual word form area, a region reproducibly located within the left occipito-temporal sulcus and encoding the abstract identity of strings of visual letters. The functional specialization of this area suggests that it is initially plastic and becomes attuned to the orthographic regularities that constrain letter combination during the acquisition of literacy. The visual word form ar
