Heart surgery in patients from high-altitude areas is more challenging than usual. Few studies have been published on this issue, and none of them have discussed the effect of an altitude change (from high to low altitude) on a patient's physiology or its effects on a patient's perioperative management. Here, we present the case of a 46-year-old man who was a long-time resident of Tibetan area in Sichuan (altitude >3000 m) who underwent Stanford type A aortic dissection emergency surgery on the plain. Anesthetic management occurred through monitoring of the bispectral index (BIS) and transesop

Mild-to-moderate closed-head injury (mmCHI) is an acute disease induced by high-altitudes. It is general practice to transfer patients to lower altitudes for treatment, but the pathophysiological changes at different altitudes following mmCHI remain unknown. The present study simulated acute high-altitude exposure (6,000 m above sea level) in rats to establish a model of mmCHI and recorded their vital signs. The rats were then randomly assigned into different altitude exposure groups (6,000, 4,500 and 3,000 m) and neurological severity score (NSS), body weight (BW), brain magnetic resonance im

At high altitudes, which typically exceed 2500 m, approximately 80 million people reside permanently, with over a million visitors annually. The primary effect of high altitude is hypobaric hypoxia, which leads to decreased oxygen availability and a cascade of physiological responses. However, inadequate or excessive responses can lead to malacclimatization, resulting in hypoxemia and various high-altitude illnesses, including acute mountain sickness (AMS), high-altitude cerebral edema (HACE), high-altitude pulmonary edema (HAPE), chronic mountain sickness (CMS), and high-altitude pulmonary hy

Oxygen is vital for cellular metabolism; therefore, the hypoxic conditions encountered at high altitude affect all physiological functions. Acute hypoxia activates the adrenergic system and induces tachycardia, whereas hypoxic pulmonary vasoconstriction increases pulmonary artery pressure. After a few days of exposure to low oxygen concentrations, the autonomic nervous system adapts and tachycardia decreases, thereby protecting the myocardium against high energy consumption. Permanent exposure to high altitude induces erythropoiesis, which if excessive can be deleterious and lead to chronic mo

It is known that high-altitude trips cause nasal congestion, impaired nasal mucociliary transport rate, and increased nasal resistance, due to decreased partial oxygen pressure and dry air. It is also known that olfactory perception is affected by barometric pressure and humidity. The aim of the present study was to investigate whether olfactory function changes in relation to high altitude in a natural setting. The present study included 41 volunteers with no history of chronic rhinosinusitis or nasal polyposis. The study group consisted of 31 men (76 %) and 10 women (24 %); the mean age of t

OBJECTIVE: This study aimed to compare 3 treatment modalities during sleep at an altitude of 5300 m to identify strategies for reducing the incidence of periodic breathing at high altitude.
METHODS: Fifteen trekkers, with identical ascent profiles and no signs or symptoms of altitude illness, served as subjects. All study participants arrived at 5300 m after a gradual ascent from 1300 m. On their second night at 5300 m, subjects were randomly assigned (with a computer-based random assignment procedure) to 1 of 4 different treatment groups: control (n = 4); 1 L/min O(2) via a demand system dur

An ascent to altitude places considerable demands on the cardiovascular system. Changes in the rate, rhythm, and morphology of the electrocardiogram reflect the fall in the partial pressure of inspired oxygen (PiO2) and the adaptive responses that the human body makes. The effect of hypoxia on the autonomic nervous system results in changes to the heart rate during rest and exercise. Although this is raised during rest and submaximal exercise, maximal heart rate at altitude is reduced as a result of changes in parasympathetic activity. Hypoxic pulmonary vasoconstriction leads to a rise in pulm

Based upon the European Union (EU)-Directive 96/29/EURATOM, legal regulations on the radiation protection of aircrew had to be implemented into the corresponding national law within the member states of the EU by 13 May 2000. In Article 42 the directive stipulates, among other things, that the exposure of the crew concerned shall be assessed. This requirement has been implemented by dose calculations for most aircrew members in the EU. Some airlines and research institutes regularly spot check the calculated doses by measuring flights. The solar minimum is a time period of particular interest

UNLABELLED: Acute mountain sickness (AMS) develops when rapidly ascending to high altitudes. However, some mountaineers will suffer from AMS even at 2,000 m and others not until 5,000 m. The awareness of the individual susceptibility for AMS would be helpful for preventive strategies. Thus, the main purpose of this paper is the comparison of existing studies dealing with the prediction of AMS susceptibility and to draw conclusions on presently most valuable tests.
DATA SOURCE: A PubMed search has been performed, and preliminary observations from our laboratory have been included. The cautious

BACKGROUND: The aim of the study was to document link between hyperventilatory capacity and risk for developing acute mountain sickness (AMS).
METHODS: This study was carried out at Karakorum Mountain ranges (Northern areas of Pakistan) from March till July 2004. 54 healthy male subjects were enrolled in this study. Arterial oxygen saturation (SpO2) of the subjects was measured by the pulse oximeter at rest and after 1 minute of voluntary hyperventilation at an altitude of 2833 meters. Symptoms of acute mountain sickness (AMS) were recorded on a questionnaire by using the Lake Louise consensu

Mercury-independent devices are increasingly being used in clinical practice as mercury will soon be removed from clinical use as a result of environmental, health and safety concerns. The aim of this study was to evaluate the accuracy of a portable aneroid device in an adult population at high altitude by following the part of the protocol of the British Hypertension Society regarding comparison between device and observer. We examined 10 subjects in Cerro de Pasco, Peru, which is situated 4370 m above sea level. The aneroid device was initially calibrated at both high altitude and at sea lev

The perinatal cardiopulmonary transition at high altitude differs from that at sea level because oxygen plays a fundamental role in the developmental changes from fetus to newborn infant. Under conditions of high altitude hypoxia, arterial oxygen saturations are lower, breathing patterns and maturation of respiratory control reflexes differ, and regression of fetal characteristics of the pulmonary vasculature proceeds more slowly. Several aspects of transition vary not only with postnatal age and altitude, but also with population group, suggesting an effect of genetic adaptation on perinatal

BACKGROUND: Total handwear insulation (I(T)) is dependent on the rate of heat transfer in air through the skin-handwear interface, handwear layers, and the surface boundary air layer. As altitude increases, the corresponding decrease in air pressure reduces convective heat loss. As convective heat losses decline, I(T), which is inversely related to the rate of heat loss, should increase. Increasing air velocity also reduces the insulation (Ia) provided by the boundary layer.
METHODS: The military issue test handwear, Light-duty glove (LD), Trigger-finger mitten (TF), and Arctic mitten (AM), w

Acute mountain sickness (AMS) has long been recognised as a potentially life-threatening condition afflicting otherwise healthy normal individuals who ascend rapidly to high altitude where the partial pressure of oxygen (pO2) in the air is reduce. The symptoms of AMS (e.g. headache, poor appetite and nausea, fatigue and weakness, dizziness or light-headedness and poor sleep) are probably a consequence of disturbances in fluid balance brought about by severe tissue hypoxia. AMS can be prevented by an adequately slow ascent, which is the best method, but for those with limited time there are sev

One of the most important factors in the acclimatization of lowlanders to high altitude is hyperventilation which helps to defend the alveolar PO2. However, how rapidly this occurs at very high altitude is poorly understood. Information can be obtained by comparing the alveolar gas values reported from the extended low pressure chamber studies, Operation Everest I and II, and the American medical research expedition to Everest (AMREE) of 1981. Rahn and Otis (1949) reported the alveolar PO2 and PCO2 values for non-acclimatized and well-acclimatized man on an O2-CO2 diagram, and pointed out that

A radioimmunoassay showed that the stay of intact animals in the highlands (the pass of Tuya-Ashu, 3200 m) was accompanied by phase changes in the blood content of the thyroid hormones and thyrotropin (TSH). An increase in the T4 concentration in the blood of healthy animals on the first days of adaptation was determined by thyroid activation. Further on thyroid activity suppression was observed in parallel with a decrease in the production of the thyroid hormones. The thyroid of animals with thyrotoxicosis reacted less to a stress situation (climbing the mountain) than that of intact animals.

Hyperventilation is one of the most important features of acclimatization to high altitude. Resting ventilation at extreme altitudes increases up to fourfold and exercise ventilation for a given work level increases to the same extent. Hypoxic stimulation of the peripheral chemoreceptors is the chief mechanism for the hyperventilation but there is also evidence that central sensitization of the respiratory centres occurs. Permanent residents of high altitude have a blunted hypoxic ventilatory response compared to acclimatized lowlanders. Cardiac output increases in responses to acute hypoxia b
