Surgical Burden of Traumatic Lower Limb Amputations in the Pediatric Age Group.

Although the burden of traumatic lower limb amputation (TLLA) has been well described when it concerns the adult population, there is an evident scarcity in literature concerning this matter in the pediatric population. Our objective is to review the surgical burden and long-term outcome of lower limb amputation among pediatric trauma victims who have experienced lower extremity amputation as the result of an accident or injury. A PICO format was utilized. The population of interest includes only children and adolescents suffering TTLA. The generation of data to be reviewed was executed using

Traumatic Amputations Treated in US Emergency Departments: A Review of the NEISS Database.

BACKGROUND: Trauma is the second most common cause of limb loss in the United States (US), second only to vascular disease. The aim of this study was to evaluate the demographics and commercial products associated with traumatic amputations in the United States. METHODS: The National Electronic Injury Surveillance System (NEISS) database was analyzed from 2012 to 2021 to identify patients presenting to the Emergency Department (ED) with the diagnosis of amputation. Additional variables included patient demographics, body part amputated, commercial products associated with amputation, and ED t

Management of Traumatic Radial and Ulnar Artery Injuries and Risk Factors for Amputation.

INTRODUCTION: Traumatic injuries to the radial and/or ulnar arteries represent a subset of arterial injuries. In the absence of injury to both forearm arteries, treatment was historically ligation if perfusion was maintained to the hand via the uninjured vessels or adequate collateral vessels. We sought to determine management of traumatic forearm arterial injuries in 2019 and to identify risk factors for major upper extremity amputation. METHODS: The American College of Surgeons Trauma Quality Improvement Program database was queried by International Classification of Diseases 10 code for pa

Investigation of effective factors on traumatic amputations due to road traffic accidents.

AIM: To determine factors associated with the locations, types and levels of traumatic limb amputations caused by road traffic accidents. MATERIALS AND METHODS: The files sent by the courts to the expert committee chaired by the authors for the determination of applicants' disability rate over the 10-year period between 2011 and 2020 were examined retrospectively. RESULTS: Of the 27 cases with traumatic amputations caused by road traffic accidents, 63% of them were male and the mean age of the cases was 29.0 ± 20.0 years. The cases comprised 12 passengers, 10 pedestrians, and 5 drivers. Of a

Replantation versus Prosthetic Fitting in Traumatic Arm Amputations: A Systematic Review.

BACKGROUND: Traumatic arm amputations can be treated with replantation or surgical formalization of the stump with or without subsequent prosthetic fitting. In the literature, many authors suggest the superiority of replantation. This systematic review compared available literature to analyze whether replantation is functionally and psychologically more profitable than formalization and prosthetic fitting in patients with traumatic arm amputation. METHODS: Functional outcome and satisfaction levels were recorded of patients with amputation levels below elbow, through elbow, and above elbow.

The use of forearm free fillet flap in traumatic upper extremity amputations.

BACKGROUND: Complete traumatic upper extremity avulsions are an infrequent but devastating injury. These injuries are usually the result of massive blunt trauma to the upper limb. Intact issue from amputated or nonsalvageable limbs may be transferred for reconstruction of complex defects resulting from trauma when the indications for replantation are not met. This strategy allows preservation of stump length or coverage of exposed joints, and provides free flap harvest for reconstruction without additional donor-site morbidity. METHODS: A retrospective review at São João Hospital was performe

Role of temporary arterial shunt in the reimplantation of a traumatic above-elbow amputation.

Although the feasibility of reimplantation has increased greatly with the development of new surgical techniques, long-term functional prognosis is unpredictable. Injury to a major nerve is an important factor in long-term outcome, and prolonged ischemia time also has a negative effect. We present a 26-year-old woman whose right arm was sectioned above the elbow in an accident. The limb presented a complete section, with soft tissue loss of the humeral artery and vein, basilic vein, and cephalic vein. Median and radial nerve tissue was missing, and a supracondylar fracture with substantial los

[The dynamics of the autonomic disorders in the upper extremities replanted after traumatic amputation].

The changes in vegetative reinnervation of replanted upper limbs were followed up in 71 patients. On posttraumatic month 1-2 there was a tendency to predominance of the replant sympathetic tone. The latter declined within the next 22 months. Vegetative reinnervation continued 1-2 years and more, depending on the time of stabilization of various vegetative abnormalities.

A case of heterotopic transplantation after bilateral traumatic above-knee amputations.

This paper describes a case in which a young woman's two lower limbs were crushed and amputated from the upper part of the thighs by a train. She was in serious hypovolemic shock at the time. With rapid infusion of intravenous fluids, she recovered from shock quickly, avoiding acute renal failure and ARDS. Heterotopic transplantation was performed. Two years later, the limb which had undergone heterotopic transplantation had partly restored function.

Traumatic amputation of the external ear: the War of Jenkins' Ear and George Washington.

Given the right circumstances, a traumatically severed ear can now successfully be reimplanted. The career of George Washington was influenced by the involvement of his half brother Lawrence Washington in a war (1739 to 1742) fought between England and Spain on account of a severed ear. It was called The War of Jenkins' Ear.

Traumatic bilateral testicular amputation in a 31 year old male: A case report.

INTRODUCTION AND IMPORTANCE: Testicular loss following assault is a very rare occurrence and uncommon in the low income countries. Testicular amputation is usually associated with blast injuries, sexual violence and self-mutilation in psychiatric cases. Management involves a multidisciplinary approach and is focused on resuscitation, aggressive debridement, testosterone supplementation, and reconstructive surgery. CASE PRESENTATION: We present a 31 year old male referred to the emergency department of a Regional Referral Hospital in western Uganda with a history of trauma to his scrotum. The

Long-term health effects after conflict-related traumatic amputation among patients in Gaza.

BACKGROUND: In the past 10-15 years, thousands of civilians in Gaza have experienced conflict-related traumatic injuries. How injuries affect survivors' risks of negative long-term health effects and serious illness is unclear. We report follow-up findings in a group of patients with traumatic amputations. METHODS: Eligible patients had traumatically amputated limbs and showed signs and symptoms of possible serious illness on standardised clinical examination. The patients were all receiving rehabilitation treatment at the Artificial Limbs and Polio Centre, Gaza, which is the main provider of

Walking speed differentially alters spinal loads in persons with traumatic lower limb amputation.

Persons with lower limb amputation (LLA) perceive altered motions of the trunk/pelvis during activities of daily living as contributing factors for low back pain. When walking (at a singular speed), larger trunk motions among persons with vs. without LLA are associated with larger spinal loads; however, modulating walking speed is necessary in daily life and thus understanding the influences of walking speed on spinal loads in persons with LLA is of particular interest here. Three-dimensional trunk-pelvic kinematics, collected during level-ground walking at self-selected (SSW) and two controll

Hospital Quality and Performance of a Complex Surgical Procedure after Traumatic Digit Amputation.

BACKGROUND: Traumatic digit amputations are prevalent injuries that have long-term disabling consequences. Although replantation after traumatic digit amputation is a complex procedure, the aesthetic, functional, and long-term economic benefits of replantation render it preferable to revision amputation when clinically indicated. The authors adapted the Donabedian quality-of-care conceptual framework to examine the association between hospital outcome quality measured by observed-to-expected mortality ratio and the treatment received after traumatic digit amputation. The authors hypothesized t

Use of buccal mucosa to simulate the coronal sulcus after traumatic penile amputation.

Traumatic penile amputation is a rare but catastrophic complication of newborn circumcision, usually involving a portion of, or the entire, glans penis. Emergent reimplantation is necessary to prevent glans loss. However, loss of the coronal sulcus might occur despite successful reimplantation, resulting in a cosmetically unacceptable appearance. We report such a case involving a newborn who suffered partial penile glans amputation and loss of the coronal sulcus after reimplantation, successfully treated with buccal mucosa grafting.
