The history of AV nodal reentry.

Though patients with AV nodal reentry are now routinely cured by catheter ablation, the basic mechanism of this disorder is still under debate. The putative mechanism of AV node reentry was first discovered by the elegant work of Gordon Moe. He demonstrated the existence of dual pathways and echo beats in rabbits. Building on these seminal observations, the mechanism of AVNRT has burgeoned to include the possibility of left atrial input into the node. The first curative nonpharmacologic procedures involved surgical dissection around the AV node and the procedure was rapidly supplanted by cathe

Dominant maxillary artery as a cause of failure in maxillary artery ligation for posterior epistaxis.

Ligation of the maxillary artery is a logical and effective method for the arrest of severe uncontrollable posterior epistaxis. The failure rates for arrest of haemorrhage are given in the literature as 10-13%. In our centre, over the last 9 years, 23 patients have undergone maxillary artery ligation to control epistaxis without a failure. Bilateral maxillary artery ligation was carried out whenever the maxillary artery of the bleeding side was found to be of small diameter. To investigate whether there is an asymmetry in the size of the maxillary arteries we performed 13 cadaveric dissections

The Musculus Uvulae Muscle and the Effect of Furlow's Palatoplasty on It.

The musculus uvulae (m. uvulae) is an often overlooked, albeit crucial anatomic structure within the human soft palatal musculature. It is one of 5 paired muscles that contribute to a variety of physiological functions, including speech, deglutination, and prevention of nasal regurgitation. Over the previous 60 years, there has been contention among surgeons and anatomists as to the functional anatomy of the musculus uvulae and its contribution to the competence of the velopharyngeal sphincter, both actively and passively. Furlow's palatoplasty is widely used in repair of both primary and seco

Strangulated Littre's direct inguinal hernia: a case report and literature review.

The rare occurrence of a Meckel's diverticulum within a hernial sac, known as a Littre's hernia, was first described by the French anatomist and surgeon Alexis Littré in the 18th century. This condition is observed in only 0.09% of strangulated hernias. Patients typically present with an irreducible lump in the groin. In this case report, we present a 66-year-old man who sought medical attention with a 72-hour history of an incarcerated inguinal hernia. Patient underwent hernia repair and intraoperatively, strangulated Meckel's diverticulum, adhered medially to the base of the sac, was identif

The Bicentenary of Bell's Description of the Neuroanatomical Basis of Facial Paralysis: Historical Remarks.

Charles Bell was a talented and versatile Scottish anatomist, neurophysiologist, artist, and surgeon. On July 12, 1821, he reported his studies regarding facial innervation in the essay "On the Nerves," read before the Royal Society in London. Since then, idiopathic peripheral facial paralysis has been named "Bell's palsy." He was the first author to describe the neuroanatomical basis of facial paralysis, in an essay enriched by beautifully self-made illustrations. The aim of this article is to trace the history of Bell's description of the neuroanatomy of the facial nerve, reexamining his 182

Does the Venus de Milo have a Spinal Deformity?

The Venus de Milo, an ancient Greek statue, has been viewed as one of the most celebrated pieces of art in Western culture. It was sculpted during the Hellenistic period between 150 and 50 BC and is believed to be the work of Alexandros of Antioch. The sculpture is thought to depict Aphrodite, the Greek goddess of love and beauty. When assembled, the two halves of the sculpture meet in an almost horizontal line that is purposefully obscured by a roll of garment around the hips. It has been noted that the midline of the statue's face is displaced slightly. German anatomist von Henke observed th

Unusual Branching Pattern of the Lateral Cord of the Brachial Plexus Associated with Neurovascular Compression: Case report.

The brachial plexus consists of a network of nerves that innervates the upper limbs and its musculature. We report a rare formation of the lateral cord of the brachial plexus observed during the dissection of a 47-year-old male cadaver at the Department of Anatomy, Vardhman Mahavir Medical College, New Delhi, India, in 2016. The lateral cord was exceptionally long with twin lateral pectoral nerves and twin lateral roots of the median nerve. The proximal lateral root of the median nerve was thin in comparison to the medial root of the median nerve. The distal lateral root of the median nerve wa

The. Thoma Ionescu - Victor Gomoiu Procedure: Cervicothoracic Sympathectomy for Angina Pectoris. The First Surgical Attempt to Treat the Coronary Heart Disease.

Cervicothoracic Sympathectomy is a common indication in the treatment of Raynaud Syndrome, Palmer Hyperhidrosis or Acute Ischemia of the superior limb. Nonetheless, almost a century ago it represented one of the first innovative attempts in curing coronary heart disease. Nowadays, this indication is no more than a footnote in a volume on the History of Medicine, and a trivia fact for medical history enthusiasts. The operation's history is rather conflicting. A young Romaninan surgeon, Victor Gomoiu seems to have come up with the idea, in the early 20th century. However, his contribution remain

Matters of priority: Herbert Mayo, Charles Bell and discoveries in the nervous system.

Between 1822 and the late 1830s a highly personal priority dispute was fought between the celebrated surgeon and anatomist Sir Charles Bell and his ex-student Herbert Mayo. The dispute was over the motor and sensory functions of the Vth and VIIth cranial nerves. Over the course of the 1820s and the 1830s, the competing claims of Bell and Mayo were presented in newspapers, journals, and textbooks. But by the time of Bell's death in 1842, Mayo had been discredited, a seemingly tragic footnote in the history of nervous discovery. And yet, with the benefit of hindsight, Bell's case was at best dis

Wilhelm His Jr. (1863-1934)--the man behind the bundle.

Wilhelm His Jr. (1863-1934) was a Swiss-born cardiologist and anatomist. In 1893, he discovered the bundle of His-the specialized tissue in the heart that transmits the electrical impulses and helps synchronize contraction. He pioneered studies in cardiac conduction and coined the term "heart block," which he identified as the cause of Adams-Stokes disease. He was a professor of medicine at the University of Berlin, became the dean of medicine in 1918 and in 1928 was elected rector of the University of Berlin. Through his long clinical and teaching career he became better known for his work on

August Knoblauch and amusia: a nineteenth-century cognitive model of music.

Early models of human cognition can be traced to nineteenth-century investigations of brain and behavior. Influential neurologists such as Wernicke, Kussmaul, and Lichtheim constructed diagrammatic models to illustrate current theories of cognition. Language was the most commonly studied cognitive function during this time; however, investigators also studied other cognitive functions, such as music and visual processing. While a number of nineteenth-century neurologists made observations about music abilities in aphasic patients, August Knoblauch, a German physician and anatomist, was the fir

A reliable method for Golgi staining of retina and brain slices.

Although the classical Golgi method is a powerful means for structural analysis of the brain, it is generally considered to be an unpredictable technique making anatomists wary of using it. Often, even when successful staining has occurred, deposits of silver chromate crystals on the surface of the tissue obscure examination. This paper describes a simple procedure for Golgi impregnation of retina and brain slices so that good, even staining is obtained and crystal formation is avoided. The most outstanding feature of the method is the consistency of results. This consistency is due to two fac

[Felix Platter (1536-1614) as ophthalmologist].

Felix Platter, physician and anatomist, had a keen interest in ophthalmology, too. By his wife, he was related to a family of surgeons; this may have brought him into closer contact with eye diseases and their cure. Platter was the first physician to attribute the sensory power of the eye to the retina and a purely optical function to the lens (humor crystallinus). The present author mentions two original observations of his: the symptomatology of posterior vitreous detachment (observed in his own myopic left eye) and a case of blindness due to compression of the optic nerves by a cranial tumo

Clinical significance and measurement of the length of the right main bronchus.

It is important for surgeons, pathologists, anesthetists and anatomists to know the length of the right main bronchus. It extends from the carina of the trachea to the origin of the right upper lobe bronchus, but an exact method for measuring it has never been described. Using bronchography, the authors measured the length of the right main bronchus in 24 patients. The posteroanterior projection taken at a standard distance (1.8 m) from the patient was used to minimize distortion due to the technique; if present, the distortion would not be more than 5% and would be an increase rather than a d

An accessory sphenoidal foramen of the middle cranial fossa detected on computed tomography.

BACKGROUND: The skull base depicts significant morphological variability, which is frequently described due to its neurosurgical significance. The middle cranial fossa's accessory foramen has rarely been described. MATERIALS: A 53-year-old female patient's computed tomography (CT) scan was further investigated for its unusual morphology. RESULTS: On the left-sided middle cranial fossa, an accessory sphenoidal foramen (ASF) was observed, located 3.3 mm posterior to the foramen rotundum (FR) and 5.5 mm anterior to the foramen ovale (FO). Extracranially, the ASF opened into the infratemporal fo

A Morphological Study of Assimilation of Atlas Vertebrae and their Clinical Implications in Indian Population.

BACKGROUND: Atlas is the first cervical vertebra. Rarely, does the atlas vertebra fuse with the occipital bone, and this occipitalization of the atlas is known as the assimilation of the atlas. Congenital fusion of the atlas with the occipital bone is the common denominator of a galaxy of skull defects, several examples of bony distortion of the foramen magnum were reported by early anatomists. MATERIALS AND METHOD: A total of 210 dry skulls of unknown sex and age from Dr. Rajendra Gode Medical College, Amravati, Department of Anatomy, and various other medical, Dental, and Ayurveda colleges

Treatment of dynamic scapholunate instability dissociation: Contribution of arthroscopy.

Scapholunate (SL) ligament injuries generally result from trauma to an extended, supinated wrist, leading to chronic instability that progresses toward osteoarthritis. They can occur together with distal radius fracture. These injuries can be difficult to diagnose, especially early on. Treating chronic SL ligament injuries before the onset of osteoarthritis is still a challenge for surgeons. Until recently, the recommended treatments consisted of open SL ligament reconstruction or repair procedures that reduce pain and improve pinch strength but also lead to wrist stiffness. The introduction o

Dermatoglyphic Pattens in OSMF and Leukoplakia - A Comparative Study.

The term dermatoglyphics was first used in 1926 by anatomist Cummins. Dermatoglyphic is a term that refers to "a skin carving." The unique carve present on the skin surface of humans and other living beings is referred to as dermatoglyphics. This research was conducted to understand the variations in patterns and assess the usefulness of dermatoglyphics among individuals diagnosed with potentially malignant diseases and mouth cancer. Three hundred subjects were selected. The study consisted of three groups. Patients were divided into the following groups: Group I: oral submucous fibrosis (OSMF

The latest anatomical discovery: the parathyroid glands or Glandulae parathyreoideae of Owen-Sandström-Gley.

In 1852, Owen, a prominent British anatomist, described the parathyroid glands. While dissecting a rhinoceros, he noted a small compact yellow body, attached to the thyroid. Virchow and later Remak described the human parathyroids around 1960, but credit for the first complete description goes to the Sandström in 1980. More than a decade later Gley, showed that it was the removal of the parathyroids that accounted for the tetany that followed thyroidectomy. The association of parathyroid pathology and skeletal abnormalities was made in 1914 by Erdheim and Schlagenhaufer, and Mandl, was the fir

Surgical anatomy of the lymphatic drainage of the salivary glands: a systematic review.

BACKGROUND: Nodal metastasis in salivary gland malignancies has important clinical implications; a good understanding of their complex anatomy is paramount to the head and neck surgeon. METHODS: A contemporary and comprehensive literature review was conducted of the lymphatic drainage of the salivary glands, with special emphasis on its surgical applications. RESULTS: The parotid gland has extraglandular and intraglandular nodes acting as a single functional drainage unit. Intraglandular parotid notes are unique to the parotid gland, and consist of a larger superficial group and a smaller de
