Neuraxial Anesthesia and Analgesia During Cardiothoracic Surgery: A Narrative Review.

PURPOSE OF REVIEW: The purpose of this review is to synthesize and examine the literature on the use of neuraxial anesthesia and analgesia during cardiothoracic surgery. As cardiothoracic procedures often require systemic anticoagulation, neuraxial techniques are quite often underutilized due to the theoretical risk of epidural hematoma. In this review, we seek to examine the literature to review the indications and contraindications and to explore if neuraxial anesthesia and analgesia has a role in cardiothoracic surgery. RECENT FINDINGS: Neuraxial techniques have multiple advantages during

[Surgical correction of acute congenital cardiopathies in the newborn infant].

The experience of the Cardiac Surgery Service at the Hospital del Niño IMAN with the deep hypothermia method and hemodynamic arrest is shown in 30 infants with severe congenital cardiac malformations. The procedures followed before and during the operation are carefully described in their three aspects; anesthesia, extracorporeal circulation and surgery, including postoperative care. Ventilatory assistance is stressed. Results are analyzed considering postoperative evolution; stress is made on the causes of morbidity and mortality. Considering the results of this and other similar reports, we

Characteristics and neurological survival following intraoperative cardiac arrest in a Swiss University Hospital: a 7-year retrospective observational cohort study.

INTRODUCTION: Little is known about intraoperative cardiac arrest during anesthesia care. In particular, data on characteristics of cardiac arrest and neurological survival are scarce. METHODS: We conducted a single-center retrospective observational study evaluating anesthetic procedures from January 2015 until December 2021. We included patients with an intraoperative cardiac arrest and excluded cardiac arrest outside of the operating room. The primary outcome was the return of spontaneous circulation (ROSC). Secondary outcomes were sustained ROSC over 20 min, 30-day survival, and favorable

Anesthetic management of a parturient implanted with a permanent pacemaker--a case report.

At the present time, a patient with a pacemaker who undergoes an anesthesia for a surgical procedure is very common, but a parturient with a permanent pacemaker scheduled for elective cesarean section (c/s) is very rare. Complete heart block in pregnancy is not a common event and it may be congenital or acquired, particularly secondary to cardiac surgery. In normal women, the heart rate is physiologically increased commensurate with need, whereas in a parturient with installment of an implanted fixed rate pacemaker of VVI mode her heart rate cannot accelerate to cope with increased physiologic

Subxiphoid pericardial window in the diagnosis of penetrating cardiac trauma.

During a 15-month period, a subxiphoid pericardial window was performed as a diagnostic method to rule out cardiac injury in 76 patients with penetrating wounds near the heart. Patients with an obvious diagnosis of cardiac tamponade or patients in severe shock were excluded. Seventy-four patients were male, and 2 were female. The average age was 26.7 years. The procedure was done under general anesthesia in every case. There were no false-positive or false-negative results. There were no deaths, and the morbidity rate was 1.3%. In 16 patients (21%), the procedure identified hemopericardium. In

Recent Developments in Catheter-based Cardiac Procedures.

Since the first transcatheter aortic valve implantation in 2002, catheter-based cardiac procedures have increased significantly. This review provides an overview of CE approved devices, updated guidelines, and the key clinical findings for TAVR as well as for AV Valve interventions. Recent studies compare balloon-expanding and self-expanding valves, highlighting differences in prosthesis mismatch and paravalvular leaks. Long-term trials confirm TAVR's efficacy, especially in low-risk patients. Challenges include paravalvular regurgitation, vascular complications, and coronary obstruction, with

A National Survey: Current Clinical Practice in Pediatric Anesthesia for Congenital Heart Surgery.

BACKGROUND: Providing anesthesia for pediatric patients undergoing congenital cardiac surgery is complex and requires profound knowledge and clinical experience. Prospective studies on best anesthetic management are missing, partially due to different standards. The aim of the present study was to survey the current standard practice in anesthetic management in pediatric cardiac surgical centers in Germany. METHODS: All 78 cardiac surgical centers in Germany were reviewed for a congenital cardiac surgery program. Centers with an active program for congenital cardiac surgery were interviewed t

Incidents and complications during pediatric cardiac catheterization.

BACKGROUND: Cardiac catheterization has revolutionized the management of pediatric cardiac disease. There has been little information on adverse events during these cases from an anesthesia viewpoint. The aim of this audit was to determine the incident rate during pediatric cardiac catheterization as contemporaneously reported by the anesthetist and to identify both the types of events and which procedures had the highest risk. METHODS: Since 1993, data have been collected prospectively on an audit form for every anesthetic given in our institution, and in-theatre events were recorded on this

Effects of regional analgesia on stress responses to pediatric surgery.

Invasive surgery induces a combination of local response to tissue injury and generalized activation of systemic metabolic and hormonal pathways via afferent nerve pathways and the central nervous system. The local inflammatory responses and the parallel neurohumoral responses are not isolated but linked through complex signaling networks, some of which remain poorly understood. The magnitude of the response is broadly related to the site of injury (greater in regions with visceral pain afferents such as abdomen and thorax) and the extent of the trauma. The changes include alterations in metab

Anesthesia for percutaneous transcatheter closure of atrial and ventricular septal defects in pediatric patients.

We aimed to investigate the anesthetic management of percutaneous closure of atrial and ventricular septal defects (ASD/VSD) in pediatric patients. A retrospective review of the anesthetic data of 351 patients who underwent transcatheter closure of ASD/VSD was conducted. The mean age was 8.42 ± 5.71 years (4 months-18 years). VSD closure was performed in 52 patients and the remaining 299 had a procedure for ASD closure. All patients were premedicated with midazolam. All procedures were performed under general anesthesia in the catheterization laboratory. After anesthesia induction with sevoflu

[Complications during implantation of 3260 hip endoprostheses under spinal anesthesia].

There is a tendency to perform increasingly severe operations on patients of advanced age. Because geriatric patients are of greater risk when under anesthesia during operation, it is necessary to be aware of the possible anesthesia-related and operation-related complications. In a retrospective study the complication rate with regard to patients (age, multimorbidity), operation (e.g. number and duration of procedures) and anesthesia was analyzed. Of particular interest were the kind and degree of certain complications at a particular time. Directly following the induction of anesthesia and th

Anesthesia during cardiologic procedures.

PURPOSE OF REVIEW: Cardiovascular diseases are counted among the most prevalent morbidities in western nations and they still are the leading cause of death in these countries. An increasingly aging population will confront all physicians with a growing number of patients having cardiovascular dysfunction. Anesthesiologists need to be familiar with upcoming new cardiologic procedures to provide hemodynamic stability during these procedures. RECENT FINDINGS: Cardiovascular diseases are often combined with severe comorbidities such as renal failure, chronic obstructive pulmonary disease, pulmon

Quality comparison of remote anesthetic consultation versus on-site consultation in children with sedation for a magnetic resonance imaging examination-A randomized controlled trial.

BACKGROUND: In the course of the corona pandemic, digital media has increasingly been used in many areas of medical practice to reduce personal contact. As it is of interest whether this can be practiced in the context of anesthesia consultations without loss of quality, we interviewed parents whose children received a cardiac or neuro magnetic resonance imaging (MRI) under sedation. Parents either received an on-site or a remote consultation conducted by an anesthesiologist. Both parents and anesthesiologist were asked to indicate their satisfaction with the respective consultation procedure

[Intraoperative coronary spasm in patients without a history of anginal chest pain].

We report three cases of intraoperative coronary spasm that developed during non-cardiac surgical procedures. None of the patients had a history of anginal chest pain. The presumed contributing factors were: 1) suction of the trachea during general anesthesia, 2) hyperventilation and hypotension during induction of general anesthesia, and 3) hyperventilation during neuroanesthesia. Coronary vasodilators were administered and all cases recovered promptly without any clinical sequelae. A review of the literature reveals that the majority of patients who developed intraoperative coronary spasm ha

The elderly patient and cardiac surgery - a mini-review.

INTRODUCTION: Due to the increase in average life expectancy and the higher incidence of cardiovascular disease with advancing age, more elderly patients present for cardiac surgery nowadays. Advances in pre- and postoperative care mean that more elderly patients can be operated on safely and with a satisfactory outcome. Currently, coronary artery bypass surgery, aortic and mitral valve surgery and surgery of the ascending aorta are performed in elderly patients. OBJECTIVE: In this review, we summarize the outcome of elderly patients undergoing various cardiac surgical procedures and give fut

Anesthesia for non-obstetric surgery during late term pregnancy in mares.

Submitting late-term pregnant mares to anesthesia for non-obstetric surgery raises concerns about the survival of the mother and fetus. This study aimed to evaluate and describe transient maternal and fetal hemodynamic changes during general inhalation anesthesia in mares during the last month of gestation. Nine adult mares in the last month of gestation were subjected to general inhalation anesthesia and dorsal recumbency for 90 minutes. Trans-anesthetic vital parameters, arterial hemogasometry, cardiac output, pulmonary arterial pressure, central venous pressure, and fetal heart rates were a

Anesthesia management of surgery for sigmoid perforation and acute peritonitis patient following heart transplantation: case report.

Here we described a case in which a patient underwent emergency laparotomy for acute peritonitis and sigmoid perforation under general anesthesia with a history of heart transplantation. A good knowledge in the physiology of the transplanted heart is critical for effective and safe general anesthesia. We chose etomidate that have a weaker impact on cardiovascular function plus propofol for induction, and propofol plus cisatracurium for maintenance with intermittently analgesics and vasoactive drugs to facilitate the anesthesia. In addition, fluid input, electrolyte and acid-base balance were w

Emergency airway management in the post anesthesia care unit.

PURPOSE OF REVIEW: Airway management is one of the most challenging aspects in the field of anesthesia. This is also the case when an emergency airway situation arises in the postoperative patient. These airway procedures are often classified as advanced with high complexity. This article aims to address emergency airway management in the post anesthesia care unit. RECENT FINDINGS: The way in which care and supervision are organized in the post anesthesia care unit (PACU) varies between hospitals. Airway management in the PACU has typical challenges related to the team, the location itself, a

Single-trocar thoracoscopic pericardio-pleural fenestration under local anesthesia for malignant pleural effusion: a case report.

BACKGROUND: Pericardio-pleural fenestration by video-assisted thoracoscopic surgery is an efficient procedure for malignant pericardial effusion, but requires general anesthesia with single-lung ventilation. CASE PRESENTATION: A 43-year-old woman was referred with complaints of deteriorating dyspnea and orthopnea. Chest computed tomography revealed right massive pleural effusion and pericardial effusion. Echocardiography demonstrated collapse of both the right atrium and right ventricle due to cardiac tamponade. Semi-rigid thoracoscopic pleural biopsy and pericardio-pleural fenestration were
