Regional anaesthesia: what surgical procedures, what blocks and availability of a "block room"?

PURPOSE OF REVIEW: With an expected rise in day care procedures with enhanced recovery programs, the use of specific regional anaesthesia can be useful. In this review, we will provide insight in the used regional block and medication so far known and its applicability in a day care setting. RECENT FINDINGS: Regional anaesthesia has been improved with the aid of ultrasound-guided placement. However, it is not commonly used in the outpatient setting. Old, short acting local anaesthetics have found a second life and may be especially beneficial in the ambulatory setting replacing more long-acti

COVID-19: Regional Anesthesia, Benefits, and Practical Recommendations.

During coronavirus disease 2019 (COVID-19) pandemic, efforts have been made to rethink the health system and provide various recommendations to the best care of patients and for the protection of health personnel. In patients with suspicion or confirmation of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) who require surgical intervention and anesthetic management, strategies must be established to minimize aerosol-generating procedures. Regional anesthesia (RA) is not considered an aerosol-generating procedure per se and is currently proposed such as a safe strategy and part of

Comparison of intravenous regional anaesthesia with lidocaine and ropivacaine in dogs.

The present study was designed to compare the effects of lidocaine and ropivacaine in intravenous regional anaesthesia (IVRA) in dogs. Twelve adult male dogs were used. Under isoflurane anaesthesia, exsanguination was performed in the target forelimb. Then, a blood pressure cuff was encircled around the limb proximal to the elbow joint with a pressure of approximately 150 mmHg above the mean arterial blood pressure. The animals then received one of the two treatments of lidocaine (3 mg/kg) or ropivacaine (1.5 mg/kg) with a final volume of 0.6 mL/kg into the cephalic vein. After 60 min, the ana

Perioperative costs of local or regional anesthesia versus general anesthesia in the outpatient setting: a systematic review of recent literature.

BACKGROUND AND OBJECTIVES: In this systematic review, we carried out an assessment of perioperative costs of local or regional anesthesia versus general anesthesia in the ambulatory setting. METHODS: A systematic literature search was conducted to find relevant data on costs and cost-effectiveness analyses of anesthesia regimens in outpatients, regardless of the medical procedure they underwent. The hypothesis was that local or regional anesthesia has a lower economic impact on hospital costs in the outpatient setting. The primary outcome was the average total cost of anesthesia calculated on

A simple guide to regional anaesthesia.

Regional anaesthesia is widely used in modern anaesthetic practice for perioperative and postoperative analgesia. In the operating theatre, regional anaesthesia is used both on its own and in combination with other techniques (general anaesthesia and sedation). Regional anaesthesia is now a core skill set in anaesthetic training. This article provides a basic outline of regional anaesthesia for surgeons and other non-anaesthetic staff working with anaesthetists, reviewing preparation, consent, basic and specialist equipment, central neuraxial blocks (spinal, epidural and caudal), trunk blocks,

Future of regional anaesthesia.

The development and refinement of regional anaesthetic techniques for various types of surgery, mainly obstetric, ophthalmic and orthopaedic surgery, and of continuous regional analgesia continues. Suitable analgesic drug mixtures, and concentrations, will be further tested in order to find the ideal analgesic regimen for each type of surgery and for the individual patient. No new local anaesthetics or equipment for clinical use are expected in the near future. Improvement therefore depends much on how the anaesthesiologists use the present drugs, needles, nerve detection devices, catheters an

Regional anaesthesia for intraocular surgery.

The role of anaesthetists in providing local anaesthesia for intraocular surgery has changed over the past decade. No longer confined to the interested few, more and more anaesthetists are involved in monitored care and/or are performing eye block anaesthesia. This review summarizes the information related to eye block anaesthesia. The salient features of the orbital anatomy important for safe conduct of eye block anaesthesia are described. The techniques for retrobulbar and peribulbar anaesthesia, including facial nerve blocks, anaesthetic mixture, types of needles, and softening the eye are

Prilocaine for Bier's block: how safe is safe?

Prilocaine has become the agent of choice for Bier's block (or intravenous regional anaesthesia--IVRA), since 1983 when the product licence of bupivacaine was withdrawn for this purpose owing to fatal or serious complications. No serious complications have been documented in the literature relating to prilocaine in IVRA and we have conducted a survey within the U.K. which indicates that about 45,000 Bier's blocks have been carried out with prilocaine without convulsion, arrhythmia or fatality. This includes cases of accidental cuff deflation or even failure to inflate the cuff, resulting in bo

Preoperative evaluation of patients with electrocardiographic conduction defects.

Prophylactic pacemakers should be recommended for all preoperative patients with second- and third-degree heart block, regardless of whether the block is in the AV node or the trifascicular conduction system. Symptomatic patients with potential for trifascicular block should also have a transvenous pacemaker. Those without symptoms should be monitored during anesthesia and the postoperative recovery period.

Choosing the anesthetic and sedative drugs for supraventricular tachycardia ablations: A focused review.

This study provides a review of the contemporary literature for the effects of most commonly used anesthetic drugs for sedation and anesthesia during adult electrophysiologic (EP) studies where supraventricular tachycardias (SVT) need to be induced for diagnostic purposes and/or catheter ablation. Some medications may affect cardiac electrophysiology and conduction, altering the ability to induce the arrhythmia, and may have negative impact on mapping and ablation treatment. The objective of the study is to determine the best sedative choice during SVT ablations. The authors searched MEDLINE,

[Long-term peridural anesthesia in fractures of pelvic bones].

A continuous peridural anesthesia was used in 20 patients with pelvic bones injuries. Puncture of the peridural space was performed in an appropriate patient's posture. During 3--15 days 5--12 ml of 2% trimecain solution were infused via a catheter. For patients being in a shock condition the anesthetic dose was 3-4 ml less. 4-5 minutes following injection of the anesthetic in the peridural space pain at the site of fracture disappeared, there was noted a stabilization of hemodynamic indices, and the ventilatory and cardiac functions were improved. Anesthesia contributes to an early restoratio

Nerve conduction studies in experimental non-freezing cold injury: I. Local nerve cooling.

In rabbits, the tibial nerve was exposed in the lower thigh under general anesthesia and cooled in a metal trough at 1 to 2 degrees C or 5 degrees C for 2, 3, or 4 hours. Nerve conduction studies showed local failure of conduction at the site of cooling which persisted after rewarming, and which was followed by distal degeneration of affected fibers. No persistent conduction block was seen. Changes in maximal velocity indicated that the fastest-conducting motor and afferent axons had been preferentially affected. Histological findings in nerves examined at different intervals after cooling con

Regional versus general anesthesia for hip fracture surgery in older adults: A focused research review.

Hip fractures in the elderly population are associated with considerable complications such as morbidity, mortality, and enduring functional deterioration. Thus, they render perioperative management an essential element of strategies aimed at fostering healthy aging. Regional anesthesia (RA) and general anesthesia (GA) constitute the two predominant methods for the surgical intervention of hip fractures; however, the most effective technique remains a subject of ongoing scholarly debate. This research review seeks to systematically assess and compare regional anesthesia to general anesthesia i

Effect of anesthesia for primary cesarean section on postoperative infectious morbidity.

It has been stated that general anesthesia is a risk factor for postcesarean infectious morbidity. A retrospective review of 252 women who had undergone primary cesarean section at the University of Iowa Hospital was conducted. Regional anesthesia was successfully administered to 170 patients, and general anesthesia was administered to 82 patients. Patients receiving general anesthesia were more likely to be indigent, to receive a vertical skin incision, and to be transfused intraoperatively and/or postoperatively. There were no statistically significant differences between the two groups with

Does the type of anaesthesia influence the outcome of percutaneous nephrolithotomy? Outcomes from a meta- analysis of randomized controlled trials.

OBJECTIVE: To systematically review the outcomes of percutaneous nephrolithotomy (PCNL) performed in local (LA) or regional anesthesia (RA) as compared to general anesthesia (GA). METHODS: Literature search was conducted on 12th April 2024 including PubMed, Medline, Embase, and Scopus database. Complications were assessed using the Cochran-Mantel-Haenszel Method with the random effect model and reported as odds ratio (OR), 95% confidence interval (CI), and p-values. Analyses were two-tailed and the significance was set at p < 0.05 and a 95% CI. Continuous variables were pooled using the inver

Neuraxial anesthesia in a parturient with common variable immunodeficiency: a case report.

Choosing whether or not to initiate neuraxial anesthesia in pregnant women with immune system defects may be challenging. Anesthesiologists have the responsibility of making the best decision in terms of anesthesia management for both mother and baby during the labor and delivery process. Whether neuraxial anesthesia is associated with an increased risk of central nervous system infection in immunocompromised compared with healthy patients is unknown. It is also unclear if maternal immune modulation required for fetal tolerance makes pregnant women susceptible to pathogens and causes an altere
