[Horner syndrome and brachial plexus blockade after epidural anesthesia for obstetric labor and cesarean section].

A 30-year-old woman (ASA II, obese) in her 40th week of a first pregnancy required epidural analgesia for labor. When the cervix had dilated to 5 cm, the epidural infusion was started with a 9-mL bolus of 0.2% ropivacaine and 50 pg of fentanyl, after a negative test dose. An infusion of 0.2% ropivacaine and 1 microg/mL of fentanyl was started at a rate of 8 mL/h. A cesarean section was required after insufficient progress was made during 8 hours of labor. Eight milliliters of 0.75% ropivacaine was administered to provide anesthesia to T4; cesarean delivery was completed without complications.

[Anesthesia in obstetric patient with sickle cell anemia and thalassemic trait after plasmapheresis: case report].

BACKGROUND AND OBJECTIVES: Plasmapheresis is the technique of choice for severe hemolytic anemia patients. A consequence is plasma cholinesterase depletion, which interferes with metabolism of some neuromuscular blockers currently used in anesthesiology. CASE REPORT: Pregnant patient, 26 years old, physical status ASA IV, 30 weeks and 3 days gestational age, with sickle cell anemia, thalassemic trait and allo-immunization for high frequency antigens. Patient presented sickling crisis being transfused with incompatible blood. Patient evolved with massive hemolysis being admitted with 3 g/dL he

Neuraxial analgesia in pregnant individuals with bleeding disorders: a retrospective descriptive study of obstetric anesthesia practices and outcomes.

BACKGROUND: Neuraxial analgesia is an effective and advantageous technique for managing labor pain. Yet, clinicians often hesitate to offer neuraxial analgesia to parturients with inherited bleeding disorders because of fear of epidural or spinal hematoma and the absence of high-quality evidence to guide decision-making in practice. OBJECTIVES: To describe obstetric analgesia practices and outcomes in pregnant individuals with bleeding disorders managed at a tertiary care center. METHODS: We performed a retrospective descriptive study of all deliveries (January 2010-July 2021) managed by the

Anesthetic and obstetric predictors of general anesthesia in urgent or emergent Cesarean delivery: a retrospective case-control study.

PURPOSE: While regional anesthesia (RA) is considered preferable to general anesthesia (GA) for Cesarean delivery (CD), certain situations necessitate GA. This study reviewed the practice patterns around the use of GA for CD to identify modifiable predictors of GA with the goal of reducing GA rates. METHODS: This was a retrospective, case-control study. Patients undergoing urgent/emergent CD over a 3-year period were identified, from which 102 patients undergoing GA and 102 patients undergoing RA were randomly selected. The data included patient characteristics, obstetrical indications for CD

Type of obstetric anesthesia administered and complications in women with preeclampsia in low- and middle-income countries: A systematic review.

BACKGROUND: Delivery is often expedited with cesarean section, necessitating anesthesia, to prevent complications in women with preeclampsia. Anesthesia-associated risks in these women from low- and middle-income countries (LMICs) are not known. METHODS: We searched major databases (until February 2017) for studies on general vs. regional anesthesia in women with preeclampsia. We summarized the association between outcomes and type of anesthesia using a random effects model and reported as odds ratio (OR) with 95% confidence intervals (95% CIs). FINDINGS: We included 14 studies (10,411 pregn

[Experiences with combined catheter-peridural anesthesia and nitrous oxide-oxygen ventilation in patients at risk in gynecology and obstetrics].

In 271 gynaecological or obstetrical patients a "combined anaesthesia method" was applied, consisting of lumbar catheter epidural analgesia and controlled ventilation with nitrous oxide/oxygen. We aim to show that by mere interaction of the local anaesthetic, with nitrous oxide, and the drugs used for premedication and during induction of anaesthesia a stage of sleep can be achieved, which, on the patient's behalf is identical with his idea of "full anaesthesia", while at the same time the use of centrally acting agents can be kept at a minimum. In incidents of sympatho-adrenergic-stimulation

Neurotoxic Myelitis Following Accidental Epidural Injection of Chlorhexidine During Obstetric Epidural Anesthesia: A Case Report.

We report a case of neurotoxic myelitis following the accidental epidural injection of chlorhexidine during obstetric anesthesia at a peripheral hospital. A 32-year-old parturient in labor received an epidural catheter for vaginal delivery, during which 3 mL of chlorhexidine was mistakenly injected into the epidural space. This led to progressive paraplegia, severe headaches, and respiratory distress requiring intubation. The patient was subsequently transferred to our anesthesia and critical care department for further management. Radiological findings revealed centromedullary edema and myeli

Pragmatic approach to neuraxial anesthesia in obstetric patients with disorders of the vertebral column, spinal cord and neuromuscular system.

Neuraxial anesthesia provides optimal labor analgesia and cesarean delivery anesthesia. Obstetric patients with disorders of the vertebral column, spinal cord and neuromuscular system present unique challenges to the anesthesiologist. Potential concerns include mechanical interference, patient injury and the need for imaging. Unfortunately, the existing literature regarding neuraxial anesthesia in these patients is largely limited to case series and rare retrospective studies. The lack of practice guidance may lead to unwarranted fear of patient harm and subsequent avoidance of neuraxial anest

Obstetric neuraxial anesthesia at low platelet counts in the context of immune thrombocytopenia: a systematic review and meta-analysis.

PURPOSE: Primary immune thrombocytopenia (ITP) is an autoimmune condition affecting women of childbearing age that is characterized by diminished platelet quantity with preserved function. Although pregnant women with ITP are often denied obstetric neuraxial anesthesia (OBNA) with low platelet counts for fear of neuraxial hematoma, the true magnitude of neuraxial hematoma for ITP parturients is unknown. The aim of this systematic review and meta-analysis was to examine OBNA outcomes in ITP parturients with platelet counts below 100 x 109·L-1. SOURCE: Articles published in MEDLINE, Embase, Web

Obstetric anesthesia for the obese and morbidly obese patient: an ounce of prevention is worth more than a pound of treatment.

BACKGROUND: The incidence of obesity has been dramatically increasing across the globe. Anesthesiologists, are increasingly faced with the care for these patients. Obesity in the pregnant woman is associated with a broad spectrum of problems, including dramatically increased risk for cesarean delivery, diabetes, hypertension and pre-eclampsia. A thorough understanding of the physiology, associated conditions and morbidity, available options for anesthesia and possible complications is therefore important for today's anesthesiologist. METHODS: This is a personal review in which different aspec

General Anesthesia for Cesarean Delivery for Thrombocytopenia in Hypertensive Disorders of Pregnancy: Findings From the Obstetric Airway Management Registry.

BACKGROUND: In resource-limited environments, spinal anesthesia (SA) is preferred for cesarean delivery. In women at risk of spinal epidural hematoma, particularly those with hypertensive disorders of pregnancy, thrombocytopenia should be excluded before neuraxial blockade. In the context of emergency surgery for fetal distress, this investigation may be hampered by laboratory services being unavailable or off-site. METHODS: The Obstetric Airway Management Registry (ObAMR) is currently active across all anesthesia training institutions affiliated with the University of Cape Town. This multice

Bridging the Evidence-Practice Gap in Breastfeeding After Anesthesia: A Case Report and Practical Framework for the Non-obstetric Provider.

Despite updated recommendations from major professional societies, outdated guidance to interrupt breastfeeding after anesthesia persists in clinical practice. This discrepancy reflects an evidence-practice gap, particularly in non-obstetric perioperative settings and in cases involving newer anesthetic agents. This report describes a 32-year-old lactating female undergoing elective laparoscopic abdominal surgery under general anesthesia using a multimodal approach, including dexmedetomidine and sugammadex. Detailed perioperative management, medication selection, and counseling strategies are

Reducing practice variability in obstetric anesthesia through a structured, team-based implementation strategy: a quality improvement study.

BACKGROUND: Practice variability in obstetric anesthesia remains common despite evidence-based recommendations, and standardizing care may be challenging in clinical practice settings. We hypothesized that a peer-led implementation process grounded in Society for Obstetric Anesthesia and Perinatology (SOAP) Centers of Excellence benchmarks recommendations, combining structured weekly meetings and a co-authored digital reference tool, could achieve clinically meaningful reductions in self-reported practice variability within a newly established obstetric anesthesia team. METHODS: This prospect
