A comparison of semantic feature analysis and phonological components analysis for the treatment of naming impairments in aphasia.

Therapy for naming impairments post-stroke typically involves semantic and/or phonologically-based tasks. However, the relationship between individuals' locus of breakdown in word retrieval and their response to a particular treatment approach remains unclear, and direct comparisons of treatments with different targets (semantics, phonology) yet similar formats are lacking. This study examined eight people with aphasia who each received 12 treatment sessions; half the sessions involved a semantically-based treatment task, Semantic Feature Analysis (SFA), and the other half involved a phonologi

Amnestic disorders and their role in cognitive theory.

Amnesia refers to a disorder of memory, and classical amnesia with continuous problems in acquiring new information is the most studied type of amnesia. The integration into a memory systems model of clinical studies of classical amnesia, cognitive theory, and studies with neuroimaging methods is one of the prime success stories in modern cognitive neuroscience. The clinical spectrum of amnesia encompasses a wide range of disorders ranging from specific encoding deficits for language or visuospatial information to psychogenic amnesia with confusion and loss of memory for personal identity. Semantic memory and reading abilities: a case report.

We document the unexpected dissociation of preserved reading skills in a patient with severely impaired semantic memory. The common co-occurrence between impairment of word meaning and surface dyslexia has not been observed. The patient (hereafter called DRN) had marked naming and word comprehension difficulties. A strong word frequency effect was observed on tests of word comprehension but was absent in a test of word reading. DRN's ability to read both regular and exception words that he failed to comprehend was remarkably well preserved. We will argue that these findings provide further sup

WAIS performance IQ in aphasia as a function of auditory comprehension and constructional apraxia.

This study investigated differences in nonverbal intelligence between diagnostic subgroups of aphasics using the Performance Tests of the Wechsler Adult Intelligence Scale (WAIS). Subjects were 98 right-handed hospitalized males with unilateral left-hemisphere damage who had unambiguous clinical diagnoses of Broca's, Anomic, Conduction, Mixed Non-Fluent, Wernicke's, or Global aphasia. Group differences were examined using the WAIS Performance IQ (PIQ), and subscores for spatial organization (Block Design and Object Assembly) and for verbalizability (Picture Completion and Picture Arrangement).

Aphasia and herpes virus encephalitis: a case study.

CONTEXT: Meningoencephalitis early in life, of any etiology, is a risk factor for development of subsequent sequelae, which may be of physical, psychiatric, behavioral or cognitive origin. Anomia is a language abnormality frequently found in such cases, and other language deficits are rarely described. The aim of this study was to describe the cognitive and linguistic manifestations following a case of herpetic meningoencephalitis in a 13-year-old patient with eight years of schooling. CASE REPORT: The patient underwent a speech-language audiology assessment nine months after the neurological

Aphasia or dementia: the cautionary tale of Dr. JJ.

This case study involves the differential diagnosis of aphasia versus dementia. However, the case of Dr. JJ is instructive from a number of aspects, presented here as "cautionary notes" which are intended to remind clinicians of sometimes-forgotten interpersonal issues in the diagnosis of disorders of speech and language. This case also is intended to illustrate that merely distinguishing between dementia and aphasia is sometimes inadequate, particularly when potentially transmittable (and in some cases, curable) dementias might be involved.

Sign language aphasia from a neurodegenerative disease.

While Alois Alzheimer recognized the effects of the disease he described on speech and language in his original description of the disease in 1907, the effects of Alzheimer's disease (AD) on language in deaf signers has not previously been reported. We evaluated a 55-year-old right-handed congenitally deaf woman with a 2-year history of progressive memory loss and a deterioration of her ability to communicate in American Sign Language, which she learned at the age of eight. Examination revealed that she had impaired episodic memory as well as marked impairments in the production and comprehens

Seeing red primes tomato: evidence for comparable priming from colour and colour name primes to semantically related word targets.

There is ample evidence that an independent processing stream exists that subserves the perception and appreciation of colour. Neurophysiological research has identified separate brain mechanisms for the processing of wavelength and colour, and neuropsychological studies have revealed selective colour disorders, such as achromatopsia, colour agnosia, and colour anomia. The aim of the present study is to investigate whether the perception of colour may, despite its independent processing, influence other cognitive functions. Specifically, we investigate the possibility that the perception of a

Hemispheric disconnection syndrome with a "crossed avoiding" reaction in a case of Marchiafava-Bignami disease.

A clinicopathological study is presented of a case of Marchiafava-Bignami disease with a hemispheric disconnection syndrome, an association that does not appear to have been reported previously. Gross and microscopic examination of the brain revealed necrosis of the corpus callosum (sparing a small area in front of the splenium) and of the anterior commissure, cortical and subcortical infarction of the right lingual gyrus, diffuse cortical lesions of the laminar sclerosis type, and lacunae in the basal ganglia and the pons. The patient was unable to grasp objects presented to the right visual

Medial Pulvinar Nucleus as a Causal Hub for Heteromodal Naming.

Our ability to retrieve the names of objects in our environment is a fundamental aspect of everyday life. This process requires a complex, dynamic network of cortical and subcortical interactions. While the cortical constituents of this network have been extensively studied with intracranial recordings, the subcortical nodes of the naming network are unclear. We probed the role of the left medial pulvinar nucleus in naming with direct intracranial recordings and stimulation in eight humans (three male, five female) as they named objects using pictures and auditory and written descriptions. We

Transcranial magnetic stimulation improves naming in Alzheimer disease patients at different stages of cognitive decline.

OBJECTIVE: Word-finding difficulty (anomia) is commonly observed in Alzheimer's dementia (AD). The aim of this study was to assess the effect of repetitive transcranial magnetic stimulation (rTMS) applied to the dorso-lateral prefrontal cortex (dlPFC) on picture naming in 24 probable AD patients with different degrees of cognitive decline. METHODS: High-frequency rTMS was applied to the left and right dlPFC during object and action naming in AD patients. A sham stimulation was used as a control condition. RESULTS: Whilst, as previously reported, stimulation to both the left and the right dlP

[Primary progressive aphasia: clinical aspects].

Primary progressive aphasia (PPA), initially described by Mesulam, is a syndrome of progressive deterioration of language, occurring in the presenium. Several classifications have been proposed, but the most useful one distinguishes non fluent and fluent forms of PPA. Both begin by anomia. In non fluent PPA, there is a progressive reduction of language, sometimes with aggramatism and articulatory impairment, but without impairment of comprehension. Fluent PPA is characterized by preserved fluency with severe impairment of single word comprehension. It is frequently confounded with semantic dem

Acquired alexia with agraphia syndrome in childhood.

The acquired alexia with agraphia syndrome is a conspicuous disorder of reading and writing in the absence of significant other language impairments that has mainly been recorded in adults. Pure cases are rare, with most patients displaying mild aphasic deficits. In children, acquired reading and writing disorders are generally reported as part of more encompassing aphasic syndromes affecting oral and written language equally, for example, Broca or Wernicke aphasia. Documented instances of predominant acquired reading and writing disorders in childhood are exceptional. We report an 11-year-old

Assessing Relative Linguistic Impairment With Model-Based Item Selection.

PURPOSE: A picture naming test is presented that reveals impairment to specific mechanisms involved in the naming process, using accuracy scores on curated item sets. A series of psychometric validation experiments are reported. METHOD: Using a computational model that enables estimation of item difficulty at the lexical and sublexical stages of word retrieval, two complimentary sets of items were constructed that challenge the respective psycholinguistic levels of representation. The difference in accuracy between these item sets yields the relative linguistic impairment (RLI) score. In a co

Assessment and treatment of short-term and working memory impairments in stroke aphasia: a practical tutorial.

BACKGROUND: Aphasia following stroke refers to impairments that affect the comprehension and expression of spoken and/or written language, and co-occurring cognitive deficits are common. In this paper we focus on short-term and working memory impairments that impact on the ability to retain and manipulate auditory-verbal information. Evidence from diverse paradigms (large group studies, case studies) report close links between short-term/working memory and language functioning in aphasia. This evidence leads to the hypothesis that treating such memory impairments would improve language functio

Transient retrograde amnesia associated with impaired naming of living categories.

A ease of pure retrograde amnesia following mild head injury is reported. The patient, who also showed a deficit in verbal fluency and a living/non-living dissociation in naming during the amnesic period, recovered progressively in about ten days post-trauma. Both a psychogenic and an organic origin could be taken into account. A mechanism (no matter if psychogenic or organic) can be hypothesized, which produces a functional inhibition also involving performances that are unlikely to be affected by psychological factors.

Autotopagnosia. Occurrence in a patient without nominal aphasia and with an intact ability to point to parts of animals and objects.

A patient with a metastasis in the left parietal lobe demonstrated difficulty in pointing to body parts although he could name them when they were pointed to by someone else. As he had no nominal aphasia, could understand complex verbal instructions and could correctly describe the functions of named body parts, his autotopagnosia is unlikely to be a result of a category-specific comprehension deficit. As he is able to point to parts of objects, plants and animals on command, his disorder is also unlikely to be part of a general inability to analyse a whole into its details. Instead, his diffi

Cortico-subcortical organization of language networks in the right hemisphere: an electrostimulation study in left-handers.

We have studied the configuration of the cortico-subcortical language networks within the right hemisphere (RH) in nine left-handers, being operated on while awake for a cerebral glioma. Intraoperatively, language was mapped using cortico-subcortical electrostimulation, to avoid permanent deficit. In frontal regions, cortical stimulation elicited articulatory disorders (ventral premotor cortex), anomia (dorsal premotor cortex), speech arrest (pars opercularis), and semantic paraphasia (dorsolateral prefrontal cortex). Insular stimulation generated dysarthria, parietal stimulation phonemic para
